Clinical management of acute liver failure: Results of an international multi-center survey

AIM To assess the practice of caring for acute liver failure(ALF) patients in varying geographic locations and medical *** Members of the European Acute Liver Failure Consortium completed an 88-item questionnaire detailing management of ALF. Responses from 22 transplantation centers in 11 countries were analyzed,treating between 300 and 500 ALF cases and performing over 100 liver transplants(LT) for ALF annually. The questions pertained to details of the institution and their clinical activity,standards of care,referral and admission,wardbased care versus intensive care unit(ICU) as well as questions regarding liver transplantation- including criteria,limitations,and perceived performance. Clinical data was also collected from 13 centres over a 3 mo period. RESULTS The interval between referral and admission of ALF patients to specialized units was usually less than 24 h and once admitted,treatment was provided by a multidisciplinary team. Principles of care of patients with ALF were similar among centers,particularly in relation to recognition of severity and care of the more critically ill. Centers exhibited similarities in thresholds for ICU admission and management of severe hepatic encephalopathy. Over 80% of centers administered n-acetyl-cysteine to ICU patients for non-paracetamolrelated ALF. There was significant divergence in the use of prophylactic antibiotics and anti-fungals,lactulose,nutritional support and imaging investigations in admitted patients and in the monitoring and treatment of intra-cranial pressure(ICP). ICP monitoring was employed in 12 centers,with the most common indications being papilledema and renal failure. Most patients listed for transplantation underwent surgery within an average waiting time of 1-2 d. Over a period of 3 mo clinical data from 85 ALF patients was collected. Gene variants of the phosphatidylcholine synthesis pathway do not contribute to RdS in the Chinese population

Background To determine population-based prevalence and disease contribution of phosphatidylcholine synthetic pathway-associated gene variants in a native southern Chinese cohort. Methods We used bloodspots from 2010 that were obtained from the Guangxi Neonatal Screening Center in Nannning China and included the Han (n = 443) and Zhuang (n = 313) ethnic groups. We sequenced the exons of cholinephosphate cytidylyltransferase (PCYT1B) lysophospholipid acyltransferase 1 (LPCAT1), and cholinephosphotransferase (CHPT1) genes, and analyzed both rare and common exonic variants. Results We obtained five mutations (G199d, A299V, G434C, Y490C, L312S) with eight alleles in the three candidate genes. The collapsed minor allele frequency for candidate genes was not significantly different between the Han and Zhuang popula-tions (0.0045 vs. 0.0064, respectively,P = 0.725). The combined Han and Zhuang pool collapsed carrier frequency of rare mutation allele was found to be 1.06%, which is much higher than previously reported for the Missouri population (0.1%). Further, we detected six exonic common variants (three in LPCAT1 and three in CHPT1), with three non-synonymous vari-ants (F162S, F341L, M427K) among them. Two of the non-synonymous exonic variants (F341L, M427K) were not found in CHB;F341L was also not previously reported in exome sequencing project. Conclusions The population-based frequency of mutations in the phosphatidylcholine synthesis pathway-associated genes PCYT1B LPCAT1,CHPT1 is low in southern Chinese newborns and there is no evidence of contribution to population-based disease burden of respiratory distress syndrome. As a population-based study of rare mutations and common variants, this work is valuable in directing future research.

An Assessment of the Potential Use of Forest Residues for the Production of Bio-Oils in the Urban-Rural Interface of Louisiana

Louisiana is endowed with forest resources. Forest wastes generated after thinning, land clearing, and logging operations, such as wood debris, tree trimmings, barks, sawdust, wood chips, and black liquor, among others, can serve as potential fuels for energy production in Louisiana. This paper aims to evaluate the potential annual volumes of forest wastes established on detailed and existing data on the forest structure in the rural-urban interface of Louisiana. It also demonstrates the state’s prospects of utilizing forest wastes to produce bio-oils. The data specific to the study was deduced from secondary data sources to obtain the annual average total residue production in Louisiana and estimate the number of logging residues available for procurement for bioenergy production. The total biomass production per year was modeled versus years by polynomial regression curve fitting using Microsoft Excel. Results of the model show that the cumulative annual total biomass production for 2025 and 2030 in Louisiana is projected to be 80000000 Bone dry Ton (BdT) and 16000000 (BdT) respectively. The findings of the study depict that Louisiana has a massive biomass supply from forest wastes for bioenergy production. Thus, the potential for Louisiana to become an influential player in the production of bio-based products from forest residues is evident. The author recommends that future research can use Geographic Information Systems (GIS) to create maps displaying the potential locations and utilization centers of forest wastes for bioenergy production in the state.

Gait, posture and transfer assessment among elderly practitioners and non-practitioners of Tai Chi Chuan

Falls may be devastating events among elderly people. Tai Chi Chuan is a type of moderate sports exercise that demonstrates improvements in balance, gait and posture. The aim here was to assess the influence of this method on the balance, gait and posture of healthy active elderly people. A controlled cross-sectional study was conducted on 76 volunteers, divided into two groups: 51 volunteers Tai Chi Chuan group mean age 76.8 years and 25 volunteers control group mean age 70.3 years. Both groups underwent the Modified Clinical Test on Sensory Interaction for Balance (mCTSIB), Walking Test (WT), Sit-to-Stand Transfer Test (SSTT) and Unipedal Assessment Test (UT), using the Neurocom Balance Master system. Result: In conditions Static balance tests: the mCTSIB-Tai-Chi Group had a lower sway velocity (°/s) on a firm surface with eyes open and medio-lateral direction, Foam Surface with eyes open medio-lateral and anteroposterior direction. Unilateral Stance Tai-Chi Group had a lower mean sway velocity (°/s) with open eyes (right and left), but with eyes closed. The Control Group showed a lower sway velocity. Under dynamic Balance: Walk Across Test Tai-Chi Group presented a walking speed greater than the Control Group. In Sit to Stand Transfer Test, Tai-Chi Group showed a better index rising to lift up and lower oscillation in the final standing position. Conclusion: The Tai Chi Group was shown to be a protection factor for preserving and maintaining the static and dynamic positions of posture and gait.
