Objective To determine whether topical applications of thiosulfinate-enriched Allium sativum extract(TASE)can accelerate acute cutaneous wound healing(WH)in a murine model.Methods Keratinocyte viability and in vitro wound closure were assessed in keratinocyte cultures.Effects of topical TASE(0.5μg/mL of allicin in 97%ethanol)on acute cutaneous WH were determined in a murine model of acute cutaneous wound.Twelve mice were alternately assigned to the vehicle-and TASE-treated groups(n=6 per group).Expression levels of mRNA for keratinocyte differentiation marker-related proteins(filaggrin,loricrin and involucrin)and lipid synthetic enzymes(elongation of very long chain fatty acids protein 4(ELOVL4),fatty acid synthase(FA2H),3-hydroxy-3-methyl-glutaryl-coenzyme A reductase(HMGCoA),and serine palmitoyltransferase(SPT))were assessed using real-time quantitative polymerase chain reaction on day 3 and 8 after wounding,while transepidermal water loss(TEWL)rates were measured in wounded areas.Results TASE accelerated WH both in vivo(40%vs.22%reduction in wound area,P<0.01)and in vitro(90%vs.65%reduction in wound area,P<0.01).Moreover,topical applications of TASE upregulated the expression levels of epidermal mRNA for ELOVL4,HMGCoA,SPT,filaggrin,loricrin and involucrin(P<0.05 vs.vehicle-treated controls)on day 3 after wounding.Likewise,TASE significantly lowered TEWL rates in comparison with vehicle alone on day 8(33.06±2.09 g/(m^2·h)vs.24.60±2.04 g/(m^2·h),P<0.01).Conclusions Topical applications of TASE stimulated keratinocyte proliferation and formation of epidermal permeability barrier function,leading to acceleration of acute cutaneous WH.Topical products containing TASE could be used to manage acute cutaneous WH.

In many species with continuous growth,body size is an important driver of life-history tactics and its relative importance is thought to reflect the spatio-temporal variability of selective pressures.We developed a deterministic size-dependent integral projection model for 3 insular neighboring lizard populations with contrasting adult body sizes to investigate how size-related selective pressures can influence lizard life-history tactics.For each population,we broke down differences in population growth rates into contributions from size-dependent body growth,survival,and feeundity.A life table response experiment(LTRE)was used to compare the population dynamics of the 3 populations and quantify the contributions of intrinsic demographic coefficients of each population to the population growth rate(z).Perturbation analyses revealed that the largest adults contributed the most to the population growth rate,but this was not true in the population with the smallest adults and size-independent fertility.Although we were not able to identify a single factor responsible for this difference,the combination of the demographic model on a continuous trait coupled with an LTRE analysis revealed how individuals from sister populations of the same species follow different life strategies and showed different compensatory mechanisms among survival,individual body growth,and fertility.Our results indicate that body size can play a contrasting role even in closely-related and closely-spaced populations.

A vast number of physical processes involving oscillations of a bounded viscous fluid are relevantly influenced by acoustic streaming. When this happens a steady circulation of fluid develops in a thin boundary adjacent to the interface. Some examples are refracted sound waves, a fluid inside a spherical cavity undergoing torsional oscillations or a pulsating liquid droplet. Steady streaming around circular interfaces consists of a hemispherically symmetric recirculation of fluid from the equatorial plane to the polar axes closely resembling the meridional circulation pattern observed in the Sun＇s convection zone that determines the solar cycle. In this paper, it is argued that the acoustic pulsations exhibited by the Sun would lead to acoustic streaming in the boundary of the convection zone. A simple estimation using a typical dominant frequency of 3 mHz and the observed surface oscillation amplitude yields a steady streaming velocity Us - 10 m s^-1, which is on the order of the meridional circulation velocity observed in the Sun＇s convection zone.

Metal halide perovskites(MHPs) are gaining increasing attention as low-cost, high-performance semiconductors for optoelectronics. In particular, their solution processing is compatible with the largescale manufacturing of thin-film devices, including solar cells and light-emitting diodes.Understanding the coordination chemistry in precursor-solvent solution and atomistic mechanisms of film formation is of great importance for optimizing the optoelectronic properties of the final films.Using the methylammonium lead triiodide(MAPbI_(3)) as an example, we study the complex evolution of the molecular species from the solution to the initial stage of the crystallization by using a combination of density functional theory(DFT) calculations and ab-initio molecular dynamics(AIMD) simulations. We focus on the widely employed solvents DMSO and DMF, analyze the structures and energies of the iodoplumbate complexes in the form of simple complex of [PbI_(m)L_(n)]^(2-m))_(x) and polymeric iodoplumbates of([PbI_(m)L_(n)]^(2-m))_(x). Based on the calculated formation enthalpies, we propose reaction schemes of MAPbI_(3) formation in DMSO, DMF and DMF-DMSO binary solvent and explain the advantages of the binary solvent.We highlight the important role of NH...O hydrogen bonds in the formation of iodoplumbates monomers.Our calculations indicate unbalanced reaction energies at several elementary reaction steps in either DMF(formation of [PbI_(4)L_(n)]^(2-) being highly favourable) or DMSO(formation of [PbI_(5)L_(n)]^(3- )being retarded).Mixing a small amount of DMSO in DMF gives rise to a better balance in the energies and, therefore,potentially better equilibria in the overall crystallization process and better quality of the final perovskite films.

