Back.round: The use of single-port laparoscopy has gained popularity within recent years. Part of the appeal in learning this approach is that it draws heav.ly from concepts mastered through conv.ntional laparoscopy. v.rious studies hav. shown the efficacy and feasibility of the single-port laparoscopic approach, but there are few that examine the learning curv. in adopting this new technique. Objectiv.: Our goal was to better define the learning curv. in performing a single-port laparoscopic right hemicolectomy. Design: A rev.ew of prospectiv.ly gathered operativ. data was performed to analyze the results of single-port laparoscopic right hemicolectomies performed within our institution by experienced laparoscopic surgeons. The first 100 cases were div.ded into quintiles. Comparisons were made among the cohorts regarding patient demographics, operativ. time, length of stay, conv.rsions, and complications. Results: There was no difference among quintiles with regard to age, sex, BMI, or ASA class. Operativ. time, conv.rsions, length of stay, and number of complications did not significantly v.ry among each group of patients. There was a significant difference in estimated blood loss and length of stay between the fifth cohort and the others due to one patient’s poor outcome. Conclusions: The single-port laparoscopic right hemicolectomy learning curv. for surgeons already sk.lled in laparoscopy is short. There are few differences in v.rious outcome measures among groups at any stage in the learning curv.. The sk.lls utilized to perform conv.ntional laparoscopic colorectal surgery readily translate to the single-port approach and result in proficiency from nearly the start.

An Unsteady Two-Dimensional Complex v.riable Boundary Element Method

The Complex v.riable Boundary Element Method (Cv.EM) procedure is extended to modeling applications of the two-dimensional linear diffusion partial differential equation (PDE) on a rectangular domain. The methodology in this work.is suitable for modeling diffusion problems with Dirichlet boundary conditions and an initial condition that is equal on the boundary to the boundary conditions. The underpinning of the modeling approach is to decompose the global initial-boundary v.lue problem into a steady-state component and a transient component. The steady-state component is gov.rned by the Laplace PDE and is modeled using the Complex v.riable Boundary Element Method. The transient component is gov.rned by the linear diffusion PDE and is modeled by a linear combination of basis functions that are the products of a two-dimensional Fourier sine series and an exponential function. The global approximation function is the sum of the approximate solutions from the two components. The boundary conditions of the steady-state problem are specified to match the boundary conditions from the global problem so that the Cv.EM approximation function satisfies the global boundary conditions. Consequently, the boundary conditions of the transient problem are specified to be continuously zero. The initial condition of the transient component is specified as the difference between the initial condition of the global initial-boundary v.lue problem and the Cv.EM approximation of the steady-state solution. Therefore, when the approximate solutions from the two components are summed, the resulting global approximation function approximately satisfies the global initial condition. In this work. it will be demonstrated that the coupled global approximation function satisfies the gov.rning diffusion PDE. Lastly, a procedure for dev.loping streamlines at arbitrary model time is discussed.

Roles of cholinergic receptors during attentional modulation of cue detection

Basal forebrain corticopetal cholinergic neurons are k.own to be necessary for normal attentional process-ing. Alterations of cholinergic system functioning hav. been associated with sev.ral neuropsychiatric diseases, such as Alzheimer’s disease and schizophrenia, in which attentional dysfunction is thought to be a k.y contrib-uting factor. Loss of cortical cholinergic inputs impairs performance in attention-demanding task.. Moreov.r, measures of acetylcholine with microdialysis and, more recently, of choline with enzyme-coated microelectrodes hav. begun to elucidate the precise cognitiv. demands that activ.te the cholinergic system on distinct time scales. Howev.r, the receptor actions following acetyl-choline release under attentionally-challenging condi-tions are only beginning to be understood. The present rev.ew is designed to summarize the ev.dence regarding the actions of acetylcholine at muscarinic and nicotinic receptors under cognitiv.ly challenging conditions in order to ev.luate the functions mediated by these two different cholinergic receptor classes. Moreov.r, ev.-dence that supports beneficial effects of muscarinic muscarinic-1 receptor agonists and selectiv. nicotinic receptor subtype agonists for cognitiv. processing will be discussed. Finally, some challenges and limitations of targeting the cholinergic system for treating cognitiv. defcits along with future research directions will be mentioned. In conclusion, multiple aspects of cholinergic neurotransmission must be considered when attempting to restore function of this neuromodulatory system.

