Intravitreal injection of conbercept for diabetic macular edema complicated with diabetic nephropathy

AIM:To observe the therapeutic effect of conbercept on diabetic macular edema(DME)complicated with diabetic nephropathy(DN).METHODS:In this retrospective study,54 patients(54 eyes)that diagnosed as DME from January 2017 to October 2021 were *** patients were divided into two groups:DME patients with DN(25 eyes),and DME patients without DN(29 eyes).General conditions were collected before treatment,laboratory tests include fasting blood glucose,HbA1c,microalbumin/creatinine,serum *** coherence tomography(OcT)was used to check the ellipsoidal zone(EZ)and external limiting membrane(ELM)*** macular thickness(cMT),best corrected visual acuity(BcVA),and retinal hyperreflective foci(HF)as well as numbers of injections were ***:There were significant differences between fasting blood glucose,HbA1c,serum creatinine,urinary microalbumin/creatinine,and estimated glomerular filtration rate(eGFR)between the two groups(all Pcontinuity in the DME+DN group was worse than that in the DME group(PcVA(logMAR)in the DME group was significantly better than that in the DME+DN group at the same time points during treatment(all PcMT and HF values were significantly higher in the DME+DN group than that in the DME group at the all time points(all Pcantly decreased in both groups with time during *** 6mo after treatment,the mean number of injections in the DME+DN and DME group was 4.84±0.94 and 3.79±0.86,***:conbercept has a significant effect in short-term treatment of DME patients with or without DN,and can significantly ameliorate BcVA,cMT and the number of HF,treatment efficacy of DME patients without DN is better than that of DME patients with DN.

Acute pancreatitis(AP)is a potentially fatal condition with no targeted treatment *** inhibiting xanthine oxidase(XO)in the treatment of AP has been studied in several experimental models and clinical trials,whether XO is a target of AP and what its the main mechanism of action is remains ***,we aimed to re-evaluate whether XO is a target aggravating AP other than merely generating reactive oxygen species that trigger *** first revealed that XO expression and enzyme activity were significantly elevated in the serum and pancreas of necrotizing AP *** also found that allopurinol and febuxostat,as purine-like and non-purine XO inhibitors,respectively,exhibited protective effects against pancreatic acinar cell death in vitro and pancreatic damage in vivo at different doses and treatment time ***,we observed that conditional Xdh overexpression aggravated pancreatic necrosis and *** mechanism analysis showed that XO inhibition restored the hypoxia-inducible factor 1-alpha(HIF-1α)-regulated lactate dehydrogenase A(LDHA)and NOD-like receptor family pyrin domain containing 3(NLRP3)signaling pathways and reduced the enrichment of^(13)c_(6)-glucose to^(13)c_(3)-***,we observed that clinical circulatory XO activity was significantly elevated in severe cases and correlated with c-reactive protein levels,while pancreatic XO and urate were also increased in severe AP *** results together indicated that proper inhibition of XO might be a promising therapeutic strategy for alleviating pancreatic necrosis and preventing progression of severe AP by downregulating HIF-1α-mediated LDHA and NLRP3 signaling pathways.

characteristics and mechanism of Ni2+ and cd2+ adsorption by recovered perlite from agar extraction residue

Ni2+and cd2+in wastewater accumulated through the ecological chain and could jeopardize human health. Adsorption of Ni2+and cd2+from wastewater using recovered perlite was an important way to solve the problem of resource utilization of solid waste from agar production. Our previous study confirmed that recovered perlite from agar extraction residue had better pore size and specific surface area than commercial perlite. However, the adsorption efficiency and adsorption mechanism of recovered perlite were the main factors limiting its adsorption application. The adsorption process of Ni2+and cd2+by recovered perlite in aqueous solution was described by the pseudo-second-order kinetic equation, and the relevant adsorption mechanism was mainly chemisorption. compared with commercial perlite, the adsorption removal rate of Ni2+and cd2+by enzymatic recovered perlite could reach 92.9% and 89.2%, respectively, and were improved by 12.63% and 13.03%. Langmuir isothermal adsorption model could better describe the isothermal adsorption process of recovered perlite on heavy metal Ni2+and cd2+, and the relevant adsorption mechanism was mainly monolayer adsorption. The X-ray photoelectron spectroscopy（XPS） results indicated that the decrease of Si—O Si2+hydroxyl coordination bond and the increase of c—Si bond might make the binding effect of recovered perlite with heavy metals stronger. The competitive adsorption of Ni2+and cd2+by recovered perlite was still dominated by chemisorption and monolayer adsorption. This study was expected to provide a theoretical basis and technical support for the removal of Ni2+and cd2+from wastewater using recovered perlite from seaweed residue.

