Reducing environmental pollution is a critical goal in global environmental economics and economic *** European Union(EU)faces environmental challenges due to its development *** we present a comprehensive approach to assess the impact of carbon dioxide(CO_(2))emissions,energy consumption(EC),population structure(pOp),economy(GDp),and policies on the environment within the EU using the environmental Kuznets curve(EKC).Our research reveals that between 1990 and 2019,the EU-27 experienced an increase of t1.18 million tonnes of oil equivalent(Mtoe)per year in energy consumption(pper year(ppirical EKC analysis shows an inverted-U shaped relationship between GDp and CO_(2)emissions in the ***,a 1%increase in GDp results in a 0.705%increase in carbon emission,while a 1%increase in GDp2 leads to a 0.062%reduction in environmental pollution in the long run(ppment within the EU has reached a stage where economic growth positively impacts the ***,this study provides insights into the effectiveness of environmental policies in mitigating degradation and promoting green growth in the EU 27 countries.

In patients with inflammatory bowel diseases (IBD) the prevalence of thrombosis is 6.2%, the average incidence of thromboembolism (TE) is 3.6 times higher compared to normal population. The TE is a common extraintestinal complication of IBD, squarely associated with the IBD activity. The application of antico-agulant and thrombolytic therapy in severe IBD is an unresolved issue. Herein we report the first case in literature of an active IBD patient with an upper limb acute arterial occlusion and successful catheter-directed thrombolysis (CDT). A 46-year-old male patient is reported who had Crohn's disease for 10 years. His right hand suddenly became cold and painful. Angiography proved acute occlusion of the brachial and radial artery. Vascular surgery intervention was not applicable. Endoscopy showed extended, severe inflammation of the colon. Despite the severe endoscopic findings, frequent bloody stools and moderate anaemia, CDT with recombinant tissue plasminogen activator was performed. The control angiography proved improvement, the radial artery pulse appeared. No bleeding complication was observed. This case supports that CDT-after careful estimation of the bleeding risk-can be effective and safe in patients with severe or life-threatening TE and active IBD.

AIM： To study molecular and morphological changes in lens epithelial cells following femtosecond laser-assisted and manually performed continuous curvilinear capsulotomy（CCC） in order to get information about these methods regarding their potential role in the induction of development of secondary cataract. METHODS： Anterior lens capsules（ALC） were removed from 40 patients with age-related cataract by manual CCC and by femtosecond laser-assisted capsulotomy（FLAC）. Samples removed by manual CCC were assorted in group 1, FLAC samples were classified in group 2. Morphology of lens epithelial cells was examined with light and electron microscopes. Following capsulotomy, expressions of p53, Bcl-2 and cyclin D1 genes were analyzed with reverse transcriptase polymerase chain reaction. Immunohistochemistry was used to detect the pro-apoptotic p53 in the epithelial cells. RESULTS： Light and electron microscopic examination showed that ALC of group 1 contained more degenerating cells following manual CCC than after FLAC. The expression level of p53 was higher after manual than laser-assisted surgery. Immunocytochemistry indicated significantly higher number of cells containing p53 protein in the manual CCC group than following FLAC. Bcl-2 and cyclin D1 gene expression levels were slightly lower following manual CCC than after FLAC, but the difference was not significant. CONCLUSION： Manually removed ALC shows slightly, but not significantly larger damage due to the mechanical stretching and pulling of the capsule than those removed using FLAC.

