In this editorial,we comment on the article by Mu et al,published in the recent issue of the World Journal of Gastrointestinal *** pay special attention to the immune tolerance mechanism caused by hepatitis B virus(HBV)infection,the pathogenesis of hepatocellular carcinoma(HCC),and the role of antiviral therapy in treating HCC related to HBV *** infection leads to systemic innate immune tolerance by directly inhibiting pattern recognition receptor recognition and antiviral signaling pathways,as well as by inhibiting the immune functions of macrophages,natural killer cells and dendritic *** addition,HBV leads to an immunosuppressive cascade by expressing inhibitory molecules to induce exhaustion of HBV-specific cluster of differentiation 8+T cells,ultimately leading to long-term viral *** loss of immune cell function caused by HBV infection ultimately leads to ***-term antiviral therapy can improve the prognosis of patients with HCC and prevent tumor recurrence and metastasis.

Infiltration by monocytes of the central nervous system and its role in multiple sclerosis: reflections on therapeutic strategies

Mononuclear macrophage infiltration in the central nervous system is a prominent feature of neuroinflammation. Recent studies on the pathogenesis and progression of multiple sclerosis have highlighted the multiple roles of mononuclear macrophages in the neuroinflammatory process. Monocytes play a significant role in neuroinflammation, and managing neuroinflammation by manipulating peripheral monocytes stands out as an effective strategy for the treatment of multiple sclerosis, leading to improved patient outcomes. This review outlines the steps involved in the entry of myeloid monocytes into the central nervous system that are targets for effective intervention: the activation of bone marrow hematopoiesis, migration of monocytes in the blood, and penetration of the blood–brain barrier by monocytes. Finally, we summarize the different monocyte subpopulations and their effects on the central nervous system based on phenotypic differences. As activated microglia resemble monocyte-derived macrophages, it is important to accurately identify the role of monocyte-derived macrophages in disease. Depending on the roles played by monocyte-derived macrophages at different stages of the disease, several of these processes can be interrupted to limit neuroinflammation and improve patient prognosis. Here, we discuss possible strategies to target monocytes in neurological diseases, focusing on three key aspects of monocyte infiltration into the central nervous system, to provide new ideas for the treatment of neurodegenerative diseases.

BACKGROUND Rebleeding after recovery from esophagogastric variceal bleeding(EGVB)is a severe complication that is associated with high rates of both incidence and *** its clinical importance,recognized prognostic models that can effectively predict esophagogastric variceal rebleeding in patients with liver cirrhosis are *** To construct and externally validate a reliable prognostic model for predicting the occurrence of esophagogastric variceal *** This study included 477 EGVB patients across 2 cohorts:The derivation cohort(n=322)and the validation cohort(n=155).The primary outcome was rebleeding events within 1 *** least absolute shrinkage and selection operator was applied for predictor selection,and multivariate Cox regression analysis was used to construct the prognostic *** validation was performed with bootstrap *** assessed the discrimination,calibration and accuracy of the model,and performed patient risk *** Six predictors,including albumin and aspartate aminotransferase concentrations,white blood cell count,and the presence of ascites,portal vein thrombosis,and bleeding signs,were selected for the rebleeding event prediction following endoscopic treatment(REPET)*** predicting rebleeding within 1 year,the REPET model ex-hibited a concordance index of 0.775 and a Brier score of 0.143 in the derivation cohort,alongside 0.862 and 0.127 in the validation ***,the REPET model revealed a significant difference in rebleeding rates(P<0.01)between low-risk patients and intermediate-to high-risk patients in both *** We constructed and validated a new prognostic model for variceal rebleeding with excellent predictive per-formance,which will improve the clinical management of rebleeding in EGVB patients.

In this paper, we extend the classical compound binomial risk model to the case where the premium income process is based on a Poisson process, and is no longer a linear function. For this more realistic risk model, Lundberg type limiting results for the finite time ruin probabilities are derived. Asymptotic behavior of the tail probabilities of the claim surplus process is also investigated.

For the Hengsha East Shoal Promoting Siltation Project in the Yangtze River Estuary, this work developed a formula for calculating sediment carrying capacity based on variable coefficients and a mathematical model for suspended sedimentation using variable saturation recovery coefficients. Not only does the model yield good verification, preferably give the hydrodynamics and the sediment concentration, but also reach a good agreement between the simulation results andlux by tide for studying the effect the measured topographical changes in the promoting siltation zone of the Hengsha East Shoal. Moreover, this article proposes a method for examing the net sediment tran- sportation fs of the project of promoting siltation and comparatively analyzes the current siltation of the Hengsha East Shoal and siltation mouth layouts, entrance widths, entrance bottom elevations, and the impact of the South Main Dike on promoting siltation through calculating the net sediment transportation flux by tide over the fixed bed and the direct movable bed numerical simulations. The results are in good agreement with each other, indicating that the method for calculating the net sedi- ment transportation flux by the tide is also useful for assessing the project in promoting siltation. Finally, we use the modeling results to analyze the water and sediment diversion ratios and the changes in silting and scouting in riverbeds and draw the conclusion that the completion of the Hengsha East Shoal Promoting Siltation Project would not adversely affect the Yangtze River Estuary Deepwater Channel Project.

Statistical and contextual information are typically used to detect moving regions in image sequences for a fixed *** this paper,we propose a fast and stable linear discriminant approach based on Gaussian single Model(GSM)and Markov Random Field(MRF).The performance of GSM is analyzed first,and then two main improvements corresponding to the drawbacks of GSM are proposed:the latest filtered data based update scheme of the background model and the linear classification judgment rule based on spatial-temporal feature specified by *** results show that the proposed method runs more rapidly and accurately when compared with other methods.

We consider an optimization problem of an insurance company in the diffusion setting, which controls the dividends payout as well as the capital injections. To maximize the cumulative expected discounted dividends minus the penalized discounted capital injections until the ruin time, there is a possibility of （cheap or non-cheap） proportional reinsurance. We solve the control problems by constructing two categories of suboptimal models, one without capital injections and one with no bankruptcy by capital injection. Then we derive the explicit solutions for the value function and totally characterize the optimal strategies. Particularly, for cheap reinsurance, they axe the same as those in the model of no bankruptcy.

Multi-label learning deals with objects associated with multiple class labels,and aims to induce a predictive model which can assign a set of relevant class labels for an unseen *** each class might possess its own characteristics,the strategy of extracting label-specific features has been widely employed to improve the discrimination process in multi-label learning,where the predictive model is induced based on tailored features specific to each class label instead of the identical instance *** a representative approach,LIFT generates label-specific features by conducting clustering ***,its performance may be degraded due to the inherent instability of the single clustering *** improve this,a novel multi-label learning approach named SENCE(stable label-Specific features gENeration for multi-label learning via mixture-based Clustering Ensemble)is proposed,which stabilizes the generation process of label-specific features via clustering ensemble ***,more stable clustering results are obtained by firstly augmenting the original instance repre-sentation with cluster assignments from base clusters and then fitting a mixture model via the expectation-maximization(EM)*** experiments on eighteen benchmark data sets show that SENCE performs better than LIFT and other well-established multi-label learning algorithms.
