New therapeutic strategy with extracorporeal membrane oxygenation for refractory hepatopulmonary syndrome after liver transplant: A case report

Background Schistosomiasis is highly endemic in sub-Saharan Africa and frequently imported to Europe. Male urogenital manifestations are often neglected. We aimed to ascertain the prevalence of genitourinary clinical signs and symptoms among long-term African migrants in a non-endemic European country using a serology *** We carried out a prospective, community-based cross-sectional study of adult male migrants from sub-Saharan Africa living in *** serology tests and microscopic urine examinations were carried out, and clinical data were obtained from an electronic medical record search and a structured *** We included 388 adult males, mean age 43.5 years [Standard Deviation (SD) = 12.0, range: 18–76]. The median time since migration to the European Union was 17 [Interquartile range (IQR): 11–21] years. The most frequent country of origin was Senegal (N = 179, 46.1%). Of the 338, 147 (37.6%) tested positive forSchistosoma. Parasite eggs were present in the urine of only 1.3%. Nine genitourinary clinical items were significantly associated with positiveSchistosoma serology results: pelvic pain (45.2%;OR = 1.57, 95%CI: 1.0–2.4), pain on ejaculation (14.5%;OR = 1.85, 95%CI: 1.0–3.5), dyspareunia (12.4%;OR = 2.45, 95%CI: 1.2–5.2), erectile dysfunction (9.5%;OR = 3.10, 95%CI: 1.3–7.6), self-reported episodes of infertility (32.1%;OR = 1.69, 95%CI: 1.0–2.8), haematuria (55.2%;OR = 2.37, 95%CI: 1.5–3.6), dysuria (52.1%;OR = 2.01, 95%CI: 1.3–3.1), undiagnosed syndromic STIs (5.4%), and orchitis (20.7%;OR = 1.81, 95%CI: 1.0–3.1). Global changes explain the long-term demographic trend of the Eurasian common lizard (Squamata: lacertidae)

The demographic trend of a species depends on the dynamics of its local populations,which can be compromised by local or by global ***,the relevance of local and global phenomena has rarely been investigated ***,we tested whether local phenomena compromised a species'demographic trend using the Eurasian common lizard Zootoca vivipara,the terrestrial reptile exhibiting the widest geographic distribution,as a model *** analyzed the species'ancient demographic trend using genetic data from its 6 allopatric genetic clades and tested whether its demographic trend mainly depended on single clades or on global *** vivipara's effective population size increased since 2.3 million years ago and started to increase steeply and continuously from 0.531 million years *** growth rate exhibited 2 maxima,both occurring during global climatic changes and important vegetation changes on the northern *** population size and growth rate were negatively correlated with global surface temperatures,in line with global parameters driving long-term demographic *** vivipara's ancient demography was neither driven by a single clade,nor by the 2 clades that colonized huge geographic areas after the last *** low importance of local phenomena,suggests that the experimentally demonstrated high sensitivity of this species to short-term ecological changes is a response in order to cope with short-term and local *** suggests that what affected its long-term demographic trend the most,were not these local changes/responses,but rather the important and prolonged global climatic changes and important vegetation changes on the northern hemisphere,including the opening up of the forest by humans.

Counter-regulatory phosphatases TNAP and NPP1 temporally regulate tooth root cementogenesis

Cementum is critical for anchoring the insertion of periodontal ligament fibers to the tooth root. Several aspects of cementogenesis remain unclear, including differences between acellular cementum and cellular cementum, and between cementum and bone. Biomineralization is regulated by the ratio of inorganic phosphate （Pi） to mineral inhibitor pyrophosphate （PPi）, where local Pi and PPi concentrations are controlled by phosphatases including tissue-nonspecific alkaline phosphatase （TNAP） and ectonucleotide pyrophosphatase/phosphodiesterase 1 （NPP1）. The focus of this study was to define the roles of these phosphatases in cementogenesis. TNAP was associated with earliest cementoblasts near forming acellular and cellular cementum. With loss of TNAP in the Alpl null mouse, acellular cementum was inhibited, while cellular cementum production increased, albeit as hypomineralized cementoid. In contrast, NPP1 was detected in cementoblasts after acellular cementum formation, and at low levels around cellular cementum. loss of NPP1 in the Enppl null mouse increased acellular cementum, with little effect on cellular cementum. Developmental patterns were recapitulated in a mouse model for acellular cementum regeneration, with early TNAP expression and later NPP1 expression. In vitro, cementoblasts expressed Alpl gene/protein early, whereas Enppl gene/protein expression was significantly induced only under mineralization conditions. These patterns were confirmed in human teeth, including widespread TNAP, and NPP1 restricted to cementoblasts lining acellular cementum. These studies suggest that early TNAP expression creates a low PPi environment promoting acellular cementum initiation, while later NPP1 expression increases PPi, restricting acellular cementum apposition. Alterations in PPi have little effect on cellular cementum formation, though matrix mineralization is affected.

Changes in Olsen P in Relation to P Balance in Contrasting Agricultural Soils

Maintaining soil phosphorus （P） at adequate levels for plant growth requires assessing how the long-term P balance （viz., the difference between P inputs and outputs） results in changes in soil test P. The hypothesis that routinely measured soil properties can help predict the conversion factor of P balance into Olsen P was tested at 39 sites in agricultural areas of the Mediterranean region in Spair~. A set of soil samples from each site was analyzed for Olsen P, inorganic P （P extracted using 0.5 mol l-1 h2SO4）, pseudototal P （P extracted using 0.5 mol l-1 h2SO4 following ignition at 550 ~C）, and organic P （the difference between pseudototal P and inorganic P）. Organic and Olsen P were uncorrelated in most of the 39 soil sets, which suggests that organic P content changed little with P inputs and outputs. The slopes of the regression lines of Olsen P against pseudototal and inorganic P, which were used as two different measures of the conversion factor, ranged widely （from 0.03 to 0.25 approximately）, with their average values （about 0.10） being similar to those found in long-term experiments conducted in temperate areas. Neither conversion factor was significantly correlated with any routinely measured soil property; however, the conversion factor for inorganic P was significantly lower for calcareous soils than for noncalcareous soils. Our negative results suggest the need to isolate the influence of soil properties from that of management systems and environmental factors relating to P dynamics in future studies.

Cadaveric liver transplantation for non-acetaminophen fulminant hepatic failure:A 20-year experience

AIM: To investigate the long-term results of liver transplantation (lT) for non-acetaminophen fulminant hepatic failure (FhF). METhODS: Over a 20-year period, 29 FhF patients underwent cadaveric whole lT. Most frequent causes of FhF were hepatitis B virus and drug-related (not acetaminophen) liver failure. All surviving patients were regularly controlled at the out-patient clinic and none was lost to follow-up. Mean follow-up was 101 mo. RESUlTS: One month, one-, five- and ten-year patient survival was 79%, 72%, 68% and 68%, respectively. One month, one-, five- and ten-year graft survival was 69%, 65%, 51% and 38%, respectively. Six patients needed early (lantation, four for primary non-function, one for early acute refractory rejection because of ABO blood group incompatibility, and one for a malignant tumor found in the donor. Two patients with hepatitis B FhF developed cerebral lesions peri-transplantion: One developed irreversible and extensive brain damage leading to death, and one suffered from deep deficits leading to continuous medical care in a specialized institution. CONClUSION: long-term outcome of patients transplanted for non-acetaminophen FhF may be excellent. As the quality of life of these patients is also particularly good, lT for FhF is clearly justified, despite lower graft survival compared with lT for other liver diseases.