Acute pancreatitis (Ap) is a serious inflammatory disease with rising incidence both in the adult and pediatric populations. It has been shown that mitochondrial injury and energy depletion are the earliest intracellular events in the early phase of Ap. Moreover, it has been revealed that restoration of intracellular ATp level restores cellular functions and defends the cells from death. We have recently shown in a systematic review and meta-analysis that early enteral feeding is beneficial in adults; however, no reviews are available concerning the effect of early enteral feeding in pediatric Ap. In this minireview, our aim was to systematically analyse the literature on the treatmentof acute pediatric pancreatitis. The preferred reporting items for systematic review(pRISMA-p) were followed, and the question was drafted based on participants, intervention, comparison and outcomes: p: patients under the age of twenty-one suffering from acute pancreatitis; I: early enteral nutrition (per os and nasogastric- or nasojejunal tube started within 48 h); C: nil per os therapy; O: length of hospitalization, need for treatment at an intensive care unit, development of severe Ap, lung injury (including lung oedema and pleural effusion), white blood cell count and pain score on admission. Altogether, 632 articles (pub Med: 131; EMBASE: 501) were found. After detailed screening of eligible papers, five of them met inclusion criteria. Only retrospective clinical trials were available. Due to insufficient information from the authors, it was only possible to address length of hospitalization as an outcome of the study. Our mini-meta-analysis showed that early enteral nutrition significantly(SD = 0.806, p = 0.034) decreases length of hospitalization compared with nil per os diet in acute pediatric pancreatitis. AIM： To determine the fasting plasma carnitine ester in patients with celiac disease. METHODS： We determined the fasting plasma carnitine ester profile using ESI triple quadrupol mass spectrometry in 33 adult patients with biopsy-confirmed maturity onset celiac disease maintained on long term gluten free diet. RESULTS： The level of free camibine did not differ as the celiac disease patients were compared with the healthy controls, whereas the acetylcarnitine level was markedly reduced （4.703± 0.205 vs 10.227 ± 0.368 nmol/mL, p〈0.01）. The level of propionylcarnitine was 61.5%, butyrylcarnitine 56.9%, hexanoylcarnitine 75%, octanoylcarnitine 71.1%, octenoylcarnitine 52.1%, decanoylcarnitine 73.1%, cecenoylcarnitine 58.3%, lauroylcarnitine 61.5%, miristoylcarnitine 66.7%, miristoleylcarnitine 62.5% and oleylcarnitine 81.1% in the celiac disease patients compared to the control values, respectively （p〈0.01）. CONCLUSION： The marked decrease of circulating acetylcarnitine with 50-80 % decrease of 11 other carnitine esters shows that the carnitine ester metabolism can be influenced even in clinically asymptomatic and well being adult celiac disease patients, and gluten withdrawal alone does not necessarily normalize all elements of the disturbed carnitine homeostasis.

AIM To understand the influence of chronic kidney disease(CKD) on mortality, need for transfusion and rebleeding in gastrointestinal(GI) bleeding *** A systematic search was conducted in three databases for studies on GI bleeding patients with CKD or endstage renal disease(ESRD) with data on outcomes of mortality, transfusion requirement, rebleeding rate and length of hospitalization(LOH). Calculations were performed with Comprehensive Meta-Analysis software using the random effects model. Heterogeneity was tested by using Cochrane's Q and I2 statistics. Mean difference(MD) and OR(odds ratio) were ***1063 articles(EMBASE: 589; pubM ed: 459; Cochrane: 15) were found in total. 5 retrospective articles and 1 prospective study were available for analysis. These 6 articles contained data on 406035 patients, of whom 51315 had impaired renal function. The analysis showed a higher mortality in the CKD group(OR = 1.786, 95%CI: 1.689-1.888, p p(OR = 2.530, 95%CI: 1.386-4.616, p = 0.002), and a rebleeding rate(OR = 2.510, 95%CI: 1.521-4.144, p patients with impaired renal function. CKD patients required more unit red blood cell transfusion(MD = 1.863, 95%CI: 0.812-2.915, p pent more time in hospital(MD = 13.245, 95%CI: 6.886-19.623, p patients. prospective patient registries and observational clinical trials are crucially needed.

BACKGROUND Obesity rates have increased sharply in recent decades. As there is a growing number of cases in which acute pancreatitis(Ap) is accompanied by obesity, we found it clinically relevant to investigate how body-mass index(BMI) affects the outcome of the *** To quantify the association between subgroups of BMI and the severity and mortality of *** A meta-analysis was performed using the preferred Reporting Items for Systematic Review and Meta-Analysis(pRISMA) protocols. Three databases(pubMed, EMBASE and the Cochrane Library) were searched for articles containing data on BMI, disease severity and mortality rate for Ap. Englishlanguage studies from inception to 19 June 2017 were checked against our predetermined eligibility criteria. The included articles reported all Ap cases with no restriction on the etiology of the disease. Only studies that classified Ap cases according to the Atlanta Criteria were involved in the severity analyses. Odds ratios(OR) and mean differences(MD) were pooled using the random effects model with the DerSimonian-Laird estimation and displayed on forest plots. The meta-analysis was registered in pROSpERO under number *** A total of 19 articles were included in our meta-analysis containing data on 9997 patients. As regards severity, a subgroup analysis showed a direct association between Ap severity and BMI.
