Objective The relationship between serum uric acid(SUA)levels and glycemic indices,including plasma glucose(FPG),2-hour postload glucose(2 h-PG),and glycated hemoglobin(HbA1 c),remains *** aimed to explore the associations between glycemic indices and SUA levels in the general Chinese *** The current study was a cross-sectional analysis using the first follow-up survey data from The china Cardiometabolic Disease and Cancer Cohort Study.A total of 105,922 community-dwelling adults aged≥40 years underwent the oral glucose tolerance test and uric acid *** nonlinear relationships between glycemic indices and SUA levels were explored using generalized additive *** A total of 30,941 men and 62,361 women were eligible for the current *** additive models verified the inverted U-shaped association between glycemic indices and SUA levels,but with different inflection points in men and *** thresholds for FPG,2 h-PG,and HbA1 c for men and women were 6.5/8.0 mmol/L,11.0/14.0 mmol/L,and 6.1/6.5,respectively(SUA levels increased with increasing glycemic indices before the inflection points and then eventually decreased with further increases in the glycemic indices).Conclusion An inverted U-shaped association was observed between major glycemic indices and uric acid levels in both sexes,while the inflection points were reached earlier in men than in women.

原发性免疫性血小板减少症(ITP)中少见但至关重要的危重出血事件,给患者的预后、生活质量和治疗决策带来严重影响。尽管有一些研究探讨了ITP中与危重出血相关的风险因素,但目前尚缺乏大样本数据、大规模多中心研究结果以及针对ITP患者致命出血事件的预测模型。本研究首次采用国际血栓与止血学会新提出的ITP致命出血标准,利用大样本数据开发了首个基于机器学习的在线应用,用于预测ITP患者的致命出血.研究中,我们使用中国各地大型多中心数据进行开发,并对全国39家医疗中心进行为期一年的外部测试,得到了较好的训练、验证和测试数据集预测能力该基于新算法的便捷网络工具能够快速识别ITP患者的出血风险,辅助临床决策,有望未来降低不良事件的发生。

目的观察双清合剂治疗卫气同病证流行性感冒的有效性及安全性。方法采用多中心、随机、双盲、双模拟平行对照、非劣效性试验设计,将240例卫气同病证流行性感冒患者采用分层区组随机化方法,使用SAS软件按1:1随机分为试验组和对照组,每组120例。试验组服用双清合剂和磷酸奥司他韦胶囊模拟剂,对照组服用磷酸奥司他韦胶囊和双清合剂模拟剂,两组疗程均为5日。分别于治疗前、治疗3日、治疗5日时进行访视,评价其主要临床症状缓解时间、完全退热时间、退热起效时间、对乙酰氨基酚使用量、中医证候积分、中医证候疗效、重要临床症状缓解时间、重症/危重流感发生率及安全性。结果试验期间,共脱落/剔除16例(试验组8例、对照组8例),最终纳入符合方案集224例,其中试验组112例,对照组112例。与对照组比较,试验组可缩短头痛缓解时间[28.00(17.75,42.99)h vs.41.00(18.75,59.00)h]、减少肌肉或关节酸痛缓解时间[37.83(12.88,52.77)h vs.38.07(13.50,69.25)h](P0.05)。两组患者不良反应发生率,试验组为4.17%和对照组为1.67%,差异无统计学意义(χ^(2)=1.36,P>0.05)。结论双清合剂可有效缓解卫气同病证流行性感冒的头痛、肌肉或关节酸痛症状,效果优于磷酸奥司他韦胶囊;在改善发热、咳嗽、汗出、面赤、咳黄痰临床症状,减少对乙酰氨基酚使用量,降低中医证候积分方面与磷酸奥司他韦胶囊疗效相当;安全性与磷酸奥司他韦胶囊相当。(中国临床试验注册中心注册***2400087831)。

There is considerable potential for integrating transarterial chemoembolization(TACE),programmed death-(ligand)1(PD-[L]1)inhibitors,and molecular targeted treatments(MTT)in hepatocellular carcinoma(HCC).It is necessary to investigate the therapeutic efficacy and safety of TACE combined with PD-(L)1 inhibitors and MTT in real-world *** this nationwide,retrospective,cohort study,826 HCC patients receiving either TACE plus PD-(L)1 blockades and MTT(combination group,n=376)or TACE monotherapy(monotherapy group,n=450)were included from January 2018 to May *** primary endpoint was progression-free survival(PFS)according to modified *** secondary outcomes included overall survival(OS),objective response rate(ORR),and *** performed propensity score matching approaches to reduce bias between two *** matching,228 pairs were included with a predominantly advanced disease *** PFS in combination group was 9.5 months(95%confidence interval[CI],8.4-11.0)versus 8.0 months(95%CI,6.6-9.5)(adjusted hazard ratio[HR],0.70,P=0.002).OS and ORR were also significantly higher in combination group(median OS,19.2[16.1-27.3]vs.15.7 months[13.0-20.2];adjusted HR,0.63,P=0.001;ORR,60.1%vs.32.0%;P<0.001).Grade 3/4 adverse events were observed at a rate of 15.8%and 7.5%in combination and monotherapy groups,*** results suggest that TACE plus PD-(L)1 blockades and MTT could significantly improve PFS,OS,and ORR versus TACE monotherapy for Chinese patients with predominantly advanced HCC in real-world practice,with an acceptable safety profile.

