Korean oriental medicine prescription is widely used for the treatment of gouty diseases. In the present study, we investigated anti-inflammatory effects of modified Korean herbal formulation, mixed extract of medicinal herbs(MEMH), and its modulatory effects on inflammatory mediators associated with gouty arthritis. Both in vitro and in vivo studies were carried out to assess the anti-inflammatory efficacy of MEMH on monosodium urate(MSU) crystals-induced gouty inflammation. MSU crystals stimulated human chondrosarcoma cell line, SW1353, and human primary chondrocytes were treated with MEMH in vitro. The expression levels of pro-inflammatory mediators and metalloproteases were analyzed. The effect of MEMH on NFκB signaling pathway in SW1353 cells was examined. Effect of MEMH on the mR NA expression level of pro-inflammatory mediators and chemotactic factor from human monocytic cell line, THP-1, was also analyzed. The probable role of MEMH in the differentiation process of osteoblast like cells, SaO S-2, after MSU treatment was also observed. To investigate the effects of MEMH in vivo, MSU crystals-induced ankle arthritic model was established. Histopathological changes in affected joints and plasma levels of pro-inflammatory mediators(IL-1β and TNFα) were recorded. MEMH inhibited NFκB signaling pathway and COX-2 protein expression in chondrocytes. MSU-induced mR NA expressions of pro-inflammatory mediators and chemotactic cytokines were suppressed by MEMH. In MSU crystals-induced ankle arthritic mouse model, administration of MEMH relieved inflammatory symptoms and decreased the plasma levels of IL-1β and TNFα. The results indicated that MEMH can effectively inhibit the expression of inflammatory mediators in gouty arthritis, demonstrating its potential for treating gouty arthritis.

In this study, we aimed to investigate the prevalence and perception of premature ejaculation （PE） in young and middle-aged Korean men. The study was conducted using an Internet-assisted questionnaire. A total of 2 037 Korean male adults, aged 20 years or older, were randomly sampled based on age and residency. The questionnaire developed by the PE Study Group of the Korean Andrological Society includes four categories （overall sexual function, symptoms, distress and treatment） with a total of 16 questions. For each question, symptoms were evaluated by a scale ranging from 0 to 10. Intravaginal ejaculation latency time was ‘5-10 min＇ in 38.6%, followed by ‘longer than 10 min＇ in 2919%, ‘2-5 min＇ in 23.6%, ‘1-2 min＇ in 5.4% and ‘shorter than 1 min＇ in 2.5%. In our series, 27.5% of respondents reported having PE. Control over ejaculation within a recent 3-month period was 6.2 points on average. Respondent complaints of PE-related stress averaged 7.1 points and stress-related complaints from sexual partners averaged 7.1 points. The effect of PE on sexual life was 6.8 points. Of the respondents determined as having PE, 42.6% responded that they were inclined to receive treatment. Results from this study suggest that the prevalence of PE diagnosed by the respondent on his own was approximately 27.5% in young and middle-aged men in Korea. PE-related stress had a significant effect on the stress, sexual activity and quality of life of the respondent and his sexual partner.

Heat stress(HS)damages livestock by adversely affecting physiological and immunological ***,fundamental understanding of the metabolic and immunological mechanisms in animals under HS remains elusive,particularly in *** understand the changes on metabolic and immune responses in steers under HS condition,we performed RNA-sequencing and proton nuclear magnetic resonance spectroscopy-based metabolomics on HS-free(temperature humidity index[THI]value:64.92±0.56)and HS-exposed(THI value:79.13±0.56)Jersey steer(n=8,body weight:559.67±32.72 kg).This study clarifies the metabolic changes in 3 biofluids(rumen fluid,serum,and urine)and the immune responses observed in the peripheral blood mononuclear cells of HS-exposed *** integrated approach allowed the discovery of HS-sensitive metabolic and immunological *** metabolomic analysis indicated that HS-exposed steers showed potential HS biomarkers such as isocitrate,formate,creatine,and riboflavin(P<0.05).Among them,there were several integrative metabolic pathways between rumen fluid and ***,HS altered mRNA expression and immune-related signaling pathways.A meta-analysis revealed that HS decreased riboflavin metabolism and the expression of glyoxylate and dicarboxylate metabolism-related ***,metabolic pathways,such as the hypoxia-inducible factor-1 signaling pathway,were downregulated in immune cells by HS(P<0.05).These findings,along with the datasets of pathways and phenotypic differences as potential biomarkers in steers,can support more in-depth research to elucidate the interrelated metabolic and immunological *** would help suggest new strategies to ameliorate the effects of HS,including disease susceptibility and metabolic disorders,in Jersey steers.

