美国放射学院(American College of Radiology,ACR)偶发病变委员会(Incidental Findings Committee,IFC)针对CT和MRI上偶然发现的肾上腺肿块,提出了新的管理建议。该建议是对美国放射学院杂志(JACR)2010版肾上腺、肾脏、肝脏及胰腺偶发病变管理白皮书中肾上腺部分的更新。由腹部放射科医师和1位内分泌外科医生组成的肾上腺亚组委员会制定了该方案。该方案结合已发表的文献和专家意见,经过反复推敲最终达成共识。该方案依据患者的特征和图像的特点对偶然发现的肾上腺肿块进行分类。对于每种特定的组合,总结良性或惰性的特征(足以终止随访),并提出后续管理建议。该方案涵盖了很多、但并非适用于所有的病理及临床情况,旨在通过提供肾上腺偶发肿块的管理意见以提升医疗质量。

We read with great interest the study of Ratti et al.(1)aiming to compare robotic,laparoscopic and open hepatectomies in terms of feasibility,safety and oncological adequacy in surgery for biliary tumours(intrahepatic cholangiocarcinoma,perihilar cholangiocarcinoma and gallbladder cancer).The authors conducted a retrospective monocentric study focusing on the quality of lymphadenectomy,operative time and postoperative morbidity and mortality up to day *** a weighted propensity score analysis by inverse probability of treatment weighting(IPTW),25 patients operated by robotic approach were compared with 97 patients operated by laparoscopy and 113 by laparotomy.

high early strength(hES)concrete pavements are commonly opened to traffic within the first 24 h after construction,making early-opening decisions critical for pavement quality and traffic ***,most state departments of transportation rely on compressive strength testing for early-opening decision ***,there laboratory tests are labor intensive,costly and not always representative of field strength *** this study,non-destructive testing(maturity and ultrasonic tomography)was explored for faster and reliable in-situ strength *** experimental section constructed using hES concrete was routinely monitored using compressive testing,maturity,and ultrasonic tomography in the first 24 h after *** shear wave velocity,measured using ultrasonic tomography,was able to capture the strength-gain variability within a single slab and between different slabs due to the ability to monitor several locations in a short period of *** results were consistently conservative in the first 24 h of *** show that both maturity testing and ultrasonic tomography are able to replace or add to conventional strength testing for hES concrete pavements to facilitate making the opening decision within the first 24 *** tomography proved more beneficial as a result of the device's portability,increased speed of testing,and accurate estimations of hES concrete strength for the entire pavement length.

Personality can play a large role in dispersal decisions, influencing how each individual estimates and values expected costs versus benefits of venturing out to explore the unknown. We contrast the impact of persistence on fitness using decision theory models to examine the relative success of prospecting movement (which is a widespread behavior used by animals to explore available breeding habitat before determining where to settle) in different landscapes among individuals with different personalities. While evaluating trends in the costs and benefits of movement we provide a quantifiable measure of the impacts of relative tenacity in prospecting behavior on fitness gains/losses. Our model results predict that, regardless of landscape composition, individuals with greater tenacity in prospecting strategy have reduced fitness gains relative to individuals who are more willing to both initiate novel strategies, but then also abandon the new strategy rapidly if initial search does not seem fruitful. We show that fitness gains from choosing an appropriate prospecting strategy (i.e. abandoning poor habitat or staying in rich habitat) can be very high, indicating a potentially large influence from personality on fitness. Importantly, the model predicts that relative fitness gains are highly dependent on the availability of high quality habitat in the landscape, even if the difference under varying fitness outcomes between poor and rich habitat was substantial.
