Pelvic region bone density,soft tissue mass,and injury frequency in female professional ballet dancers and soccer athletes

We recently observed a high prevalence of low pelvic bone mineral density(BMD)in female professional ballet *** this population is susceptible to musculoskeletal overuse injuries,we aimed to determine which regions of the pelvis may be at greatest risk compared to general population females(GENPOP)as well as professional female soccer players(SOccER,a comparison to other elite athletes regularly subjected to high degrees of loading).Three groups of age-matched females[(GENPOP;n=38,27±1yrs),(BALLET;single com-pany,n=36,26±3yrs),(SOccER;single NWSL■club,n=34,25±1yrs)]consented to have their BMD and body composition assessed(DEXA,GE■).In addition to soft tissue and total and regional BMD analyses,a segmental analysis of the pelvis was performed to determine site-specific BMD for the iliac fossa,iliac fossa/iliac crest/ilium combined,pubic bone,ischium,and sacrum.A mixed-model ANOVA followed by a Tukey's post-hoc test was used to compare the groups(Type-I error;α=0.05).The BALLET group had lower pelvic BMD for all measures(Avg.%Diff.=15%-27%,pcompared to the SOccER group and for the ischium(Avg.%Diff.=8%;p=0.007)and sacrum(Avg.%Diff.=7%;p=0.028)compared to the GENPOP *** BALLET group had lower lean mass for all measures compared to the other groups(Avg.%Diff.=12%-18%;pced pelvic region soft tissue and site-specific BMD not previously detected using standard DEXA *** findings highlight which pelvic regions may benefit from preventative strength training and/or nutritional interventions.

Optical coherence Tomography (OcT) Feature Identifying Niche for Dexamethasone (FIND) Intravitreal Implant in the Treatment of Anti-VEGF Slow Responders with Diabetic Macular Edema

This paper raises the question if intravitreal dexamethasone implant deserves to be utilized more effectively in a select subset of eyes with diabetic macular edema (DME). If so, what is the OcT morphology of such eyes? A retrospective consecutive case series is employed to answer these questions. Twenty consecutive eyes were studied: ten that have been treated with intravitreal anti-VEGF (Group A) injections and ten which have been treated with the steroidal implant (Group O) because they failed or were slow to respond to multiple injections of anti-VEGF medications. Specifically, 1) macular edema in the eyes were categorized for the type of OcT morphology and 2) their response to the respective treatments in terms of the resolution of the OcT morphology was determined. Results show that the OcT morphology of eyes that were in Group O predominantly (7/10) had the feature of posterior retinal leakage (subretinal fluid and large outer retinal cysts);this feature was rare in Group A (2/10). Further, each of these eyes (7/7) in Group O had a complete resolution of the macular edema after a single treatment with the dexamethasone intravitreal implant whereas neither eye with this feature (0/2) responded to the (anti-VEGF) treatment in Group A. This leads to the conclusion that there exists an OcT Feature that Identifies a Niche for Dexamethasone Intravitreal implant (FIND) in the treatment of anti-VEGF slow responders in DME. The clinical significance of the study is that selecting eyes with a priori FIND morphology on the OcT for treatment with dexamethasone implant prior to, or at the outset of, a series of anti-VEGF treatment may resolve DME promptly and lower the treatment burden for patients and cost to society.

Microglia shape the synaptic environment in health and disease,but synapses do not exist in a ***,pre-and postsynaptic terminals are surrounded by extracellular matrix(EcM),which together with glia comprise the four elements of the contemporary tetrapartite synapse *** research in this area is still just beginning,accumulating evidence points toward a novel role for microglia in regulating the EcM during normal brain homeostasis,and such processes may,in turn,become dysfunctional in *** it relates to synapses,microglia are reported to modify the perisynaptic matrix,which is the diffuse matrix that surrounds dendritic and axonal terminals,as well as perineuronal nets(PNNs),specialized reticular formations of compact EcM that enwrap neuronal subsets and stabilize proximal *** interconnected relationship between synapses and the EcM in which they are embedded suggests that alterations in one structure necessarily affect the dynamics of the other,and microglia may need to sculpt the matrix to modify the synapses ***,we provide an overview of the microglial regulation of synapses,perisynaptic matrix,and PNNs,propose candidate mechanisms by which these structures may be modified,and present the implications of such modifications in normal brain homeostasis and in disease.

