Catheter ablation of atrial fibrillation in elderly population

Background Although elderly patients have been included in published series of catheter ablation for atrial fibrillation （AF）, clinicalbenefit and safety remain still less defined in this population. A retrospective analysis of the results of catheter ablation for AF in a large volumecenter focused on comparison of elderly patients with the rest of the patient cohort was conducted in this study. Methods Consecutive patientswho underwent catheter ablation for AF between January 2001 and December 2016 were analysed. A total population of 3197 patients wasdichotomized by the age of 70 years （394 elderly vs. 2803 younger subjects）. Patients were followed in terms of arrhythmia status and sur-vival for a median period of 18 vs. 21 and 35 vs. 57 months, respectively. Results Elderly patients were more frequently females （49% vs.29%, P 〈 0.0001）, had a history of hypertension （79% vs. 57%, P 〈 0.0001）, diabetes （16% vs. 11%, P 〈 0.01）, stroke （9% vs. 6%, P 〈 0.01）,coronary/peripheral artery disease （14% vs. 8%, P 〈 0.0001）, and CHAzDs2-VAsc score （3.1 ± 1.3 vs. 1.5 ± 1.2 s, P 〈 0.0001）. Major com-plications were more frequent in elderly （5.3% vs. 3.2%, P = 0.03）; however, this difference was driven by vascular complications （3.6% vs.1.9%, P = 0.04）. There were comparable rates of cerebrovascular （0.3 vs. 0.3%） or nonvascular complications （1.8 vs. 1.2%）. Good arrhyth-mia control was inferior in elderly patients as compared with the rest of the cohort, both without and with antiarrhythmic drugs： 44.2% vs.58.2% （P 〈 0.0001） and 78.2 vs. 83.2% （P 〈 0.01）, respectively. Poor arrhythmia control was associated with relative risk of all-cause mor-tality of 2.7 （95% CI： 1.1-6.4） in elderly patients and 1.4 （95% CI： 0.9-2.0） in younger subjects. Conclusions Catheter ablation for AF inelderly patients is safe although somewhat less effective. Good arrhythmia control is associated with better survival, especially in elderly patients.

Periodontal disease is associated with chronic oxidative stress and inflammation. Caffeic acid phenethyl ester(CAPE), which is a potent inducer of heme oxygenase 1(HO1), is a central active component of propolis, and the application of propolis improves periodontal status in diabetic patients. Here, primary murine macrophages were exposed to CAPE. Target gene expression was assessed by whole-genome microarray, RT-PCR and Western blotting. The antioxidative and anti-inflammatory activities of CAPE were examined by exposure of the cells to hydrogen peroxide, saliva and periodontal pathogens. The involvement of HO1 was investigated with the HO1 inhibitor tin protoporphyrin(snPP) and knockout mice for Nrf2, which is a transcription factor for detoxifying enzymes. CAPE increased HO1 and other heat shock proteins in murine macrophages. A p38 MAPK inhibitor and Nrf2 knockout attenuated CAPE-induced HO1 expression in macrophages. CAPE exerted strong antioxidative activity. Additionally, CAPE reduced the inflammatory response to saliva and periodontal pathogens. Blocking HO1 decreased the antioxidative activity and attenuated the anti-inflammatory activity of CAPE. In conclusion, CAPE exerted its antioxidative effects through the Nrf2-mediated HO1 pathway and its anti-inflammatory effects through NF-κB inhibition. However, preclinical models evaluating the use of CAPE in periodontal inflammation are necessary in future studies.

AIM To perform a comprehensive review and provide an up-to-date synopsis of the incidence and trends of inflammatory bowel disease(IBD). METHODs We systematically searched the MEDLINE(source Pub Med), EMBAsE and Cochrane Library databases in accordance with the Preferred Reporting Items for systematic Reviews and Meta-Analyses guidelines(period: 1985-2018) to identify studies reporting population-based data on the incidence of pediatriconset(s at diagnosis) IBD in full manuscripts. Two authors carried out screening and data extraction. Choropleth interactive maps and temporal trends were used to illustrate the international differences and incidences of and changes in IBD and *** In total, one hundred forty studies reporting data from 38 countries were considered in this review. The highest annual pediatric incidences of IBD were 23/100000 person-years in Europe, 15.2/100000 in North America, and 11.4/100000 in Asia/the Middle East and Oceania. The highest annual incidences of Crohn's disease(CD) were 13.9/100000 in North America and 12.3/100000 in Europe. The highest annual incidences of ulcerative colitis(UC) were 15.0/100000 in Europe and 10.6/100000 in North America. The highest annual incidences of IBD-unclassified(IBD-U) were 3.6/100000 in Europe and 2.1/100000 in North America. In the time-trend analyses, 67% of CD, 46% of UC and 11% of IBD-U studies reported an increasing incidence(P sk of IBD is increasing among firstgeneration of migrant *** Globally, the incidence of IBD varies greatly by geographical areas. The steadily increasing incidence of pediatric IBD over time indicates its emergence as a global disease, suggesting that studies should investigate the environmental risk factors among pediatric cohorts.

spatial body patterning is widely observed throughout the phylogenetic tree and is used for a variety of functions. Body colours in general and camouflaging patterns in particular have been extensively studied for their role in stealth and crypsis. Particular interest has focused on the diverse skin patterns surrounding animals’ eyes (Peri-Ocular Eye Patterning-POEP). These patterns have been suggested to aid in high brightness conditions, help camouflage an organism’s eyes or ornament and emphasize bright head colorations. In this work I demonstrate the apparent widespread use of POEP among various marine and terrestrial organisms (both vertebrates and invertebrates) and discuss the trait’s abundance, variations, and possible roles.

In the era of economic globalization,the concept of Economic Polarized Area comes into being as an effective vehicle to agglomerate the economic elements and sustain the economic lifeline of the region. Based on the region's specific development mode and construction form the concept is working in such a way that it will contribute to guide the economic development of the country and will play an important role in competing with other regions or countries in the world. Due to the high speed development of the last 30 years,the Yangtze Delta Region starts to show the features of Economic Polarized Area. But,compared with other world-class Economic Polarized Areas,the economic strength and the ability of the Yangtze Delta Region to drive the country's economic development is still very low and the competitive power is still very limited. Expanding the boundaries of the Economic Polarized Area of the Yangtze Delta may extend the economic hinterland of the core area of the Yangtze Delta Region,will lighten the pressures from the limited resource and promote the rationalization of the industrial structure in the Yangtze Delta Region's core area. With regard to the reasonable boundaries of the Yangtze Delta Region,there are different opinions and controversial arguments in political and academic circles. starting from the idea of increasing the competitive power and improving the economic strength of the Yangtze Delta Region,the paper firstly is discussing the requirements to become a world-class Economic Polarized Area. In a second step an analysis of functional complementation and economic collaboration between the cities of an "extended" Yangtze Delta Region has been carried out by in particular considering the feasibility of integrating these "newly included" cities.
