Evaluation and treatment of internal impingement of the shoulder in overhead athletes

One of the most common pathologic processes seen in overhead throwing athletes is posterior shoulderpain resulting from internal impingement. "Internal impingement" is a term used to describe a constellation of symptoms which result from the greater tuberosity of the humerus and the articular surface of the rotator cuff abutting the posterosuperior glenoid when the shoulder is in an abducted and externally rotated position. The pathophysiology in symptomatic internal impingement is multifactorial,involving physiologic shoulder remodeling,posterior capsular contracture,and scapular dyskinesis. Throwers with internal impingement may complain of shoulder stiffness or the need for a prolonged warm-up,decline in performance,or posterior shoulder pain. On physical examination,patients will demonstrate limited internal rotation and posterior shoulder pain with a posterior impingement test. Common imaging findings include the classic "Bennett lesion" on radiographs,as well as articular-sided partial rotator cuff tears and concomitant SlAP lesions. Mainstays of treatment include intense non-operative management focusing on rest and stretching protocols focusing on the posterior capsule. Operative management is variable depending on the exact pathology,but largely consists of rotator cuff debridement. Outcomes of operative treatment have been mixed,therefore intense non-operative treatment should remain the focus of treatment.

Ulnar nerve(UN) injuries are a common complaint amongst overhead athletes. The UN is strained during periods of extreme valgus stress at the elbow, especially in the late-cocking and early acceleration phases of throwing. Although early ulnar collateral ligament(UCl) reconstruction techniques frequently included routine submuscular UN transposition, this is becoming less common with more modern techniques. We review the recent literature on the sites of UN compression, techniques to evaluate the UN nerve, and treatment of UN pathology in the overhead athlete. We also discuss our preferred techniques for selective decompression and anterior transposition of the UN when indicated. More recent studies support the use of UN transpositions only when there are specific preoperative symptoms. Athletes with isolated ulnar neuropathy are increasingly being treated with subcutaneous anterior transposition of the nerve rather than submuscular transposition. When ulnar neuropathy occurs with UCl insufficiency, adoption of the muscle-splitting approach for UCl reconstructions, as well as using a subcutaneous UN transposition have led to fewer postoperative complications and improved outcomes. Prudent handling of the UN in addition to appropriate surgical technique can lead to a high percentage of athletes who return to competitive sports following surgery for ulnar neuropathy.

Red cabbage washing with acidic electrolysed water:effects on microbial quality and physicochemical properties

The effects of acidic electrolysed water(AEW)as‘green’technology on the microbiological and physicochemical properties of fresh-cut red cabbages were ***-cut red cabbages and artificially inoculated red cabbages with Salmonella typhimurium DT104 were washed with distilled water(DW)and different available chlorine concentrations(ACC)of AEW for different *** treatments significantly reduced the populations of native aerobic bacteria,molds,and yeasts,and artificially inoculated *** DT104 compared with the DW-treated and untreated red cabbage *** effectiveness of AEW treatments was greatly enhanced with increasing ACC and treatment *** DT104 were not detected in the washing water that were collected after the red cabbages treated by *** surface colour,pH,and total phenolic contents did not significantly change when the red cabbages were washed with DW and AEW containing 100 mg/l available chlorine for 3 *** anthocyanin contents and antioxidant activities of red cabbage were significantly reduced by 18.5 per cent for cyanidin,22.1 per cent for pelargonidin,and 11.2 per cent for 2,2-diphenyl-1-picrylhydrazyl(DPPH)radical scavenging activity,however,the impacts on the nutritional benefits of red cabbage were considered as limited and *** optimal process condition of AEW for washing red cabbage was 100 mg/l ACC for 3 *** these conditions,most of the native microflora were inactivated,and artificially inoculated *** DT104 on the red cabbage were reduced by 40.2 per cent[3.67 log CFU/g(log10 colony-forming units per gram)]and with minimal losses of nutrients and antioxidant activity,as well as no requirement of decontamination treatment on the washing water after AEW treatment.

