Management of end-stage erectile dysfunction and stress urinary incontinence after radical prostatectomy by simultaneous dual implantation using a single trans-scrotal incision： surgical technique and outcomes

Stress urinary incontinence （SUi） and end-stage erectile dysfunction （ED） after radical prostatectomy （RP） can decrease a patient＇s quality of life （QoL）. We describe a surgical technique involving scrotal incision for simultaneous dual implantation of an artificial urinary sphincter （AUS） and an inflatable penile prosthesis （iPP）. Patients with moderate to severe SUi （〉3 pads per day） and end-stage ED following RP were selected for dual implantation. An upper transverse scrotal incision was made, followed by bulbar urethra dissection and AUS cuff placement. Through the same incision, the corpora cavernosa was exposed, and an iPP positioned. Followed by extraperitoneal reservoirs placement and pumps introduced in the scrotum. Short-term, intra- and post-operative complications; continence status and erectile function; and patient satisfaction and QoL were recorded. A total of 32 patients underwent dual implantation. Early AUS-related complications were： AUS reservoir migration and urethral erosion. One case of distal corporal extrusion occurred. No prosthetic infection was reported. Over 96% of patients were socially the continent （≤1 pad per day） and 〉95% had sufficient erections for intercourse. Limitations of the study were the small number of patients, the lack of the control group using a perineal approach for AUS placement and only a 12 months follow-up. iPP and AUS dual implantation using a single scrotal incision technique is a safe and effective option in patients with SUi and ED after RP. Further studies on larger numbers of patients are warranted.

Objective: To evaluate transperineal laser ablation (TPLA) with Echolaser® (Echolaser® TPLA, Elesta S.p.A., Calenzano, italy) as a treatment for benign prostatic hyperplasia (BPH) and prostate cancer (PCa) using the Delphi consensus ***: italian and international experts on BPH and PCa participated in a collaborative consensus project. During two rounds, they expressed their opinions on Echolaser® TPLA for the treatment of BPH and PCa answering online questionnaires on indications, methodology, and potential complications of this technology. Level of agreement or disagreement to reach consensus was set at 75%. if the consensus was not achieved, questions were modified after each round. A final round was performed during an online meeting, in which results were discussed and ***: Thirty-two out of forty invited experts participated and consensus was reached on all topics. Agreement was achieved on recommending Echolaser® TPLA as a treatment of BPH in patients with ample range of prostate volume, from 80 mL (80%), comorbidities (100%), antiplatelet or anticoagulant treatment (96%), indwelling catheter (77%), and strong will of preserving ejaculatory function (100%). Majority of respondents agreed that Echolaser® TPLA is a potential option for the treatment of localized PCa (78%) and recommended it for low-risk PCa (90%). During the final round, experts concluded that it can be used for intermediate-risk PCa and it should be proposed as an effective alternative to radical prostatectomy for patients with strong will of avoiding urinary incontinence and sexual dysfunction. Almost all participants agreed that the transperineal approach of this organ-sparing technique is safer than transrectal and transurethral approaches typical of other techniques (97% of agreement among experts). Work Ability index and Work Ability Score: A Comparation between both Scores in a Persistent COViD-19 Cohort