目的：探讨下腹正中切口入路腹膜前间隙腹股沟疝无张力修补术的临床应用价值。方法回顾性分析2010年4月至2012年10月哈尔滨医科大学附属第四医院普外科收治的120例腹股沟疝病人的临床资料，均采用补片行下腹正中切口入路腹膜前间隙腹股沟疝无张力修补术。其中，单侧疝98例，双侧疝22例；斜疝85例，直疝30例，复发疝5例。观察手术时间、术后舒适性、并发症和复发率。结果120例病人手术时间为20～60 min，单侧疝平均30 min，双侧疝平均50 min。术后6 h下床活动。平均住院时间为4 d。切口积液2例，无阴囊血肿或积液，无尿潴留。术后随访6～24个月无复发。结论下腹正中切口入路腹膜前间隙腹股沟疝修补术学习曲线短、操作简便、安全性高、术后复发率低、恢复快、异物不适感轻微、术后慢性疼痛发生率低，尤其适合于双侧疝及复发疝。

The incidence of pancreatic cancer has been rising worldwide,and its clinical diagnosis and treatment remain a great *** present the update and improvements in the clinical diagnosis and treatment of pancreatic cancer in recent years,Chinese Pancreatic Association,the Chinese Society of surgery,Chinese medical Association revised the Guidelines for the Diagnosis and Treatment of Pancreatic Cancer in china(2014)after reviewing evidence-based and problem-oriented literature published during 2015-2021,mainly focusing on highlight issues regarding diagnosis and surgical treatment of pancreatic cancer,conversion strategies for locally advanced pancreatic cancer,treatment of pancreatic cancer with oligo metastasis,adjuvant and neoadjuvant therapy,standardized processing of surgical specimens and evaluation of surgical margin status,systemic treatment for unresectable pancreatic cancer,genetic testing,as well as postoperative follow up of patients with pancreatic *** recommendation items were finally proposed based on the above issues,and the quality of evidence and strength of recommendations were graded using the Grades of Recommendation,Assessment,Development,and Evaluation *** guideline aims to standardize the clinical diagnosis and therapy,especially surgical treatment of pancreatic cancer in china,and further improve the prognosis of patients with pancreatic cancer.

根据国内外循证医学指南制定标准化方法与步骤,组建多学科指南制定专家工作组,先后经过指南注册及指南计划书撰写、相关指南评价分析、临床问题遴选和确定、临床证据检索及评价、形成推荐意见等流程,经共识专家组3轮讨论,最终制定中国老年骨质疏松症诊疗指南(2018)。使用GRADE系统对证据体和推荐意见进行分级。同时考虑中国患者的偏好与价值观、干预措施的成本和利弊平衡为老年骨质疏松症的诊疗提供15条推荐意见。该指南涵盖了老年骨质疏松症的筛查、风险评估、诊断、基础措施、多种抗骨质疏松药物、疗效监测和评估等方面。本指南旨在为我国广大临床医生和患者提供科学依据。

BACKGROUND In addition to insulin resistance,impaired insulin secretion has recently been identified as a crucial factor in the pathogenesis of type 2 diabetes mellitus(T2DM).Scarce clinical data exist for pediatric *** To investigate the association ofβ-cell function and insulin resistance with pediatric T2DM in the first Chinese multicenter *** This multicenter cross-sectional study included 161 newly diagnosed T2DM children and adolescents between January 2017 and October *** with normal glycemic levels(n=1935)were included as healthy control *** homeostasis models(HOMAs)were used to assess theβ-cell function(HOMA2-%B)and insulin resistance(HOMA2-IR)*** HOMA index was standardized by sex and *** performed logistic regression analysis to obtain odds ratios(ORs)for T2DM risk using the standardized HOMA index,adjusted for confounding factors including sex,Tanner stage,T2DM family history,body mass index z-score,and lipid *** The male-female ratio of newly diagnosed T2DM patients was 1.37:1(OR=2.20,P=0.011),and the mean ages of onset for boys and girls were 12.5±1.9 years and 12.3±1.7 years,*** prevalence of related comorbidities including obesity,elevated blood pressure,and dyslipidemia was 58.2%,53.2%,and 80.0%,*** T2DM group had lower HOMA2-%B levels(P<0.001)and higher HOMA2-IR levels(P<0.001)than the control *** the decrease in HOMA2-%B z-score(OR=8.40,95%CI:6.40-11.02,P<0.001)and the increase in HOMA2-IR z-score(OR=1.79,95%CI:1.60-2.02,P<0.001)were associated with a higher risk of T2DM,and the decrease in HOMA2-%B z-score always had higher ORs than the increase in HOMA2-IR z-score after adjusting for confounding *** Besides insulin resistance,β-cell function impairment is also strongly associated with Chinese pediatric *** difference in susceptibility and high comorbidities warrant specific T2DM screening and prevention strategies in Chinese children.