BACKGROUND The Black/African Ancestry(AA)population has a higher prevalence of type 2 diabetes mellitus(T2DM)and a higher incidence and mortality rate for colorectal cancer(CRC)than all other races in the United States.T2DM has been shown to increase adenoma risk in predominantly white/European ancestry(EA)populations,but the effect of T2DM on adenoma risk in Black/AA individuals is less *** hypothesize that T2DM has a significant effect on adenoma risk in a predominantly Black/AA *** To investigate the effect of T2DM and race on the adenoma detection rate(ADR)in screening colonoscopies in two disparate *** A retrospective cohort study was conducted on ADR during index screening colonoscopies(age 45-75)performed at an urban public hospital serving a predominantly Black/AA population(92%)(2017-2018,n=1606).Clinical metadata collected included basic demographics,insurance,body mass index(BMI),family history of CRC,smoking,diabetes diagnosis,and aspirin *** dataset was combined with a recently reported parallel retrospective cohort data set collected at a suburban university hospital serving a predominantly White/EA population(87%)(2012-2015,n=2882).RESUlTS The ADR was higher in T2DM patients than in patients without T2DM or prediabetes(35.2%vs 27.9%,P=0.0166,n=981)at the urban public *** analysis of the combined datasets showed that T2DM[odds ratio(OR)=1.29,95%confidence interval(CI):1.08-1.55,P=0.0049],smoking(current vs never OR=1.47,95%CI:1.18-1.82,current vs past OR=1.32,95%CI:1.02-1.70,P=0.0026),older age(OR=1.05 per year,95%CI:1.04-1.06,Ple sex(OR=1.87,95%CI:1.62-2.15,Ply associated with increased ADR on index screening colonoscopy while controlling for other factors.

Objective: To identify the extent to which *** videos targeting prescription drug abuse include health behavior theory in their content and design. Methods: This study used a content analysis of 209 *** videos. Coders used an established theory-based instrument to rate each video’s inclusion of theoretical constructs from four prominent health behavior theories. Each *** video was coded for twenty-two theoretical items, which were summed to create a total theory score. Results: The mean theory score of videos was 3.26 out of a possible 22. Among the top 10% of videos with highest theory scores, social cognitive theory constructs were coded most often and transtheoretical constructs were coded least often. Conclusion: It is not unexpected that *** videos are largely void of theoretical content since these videos are made by a variety of organizations and sources that may not be trained in the application of health behavior theories. There is an opportunity for the organizations that create these videos to collaborate with health and behavior change experts in creating videos with more theoretical constructs, possibly resulting in better health outcomes.

以CoCl_(2)刺激人脐静脉内皮细胞(HUVEC)建立细胞异常缺氧损伤模型,探究肝素寡糖(HDO)对HUVEC细胞中糖酵解的影响及分子机制。实验分为对照组(无血清DMEM培养基)、模型组(无血清DMEM培养基+50μmol/l CoCl_(2))及HDO给药组(模型组基础上分别添加0.01、0.1、1μmol/l HDO)。采用生化试剂盒检测HDO对HUVEC细胞葡萄糖摄取及乳酸积累的影响;采用Western blot及qPCR实验检测HIF-1α、GlUT-1及lDha基因转录和蛋白表达的影响,并对PI3K/Akt信号通路进行检测。结果显示:HDO可以抑制HUVEC细胞的葡萄糖摄取及乳酸生成,下调HIF-1α、GlUT-1及lDha的表达水平,影响PI3K/Akt信号通路的激活。结果表明,HDO可以通过抑制PI3K/Akt/HIF-1α信号轴的激活从而调控HUVEC细胞的糖酵解水平。

Given the interdependence of multiple factors in age-related vestibular loss (e.g., balance, vision,cognition), it is important to examine the individual contributions of these factors with ARVl. While therelationship between the vestibular and visual systems has been well studied (Bronstein et al., 2015),little is known about the association of the peripheral vestibular system with neurodegenerative disorders (Cronin et al., 2017). Further, emerging research developments implicate the vestibular system asan opportunity for examining brain function beyond balance, and into other areas, such as cognition andpsychological functioning. Additionally, the bidirectional impact of psychological functioning is understudied in ARVl. Recognition of ARVl as part of a multifaceted aging process will help guide thedevelopment of integrated interventions for patients who remain at risk for decline. In this review, wewill discuss a wide variety of characteristics of the peripheral vestibular system and ARVl, how it relatesto neurodegenerative diseases, and correlations between ARVl and balance, vision, cognitive, and psychological dysfunction. We also discuss clinical implications as well as future directions for research, withan emphasis on improving care for patients with ARVl.