Return to sports after shoulder arthroplasty

Many patients prioritize the ability to return to sports following shoulder replacement surgeries, including total shoulder arthroplasty(TSA), reverse total shoulder arthroplasty(RTSA), and hemiarthroplasty(HA). While activity levels after hip and knee replacements have been well-established in the literature, studies on this topic in the field of shoulder arthroplasty are relatively limited. A review of the literature regarding athletic activity after shoulder arthroplasty was performed using the PubMed database. All studies relevant to shoulder arthroplasty and return to sport were included. The majority of patients returned to their prior level of activity within six months following TSA, RTSA, and shoulder ***, low demand activities are permitted by most surgeons postoperatively and generally have higher return rates than contact sports or high-demand activities. In some series, patients reported an improvement in their ability to participate in sports following the arthroplasty procedure. The rates of return to sports following TSA(75%-100%) are slightly higher than those reported for HA(67%-76%) and RTSA(75%-85%). Patients undergoing TSA, RTSA, and shoulder HA should be counseled that there is a high probability that they will be able to return to their preoperative activity level within six months postoperatively. TSA has been associated with higher rates of return to sports than RTSA and HA,although this may reflect differences in patient population or surgical indication.

The nuclear factor kappaB(NF-κB)family of transcription factors regulates immune and inflammatory responses and controls cell survival,proliferation,differentiation,and apoptosis.1 The five subunits of the NF-κB family,(RelA,RelB,c-Rel,p105/p50,and p100/p52),are categorized as activating(RelA,RelB,and c-Rel),or inhibitory(p105/p50 and p100/p52),based on the presence or absence of the transactivation domain and form homo-and ***-dependent activation of NF-κB,either by the classical pathway involving the degradation of the inhibitor of NF-κB(IκB)or by the alternative pathway involving the partial degradation of p100 to p52 and formation of the p52/RelB dimer,has emerged as a central theme in the regulation of NF-κBdependent genes.2–4 However,in Sertoli cells and some neurons,lymphocytes,and malignant cells,NF-κB has been shown to be constitutively active in the nucleus5,6(***/nf-kb).Moreover,several recent studies have also identified disease-associated mutations in the NF-κB subunits that affect their expression level and transcriptional activity.2,7 Our understanding of the ways in which the changes in the basal level of NF-κB affect the subsequent basal regulation of the expression of NF-κBdependent genes remain poorly defined.

The Global Weather Research and Forecasting (GWRF) Model: Model Evaluation, Sensitivity Study, and Future Year Simulation

Global WRF (GWRF) is an extension of the mesoscale Weather Research and Forecasting (WRF) model that was developed for global weather research and forecasting applications. GWRF is being expanded to simulate atmospheric chemistry and its interactions with meteorology on a global scale. In this work, the ability of GWRF to reproduce major boundary layer meteorological variables that affect the fate and transport of air pollutants is assessed using observations from surface networks and satellites. The model evaluation shows an overall good performance in simulating global shortwave and longwave radiation, temperature, and specific humidity, despite large biases at high latitudes and over-Arctic and Antarctic areas. Larger biases exist in wind speed and precipitation predictions. These results are generally consistent with the performance of most current general circulation models where accuracies are often limited by a coarse grid resolution and inadequacies in sub-filter-scale parameterizations and errors in the specification of external forcings. The sensitivity simulations show that a coarse grid resolution leads to worse predictions of surface temperature and precipitation. The combinations of schemes that include the Dudhia shortwave radiation scheme or the Purdue Lin microphysics module, or the Grell-Devenyi cumulus parameterization lead to a worse performance for predictions of downward shortwave radiation flux, temperature, and specific humidity, as compared with those with respective alternative schemes. The physical option with the Purdue Lin microphysics module leads to a worse performance for precipitation predictions. The projected climate in 2050 indicates a warmer and drier climate, which may have important impacts on the fate and lifetime of air pollutants.
