Usefulness of applying lidocaine in esophagogastroduodenoscopy performed under sedation with propofol

AIM:To determine whether topical lidocaine benefits esophagogastroduoduenoscopy(EGD) by decreasing propofol dose necessary for sedation or procedurerelated ***:The study was designed as a prospective,single centre,double blind,randomised clinical trial and was conducted in 2012 between January and May(NCT01489891).Consecutive patients undergoing EGD were randomly assigned to receive supplemental topical lidocaine(l;50 mg in an excipient solution which was applied as a spray to the oropharynx) or placebo(P;taste excipients solution without active substance,similarly delivered) prior to the standard propofol sedation *** propofol was administered as a bolus intravenous(iv) dose,with patients in the l and P groups receiving initial doses based on the patient’s American Society of Anaesthesiologists(ASA) classification(ASAⅠ-Ⅱ:0.50-0.60 mg/kg;ASA Ⅲ-Ⅳ:0.25-0.35 mg/kg),followed by 10-20 mg iv dose every 30-60 s at the anaesthetist’s *** signs,anthropometric measurements,amount of propofol administered,sedation level reached,examination time,and the subjective assessments of the endoscopist’s and anaesthetist’s satisfaction(based upon a four point likert scale) were *** statistical tests were performed by the Stata statistical software suite(Release 11,2009;StataCorp,lP,College Station,TX,United States).RESUlTS:No significant differences were found between the groups treated with lidocaine or placebo in terms of total propofol dose(310.7 ± 139.2 mg/kg per minute vs 280.1 ± 87.7 mg/kg per minute,P = 0.15) or intraprocedural propofol dose(135.3 ± 151.7 mg/kg per minute vs 122.7 ± 96.5 mg/kg per minute,P = 0.58).Only when the l and P groups were analysed with the particular subgroups of female,< 65-year-old,and lower anaesthetic risk level(ASA Ⅰ-Ⅱ) was a statistically significant difference found(l:336.5 ± 141.2 mg/kg per minute vs P:284.6 ± 91.2 mg/kg per minute,P = 0.03) for greater total propofol requirements).The total i

We present a case of keratitis caused by Stenotrophomonas maltophilia in a therapeutic contact lens user with trichiasis and symblepharon. This keratitis was initially diagnosed as caused by Achromobacter xylosoxidans, but the strain was sent for species confirmation and the isolate was finally identified as S. maltophilia by means of 16S rDNA sequencing. The patient rapidly improved on administration of fortified ceftazidime. Physicians should be aware that the definitive identification of the pathogenic agent and prolonged antimicrobial treatment according to culture sensitivities in keratitis are mandatory as treatment success depends greatly on them.

The Effect of Weather and Agronomic Practice on Deoxynivalenol Mycotoxin in Durum Wheat

Deoxynivalenol （DON） produced by Fusarium graminearum and F. culmorum in durum wheat （Triticum turgidum l. var. durum） is an important food safety issue for the cereal chain. A number of agronomic practices are known to affect the infection and mycotoxin production by these Fusarium species in cereal grains. A field study was conducted to compare the natural occurrence of DON in durum wheat grown under conventional （n = 67 samples） and organic （n = 50） farming conditions during two consecutive crop years. DON mean levels were 95 and 194 μg/kg in conventional and organic durum wheat, respectively. The incidence and levels of DON were higher in the second crop year due to higher rainfall （〉 100 mm） during flowering. Overall, organically managed farming system may provide lsome reduction in DON levels in durum wheat, which could be attributable to the lower intensity of cultivation in addition to the different system of cropping rotation. However, favourable weather conditions are likely to be more important for DON contamination than crop system.

Maximum Entropy Ecological Niche Prediction of the Current Potential Geographical Distribution of EimeriaSpecies of Cattle, Sheep and Goats in Mexico

Coccidiosis is a gastrointestinal disease caused by parasites of the genus Eimeria. To produce the ecological niche model for the geographic distribution of Eimeria species, the maximum entropy algorithm (MaxEnt) was used and 19 bioclimatic variables with a spatial resolution of 30 arc-seconds (approximately 1 km2) were downloaded from the World Climate Database. These were reduced to BIO2, BIO3, BIO4, BIO7 and BIO15 for each species after examining cross-correlations among them to account multicollinearity. A jackknife analysis was included to assess the contribution of five bioclimatic variables and the fit of the model was evaluated with the area under receiver operating characteristic curve (AUC). Under a current climate scenario, the jackknife evaluation of the MaxEnt model showed that BIO4 (temperature seasonality) made the greatest contribution to the distribution model for 22 Eimeria species;whereas BIO7 (temperature annual range) was the most important factor that contributes to the distribution model of 10 species. The habitat suitability model based on the maximum entropy theory was supported by AUC values higher than 0.9 and predicted that the suitable habitats for different species of Eimeria are present in southern, eastern and western areas of Mexico. Our study may support future studies exploring factors that constrain the distribution of Eimeria as well as strategies aimed at reducing the disease prevalence.