Aims: The present study aims to compare the assessment of work ability based on the use of the Work Ability index (WAi) with another questionnaire base only on the use of WAi’s first item, termed as the “Work Ability Score” (WAS). Study design: A cohort of 384 Spanish workers included in a Post COViD-19 condition or persistent COViD-19 multicenter research was utilized. Place and Duration of Study: This cohort was enlisted in four hospitals (Hospital Universitario 12 de Octubre, Madrid;Hospital Universitario Virgen Macarena, Sevilla, Andalucía;Hospital Universitario Gregorio Marañón, Madrid and Complejo Asistencial Universitario de salamanca, Castilla y León), since 2021 until 2022. Methodology: 384 Spanish workers (176 men and 208 women;aged 20 to 70 years) with Post COViD-19 condition or persistent COViD-19 were included. Descriptive analysis of primary scores was conducted. Given the non-normal distribution of data, the Mann-Whitney and Kruskal-Wallis tests were employed. Spearman and Kendall correlations were employed to assess the relationship between WAi and WAS, also used weighted Kappa to estimate the degree agreement between WAi and WAS. Logistic regression models were utilized to study determinants influencing WAi and WAS, categorized as poor or moderate. Results: WAi had an average score of 32.98 (SD = 10.28), whereas WAS had an average of 5.95 (SD = 2.77). Significant differences were observed in both WAi and WAS across the same variables. Strong and statistically significant correlations were evident between WAi and WAS (rs = 0.83, p iables used in the logistic regression model (gender, the sector employment, and previous chronic diseases) were statistically significant in both questionnaires. Conclusion: WAS questionnaire could be used as a tool for reliable assessment of work ability among Spanish workers with Post COViD-19 condition or Persistent CO-ViD-19.

in recent years, graphene field-effect-transistors(GFETs) have demonstrated an outstanding potential for terahertz(THz) photodetection due to their fast response and high-sensitivity. Such features are essential to enable emerging THz applications, including 6G wireless communications, quantum information, bioimaging and security. However, the overall performance of these photodetectors may be utterly compromised by the impact of internal resistances presented in the device, so-called access or parasitic resistances. in this work, we provide a detailed study of the influence of internal device resistances in the photoresponse of high-mobility dual-gate GFET detectors. Such dual-gate architectures allow us to fine tune(decrease) the internal resistance of the device by an order of magnitude and consequently demonstrate an improved responsivity and noise-equivalent-power values of the photodetector, respectively. Our results can be well understood by a series resistance model, as shown by the excellent agreement found between the experimental data and theoretical calculations. These findings are therefore relevant to understand and improve the overall performance of existing high-mobility graphene photodetectors.

Multi-EAP: Extended EAP for multi-estimate extraction for SMC-PHD filter

The ability to extract state-estimates for each target of a multi-target posterior, referred to as multi-estimate extraction（MEE）, is an essential requirement for a multi-target filter, whose key performance assessments are based on accuracy, computational efficiency and reliability. The probability hypothesis density（PHD） filter, implemented by the sequential Monte Carlo approach,affords a computationally efficient solution to general multi-target filtering for a time-varying number of targets, but leaves no clue for optimal MEE. in this paper, new data association techniques are proposed to distinguish real measurements of targets from clutter, as well as to associate particles with measurements. The MEE problem is then formulated as a family of parallel singleestimate extraction problems, facilitating the use of the classic expected a posteriori（EAP） estimator, namely the multi-EAP（MEAP） estimator. The resulting MEAP estimator is free of iterative clustering computation, computes quickly and yields accurate and reliable estimates. Typical simulation scenarios are employed to demonstrate the superiority of the MEAP estimator over existing methods in terms of faster processing speed and better estimation accuracy.

Collaborative learning via social computing

Educational innovation is a field that has been greatly enriched by using technology in its processes, resulting in a learning model where information comes from numerous sources and collaboration takes place among multiple students. One attractive challenge within educational innovation is the design of collaborative learning activities from the social computing point of view, where collaboration is not limited to student-to-student relationships, but includes student-to-machine interactions. At the same time, there is a great lack of tools that give support to the whole learning process and are not restricted to specific aspects of the educational task. in this paper, we present and evaluate context-aware framework for collaborative learning applications(CAFCLA) as a solution to these problems. CAFCLA is a flexible framework that covers the entire process of developing collaborative learning activities, taking advantage of contextual information and social interactions. its application in the experimental case study of a collaborative WebQuest within a museum has shown that, among other benefits, the use of social computing improves the learning process, fosters collaboration, enhances relationships, and increases engagement.
