Colorectal cancer screening from 45 years of age: Thesis, antithesis and synthesis

Colorectal cancer incidence and mortality in patients younger than 50 years are increasing, but screening before the age of 50 is not offered in Europe. Advancedstage diagnosis and mortality from colorectal cancer before 50 years of age are increasing. This is not a detection-bias effect;it is a real issue affecting the entire population. Three independent computational models indicate that screening from 45 years of age would yield a better balance of benefits and risks than the current start at 50 years of age. Experimental data support these predictions in a sex- and race-independent manner. Earlier screening is seemingly affordable, with minimal impediments to providing younger adults with colonoscopy. Indeed, the American Cancer Society has already started to recommend screening from 45 years of age in the United States. Implementing early screening is a societal and public health problem. The three independent computational models that suggested earlier screening were criticized for assuming perfect compliance. Guidelines and recommendations should be derived from well-collected and reproducible data, and not from mathematical predictions. In the era of personalized medicine, screening decisions might not be based solely on age, and sophisticated prediction software may better guide screening. Moreover, early screening might divert resources away from older individuals with greater biological risks. Finally, it is still unknown whether early colorectal cancer is part of a continuum of disease or a biologically distinct disease and, as such, it might not benefit from screening at all. The increase in early-onset colorectal cancer incidence and mortality demonstrates an obligation to take actions. Earlier screening would save lives, and starting at the age of 45 years may be a robust screening option.

Multiple primary colorectal cancer: Individual or familial predisposition?

Colorectal carcinoma(CRC) is one of the most frequent cancers. Along the surface of the large bowel, several foci of CRC may appear simultaneously or over the time. The development of at least two different tumours has been defined as multiple primary CRC(MPCRC):When more than one tumour is diagnosed at the same time, it is known as synchronous CRC(SCRC), while when a second neoplasm is diagnosed some time after the resection and/or diagnosis of the first lesion, it is called metachronous CRC(MCRC). Multiple issues can promote the development of MPCRC, ranging from different personal factors, such as environmental exposure, to familial predisposition due to hereditary factors. However, most studies do not distinguish this dichotomy. High- and low-pentrance genetic variants are involved in MPCRC. An increased risk for MPCRC has been described in Lynch syndrome, familial adenomatous polyposis, and serrated polyposis. Non-syndromic familial CRCs should also be considered as risk factors for MPCRC. Environmental factors can promote damage to colon mucosae that enable the concurrence of MPCRC. Epigenetics are thought to play a major role in the carcinogenesis of sporadic MPCRC. The methylation state of the DNA depends on multiple environmental factors(e.g., smoking and eating foods cooked at high temperatures), and this can contribute to increasing the MPCRC rate. Certain clinical features may also suggest individual predisposition for MPCRC. Different etiopathogenic factors are suspected to be involved in SCRC and MCRC, and different familial vs individual factors may be implicated. MCRC seems to follow a familial pattern, whereas individual factors are more important in SCRC. Further studies must be carried out to know the molecular basis of risks for MPCRC in order to modify, if necessary, its clinical management, especially from a preventive point of view.

Surrogacy in Reproductive Medicine: Ethical and Legal Analysis of the Procedure

This text sets out the different arguments regarding surrogacy as a reproductive medicine tech-nique in Colombia. The procedure is described, as well as the legal matters that this issue generates. Gestational carriers appear as an alternative for couples who want to have a child but they are not able to conceive because of infertility issues and do not want to go through an adoption process. Therefore, they adhere to surrogacy as an alternative of assisted human reproduction;a procedure that in Colombia faces serious legal gaps and causes ethical dilemmas in the collective imagery. These situations suggest an open discussion through a dialectics between the scientific progress advances, the lack of legal regulation and the constants against ethical positions that the surrogacy procedure causes in Colombia and other parts of the world.

Early life investment in antlers and body growth reflects adult performance in roe deer population under supplementary feeding conditions

Recent research has challenged the idea that cervid antlers are such costly traits,supporting the assertion early-life antler investment is an honest signal of adult phenotypic ***,inferences were made based on antler measurements while growing(velvet)and thus,studies on fully-formed clean antlers are needed to avoid possible bias caused by the inter-individual variation in antler growth *** studied a semi-captive population of European roe deer inhabiting a sub-Mediterranean area(Valsemana research station)and living under optimal conditions(ad libitum food supply and veterinary care).Based on repeated measurements taken from 146 individuals,we assessed whether allocation to secondary sexual traits during early life affected body mass or antler development during ***,we evaluated which body measurements better predicted future male ***,using 488 individuals,we described age-class-specific variation in male body measurements and assessed the decline in antler size between adult and senescent stages(*** of senescence).Results agree with the assertion that there is no negative effect of a high investment in antler during early life on adult antler size or body mass,but we suggest the use of clean antlers as a more robust and reliable *** variables that better predicted body mass during adulthood were yearling body mass and body size at 6 *** score between 10 and 18 months resulted in the best indicator of adult antler ***,we support the idea that the magnitude of senescence in antler size is smaller in environments with higher resource availability during winter.

The extant Cabrera’s vole,Microtus cabrerae,differs in morphology and evolutionary history from the other species of *** arvicoline has unique derived features in the cranium,mandible and ***bly its most conspicuous features are its large size,the high skull in lateral view,the long and distally broad nasals,and the triangle shape of the anteroconid complex,with a marked labio-lingual asymmetry of the occlusal surface of the first lower *** this study,we propose a phylogenetic lineage that includes Cabrera’s vole in what until now has been the Microtus subgenus *** information and several life history traits support the elevation of Iberomys to the rank of *** Iberomys comprises species that have appeared in succession during the Quaternary:in the Early Pleistocene,the extinct *** in the Middle Pleistocene,the extinct *** and in the Late Pleistocene,the extant ***,the extant species shows several biological singularities,such as multiple polymorphic copies of the SRY male-specific gene in both males and females,and the lowest basal metabolic rate in relation to weight among arvicoline ***,its habitat requirement is unique among the Iberian ***,the biological and paleontological data that we present in this work support the elevation of its taxonomic rank to that of ge*** study also suggests a modification of nomenclature:Microtus(Iberomys)brecciensis is replaced with *** and the common name of the extant M.(I.)cabrerae changed from‘topillo’to‘iberon’to improve conservation and protection actions.
