Magnetically controlled growing instrumentation for early onset scoliosis: Caution needed when interpreting the literature

BACKGROUND Magnetically controlled growing rods(MCGR) are a novel treatment option for early onset scoliosis(EOs). Although the complication profile with MCGR use has been reviewed, these reviews do not take into account important implants modifications, termed iterations, that were made due to early on postoperative complications is not well reported or *** To assess the effect of MCGR implant iterations on post-operative complications in *** A systematic review was performed to identify studies investigating MCGR specifically for the treatment of EOs, refined to those reporting the implant iteration, specifically the incorporation of the keeper plate to the implant *** with mixed implant iteration usage were excluded. Complications following surgery were recorded as well as potential risk factors and compared between implant *** Although 20 articles were identified for inclusion, 5 included mixed implant iteration leaving a total of 271 patients identified through 15 clinical studies thatmet inclusion criteria. The average follow-up was 25.4-mo. Pre-keeper plate implants were utilized in 3 studies with a total of 49 patients. Overall, 115(42.4%)post-operative complications were identified, with 87% defined as major. The addition of the keeper plate significantly decreased the rate of post-operative complications per study(35.7% vs 80.6%, P = 0.036), and the rate of distraction failure(8.1% vs 40.8%, P = 0.02). Unplanned reoperation occurred in 69(26.7%)patients but was not different between implant iteration cohorts(25.5% without keeper plate vs 27.1% with keeper plate, P = 0.92).CONCLUsION MCGR for EOs has a cumulative complication rate of 42.4% but this is significantly reduced to 35.7% when reviewing only keeper-plate enabled implants. However, 25% of published articles included mixed implant *** studies should discern between implants iterations when reporting on the usage of MCGR for EOs.

Complementary and Alternative Medicine in the Treatment of Menopausal symptoms

A large number of women will pass through menopause each year. Women in menopausal transition experience a variety of menopausal symptoms. Although hormonal therapy remains the most effective treatment, side effects have been reported by several large studies. An increased number of women seek the use of complementary and alternative medicine （CAM） for treating menopausal symptoms. This review analyzes the evidence from systematic reviews, randomized controlled trials and epidemiological studies of using herbal medicine （Black cohosh, Dong quai, st John＇s wart, Hops, Wild yam, Ginseng, and evening primrose oil） and acupuncture for the treatment of menopausal symptoms. Evidence supporting the efficacy and safety of most CAM for relief of menopausal symptoms are limited. Future larger and better controlled studies testing the effectiveness of these treatments are needed.

The ghosts of parasitism past:lingering frontline anti-brood parasite defenses in a former host

Coevolutionary arms races between brood parasites and hosts provide tractable systems for understanding antagonistic coevolution in nature;however,little is known about the fate of frontline antiparasite defenses when the host“wins”the coevolutionary arms *** recreating bygone species interactions,using artificial parasitism experiments,lingering defensive behaviors that evolved in the context of parasitism can be understood and may even be used to identify the unknown agent of parasitism *** we present the first study of this type by evaluating lingering“frontline”nest defenses that have evolved to prevent egg laying in a former brood parasite *** Australian reed warbler Acrocephalus australis is currently not parasitized but is known to exhibit fine-tuned egg discrimination—a defensive behavior indicative of a past brood parasite–host arms race and common in closely related parasitized ***,using 3D-printed models of adult brood parasites,we examined whether the Australian reed warbler also exhibits frontline defenses to adult brood parasites,and whether we could use these defenses to identify the warbler’s“ghost of parasitism past.”Our findings provide evidence that the Australian reed warbler readily engages in frontline defenses that are considered adaptive specifically in the context of brood ***,individuals were unable to discriminate between adults of different brood parasite species at their ***,our results demonstrate that despite a relaxation in selection,defenses against brood parasitism can be maintained across multiple stages of the host’s nesting cycle,and further suggest that,in accordance with previous findings,that learning may be important for fine-tuning frontline defense.