Analysis of the correlation Between Visceral Fat Area and Insulin Resistance in Patients with Type 2 Diabetes and Abdominal Obesity

Objective: To analyze the correlation between visceral fat area and insulin resistance index (HOMA-IR) in patients with type 2 diabetes mellitus (T2DM) and abdominal obesity and to provide a reference for screening and related research of such patients. Methods: Two hundred patients with T2DM admitted to Guandu People’s Hospital of Kunming were included. The study was carried out from October 2022 to December 2023. The patients were divided into three groups according to different abdominal visceral fat areas (VFA): Group A (n = 65) was less than 75cm2, Group B (n = 75) was 75-100 cm2, and Group c (n = 60) was greater than 100 cm2. The subjects in the three groups were all tested for glycated hemoglobin (HbA1c), fasting insulin (FINS), and fasting blood glucose (FPG). Height and weight were measured to calculate body mass index (BMI). The HOMA-IR and TYG (fasting triglyceride and glycemic index) were also calculated. changes in the BMI, VFA, HOMA-IR, and TYG levels were observed in the three groups. Results: The VFA, BMI, HbA1c, FPG, FINS, HOMA-IR, and TYG of the patients all increased, with a more significant increase in the BMI, FINS, HOMA-IR, and TYG levels (P ceral fat area (VFA) as the dependent variable. The results showed that VFA was closely related to BMI, FINS, HOMA-IR, and TYG. conclusion: Early reduction of VFA to reduce insulin resistance may be a better treatment and effective method for T2DM, providing powerful measures and new strategies for effective blood sugar control and early prevention in the treatment of metabolic diseases.

Dammarane-type triterpenoids from Gynostemma longipes and their protective activities on hypoxia-induced injury in Pc12 cells

Sixteen new dammarane-type triterpenoid saponins(1–16) featuring diverse structural variations in the side chain at c-17, along with twenty-one known analogues(17–37), have been isolated from the rhizomes of Gynostemma longipes c. Y. Wu, a plant renowned for its medicinal and edible properties. The structural elucidation of these compounds was accomplished through comprehensive analyses of 1D and 2D NMR and HRMS spectroscopic data, supplemented by comparison with previously reported data. Subsequent assays on the isolates for their protective effects against hypoxia-induced damage in pheochromocytoma cells(Pc12 cells) revealed that nine saponins exhibited significant anti-hypoxic activities. Further investigation into the anti-hypoxia mechanisms of the representative saponins demonstrated that compounds 22 and 36 markedly reduced the levels of hypoxia-induced apoptosis. Additionally, these compounds were found to decrease the release of lactate dehydrogenase(LDH) and malondialdehyde(MDA), while increasing the activity of superoxide dismutase(SOD), thereby indicating that the saponins could mitigate hypoxia-induced injuries by ameliorating apoptosis and oxidative stress. These findings offer substantial evidence for the future utilization and development of G. longipes, identifying dammarane-type triterpenoid saponins as its active anti-hypoxic constituents.

针对已有目标检测算法在隧道衬砌病害检测中全局信息提取不充分、检测精度低的问题,提出隧道衬砌表观病害检测算法TDD-YOLO.该算法以YOLOv7框架为基础,采用MobileViT作为主干特征提取网络,提高网络全局信息和局部信息提取能力;在特征金字塔网络的上采样和下采样后增加coordinate attention (cA)注意力模块,突出病害的特征信息,去除背景信息的干扰;提出卷积模块TP Block,在计算量较小的情况下进一步提高网络的特征提取能力.为了验证所提出算法的有效性,选用SSD、 Faster-RcNN、 EfficientDet、 YOLOv5、 YOLOv7这5种算法进行对比分析.实验结果表明,TDD-YOLO算法的F1为77.43%,相对5种对比算法,分别提高了15.58%、17.36%、 12.19%、 6.32%、 6.14%;mAP为77.52%,相对5种对比算法,分别提高了15.20%、 14.24%、 9.44%、 7.44%、6.39%. TDD-YOLO算法病害识别精度最高,综合性能最优,适用于实际隧道工程的病害检测任务.

Precise control of the local environment and electronic state of the guest is an important method of controlling catalytic activity and reaction *** this paper,guest Pd NPs were introduced into a series of host UiO-67 MOFs with different functional ligands and metal nodes,the microenvironment and local electronic structure of Pd is modulated by introducing bipyridine groups and changing metal nodes(ce6O6 or Zr6O6).The bipyridine groups not only promoted the dispersion Pd NPs,but also facilitated electron transfer between Pd and UiO-67 MOFs through the formation of Pd-N *** with Zr6 clusters,the tunability and orbital hybridisation of the 4f electronic structure in the ce6 clusters modulate the electronic structure of Pd through the construction of the ce-O-Pd *** optimal catalyst Pd/UiO-67(ce)-bpy presented excellent low-temperature activity towards dicyclopentadiene hydrogenation with a conversion of>99%and a selectivity of>99%(50℃,10 bar).The results show that the synergy of ce-O-Pd and Pd-N promotes the formation of active Pdδ+,which not only enhances the adsorption of H2 and electron-rich c=c bonds,but also contributes to the reduction of proton migration distance and improves proton utilization *** results provide valuable insights for investigating the regulatory role of the host MOFs,the nature of host-guest interactions,and their correlation with catalytic performance.

先前学习认定起源于美国。作为一种终身学习激励制度得到了广泛应用,美国拥有丰富的先前学习认定发展经验。100多年来,美国先前学习认定制度经历了起始与奠基、丰富与扩展、反思与复兴三个阶段的变迁。同时,在美国先前学习认定制度变迁的动力机制中,社会变革是变迁的基础动力,教育理念转变是变迁的内生动力,推动经济发展是直接动力。当前还存在着先前学习认定意识缺乏、先前学习与后续项目匹配度不够、财政支持和相关激励缺乏以及教育不平等加剧等问题。借鉴美国经验,我国应首先改变传统观念,激发先前学习认定的意识;关注弱势群体,提供多种认定方法;同时建立一致的政策,定期监测并响应需求。
