Autologous bone marrow derived stem cell therapy in patients with type 2 diabetes mellitus-defining adequate administration methods

AIM To carry out randomized trial for evaluating effects of autologous bone marrow derived stem cell therapy(ABMSCT) through different *** Bone marrow aspirate was taken from the iliac crest of patients. Bone marrow mononuclear cells were separatedand purified using centrifugation. These cells were then infused in a total of 21 patients comprising three groups of 7 patients each. Cells were infused into the superior pancreaticoduodenal artery(Group Ⅰ), splenic artery(Group Ⅱ) and through the peripheral intravenous route(Group Ⅲ). Another group of 7 patients acted as controls and a sham procedure was carried out on them(Group Ⅳ). The cells were labelled with the PET tracer F18-FDG to see their homing and in vivo distribution. Data for clinical outcome was expressed as mean ± SE. All other data was expressed as mean ± SD. Baseline and post treatment data was compared at the end of six months, using paired t-test. Cases and controls data were analyzed using independent t-test. A probability(P) value of < 0.05 was regarded as statistically significant. Measures of clinical outcome were taken as the change or improvement in the following parameters:(1) C-peptide assay;(2) HOMA-IR and HOMA-B;(3) reduction in Insulin dose; subjects who showed reduction of insulin requirement of more than 50% from baseline requirement were regarded as responders; and(4) reduction in HbA 1c. RESULTS All the patients, after being advised for healthy lifestyle changes, were evaluated at periodical intervals and at the end of 6 mo. The changes in body weight, body mass index, waist circumference and percentage of body fat in all groups were not significantly different at the end of this period. The results of intra-group comparison before and after ABMSCT at the end of six months duration was as follows:(1) the area under C-peptide response curve was increased at the end of 6 mo however the difference remained statistically non-significant(P values for fasting C-peptide were 0.973, 0

Nonalcoholic fatty liver disease(NAFLD)affects about a quarter of the world’s population and poses a major health and economic burden ***,there have been hasty attempts to rename NAFLD to metabolic-associated fatty liver disease(MAFLD)despite the fact that there is no scientific rationale for *** for a“positive criterion”to diagnose the disease and destigmatizing the disease have been the main reasons put forth for the name change.A close scrutiny of the pathogenesis of NAFLD would make it clear that NAFLD is a heterogeneous disorder,involving different pathogenic mechanisms of which metabolic dysfunction-driven hepatic steatosis is only *** NAFLD with MAFLD would neither enhance the legitimacy of clinical practice and clinical trials,nor improve clinical care or move NAFLD research *** than changing the nomenclature without a strong scientific backing to support such a change,efforts should be directed at understanding NAFLD pathogenesis across diverse populations and ethnicities which could potentially help develop newer therapeutic options.

The Impact of Delays during the Pandemic Months on Survival of Lung Cancer Patients in Canada in 2020

Background and Purpose: Most cancer deaths in the world are due to lung cancer and diagnosis and treatment delays sharply reduce survival in lung cancer patients. This study examined the impact of delays during the early months of the pandemic on the survival of newly identified lung cancer patients in Canada in 2020. Method: This was a secondary data analysis from published literature and openly available data sources. Cancer Statistics from existing literature were used as a proxy for the month-wise distribution of lung cancer cases in Canada. The incidence of lung cancer, using population statistics from Statistics Canada and incidence rates from the Canadian Cancer Statistics in 2020, was estimated. The population-based Excel model employed compounded cuts on the incidence to arrive at the outcomes. *** tool was used to digitize the survival versus time curves for each stage from secondary sources. Stage-wise incidences for each sex were calculated for each age group for each month of 2020. Using delay impact on each stage the final results were calculated. Results: A total of 5004 life years would have been lost due to 448 deaths in the long term (40 months) attributed to the delays caused during March, April, May and June in Canada. The estimated incidence for all stages of lung cancer for these months was 9801 although the observed incidence was expected to be 6571 due to reduced screenings. Hence, it was within the missing 3231 cases that delays would occur. Over the short term (10 months) there are expected to be 151 early deaths and 273 deaths in the intermediate-term (20 months). Conclusions: This study using a mathematical model showed that in 2020, the COVID epidemic resulted in higher mortality and fewer lung cancer diagnoses in Canada. Experimental investigations on mechanical performance of rocks under fatigue loads and biaxial confinements

