Background:Family history is a risk factor for the development of hepatocellular carcinoma(HCC).The aim of the current study was to investigate the association between family history of HCC and long-term oncologic prognosis among patients undergoing curative liver resection for hepatitis B virus(HBV)-related ***:Patients who underwent curative liver resection of HBV-related HCC between 2003 and 2013 were consecutively *** history was defined as a self-reported history of HCC in a first-degree *** score matching(PSM)and multivariable Cox-regression analyses were performed to compare overall survival(OS)and recurrence-free survival(RFS)among patients with and without a family ***:Among 1,112 patients,183(16.5%)patients had a family history of *** PSM,179 pairs of patients with and without a family history were created that had no differences in the baseline characteristics and operative *** matched analysis,family history was associated with decreased OS and RFS after curative-intent resection of HBV-related HCC in the propensity matching cohort(P=0.042 and 0.006,respectively).On multivariable Cox-regression analyses,a family history of HCC was associated with decreased OS(HR:1.574;95%CI:1.171–2.116;P=0.003)and RFS(HR:1.534;95%CI:1.176–2.002;P=0.002)after adjusting for other prognostic risk ***:Family history was associated with decreased OS and RFS rates among patients undergoing curative liver resection of HBV-related HCC.

Background:Preoperative prediction of microvascular invasion(MVI)in hepatocellular carcinoma(HCC)may optimize individualized treatment *** study aimed to investigate the prognostic differences between HCC patients undergoing liver resection(LR)and liver transplantation(LT)based on predicted MVI ***:We analysed 905 patients who underwent LR,including 524 who underwent anatomical resection(AR)and 117 who underwent LT for HCC within the Milan criteria using propensity score matching.A nomogram model was used to predict preoperative MVI ***:The concordance indices of the nomogram for predicting MVI were 0.809 and 0.838 in patients undergoing LR and LT,*** on an optimal cut-off value of 200 points,the nomogram defined patients as high-or low-risk MVI *** resulted in a lower 5-year recurrence rate and higher 5-year overall survival(OS)rate than LR among the high-risk patients(23.6%vs 73.2%,P<0.001;87.8%vs 48.1%,P<0.001)and low-risk patients(19.0%vs 45.7%,P<0.001;86.5%vs 70.0%,P=0.002).The hazard ratios(HRs)of LT vs LR for recurrence and OS were 0.18(95%confidence interval[CI],0.09–0.37)and 0.12(95%CI,0.04–0.37)among the high-risk patients and 0.37(95%CI,0.21–0.66)and 0.36(95%CI,0.17–0.78)among the low-risk *** also provided a lower 5-year recurrence rate and higher 5-year OS rate than AR among the high-risk patients(24.8%vs 63.5%,P=0.001;86.7%vs 65.7%,P=0.004),with HRs of LT vs AR for recurrence and OS being 0.24(95%CI,0.11–0.53)and 0.17(95%CI,0.06–0.52),*** 5-year recurrence and OS rates between patients undergoing LT and AR were not significantly different in the low-risk patients(19.4%vs 28.3%,P=0.129;85.7%vs 77.8%,P=0.161).Conclusions:LT was superior to LR for patients with HCC within the Milan criteria with a predicted high or low risk of *** significant differences in prognosis were found between LT and AR in patients with a low risk of MVI.

Background Mangrove tannins can participate in wetland biogeochemical ***,their fate and dynamics during leaf litter leaching have yet to be elucidated in coastal aquatic *** By using a simulated microcosm experiment,changes in leaf litter mass and tannin compounds,including total phenolics(TP),extractable condensed tannins(ECT),and bound condensed tannins(BCT),were examined in the litters and leachates of four common mangrove species:Kandelia obovata(Ko),Aegiceras corniculatum(Ac),Sonneratia apetala(Sa),and Avicennia marina(Am).Results Leaching caused a notable decline in litter mass,TP,and ECT in the leaf litter of Ko,Ac,Sa,and Am,while BCT increased significantly in Ko and *** in foliar TP was higher than in leaf litter mass,and loss in foliar ECT was higher than in TP of Ko and Ac,but Sa showed the opposite *** temporal changes of TP and ECT concentrations in leachates followed a similar trend,with an initial increase followed by a *** dynamics in Ko and Ac leachates correlated with their TP concentrations,while in Sa leachate,the peak ECT occurred 72 h later than its TP *** leachate ECT concentrations were highest in Ac,followed by Ko,and significantly lower in *** peak TP and ECT proportions in leachates accounted for 9.2-23.9%and 7.7-9.4%of the total decreases in foliar TP and ECT,*** During the leaching process,tannins’fate was species-specific,while the dynamics were almost similar.

