Laparoscopic management of remnant gall bladder with stones: Lessons from a tertiary care centre's experience

Objective:Laparoscopic cholecystectomy is currently the gold standard for treating symptomatic gallstone *** its success,approximately 10%of patients may experience persistent biliary symptoms,leading to the post-cholecystectomy syndrome.A remnant gallbladder with cystic duct or bile duct stones is one of the causes of this *** objective of this study was to shed light on the clinical manifestations,evaluation,therapeutic strategies,and outcomes associated with laparoscopic management of symptomatic remnant ***:This was a retrospective study,conducted over a five-year period(January 2017 to December 2022)at Apollo Hospitals in south *** patients who underwent laparoscopic completion cholecystectomy for a remnant gall bladder were *** following data were collected:patient demographics,symptoms,preoperative investigations,intraoperative details and post operative ***:In total,36 patients were included and *** majority of patients were male(25,69.4%),with a mean age of 50.7±12.1 *** most common presentation was pain in the upper abdomen or right upper quadrant region(24,66.7%).The laparoscopic approach was attempted in all patients,with a success rate of 94.4%.Two patients required conversion to open *** fistula to the colon was observed in one *** was observed in 7 patients(19.4%),and stone clearance was successfully achieved using endoscopic retrograde cholangiopancreatography in all patients ***:Incomplete gall bladder removal either intentionally or unintentionally leaves a remnant gall bladder that is at risk for stone formation and *** who have this clinical entity with symptoms require a redo or complete cholecystectomy,a complex procedure associated with certain *** study highlights the feasibility and safety of laparoscopic completion cholecystectomy for the management of remnant gallbladder

Vibration energy harvesting has emerged as a promising method to harvest energy for small-scale *** the performance of a vibration energy harvester(VEH)incorporating nonlinear techniques,for example,the snap-through VEH with geometric non-linearity,has gained attention in recent years.A conventional snap-through VEH is a bi-stable system with a time-invariant potential function,which was investigated extensively in the *** this work,a modified snap-through VEH with a time-varying potential function subject to harmonic and random base excitations is *** snap-through VEHs,such as the one considered in this study,are used in wave energy ***,the studies on their dynamics and energy harvesting under harmonic and random excitations are *** dynamics of the modified snap-through VEH is represented by a system of differential algebraic equations(DAEs),and the numerical schemes are proposed for its *** a harmonic excitation,the system exhibits periodic and chaotic motions,and the energy harvesting is superior compared with the conventional *** dynamics under a random excitation is investigated by the moment differential method and the numerical scheme based on the modified Euler-Maruyama *** Fokker-Planck equation representing the dynamics is derived,and the marginal and joint probability density functions(PDFs)are obtained by the Monte Carlo *** study shows that the modified snap-through oscillator based VEH performs better under both harmonic and random *** dynamics of the system under stochastic resonance(sR)is investigated,and performance enhancement is *** results from this study will help in the development of adaptive VEH techniques in the future.

At present,hundreds of cloud vendors in the global market provide various services based on a customer’s *** cloud vendors are not the same in terms of the number of services,infrastructure availability,security strategies,cost per customer,and reputation in the ***,software developers and organizations face a dilemma when choosing a suitable cloud vendor for their developmental ***,there is a need to evaluate various cloud service providers(CsPs)and platforms before choosing a suitable *** existing solutions are either based on simulation tools as per the requirements or evaluated concerning the quality of service ***,they require more time to collect data,simulate and evaluate the *** proposed work compares various CsPs in terms of major metrics,such as establishment,services,infrastructure,tools,pricing models,market share,etc.,based on the comparison,parameter ranking,and weightage ***,the parameters are categorized depending on the priority *** weighted average is calculated for each CsP,after which the values are sorted in descending *** experimental results show the unbiased selection of CsPs based on the chosen *** proposed parameter-ranking priority level weightage(PRPLW)algorithm simplifies the selection of the best-suited cloud vendor in accordance with the requirements of software development.

