Prevalence of coronary artery ectasia in older adults and the relationship with epicardial fat volume by cardiac computed tomography angiography

Objective Coronary artery ectasia (CAE) refers to abnormal dilation of coronary artery segments to 1.5 times of adjacent normal ones. Epicardial fat is associated with cardiovascular risk factors. The relationship between CAE and epicardial fat has not yet been investigated. This study aimed to assess the relationship between CAE and epicardial fat volume (EFV) in older people by dual-source computed tomography coronary angiography (CTCA). Methods We prospectively enrolled 1400 older adults who were scheduled for dual-source CTCA. Under reconstruction protocols, patients with abnormal segments 1.5 times larger than the adjacent segments were accepted as CAE. EFV was measured by semi-automated software. Traditional risk factors in CAE patients, as well as the extent of EFV, were analyzed and compared to non-CAE group. Results A total of 885 male and 515 female older patients were enrolled. CAE was identified by univariable analysis in 131 patients and significantly correlated to hypertension, smoking, hyperlipidemia, prior percutaneous coronary intervention and ascending aorta aneurysm. EFV was shown to be significantly higher inCAE patients than patients without ectasia. Inmultivariable analyses, EFV (P = 0.018), hypertension (P < 0.001) and hyperlipidemia (P < 0.001) were significantly correlated to CAE. There was a significant negative correlation between EFV and Markis classification. Conclusions CAE can be reliably recognized by dual-source CTCA. Epicardial fatmight play a role in etiopathogenesis and progression of CAE, providing a newtarget for treating ectasia.

AIM： To retrospectively evaluate the clinical relevance, perioperative risk factors, outcome of different pharmacological prophylaxis, and short-term prognostic value of atrial fibrillation （AF） after surgery for esophageal carcinoma. METHODS： We retrospectively studied 63 patients with AF after surgery for esophageal carcinoma in comparison with 126 patients without AF after esophagectomy during the same time. Postoperative AF incidence was related to different clinical factors possibly involved in its occurrence and short-term survival. RESULTS： A strong relationship was observed between AF and postoperative hypoxia, history of chronic obstructive pulmonary disease （COPD）, postoperative thoracic-gastric dilatation, age older than 65 years, male sex and history of cardiac disease. No difference was observed between the two groups with regard to shortterm mortality and length of hospital stay. CONCLUSIONS： AF occurs more frequently after esophagectomy in aged and male patients. Other factors contributing to postoperative AF are history of COPD and cardiac disease, postoperative hypoxia and thoracicgastric dilatation.

The coronary artery calcification score and pericardial fat volume have recently been reported to be strongly associated with the severity and presence of coronary atherosclerosis. However, no studies have explored the outcome of phased progression of atherosclerosis by non-contrast computed tomography in asymptomatic people in China. The population-based cohort study in outcome of phased progression of atherosclerosis in China （PERSUADE）, an observational, longitudinal and prospective cohort study in a target population of healthy sub- jects based in Jidong Oilfield （China）, prospectively analyzes the outcome of phased progression of atherosclerosis by non-contrast computed tomography in healthy population. The results of this study are expected to be of value for utilizing noninvasive imaging combine with tradi- tional cardiovascular risk factors to create a risk stratification and find pertinent biomarkers associated with the outcome of phased progres- sion of atherosclerosis in healthy people, thereby could help to establish a more personalized treatment of clinical practice.

Background In order to acquire a high quality image with a low radiation dose, prospective electrocardiogram （ECG）-triggered computed tomography coronary angiography （CTCA） requires a stable heart rate （HR）〈 65 beats/min. Esmolol has the advantage of reduc-ing HR. The objective of this article is to assess the value of intravenous esmolol treatment before prospective ECG-tr/ggered high-pitch spiral acquisition for CTCA. Methods From March 2013 to june 2013, 313 patients underwent prospective ECG-triggered CTCA. Two hundred and thirty two of them received esmolol before angiography. We retrospectively analyzed clinical characteristics, esmolol dose, radiation exposure dose, and the change in HR and blood pressure in these 232 patients. Results A total of 232 patients with a HR 〉 65 beats/rain before CTCA examination received intravenous esmolol treatment （mean dose of 57.26±15.39 rag）, The mean initial HR （HR1）, slowest HR （HR2）, and the HR 30 min after HR2 （HR3） were 75.06± 5.59, 60.75 ±4.00, and 75.54 ± 5.96 beats/min, respectively （HR1 vs. HR2, P 〈 0.0001; HRI vs. HR3, P = 0.377）. The mean time from esmolol administration to HR2 was 24.25 ± 4.97 s and the mean effective radiation dose was 2.28 ± 0.02 mSv. Conclusions HR could be rapidly controlled at an optimum level with intravenous esmolol before prospective ECG-triggered high-pitch spiral acquisition for CTCA. Consequently, the patients received a very low radiation dose.

Spatial Variation of P and N in Water and Sediments of Dianchi Lake, China

Dianchi Lake is one of the most eutrophic lakes in China. In order to understand this eutrophication and to help control the pollution, this research investigated the spatial distribution of Kjeldahl nitrogen (K-N) and total phosphorus(TP) through analysis of bottom water and sediment (3 depths) samples collected at 118 sites around Dianchi Lake. The concentrations of K-N and TP for the lake bottom water in the Caohai part of the lake were much higher than those in the Waihai part, generally decreasing from north to south. In the sediments, the K-N concentration was higher in the Caohai part and the middle of the Waihai part. On the other hand, TP in the sediments was greater in the southern and western parts. Both K-N and TP had similar spatial distributions for the sediment samples of three different ***, the K-N and TP concentration in the sediments decreased with an increase in depth. This was evidence that eutrophication and pollution of Dianchi Lake was becoming gradually more severe. Exterior factors including uncontrolled input of domestic and industrial effluents as well as non-point pollution around the lake were the main reasons for serious eutrophication; therefore, controlling these was the first step in reducing eutrophication of Dianchi Lake.

AIM To establish the surgical flow for anatomic isolated caudate lobe resection. METHODS The study was approved by the ethics committee of the Second Affiliated Hospital Zhejiang University School of Medicine(SAHZU). From April 2004 to July 2014, 20 patients were enrolled who underwent anatomic isolated caudate lobectomy at SAHZU. Clinical and postoperative pathological data were analyzed. RESULTS Of the total 20 cases, 4 received isolated complete caudate lobectomy(20%) and 16 received isolated partial caudate lobectomy(80%). There were 4 caseswith the left approach(4/20, 20%), 6 cases with the right approach(6/20, 30%), 7 cases with the bilateral combined approach(7/20, 35%), 3 cases with the anterior approach(3/20, 15%), and the hanging maneuver was also combined in 2 cases. The median tumor size was 5.5 cm(2-12 cm). The median intraoperative blood loss was 600 m L(200-5700 m L). The median intra-operative blood transfusion volume was 250 m L(0-2400 m L). The median operation time was 255 min(110-510 min). The median post-operative hospital stay was 14 d(7-30 d). The 1-and 3-year survival rates for malignant tumor were 88.9% and 49.4%, respectively. CONCLUSION Caudate lobectomy was a challenging procedure. It was demonstrated that anatomic isolated caudate lobectomy can be done safely and effectively.