AIM:To investigate the efficacy of amitriptyline with proton pump inhibitor(PPI)for the treatment of functional chest pain(FCP).METHODS:This was a randomized,open-label trial investigating the addition of low dose amitriptyline(10 mg at bedtime)to a conventional dose of rabeprazole(20 mg/d)(group A,n = 20)vs a double-dose of rabeprazole(20 mg twice daily)(group B,n = 20)for patients with FCP whose symptoms were refractory to *** primary efficacy endpoints were assessed by global symptom score assessment and the total number of individuals with > 50% improvement in their symptom ***:The between-group difference in global symptom scores was statistically significant during the last week of treatment(overall mean difference;3.75 ± 0.31 vs 4.35 ± 0.29,the between-group difference;P 50%,whereas only 26.3% of patients in group B had a similar treatment response(70.6% vs 26.3%,P = 0.008).Specifically,patients in group A had a significantly greater improvement in the domains of body pain and general health perception than did patients in group B(52.37 ± 17.00 vs 41.32 ± 12.34,P = 0.031 and 47.95 ± 18.58 vs 31.84 ± 16.84,P = 0.01,respectively).CONCLUSION:Adding amitriptyline to a PPI was more effective than a double-dose of PPI in patients with FCP refractory to a conventional dose of PPI.

AIM: To access the short-term outcomes of simultaneous laparoscopic surgery combined with resection for synchronous lesions in patients with colorectal cancer. METHODS: Between March 1996 and April 2010 prospectively collected data were reviewed from 93 consecutive patients who had colorectal cancer and underwent simultaneous multiple organ resection (combined group) and 1090 patients who underwent conventional laparoscopic right hemicolectomy or laparoscopic low/ anterior resection for colorectal cancer (non-combined group). In the combined group, there were nine gastric resections, three nephrectomies, nine adrenalectomies, 56 cholecystectomies, and 21 gynecologic resections. In addition, f ive patients underwent simultaneous laparoscopic resection for three organs. The patient demographics, intra-operative outcomes, surgical morbidity, and short-term outcomes were compared between thetwo groups (the combined and non-combined groups). RESULTS: There were no signifi cant differences in the clinicopathological variables between the two groups. The operating time was signifi cantly longer in the combined group than in the non-combined group, regardless of tumor location (laparoscopic right hemicolectomy and laparoscopic low/anterior resection groups; P = 0.048 and P < 0.001, respectively). The other intraoperative outcomes, such as the complications and open conversion rate, were similar in both groups. The rate of post-operative morbidity in the combined group was similar to the non-combined group (combined vs non-combined, 15.1% vs 13.5%, P = 0.667). Oncological safety for the colon and synchronous lesions were obtained in the combined group. CONCLUSION: Simultaneous laparoscopic multiple organ resection combined with colorectal cancer is a safe and feasible option in selected patients.

Predictors of pain resolution after varicocelectomy for painful varicocele

Varioocelectomny is a management option for patients with painful varicocele. In this study, we assessed the effectiveness of varicocelectomy for painful varicocele and examined the factors that might be predictive of outcome. All patients who underwent a varicocelectomy for pain between February 2007 and July 2009 were includecL A review of patient medical records was conducted; patient age,body mass index （BMI）, uede, iocatien of the vericocele, tasticular volume, duration and quality of the pain （dull, draaing, throbbing or sharp） and surgical technique （ingulnal versus subinguinal） were documented. All parameters were compared with the rasolution of pain （complete, partial or failure）. We followed up on 53 of 104 patients （51.0%）. Complete postopanntive resolution of pain was reported by 28 patients （52.8%）, whereas 22 （41.5%） reported partial resolution, Only three patients （5.7%） mpaded failure. No relationship was observed between postoperative pain resolution and age, BMI, grade of varicoceio, location of varicocele, ipsilateral testicular, quality of pain or surgical technique. The duration of pain before surllery was the only factor that correlated with postopaative pain resolution （univariate, P=-0.004; multivariate, P=0.002）. Our rasults indicatethat varicocelectomy isan effective treatmentfar painful varicocele in properly selected patients, and that duration of pain before surgery may be predictive of outcome.