This review presents a conceptual framework and supporting evidence that links impaired motor control after sport-related concussion(sRC)to increased risk for musculoskeletal *** studies have found that athletes who are post-sRC have higher risk for musculoskeletal injury compared to their counterparts.A small body of research suggests that impairments in motor control are associated with musculoskeletal injury *** control involves the perception and processing of sensory information and subsequent coordination of motor output within the central nervous system to perform a motor *** control is inclusive of motor planning and motor *** sensory information is not accurately perceived or there is interference with sensory information processing and cognition,motor function will be altered,and an athlete may become vulnerable to injury during sport *** with sRC show neuroanatomic and neurophysiological changes relevant to motor control even after meeting return to sport criteria,including a normal neurological examination,resolution of symptoms,and return to baseline function on traditional concussion *** conjunction,altered motor function is demonstrated after sRC in muscle activation and force production,movement patterns,balance/postural stability,and motor task performance,especially performance of a motor task paired with a cognitive task(i.e.,dual-task condition).The clinical implications of this conceptual framework include a need to intentionally address motor control impairments after sRC to mitigate musculoskeletal injury risk and to monitor motor control as the athlete progresses through the return to sport continuum.

A novel tri-layer approach for immobilizing metal nanoparticles in siO2 supports is *** this work,we show that under rapid heating to temperatures of approximately 1,000 ℃,metal nanoparticles less than 15 nm in size will entrench in the siO2 layer on a silicon wafer to create pores as deep as 250 *** studied and characterized this entrenching behavior and subsequent nanopore formation for a wide variety of metal nanoparticles,including Au,Ag,Pt,Pd,and *** also demonstrate that an Al2O3 layer acts as a barrier to such pore ***,by creating a tri-layer architecture consisting of siO2 on Al2O3 on silicon wafers,we can control the depth to which nanoparticles entrench between 3-5 *** small range allows one to entrench particles for the purpose of immobilization but still present them above the *** two advances of moving into the sub-15 nm size regime and of controlled particle immobilization through entrenchment have important implications in studying site-isolated and stabilized metal nanoparticles for applications in sensing,separations,and catalysis.

Contaminated open fracture and crush injury: a murine model

Modern warfare has caused a large number of severe extremity injuries, many of which become infected. In more recent conflicts, a pattern of co-infection with Acinetobacter baumannii and methicillin-resistant staphylococcus aureus has emerged. We attempted to recreate this pattern in an animal model to evaluate the role of vascularity in contaminated open fractures. Historically, it has been observed that infected bones frequently appear hypovascular, but vascularity in association with bone infection has not been examined in animal models. Adult rats underwent femur fracture and muscle crush injury followed by stabilization and bacterial contamination with A. baumannii complex and methicillin-resistant staphylococcus *** and perfusion were assessed by micro CT angiography and sPECT scanning, respectively, at 1, 2 and 4 weeks after injury. Quantitative bacterial cultures were also obtained. Multi-bacterial infections were successfully created, with methicillin-resistant s. aureus predominating. There was overall increase in blood flow to injured limbs that was markedly greater in bacteria-inoculated limbs. Vessel volume was greater in the infected group. Quadriceps atrophy was seen in both groups, but was greater in the infected group. In this animal model, infected open fractures had greater perfusion and vascularity than non-infected limbs.

With the goal of accelerating the design and discovery of metal–organic frameworks(MOFs)for electronic,optoelectronic,and energy storage applications,we present a dataset of predicted electronic structure properties for thousands of MOFs carried out using multiple density functional *** to more accurate hybrid functionals,we find that the widely used PBE generalized gradient approximation(GGA)functional severely underpredicts MOF band gaps in a largely systematic manner for semi-conductors and insulators without magnetic ***,an even larger and less predictable disparity in the band gap prediction is present for MOFs with open-shell 3d transition metal *** regards to partial atomic charges,we find that different density functional approximations predict similar charges overall,although hybrid functionals tend to shift electron density away from the metal centers and onto the ligand environments compared to the GGA point of *** more significant differences in partial atomic charges are observed when comparing different charge partitioning *** conclude by using the dataset of computed MOF properties to train machine-learning models that can rapidly predict MOF band gaps for all four density functional approximations considered in this work,paving the way for future high-throughput screening *** encourage exploration and reuse of the theoretical calculations presented in this work,the curated data is made publicly available via an interactive and user-friendly web application on the Materials Project.
