Free air or gas in mediastinum is defined as pneumomediastinum. This is a rare condition which orginates from over distention of alveolus and alveolar rupture by barotrauma. A 6-year-old boy was admitted to our department with sudden onset, swelling of neck.and face that developed during drink.ng water from tap by using his mouth. Physical examination revealed the presence of subcutaneous emphysema over the two side of the face that extended toward the neck.bilaterally. The chest X-ray and CT of the thorax, when performed, revealed the diagnosis of pneumomediastinum and extrathoracic subcutaneous emphysema. Interestingly neither trachea-bronchial nor esophageal pathology was found by emergent rigid bronchoscopy and endoscopy for etiology of pneumomediastinum. Antibiotic treatment and oxygen therapy were given to the patient with chest pain and dyspnea. During the following days, the patient’s condition improved notably, with almost total resolution of the cervical emphysema and pneumomediastinum confirmed by daily chest X-ray and control thorax CT. He was discharged home after 6 days. The emphysema gradually resolved. Pneumomediastinum caused by barotrauma is a rare condition and only conservative treatment is required when the other causes are ruled out.

Virological Therapeutic Failure and Associated Factors in People Living with HIV Followed up at the Regional Hospital and University of Borgou (RHU-B) in 2019

Introduction: Antiretroviral treatment has made a considerable reduction in morbidity and mortality in HIV patients, but its effectiveness is compromised by the emergence of cardiovascular and metabolic diseases and also treatment failure. The goal is to study Virological therapeutic failure and the factors associated with HIV patients followed at RUH-B. Methods: This was a cross-sectional, descriptive and analytical study covering the period from 21st July to 21st September 2019. The study enrolled patients 18 years old and above, on antiretroviral treatment for at least 6 months. For the survey, a questionnaire and review of the medical records were used. The variable studied was virological treatment failure. Data analysis was performed using EPI-INFO version 7 and STATA 11 software. The significance level was 5%. Results: A total of 498 patients were included in the study. The mean age was 40 ± 9.90 years. The sex ratio was 0.3. The frequency of treatment failure was 15.3%. The factors associated with the occurrence of treatment failure were: phytotherapy, occurrence of opportunistic infections during follow-up, poor compliance with antiretrovirals, lack.of weight gain and anemia during follow-up. Conclusion: This study had shown that treatment failure was frequent in patients. The systematic investigation for failure should be done in all people living with HIV patients on antiretrovirals.

Frequency and Associated Factors of Erectile Dysfunction among Patients with Liver Cirrhosis in Parak.u, Republic of Benin in 2022

Introduction: Erectile dysfunction is a pathology less expressed by patients, but it affects their quality of life. The objective of this work.is to study erectile dysfunction among patients with cirrhosis in Parak.u in 2022. Patients and Methods: This was a descriptive and analytical cross-sectional study with prospective data collection, conducted from February 1 to June 30, 2022 at the Teaching Hospital of Borgou/Alibori and the Military Teaching Hospital of Parak.u. Men with liver cirrhosis who gave their informed verbal consent were included. Erectile dysfunction was diagnosed using IIEF-5 score. The prognosis of cirrhosis was assessed using Child-Pugh score. The data were analyzed by Epi Data analysis 2.3 software. Results: A total of 64 patients were included. Their mean age was 43.53 ± 13.13 years. Cirrhosis was secondary to chronic hepatitis B virus infection in 55 patients (85.94%). In this study, 42 patients (65.63%) had at least one decompensation of cirrhosis. Among the 64 patients included, 27 (42.18%) had erectile dysfunction. This erectile dysfunction was moderate in 12 patients (44.44%). The other sexual disorders found in these patients were decreased libido and ejaculation disorders. After multivariate analysis, the factors statistically associated with erectile dysfunction were: age (p Conclusion: Erectile dysfunction is common in patients with liver cirrhosis. It is more frequent when the cirrhosis is complicated and the patients are elderly. Nevertheless, it should be systematically sought in any patient with liver cirrhosis.