分别以1,3,5-三苯甲酰基-α-D-核糖、3,5-二苯甲酰基-2-脱氧-2,2-二氟戊呋喃糖-1-酮和D-木糖为原料,经由烟酰胺核苷及烟酰胺核苷酸中间体,合成了系列糖环经氟原子取代的烟酰胺腺嘌呤二核苷酸(NAD)类CD38抑制剂.基于对CD38的水解抑制能力的考察,评价了所合成氟代NAD类似物的活性.结果表明,糖环上氟原子取代的数目和位置对抑制剂活性的影响十分明显,烟酰胺核苷的端基构型对活性的影响较大.2个化合物均显示出非常好的CD38抑制活性,其中化合物2a的抑制活性高出阳性对照物阿糖型氟代烟酰胺腺嘌呤二核苷酸2个数量级.

Cap polyposis: A rare cause of rectal bleeding in children

AIM: To evaluate the clinicopathological features and treatment outcomes of cap polyposis in the pediatric population. METHODS: All pediatric patients with histologically proven diagnosis of cap polyposis were identified from our endoscopy and histology database over a 12 year period from 2000-2012 at our tertiary pediatric center, KK Women's and Children's Hospital in Singapore. The case records of these patients were retrospectively reviewed. The demographics, clinical course, laboratory results, endoscopic and histopathological features, treatments, and outcomes were analyzed. The study protocol was approved by the hospital institutional review board. The histological slides were reviewed by a pediatric histopathologist to confirm the diagnosis of cap polyposis. RESULTS: Eleven patients were diagnosed with cap polyposis. The median patient age was 13 years (range 5-17 years); the sample included 7 males and 4 females. All of the patients presented with bloody stools. Seven patients (63%) had constipation, while 4 patients (36%) had diarrhea. All of the patients underwent colonoscopy and polypectomies (excluding 1 patient who refused polypectomy). The macroscopic findings were of polypoid lesions covered by fibrinopurulent exudates with normal intervening mucosa. The rectum was the most common involvement site (n = 9, 82%), followed by the rectosigmoid colon (n = 3, 18%). Five (45%) patients had fewer than 5 polyps, and 6 patients (65%) had multiple polyps. Histological examination of these polyps showed surface ulcerations with a cap of fibrin inflammatory exudate. Four (80%) patients with fewer than 5 polyps had complete resolution of symptoms following the polypectomy. One patient who did not consent to the polypectomy had resolution of symptoms after being treated with sulphasalazine. Hepatocellular carcinoma(HCC) is the sixth most common cancer and the third most common cause of cancer-related deaths worldwide. The Barcelona Clinic Liver Cancer(BCLC) classification has been endorsed as the optimal staging system and treatment algorithm for HCC by the European Association for the Study of Liver Disease and the American Association for the Study of Liver Disease. However, in real life, the majority of patients who are not considered ideal candidates based on the BCLC guideline still were performed hepatic resection nowadays, which means many hepatic surgeons all around the world do not follow the BCLC guidelines. The accuracy and application of the BCLC classification has constantly been challenged by many clinicians. From the surgeons' perspectives, we herein put forward some comments on the BCLC classification concerning subjectivity of the assessment criteria, comprehensiveness of the staging definition and accuracy of the therapeutic recommendations. We hope to further discuss with peers and colleagues with the aim to make the BCLC classification more applicable to clinical practice in the future.

Laser powder bed fusion(LPBF),like many other additive manufacturing techniques,offers flexibility in design expected to become a disruption to the manufacturing *** current cost of LPBF process does not favor a try-anderror way of research,which makes modelling and simulation a field of superior importance in that area of *** this work,various methods used to overcome challenges in modeling at different levels of approximation of LPBF process are *** efforts made towards a reliable and computationally effective model to simulate LPBF process using finite element(FE)codes are presented.A combination of ray-tracing technique,the solution of the radiation transfer equation and absorption measurements has been used to establish an analytical equation,which gives a more accurate approximation of laser energy deposition in powder-substrate *** this new analytical energy deposition model is used in in FE simulation,with other physics carefully set,it enables us to get reliable cooling curves and melt track morphology that agree well with experimental *** use of more computationally effective approximation,without explicit topological changes,allows to simulate wider geometries and longer scanning time leading to many applications in real engineering *** applications are herein presented including:prediction of printing quality through the simulated overlapping of consecutive melt tracks,simulation of LPBF of a mixture of materials and estimation of martensite inclusion in printed steel.