In this research,a series of biaxial compression and biaxial fatigue tests were conducted to investigate the mechanical behaviors of marble and sandstone under biaxial *** results demonstrate that the biaxial compressive strength of rocks under biaxial compression increases firstly,and subsequently decreases with increase of the intermediate principal *** fatigue failure characteristics of the rocks in biaxial fatigue tests are functions of the peak value of fatigue loads,the intermediate principal stress and the rock *** the increase of the peak values of fatigue loads,the fatigue lives of rocks *** intermediate principal stress strengthens the resistance ability of rocks to fatigue loads except considering the strength increasing under biaxial *** fatigue lives of rocks increase with the increase of the intermediate principal stress under the same ratio of the fatigue load and their biaxial compressive *** acoustic emission(AE)and fragments studies showed that the sandstone has higher ability to resist the fatigue loads compared to the marble,and the marble generated a greater number of smaller fragments after fatigue failure compared to the ***,it can be inferred that the rock breaking efficiency and rock burst is higher or severer induced by fatigue loading than that induced by monotonous quasi-static loading,especially for hard rocks.

Mechanical Property Estimation of Similar Weld using Ball Indentation Technique

A weld joint is composed of three principal zones viz., base metal, Heat Affected Zone (HAZ), and weld zone. Thus, the variation in mechanical behavior exists not only among these zones, but also from point to point in each individual zone. Being destructive in nature, the conventional method of mechanical testing cannot successfully used to estimate the variation in the mechanical behavior at different zones of the weld joint. Moreover, the conventional method of mechanical testing cannot characterize the material using small amount of material. In this respect, Ball Indentation (BI) methodology was considered to be useful approach, since it can characterize the mechanical properties of a material using very small amount of material in non destructive manner. The present work is an attempt to characterize the variation in the mechanical properties among each zone (global variation), and from point to point in each zone (local variation) of the similar weld joint used in nuclear application using BI approach. For this purpose, the similar weld joint of two SS-304 LN pipe lines was investigated using BI approach.

Radiation dose analysis of computed tomography coronary angiography in Children with Kawasaki disease

Assay method for quality control and stability studies of a new antimalarial agent (CDRI 99/411)

CDRI compound no. 99/411 is a potent 1,2,4-trioxane antimalarial candidate drug under development at our Institute. An HPLC method for determination of CDRI 99/411 with its starting material and intermediates has been developed and validated for in process quality control and stability studies. The analytical performance parameters such as linearity, precision, accuracy, specificity, limit of detection （LOD） and lower limit of quantification （LLOQ） were determined according to International Conference on Harmonization ICH Q2（R1） guidelines. HPLC separation was achieved on a RP-select B Lichrosphere~ column （250 mm x 4 ram, 5 lam, Merck） using water containing 0.1% glacial acetic acid and acetonitrile as the mobile phase in a gradient elution. The eluents were monitored by a photo diode array detector at 245 and 275 nm. Based on signal to noise ratio of 3 and 10 the LOD of CDRI 99/411 was 0.55 μg/mL, while the LLOQ was 1.05 μg/mL. The calibration curves were linear in the range of 1.05- 68 μg/mL. Precision of the method was determined by inter- and intra-assay variations within the acceptable range.

Stroke in COVID-19: a single-centre initial experience in a hotspot of the pandemic

Background COVID-19,caused by SARS-CoV-2,is a global pandemic that has been an immense burden on healthcare systems all over the *** patients may be at higher risk for acute ischaemic stroke(AIS).We present our experience with AIS in patients with *** We reviewed all patients admitted to our hospital during a 6-week period with a positive nasopharyngeal swab test for *** these patients,we identified *** reviewed the demographics,clinical,laboratory,imaging characteristics,treatments received and outcomes of AIS in patients with *** We identified 683 patients admitted with COVID-19 during the study period,of which 20 patients had ***-vessel occlusion(LVO)was noted in 11 patients(55%).Intravenous alteplase was administered in four patients(20%)and mechanical thrombectomy was performed in five patients(25%).Respiratory symptoms preceded the onset of AIS in most of the patients(70%)by 1 to 21 *** in patients with AIS was 50%compared with 26%of all COVID-19 *** of these patients died due to non-neurological causes(70%).Three patients with AIS had clinical and imaging findings consistent with COVID-19,but were negative for multiple nasopharyngeal swab *** LVO was more common in patients with AIS and *** had more severe disease and higher mortality *** of the patients had respiratory symptoms preceding AIS by days to *** could explain certain patients with clinical picture of COVID-19 but negative nasopharyngeal swab tests.