背景社区获得性肺炎(CAP)风险在初级保健中的信息是有限的。我们评估了不同生活方式和共病条件作为初级保健成人CAP的危险因素。方法一项回顾性观察对照研究。使用国家初级保健数据监测系统检索了2009至2013年在西班牙初级保健部门诊断的成人CAP病例。匹配年龄和匹配性别的对照组采用发病率密度抽样(比例2∶1)。以百分比和比值比来表示关联分析。为了避免偏倚效应,建立二项回归模型。结果51 139例患者和102 372例对照组做比较。平均年龄为61.4(19.9)岁。危险因素与CAP的显著相关性如下:艾滋病[比值比(95%可信区间)5.21(4.35~6.27)]、慢性阻塞性肺病(COPD)[2.97(2.84~3.12)]、哮喘[2.16(2.07~2.26)]、吸烟[1.96(1.91~2.02)]和口腔卫生问题[1.45(1.41~1.49)]。患者组中有危险因素存在的平均患病率为82.2%,对照组为69.2%。CAP患病率的增加与危险因素和年龄的累积有关:1个危险因素时,在18~60岁老人中的风险相关性是1.42,在>60岁人中是1.57;2个危险因素时,是1.88和2.35;≥3个危险因素时,是3.11和4.34。结论成人CAP初级保健中的危险因素普遍较高。主要与危险因素相关的是艾滋病、COPD、哮喘、吸烟、口腔卫生问题。我们的风险叠加结果可以帮助临床医生确定肺炎风险较高的患者。

Diagnosis and management of bacterial infections in decompensated cirrhosis

Bacterial infections are one of the most frequent complications in cirrhosis and result in high mortality rates.Patients with cirrhosis have altered and impaired immunity,which favours bacterial translocation.Episodes of infections are more frequent in patients with decompensated cirrhosis than those with compensated liver disease.The most common and life-threatening infection in cirrhosis is spontaneous bacterial peritonitis followed by urinary tract infections,pneumonia,endocarditis and skin and soft-tissue infections.Patients with decompensated cirrhosis have increased risk of developing sepsis,multiple organ failure and death.Risk factors associated with the development of infections are severe liver failure,variceal bleeding,low ascitic protein level and prior episodes of spontaneous bacterial peritonitis (SBP).The prognosis of these patients is closely related to a prompt and accurate diagnosis.An appropriate treatment decreases the mortality rates.Preventive strategies are the mainstay of the management of these patients.Empirical antibiotics should be started immediately following the diagnosis of SBP and the first-line antibiotic treatment is third-generation cephalosporins.However,the efficacy of currently recommended empirical antibiotic therapy is very low in nosocomial infections including SBP,compared to community-acquired episodes.This may be associated with the emergence of infections caused by Enterococcus faecium and extended-spectrum β-lactamaseproducing Enterobacteriaceae,which are resistant to the first line antimicrobial agents used for treatment.The emergence of resistant bacteria,underlines the need to restrict the use of prophylactic antibiotics to patients with the greatest risk of infections.Nosocomial infections should be treated with wide spectrum antibiotics.Further studies of early diagnosis,prevention and treatment are needed to improve the outcomes in patients with decompensated cirrhosis.

Artificial intelligence(AI)is the process of having a computational program that can perform tasks of human intelligence by mimicking human thought processes.AI is a rapidly evolving transdisciplinary field which integrates many elements to develop algorithms that aim to simulate human intuition,decision-making,and object recognition.The overarching aims of AI in cardiovascular medicine are threefold:To optimize patient care,improve efficiency,and improve clinical outcomes.In cardiology,there has been a growth in the potential sources of new patient data,as well as advances in investigations and therapies,which position the field well to uniquely benefit from AI.In this editorial,we highlight some of the main research priorities currently and where the next steps are heading us.

Phenolic compounds of green tea: Health benefits and technological application in food

Green tea has been an important beverage for humans since ancient times, widely consumed and considered to have health benefits by traditional medicine in Asian countries. Green tea phenolic compounds are predominately composed of catechin derivatives, although other compounds such as flavonols and phenolic acids are also present in lower proportion. The bioactivity exerted by these compounds has been associated with reduced risk of severe illnesses such as cancer, cardiovascular and neurodegenerative diseases. Particularly, epigallocatechin gallate has been implicated in alteration mechanisms with protective effect in these diseases as indicated by several studies about the effect of green tea consumption and mechanistic explanation through in vitro and in vivo experiments. The biological activity of green tea phenolic compounds also promotes a protective effect by antioxidant mechanisms in biological and food systems, preventing the oxidative damage by acting over either precursors or reactive species. Extraction of phenolic compounds influences the antioxidant activity and promotes adequate separation from green tea leaves to enhance the yield and/or antioxidant activity. Application of green tea phenolic compounds is of great interest because the antioxidant status of the products is enhanced and provides the product with additional antioxidant activity or reduces the undesirable changes of oxidative reactions while processing or storing food.In this scenario, meat and meat products are greatly influenced by oxidative deterioration and microbial spoilage, leading to reduced shelf life. Green tea extracts rich in phenolic compounds have been applied to increase shelf life with comparable effect to synthetic compounds, commonly used by food industry. Green tea has great importance in general health in technological application, however more studies are necessary to elucidate the impact in pathways related to other diseases and food applications.

Pancreaticoduodenectomy(PD) is the standard surgical treatment for tumors of the pancreatic head,proximal bile duct,duodenum and ampulla,and represents the only hope of cure in cases of malignancy.Since its initial description in 1935 by Whipple et al,this complex surgical technique has evolved and undergone several modifications.We review three key issues in PD:(1) the initial approach to the superior mesenteric artery,known as the artery-first approach;(2) arterial complications caused by anatomic variants of the hepatic artery or celiac artery stenosis;and(3) the extent of lymphadenectomy.
