Cardiorespiratory fitness measured with cardiopulmonary exercise testing and mortality in patients with cardiovascular disease:A systematic review and meta-analysis

Background Cardiorespiratory fitness(CRF)is inversely associated with mortality in apparently healthy subjects and in some clinical populations,but evidence for the association between CRF and all-cause and/or cardiovascular disease(CVD)mortality in patients with established CVD is *** study aimed to quantify this *** We searched for prospective cohort studies that measured CRF with cardiopulmonary exercise testing in patients with CVD and that examined all-cause and CVD mortality with at least 6 months of *** hazard ratios(HRs)were calculated using random-effect inverse-variance *** Data were obtained from 21 studies and included 159,352 patients diagnosed with CVD(38.1%female).Pooled HRs for all-cause and CVD mortality comparing the highest *** category of CRF were 0.42(95%confidence interval(95%CI):0.28–0.61)and 0.27(95%CI:0.16–0.48),*** HRs per 1 metabolic equivalent(1-MET)increment were significant for all-cause mortality(HR=0.81;95%CI:0.74–0.88)but not for CVD mortality(HR=0.75;95%CI:0.48–1.18).Coronary artery disease patients with high CRF had a lower risk of all-cause mortality(HR=0.32;95%CI:0.26–0.41)than did their unfit *** 1-MET increase was associated with lower all-cause mortality risk among coronary artery disease patients(HR=0.83;95%CI:0.76–0.91)but not lower among those with heart failure(HR=0.69;95%CI:0.36–1.32).Conclusion A better CRF was associated with lower risk of all-cause mortality and *** study supports the use of CRF as a powerful predictor of mortality in this population.

Long Coronary Lesions: Challenging Cases for Percutaneous Coronary Intervention

Long coronary lesions are associated with adverse outcomes after percutaneous coronary intervention since the era of plain balloon angioplasty. Long lesion and long stent length are considered as important predictors of restenosis after percutaneous coronary intervention. With the advent of newer generation drug eluting stents, there has been dramatic reduction in the rates of restenosis and repeat revascularization, even in complex cohort of patients with long coronary lesions. We report one such case of long coronary lesion which was intervened successfully with newer generation, thin strut, biodegradable polymer coated sirolimus-eluting stents.

Objective To assess the prognostic impact of a routine invasive strategy according to the frailty burden in patients with non-ST-segment elevation myocardial infarction(nSTEMI)from the MOSCA-FRAIL clinical *** The MOSCA-FRAIL trial randomized 167 frail patients,defined by a Clinical Frailty Scale(CFS)≥4,with nSTEMI to an invasive or conservative *** primary endpoint was the number of days alive and out of hospital(DAOH)one year after *** this subanalysis,we compared the impact of an invasive strategy on the outcomes between vulnerable(CFS=4,n=43)and frail(CFS>4,n=124)*** Compared to vulnerable patients,frail patients presented lower values of DAOH(289.8 vs.320.6,P=0.146),more read-missions(1.03 vs.0.58,P=0.046)and higher number of days spent at the hospital during the first year(10.8 vs.3.8,P=0.014).The cau-ses of readmission were mostly non-cardiac(56%).Among vulnerable patients,DAOH were similar regardless of strategy(invasive ***:325.7 vs.314.7,P=0.684).Among frailest patients,the invasive group tended to have less DAOH(267.7 vs.311.1,P=0.117).Indeed,patients with CFS>4,invasively managed lived 29 days less than their conservative *** contrast,the-re were no differences in the subgroup with CFS=*** Adult patients with frailty and nSTEMI showed different prognosis according to the degree of frailty.A routine in-vasive strategy does not improve outcomes and might be harmful to the frailest patients.

High-pressure scheelite-type polymorph of SmCrO_4: synthesis, structural characterization and magnetic properties

The new scheelite form of SmCrO4 oxide was obtained by heating the zircon-type SmCrO4 oxide at 4 GPa and 803 K. X-ray diffraction revealed that this scheelite SmCrO4 phase crystallized with tetragonal symmetry, S.G. I41/a and lattice parameters: a=0.50776(3) nm and c=1.15606(2) nm. This structural phase transition from zircon to scheelite involved a decreasing of around 10% in the unit cell volume. Although the Cr-O and Sm-O distances did not change very much in both zircon and scheelite polymorphs, the changes occurred in the bond angles were remarkable that appear to support the proposed reconstructive model to explain this structural zircon-scheelite phase transition. Magnetic susceptibility and magnetization measurements revealed that the scheelite SmCrO4 oxide behaved an antiferromagnetic material, where the Sm3+ and Cr5+ were simultaneously ordered. The estimated néel temperature, Tn, was 16 K and the critical field at 12 K associated with the metamagnetic transition was 3.2 T.