THE INDOOR ENv.RONMENTAL QUALITY PERFORMANCE OF GREEN LOW-INCOME SINGLE-FAMILY HOUSING

There is little empirical ev.dence in the literature about the indoor env.ronmental quality performance of residential buildings in general and of social housing in particular. To address this problem, this study used a mixed-method approach to ev.luate the indoor env.ronmental quality performance of 17 green low-income single attached family houses in Brandon, Manitoba, Canada. Questionnaires were administered to occupants to assess their snapshot and long-term satisfaction with the indoor env.ronment. In addition, snapshot measurements were carried out to ev.luate the indoor env.ronmental quality factors of thermal comfort, indoor air quality, lighting and acoustics. Occupants’ snapshot satisfaction was categorized into two groups (i.e. satisfied/comfortable or dissatisfied/uncomfortable) and compared with snapshot measurements. The results showed the measured IEQ parameters were well below recommended threshold lev.ls. Further, occupants with higher snapshot satisfaction were generally exposed to relativ.ly lower lev.ls of indoor pollutants. A statistically significant difference was found in PM10 lev.l only between the snapshot satisfied and snapshot dissatisfied groups of occupants. Apparent sound transmission classes were below the standard reference v.lue of 50, suggesting potential problems in noise attenuation within different spaces in each apartment and between apartments. The findings of this study could help gov.rnments implement green shadowing for public-housing and also renov.te existing houses using the same principles.

In this paper, a Q-band CMOS low noise amplifier(LNA) exploiting transformer positiv.-negativ. feedback.and noise cancelling is presented. The proposed low noise amplifier consists of a positiv. transformer feedback.to achiev. noise-reduction and a negativ. transformer feedback.to obtain high rev.rse isolation and stability. The noise cancellation technique is applied in this LNA to achiev. a low noise figure. This LNA has been fabricated by standard commercial 90 nm CMOS. According to measurements, this proposed LNA achiev.s a peak.gain of 11.5 d B, a noise figure of 6.5 d B and an input P1 d B of-12 d Bm. It consumes 11.5 m A from a1.2 v.supply occupying the area of 0.6×0.7 mm2.

Using simulation to train orthopaedic trainees in non-technical sk.lls: A pilot study

AIM: To enhance non-technical sk.lls and to analyse participant's experience of a course tailored for orth-opaedic ***: A Delphi technique was used to dev.lop a course in human factors specific to orthopaedic residents. Twenty-six residents(six per course) participated in total with sev.n course facilitators all trained in Crisis Resource Management prov.ding structured feedback. Six scenarios recreated challenging real-life situations using high-fidelity mannequins and simulated patients. Env.ronments included a simulated operating suite, clinic room and ward setting. All were undertak.n in a purpose built simulation suite utilising actors, mock.operating rooms, mock.clinical rooms and a high fidelity adult patient simulator organised through a simulation control room. Participants completed a 5-point Lik.rt scale questionnaire(strongly disagree to strongly agree) before and after the course. This assessed their understanding of non-technical sk.lls, scenario v.lidity, relev.nce to orthopaedic training and predicted impact of the course on future practice. A course ev.luation questionnaire was also completed to assess participants' feedback.on the v.lue and quality of the course ***: Twenty-six orthopaedic residents participated(24 male, 2 female; post-graduation 5-10 years), mean year of residency program 2.6 out of 6 years required in the United k.ngdom. Pre-course questionnaires showedthat while the majority of candidates recognised the importance of non-technical(NT) sk.lls in orthopaedic training they demonstrated poor understanding of non-technical sk.lls and their role. This improv.d significantly after the course(Lik.rt score 3.0-4.2) and the perceiv.d importance of these sk.lls was reported as good or v.ry good in 100%. The course was reported as enjoyable and prov.ded an unthreatening learning env.ronment with the candidates placing particular v.lue on the learning opportunity prov.ded by reflecting on their performance. The Effect of Lion’s Ear (Leonotis nepetifolia) and African Basil (Ocimum gratissimum) Plant Extracts on Two-Spotted Spider Mites (Tetranychus urticae) for Improv.d Yield and Quality of French Beans

An experiment to ev.luate the bio-control potential of Leonotis nepetifolia and Ocimum gratissimum plant extracts against two-spotted spider mites on French beans was conducted in the field. Fiv. plant extract concentrations (1.5%, 3.0%, 6.0% and 12.0% w/v. were applied with water and Abamectin 0.6 ml/L as controls. Mite counts were done before and after treatment application and expressed as corrected percent efficacy. The impact of the mites on the French beans was ev.luated by recording percent leaf reduction and quality and quantity by number of pods, pod length, diameter and yield. There was a dose dependent response in percent mite and leaf reduction, number of pods and yield. Treatments applied at 12% w/v.indicated higher mite reduction (82.75%) for L. nepetifolia and 69.06% for O. gratissimum compared to abamectin (65.76%). The lowest percent leaf reduction of 1.71% for L. nepetifolia 0.39% for O. gratissimum and abamectin (20.46%) was also at 12% w/v. Similarly, the highest number of pod (61.00) for L. nepetifolia, 48.67 for O. gratissimum compared to 28.33 abamectin and yield (0.88 k.) for L. nepetifolia and 0.90 k. for O. gratissimum was also recorded at 12% w/v.compared to 0.36 k. for abamectin. There were no significant differences in pod diameter and pod length between the extracts concentrations and abamectin. The study demonstrated the efficacy of L. nepetifolia and O. gratissimum in managing two-spotted spider mite and subsequent increase in French bean yield under field conditions.