In this paper, a modified Newton type iterative method is considered for ap- proximately solving ill-posed nonlinear operator equations involving m-accretive mappings in Banach space. Convergence rate of the method is obtained based on an a priori choice of the regularization parameter. Our analysis is not based on the sequential continuity of the normalized duality mapping.

In the current study,we examined every possible cluster-daughter combination in the heavy-particle decay of isotopes ^(297-300)119 and computed the decay half-lives using the modified generalized liquid drop model(MGLDM)with the preformation factor depending on the disintegration *** predicted half-life of every heavy cluster(Z_(C)≥32)was within the experimentally observable *** results aligned with the predictions of Poenaru et al.[***.107,062503(2011)]that superheavy nuclei(sHN)with Z>110 will release heavy particles with a penetrability comparable to or greater than theα-*** half-lives predicted using the MGLDM for clusters^(89)Rb,^(91)Rb,and^(92)Rb from parents^(297)119,^(299)119,and^(300)119,respectively,agreed with the predictions of Poenaru et al.[***.J.A 54,14(2018)].It was found that the isotopes of heavy clusters kr,Rb,sr,Pa,In,and Cd had half-lives comparable to theαhalf-life;and isotopes of clusters I,Xe,and Cs had the minimum half-life(10^(-14)s).These observations revealed the role of the shell closure(Z=82,N=82,and N=126)of the cluster and daughter nuclei in heavy-cluster *** predicted that isotope ^(297,299)119 decayed by 4αdecay chains and isotope^(300)119 decayed by 6αdecay chains,while^(298)119 decayed by continuousαdecay *** predicted half-lives and modes of decay of the nuclei in the decay chains of^(297-300)119 agreed with the experimental data,proving the reliability of our *** present study determined the most favorable heavy-cluster emissions from these nuclei and provided suitable projectile-target combinations for their synthesis.

The purpose of the study is to obtain a cement grout with improved performance. The grout mixes of the present study contain cement, ultra-fine slag （UFs）, super plasticizer and water. Properties like flowability, bleeding, compressive strength and shrinkage of cement grouts have been studied. Rheological parameters were also studied in order to explain the grout workability. The results show that, cement replacement with slag in grouts could reduce bleeding substantially without affecting the workability of the mixes. Introduction of slag enhanced the compressive strength and reduced shrinkage reasonably. Ultra-fine slag can be used as a supplementary cementitious material in cementitious grouts in order to improve the grout behavior.

A multigrid-assisted solver tbr the three-dimensional time-dependent incompressible Navier-stokes equations on graded Cartesian meshes is developed. The spatial accuracy is third-order for the convective terms and fourth-order for the viscous terms, and a fractional-step strategy ensures second-order time accuracy. To achieve good time-wise efficiency a multigrid technique is used to solve the highly time-consuming pressure-Poisson equation that requires to be solved at every time step. The speed-up achieved by multigrid is shown in tabular form. The performance and accuracy of the code are first ascertained by computing the flow in a single-sided lid-driven cubic cavity with good grid-economy and comparing the results available in the literature. The code, thus validated, is then applied to a new test problem we propose and various transient and asymptotically obtained steady-state results are presented. Given the care taken to establish the credibility of the code and the good spatio-temporal accuracy of the discretization, these results are accurate and may be used for ascertaining the performance of any computational algorithm applied to this test problem.

Alcohol abuse and chronic hepatitis C virus(HCV)infection are two major causes of chronic liver disease in the United *** 10%-15%of liver transplants performed in the United states are for patients with cirrhosis due to combined alcohol and HCV *** on outcomes on graft and patient survival,HCV recurrence,and relapse of alcohol use comparing transplants in hepatitis C positive drinkers compared to alcohol abuse or hepatitis C alone are conflicting in the *** studies report a slightly better overall outcome in patients who were transplanted for alcoholic cirrhosis vs those transplanted for HCV alone or for combined HCV and alcohol related ***,some other studies do not support these ***,most studies are limited to a retrospective design or small sample *** prospective multicenter studies are needed to better define the outcomes in hepatitis C drinkers.
