BACKGROUND magnetically controlled growing rods(mCGR) are a novel treatment option for early onset scoliosis(EOS). Although the complication profile with mCGR use has been reviewed, these reviews do not take into account important implants modifications, termed iterations, that were made due to early on postoperative complications is not well reported or *** To assess the effect of mCGR implant iterations on post-operative complications in *** A systematic review was performed to identify studies investigating mCGR specifically for the treatment of EOS, refined to those reporting the implant iteration, specifically the incorporation of the keeper plate to the implant *** with mixed implant iteration usage were excluded. Complications following surgery were recorded as well as potential risk factors and compared between implant *** Although 20 articles were identified for inclusion, 5 included mixed implant iteration leaving a total of 271 patients identified through 15 clinical studies thatmet inclusion criteria. The average follow-up was 25.4-mo. Pre-keeper plate implants were utilized in 3 studies with a total of 49 patients. Overall, 115(42.4%)post-operative complications were identified, with 87% defined as major. The addition of the keeper plate significantly decreased the rate of post-operative complications per study(35.7% vs 80.6%, P = 0.036), and the rate of distraction failure(8.1% vs 40.8%, P = 0.02). Unplanned reoperation occurred in 69(26.7%)patients but was not different between implant iteration cohorts(25.5% without keeper plate vs 27.1% with keeper plate, P = 0.92).CONCLUSION mCGR for EOS has a cumulative complication rate of 42.4% but this is significantly reduced to 35.7% when reviewing only keeper-plate enabled implants. However, 25% of published articles included mixed implant *** studies should discern between implants iterations when reporting on the usage of mCGR for EOS.

Perioperative Outcomes Following Surgical Treatment of the Neuromuscular Hip: An Analysis of the National Surgical Quality Improvement Program—Pediatrics

Background: Surgical treatment for neuromuscular (Nm) hip subluxation or dislocation is undertaken to maintain functionality and decrease pain. Longterm complications are well described;however, the acute complication profile is poorly understood. Questions/Purpose: The aim of this study was to identify the rate of and risk factors for complications following surgical treatment of hip subluxation/dislocation, especially as it relate to Nm children. methods: Hip reconstruction cases in patients with a Nm diagnosis and non-Nm patients were obtained from the 2015 American College of Surgeons NSQIP-Pediatric database by CPT code. 30-day postoperative complications were classified according to the Clavien-Dindo system as minor (grade 1 or 2) or major (grade 3+). Patient and surgical factors were assessed in univariate and multivariate logistic regression analyses for association with post-operative complications. Results: 1081 cases were identified (median age 7.7, 55% female), of whom 420 (39%) had a Nm diagnosis. Overall complication rate was significantly higher in Nm patients (33% vs. 19%, p merous factors were associated with postoperative complication on univariate analysis. multivariate analysis identified Nm diagnosis (OR 1.5), age > 6 years (OR 1.5), or pelvic osteotomy (OR 1.9) as independent risk factors for complication. Conclusion: In pediatric reconstructive hip surgery requiring pelvic osteotomy, a Nm diagnosis is associated with an increased risk of 30-day postoperative complications. Older age and increasing surgical complexity were also independently associated with complications. These findings support special attention for the older patients undergoing concomitant pelvic osteotomies to minimize complication rate.

Evaluation of a hepatic biomarker of nutritional imbalance in juvenile red drum(Sciaenops ocellatus) fed 60% soybean meal-based diets using NmR-based metabolomics