Optimizing urban traffic control using a rational agent

This paper is devoted to developing and evaluating a set of technologies with the objective of designing a methodology for the implementation of sophisticated traffic lights by means of rational agents. These devices would be capable of optimizing the behavior of a junction with multiple traffic signals, reaching a higher level of autonomy without losing reliability, accuracy, or efficiency in the offered services. In particular, each rational agent in a traffic signal will be able to analyze the requirements and constraints of the road, in order to know its level of demand. With such information, the rational agent will adapt its light cycles with the view of accomplishing more fluid traffic patterns and minimizing the pollutant environmental emissions produced by vehicles while they are stopped at a red light, through using a case-based reasoning(CBR) adaptation. This paper also integrates a microscopic simulator developed to run a set of tests in order to compare the presented methodology with traditional traffic control methods. Two study cases are shown to demonstrate the efficiency of the introduced approach, increasing vehicular mobility and reducing harmful activity for the environment. For instance, in the first scenario, taking into account the studied traffic volumes, our approach increases mobility by 23% and reduces emissions by 35%. When the roads are managed by sophisticated traffic lights, a better level of service and considerable environmental benefits are achieved, demonstrating the utility of the presented approach.

Electrochemical reduction of Cr(Ⅵ)in the presence of sodium alginate and its application in water purification

Chromium(Cr)is used in many manufacturing processes,and its release into natural waters is a major environmental problem *** concentrations of Cr(Ⅵ)are toxic to human health and living organisms due to the carcinogenic and mutagenic nature of this *** work examined the conversion of Cr(Ⅵ)to Cr(Ⅲ)via electrochemical reduction using gold electrode in an acidic sodium alginate(SA)solution and subsequent removal of the produced Cr(Ⅲ)-SA by the polymer-enhanced ultrafiltration(PEUF)technique.A solution of SA in nitric acid was used both as an electrolytic medium during the voltammetric measurements and bulk electrolysis and as an extracting agent during the PEUF *** electroanalysis of Cr(Ⅵ)was performed by linear sweep voltammetry in the presence of acidic SA solution to study its voltammetric behavior as a function of the Cr(Ⅵ)concentration,pH,presence of Cr(Ⅲ),SA concentration and scan *** addition,the quantitative reduction of Cr(Ⅵ)to Cr(Ⅲ)was studied through the bulk electrolysis *** results showed efficient reduction with well-defined peaks at approximately 0.3 V ***/AgCl,using a gold working *** the pH increased,the reduction signal strongly decreased until its *** optimum SA concentration was 10 mmol/L,and it was observed that the presence of Cr(Ⅲ)did not interfere in the Cr(Ⅵ)*** the quantitative reduction by bulk electrolysis in the presence of acidic SA solution,it was possible to reduce all Cr(Ⅵ)to Cr(Ⅲ)followed by its removal via PEUF.

Effect of Organo-Modified nanosepiolite on Fire Behaviors and Mechanical Performance of Polypropylene Composites

The objective of the study was to investigate the effect of the organo-modified nanosepiolite(OnSep)on improving the fire safety of polypropylene(PP).The composites based on PP,flame retardant master batch(MB-FR,25 wt%PP+50 wt%decabromodiphenyl ether(DBDPE)+25%antimony trioxide(ATO))and OnSep were prepared via melt *** results of the limiting oxygen index(LOI)and vertical burning rating(UL-94)test indicated that PP/40 wt%MB composites had no rating with seriously dripping phenomenon,while the LOI value was only ***,as 4 wt%OnSep was added in PP/40 wt%MB composites,the composites achieved UL94 V-0 rating and the LOI value was *** comparison,PP/50 wt%MB composites could not reach the V-0 rating *** TGA results revealed that the addition of OnSep enhanced the thermal stability of the PP/MB-FR *** cone calorimeter results indicated that the heat release rate,average mass loss rate,smoke production rate and smoke temperature of the PP/40 wt%MB-FR/4 wt%OnSep composites decreased in comparison with those of PP/40 wt%MB-FR ***,the Young modulus and impact strength were also much better improved with the increase of OnSep ***,the synergistic flame retardancy of OnSep in PP/MB-FR matrix significantly containing a halogen based flame retardant(DBDPE)significantly improved the fire safety and mechanical properties of the composites,and allowed to decrease the total amount of brominated fire retardants.