Introduction: Hearing impairments are common among nightclub work.rs. The present work.aims to study the hearing clinical profile of these employees in the city of Parak.u in 2021. Methods: This was a cross-sectional, descriptive analytical study from March 14 to June 15, 2021. It focused on employees of 13 nightclubs in the city of Parak.u. Were included in the study, employees with at least 03 months of practice and aged at least 18 to 50 years at most on the date of the survey and who had given their informed consent to participate in the study. Each employee received a questionnaire, an otoscopy and an acoumetry in tune. Noise pollution was measured at each work.tation using a sound level meter. Results: A total of 114 employees were collected. The mean age was 26.38 years ± 4.81 and the sex ratio 0.87. The server position was occupied by 65 employees (57%) followed by managers, 19 or 16.70%- and 17-disc jock.ys, 14.90%. Among the employees 31.60% exercised between 03 and 09 months. 76.32% were exposed to noise for more than 8 hours per day and 81.58% were exposed to noise for more than 40 hours per week. The sound level at the work.tation was for 59 employees (51.75%) between 90 and 100 dB. Tinnitus was the most expressed complaint, respectively 21 (18.40%) on the right and 22 (19.30%) on the left. Rinne’s test was positive in 85.10% of employees. Weber’s test was indifferent to all frequencies in the majority of respondents. Conclusion: Hearing damage among nightclub employees in the city of Parak.u is a reality. It is important to put preventive measures in place.

目的探讨ICU护士工作心理负荷与心理资本之间的关系。方法采用便利抽样法,于2016年12月—2018年1月选取上海市7所综合医院的339名ICU护士为研究对象,采用NASA任务负荷指数量表、心理资本量表(PCQ-24)对护士进行问卷调查,采用单因素分析和多元分层回归分析调查ICU护士工作心理负荷与心理资本的关系。结果ICU护士工作心理负荷的总均分为(68.14±12.01)分,心理资本总分为(98.49±17.47)分。Pearson相关分析显示,ICU护士工作心理负荷与心理资本呈负相关(r=-0.462,P<0.01)。多元分层回归分析表明,心理资本能独立预测护士工作心理负荷,可独立解释20.2%的变异性。结论ICU护士工作心理负荷与心理资本呈负相关。护理管理者可通过心理资本培训等形式,提高ICU护士的心理资本水平,从而减轻其工作心理负荷。

Introduction: Sick.e cell disease (SCD) causes chronic hemolysis which is a risk.factor for cholelithiasis. Its development may lead to severe and life-threatening complications. Objective: Determine the prevalence of cholelithiasis, the conditions of diagnosis and related factors. Materials and Method: We retrospectively reviewed records of 110 patients with sick.e cell disease followed up in Charles de Gaulle University Pediatric Hospital from January 2003 to December 2013, including 103 patients who had abdominal ultrasonography. Results: Cholelithiasis prevalence was 24.3%. The mean age of patients was 10.8 years, (range 3 to 15 years). Sex ratio was 2.1. In 88% cases, cholelithiasis was diagnosed based on the characteristic symptoms of right hypocondrial pain, fever and icterus. Most factors associated with cholelithiasis were as follows: age above 10 years (OR = 4), occurrence of at least three (03) vaso-occlusive crises per year (OR = 7.6), history of blood transfusion (OR = 8), right hypochondrial pain (OR = 4.5) and icterus (OR = 15). Only 20% of patients suffering from a symptomatic cholelithiasis underwent laparoscopic cholecystectomy and results were conclusive. Conclusion: Patients with sick.e cell disease, especially those aged above 10, should be routinely tested for cholelithiasis using abdominal ultrasonography at least once a year. Because of the difficulties in managing evolutive complications in case of an emergency in our context, we advocate laparoscopic cholecystectomy of any cholelithiasisas soon as it is diagnosed in children with sick.e cell disease.