Parameters Influencing the Optimization Process in Airborne Particles PM10 Using a Neuro-Fuzzy Algorithm Optimized with Bacteria Foraging (BFOA)

The airborne pollutants monitoring is an overriding task for humanity given that poor quality of air is a matter of public health, causing issues mainly in the respiratory and cardiovascular systems, specifically the PM10 particle. In this contribution is generated a base model with an Adaptive Neuro Fuzzy Inference System (ANFIS) which is later optimized, using a swarm intelligence technique, named Bacteria Foraging Optimization Algorithm (BFOA). Several experiments were carried with BFOA parameters, tuning them to achieve the best configuration of said parameters that produce an optimized model, demonstrating that way, how the optimization process is influenced by choice of the parameters.

Isolated Gross Hematuria or Associated to Acute Retention of Urine as a Sign of Urologic Malignant Pathology

The causes oflGH （Isolated gross hematuria）, as the only symptom or associated with ARU （acute retention of urine） caused by clots in the urinary tract in patients who do not have a previous diagnosis of urological malignant pathology and to study the immediate mortality in patients with that sign are described. A descriptive observational study of the etiologies of patients shows the above mentioned symptomatology from the 1 st of January to the 30th of June of 2008 in the Emergency Department which received follow-up treatment up to December 2010. It also describes the mortality before the definitive diagnosis and the days between receiving attention in the Emergencies Department and the diagnostic tests. One hundred and four cases with the criteria described above were evaluated. Of these, 20.0% turned out to have bladder, prostate or kidney cancer; the bladder being the most affected organ. Of all the pathology, benign and malignant, the most frequent one is the benign hypertrophy of the prostate （49.8% of the total）. Of the total, 4.8% of the patients died in the following weeks before finishing the diagnostic study. Isolated gross hematuria is a clinical symptom closely related to urological malignant pathology and it does not have an insignificant mortality rate in a short period of time. For these reasons it is necessary to accelerate the diagnostic tests as soon as the patient presents the symptom.

luminescence Characteristics of laAlO3:Eu3+Obtained by Modified Pechini’s Method

The aim of the present work was to investigate the luminescent properties of europium-doped lanthanum aluminate (laAlO3) powder prepared by Modified Pechini (MP) method. Samples were prepared at different Eu3+ concentrations and calcined at 1600°C. Structural and morphology characterizations were obtained by X-ray diffraction (XRD), scanning electron microscopy (SEM), and transmission electron microscope (TEM). luminescent properties were analyzed by photoluminescence (Pl) and thermoluminescent phenomena. The XRD pattern shows pure lanthanum aluminate oxide laAlO3 with a rhombohedral structure. Photoluminescence studies of the powders was related to 4f → 4f(5D0 → 7Fi) (i = 1, 2, 3, 4) transitions of Eu3+ ion, the most intense emission of Eu3+ in laAlO3 was registered for the transition 5D0 → 7F1, for 5% of Eu3+. The recorded Tl glow curves of excited samples showed a main Tl peak, the main peak appeared its maximum as a function of excitation energy. The investigation demonstrates that polycrystalline laAlO3:Eu3+ powders calcined at 1600°C have interesting luminescent characteristics.

In the current socio-economic scenario characterized by a growing shortage of resources and progressive budget constraints, the need to better coordinate processes in health institutions appears as a relevant aspect to ensure the future sustainability of system. In this sense, Relational Coordination(RC) provides a valuable opportunity for the reconfiguration of clinical guidelines concerning isolated single-level considerations. In this research the RC model has been applied to explain best results in the process of diagnosing and offering clinical treatments for lung cancer. lung cancer presents the higher rates of tumor's mortality worldwide. Through unstructured and informal interviews with clinicians at both levels(Primary/Specialist Care), a diagnosis of the situation in relation to joint management of lung cancer is provided. Solutions of continuity in terms of coordination are explained due to the observation of lack of effective knowledge transfer between the two levels. It is this disconnection which justifies the introduction of a modified model of RC for the study and implementation of transfer relations between the knowledge holders, in order to structure consolidated and cooperative evidence-based models that lead to a substantial shortening in the response times with a marked outcomes improvement. To our knowledge, the application of this model to a Public Health problem bringing together both levels of care, hasn't been made till now.
