Beta-blocker therapy in elderly patients with renal dysfunction and heart failure

OBJECTIVE To assess the role of beta-blockers(BB)in patients with chronic kidney disease(CKD)aged≥75 *** AND RESULTS From January 2008 to July 2014,we included 390 consecutive patients≥75 years of age with ejection fraction≤35%and glomerular filtration rate(GFR)≤60 m L/min per 1.73 m^*** analyzed the relationship between treatment with BB and mortality or cardiovascular *** mean age of our population was 82.6±4.1 *** ejection fraction was 27.9%±6.5%.GFR was 60-45 m L/min per 1.73 m^2 in 50.3%of patients,45-30 m L/min per 1.73 m^2 in 37.4%,and<30 m L/min per 1.73 m^2 in 12.3%.At the conclusion of follow-up,67.4%of patients were receiving *** median follow-up was28.04(IR:19.41-36.67)*** the study period,211 patients(54.1%)died and 257(65.9%)had a major cardiovascular event(death or hospitalization for heart failure).BB use was significantly associated with a reduced risk of death(HR=0.51,95%CI:0.35-0.74;P<0.001).Patients receiving BB consistently showed a reduced risk of death across the different stages of CKD:stage IIIa(GFR=30-45 m L/min per 1.73 m^2;HR=0.47,95%CI:0.26-0.86,P<0.0001),stage IIIb(GFR 30-45 m L/min per 1.73 m^2;HR=0.55,95%CI:0.26-1.06,P=0.007),and stages IV and V(GFR<30 m L/min per 1.73 m~2;HR=0.29,95%CI:0.11-0.76;P=0.047).CONCLUSIONS The use of BB in elderly patients with HFr EF and renal impairment was associated with a better *** of BB should be encouraged when possible.

Objective: We describe patients with MCS, the evolution of the Quick Environmental Exposure and Sensitivity Inventory (QEESI) score with a special focus on people whose fillings were removed. Methods: We have conducted a retrospective longitudinal cohort study in patients diagnosed with MCS and attended in the outpatient Internal Medicine department of the University Hospital of San juan de Alicante, from January 1, 2008 to January 1, 2021. Sociodemographic, clinical, QEESI and treatment-related variables were collected. We performed descriptive and inferential analyses. Mixed linear models were used to analyze the QEESI. Calculations were carried out with an α error of 5%. Results: Thirty-three patients were included (72.7% women, mean age 56.2). MCS was mainly triggered by mercury (N = 20) and food intolerance (N = 22). The mean interval from symptoms onset was 120 months (SD 81.6). 114 QEESIs were analyzed: 82 (N = 17 without amalgams) and 32 (N = 16 with amalgams). In patients without amalgams, severity scores increased across all subscales except the masking index (vs. with amalgams). Mean scores for the group without amalgams (vs. with amalgams) were: chemical intolerance, 62.8 points (vs. 63.4 and 46.7);other intolerances, 52.7 points (vs. 62.8 and 50.3);symptom severity, 63.2 (vs. 76.7 and 63.3);masking index, 3.9 (vs. 3.2 and 2.8);and life impacts, 63.1 (vs. 58.4 and 49.8). Conclusion: The profile of patient with MCS is a middle-aged woman who is a frequent user of healthcare services, presents a long diagnostic delay and has borne a great personal, work and socioeconomic impact. The QEESI is useful for the clinical follow-up of patients, including the optimal treatment response in the case of amalgams. Clinical Significance: People affected by Multiple Chemical Sensitivity deserve the attention, understanding and help of health professionals and family members, to face an invisible illness for those who do not suffer from it. Implications of denitrification in the ecological status of an urban river using enzymatic activities in sediments as an indicator

A better understanding of the effects of a number of environmental factors on denitrification is vital for analyzing its role as nitrogen sink and providing deeper knowledge about the ecological status of a nitrate-rich ecosystem. Since few studies have addressed the occurrence and implications of denitrification in river sediments, and complexity of interactions among all these environmental factors makes comprehension of the process difficult, the potential of sediments from the Deba River to attenuate nitrate excess through denitrification was investigated. For this purpose, we adapted an in vitro method to measure activities of two enzymes contributing to the entire multiple-step nitrate reduction: Nitrate Reductase and Nitrite Reductase. The environmental features that influence both or single enzymatic activities were identified as oxygen availability,regulated directly by the moisture content or indirectly through the aerobic respiration,organic matter and nitrate content of sediments, and electrical conductivity and exchangeable sodium percentage of water. Additionally, our results showed that Nitrate Reductase catalyzes the principal limiting step of denitrification in sediments. Therefore,taking this enzymatic activity as an indicator, the southern part of the Deba River catchment presented low potential to denitrify but nitrate-limited sediments, whereas the middle and northern parts were characterized by high denitrification potential but nitraterich sediments. In general, this study on denitrifying enzymatic activities in sediments evaluates the suitability of the management of the effluents from wastewater treatment plants and municipal sewages to ensure a good ecological status of the Deba River.

Mushroom Production as an Alternative for Rural Development in a Forested Mountainous Area

Wild mushrooms are recognized as important non-wood forest products in mountainous ecosystems, but their real potential for generating rural economies has not been fully evaluated due to the difficulties in obtaining reliable productivity data, minimizing their true potential as contributor to rural economies. Mushroom yield models based on large data series from Pinus forest ecosystems in the region of Catalonia(Spain), combined with data from the Spanish National Forest Inventory allow us to estimate the potential mushroom productivity by forest ecosystems. The results of 24,500 tons/yr of mushrooms of which 16,300 tons are classified as edible and 7,900 tons are commonly marketed demonstrate the importance of mushroom productions in Catalonian pine forests, mostly located in mountainous areas where the development of agricultural activities is limited. Economic mushroom value is estimated at 48 million € for the edible mushroom and 32 million € for those corresponding to marketable yields, confirming the potential of this non-wood forest product. These production results and corresponding economic values provide a basis for the incorporation of wild mushrooms as significant non-wood forest products in the development of forest policies in mountainous areas.

Large quantities of natural aggregates(NA)are required in the manufacturing of asphalt mixtures for *** extraction of NA generates high environmental impacts that negatively affect environmental preservation and *** alternative materials obtained in construction and demolition processes in civil construction worked have been studies as possible replacements for NA,with the purpose of reducing these environmental *** foregoing study presents a state of knowledge review referring to the use of recycled concrete aggregates(RCA)when these are used in the manufacturing of asphalt mixtures(mainly hot-mix asphalt HMA type).Environmental aspects are presented,as well as possible benefits and limitations of using RCA as replacements for NA in asphalt ***,it is not possible to establish a behavior pattern,since the characteristics of the RCA are very heterogeneous and vary from the original source of the *** is highlighted from the literature that RCA has a wide potential for use in the manufacture of asphalt mixtures,since in most studies,the mixtures with these materials comply the quality requirements contemplated by the construction specifications,mainly in low-volume ***,based on the literature review,some recommendations and suggestions are presented for future research.