A 12-week feeding trial with juvenile red drum(Sciaenops ocellatus) fed high-soybean meal(SBm) diets was conducted to investigate a putative biomarker of nutritional imbalance, N-formimino-L-glutamate(FIGLU). Three fishmeal-free, 60% SBm pelleted diets(named B12, Fol, and met, respectively) were tested to evaluate the effects on growth performance and tissue metabolite profiles of supplementation of vitamin B12(0.012 mg/kg), folate(10 mg/kg), methionine(1 g/kg) respectively, above basal supplementation levels. A fourth SBm-based diet(named B12/Fol/met) was formulated with a combination of B12, folate, and methionine to attain the above-mentioned target concentrations. A fifth 60% SBm diet(named FWS) with methionine supplementation(1 g/kg above basal supplementation levels), enriched with taurine, lysine and threonine as well as minerals, was also tested. This diet contained formulation targets and additives which have allowed for replacing fishmeal with plant proteins in rainbow trout feeds. Control diets included a fishmeal-based diet(named Fm), an unsupplemented basal 60% SBm diet(named SBm60), and a “natural” diet(named N) made up of equal parts of fish(cigar minnows), squid and shrimp as a positive reference for growth performance. Formulated feeds contained approximately 37% total crude protein, approximately 14% total crude lipid and were energetically balanced. Standard growth performance metrics were measured, and tissues(liver, muscle) were collected at week 12 to evaluate diet-induced metabolic changes using nuclear magnetic resonance(NmR)-based *** results show that the FWS diet outperformed all other SBm diets and the Fm diet under all performance metrics(Pm diets and the Fm diet. Building-Integrated Photovoltaic(BIPV)on vertical façades is a potential PV application in today’s *** performance of BIPV on façades is significantly influenced by the façade *** tropical cities,the optimum façade orientation,in terms of maximum energy yield and daylight performance,cannot be simply determined,due to relatively symmetrical sun path throughout the *** study therefore aims to determine the optimum orientation for BIPV on tropical building faç*** achieve the objective,experiment,modelling,and computational simulation are conducted to evaluate the BIPV energy yield and to predict the indoor daylight performance in a scale-model building with a 105Wp monocrystalline silicon PV,facing each cardinal orienta-tion in Bandung,*** South orientation yields practically zero ASE_(1000,250),providing the best annual daylight performance,and yielding the most desirable value in four out of five daylight *** greatest annual energy yield is at the North orientation,providing 179-186 kWh(95%prediction interval)per year,but with larger uncertainty compared to that at the South,due to direct sunlight *** on three different objective functions,the South orientation is considered optimum for placing the BIPV panel on the prototype façade in the location.

The presence of organic matter in lakes profoundly impacts drinking water supplies,yet treatment processes involving coagulants and disinfectants can yield carcinogenic disinfection *** assessments of organic matter,such as chemical oxygen demand(COD_(mn))and biochemical oxygen demand(BOD_(5)),are often ***,optical measurements of dissolved organic matter(DOm)offer a rapid and reliable means of obtaining organic matter composition *** we employed DOm optical measurements in conjunction with parallel factor analysis to scrutinize COD_(mn)and BOD_(5)*** was performed using an independent dataset encompassing six lakes on the Yungui Plateau from 2014 to 2016(n=256).Leveraging multiple linear regressions(mLRs)applied to DOm absorbance at 254 nm(a254)and fluorescence components C1-C5,we successfully traced COD_(mn)and BOD_(5)variations across the entire plateau(68 lakes,n=271,R^(2)>0.8,Pm optical indices yielded superior estimates(higher R^(2))of COD_(mn)and BOD_(5)during the rainy season compared to the dry season and demonstrated increased accuracy(R^(2)>0.9)in mesotrophic lakes compared to oligotrophic and eutrophic *** study underscores the utility of mLR-based DOm indices for inferring COD_(mn)and BOD_(5)variability in plateau lakes and highlights the potential of integrating in situ and remote sensing platforms for water pollution early warning.

A Rare Case of Aortic Valve Endocarditis and Acute meningitis Due to Haemophilus influenzae

HACEK organisms represent a rare but important group of causative pathogens in endocarditis. These bacteria have historically been associated with culture-negative endocarditis;however, modern laboratory techniques have made this less common. In this case, we present a 74-year-old man who presented with acute onset altered mentation, fever, and sepsis. He was ultimately found to have Haemophilus influenzae meningitis, cerebral empyema, aortic valve endocarditis, psoas myositis, and L2 - L3 diskitis with osteomyelitis. Although HACEK organisms are commonly found in the oropharynx and upper respiratory tract in humans, our patient did not report recent preceding dental or ENT procedures. H. influenzae is responsible for approximately 0.16% of all cases of bacterial endocarditis, representing a very limited subset. Although generally considered low virulent pathogens, this case demonstrates the unusual extent of infection from a HACEK organism, H. influenzae, causing aortic valve endocarditis as well as atypical non-cardiac sequelae, including acute meningitis.
