Foley Balloon Catheter versus Oral Misoprostol for Induction of Labour after Prelabour Rupture of Membranes: A Retrospective Data Analysis

Obj.ctives: The Foley balloon catheter (FC) is a viable method for cervical ripening, but concerns about infection risk restrict its use in cases of prolonged prelabour rupture of membranes (PROM). This study aims to evaluate the efficacy and safety of the FC compared to oral misoprostol for cervical ripening after PROM. Study Design: A retrospective data-analysis of 128 pregnant women was conducted. Of these, 49 underwent cervical ripening with an FC and 79 with oral misoprostol. We included all women with a vital singleton pregnancy at 37 - 42 weeks of gestation who underwent cervical ripening after ≥ 24 hours of PROM in specific time frames in two Dutchsecondary care and teaching hospitals. The primary outcome was the incidence of intrapartum infection, a composite of maternal and neonatal infection. In addition, we evaluated the mode of delivery, duration of priming and priming-to-delivery interval. Secondary endpoints included uterine hyperstimulation, umbilical cord prolapse, birth weight, Apgar scores, length of admission to the neonatal low dependency unit, admission to the (neonatal) Intensive Care Unit (ICU) and mortality. Statistical analyses included bivariate and multivariate techniques. Results: Cervical ripening with FC, compared with oral misoprostol, showed a higher incidence of intrapartum infection, respectively 32.7% (n = 16) vs. 12.7% (n = 10) (p = 0.006). However, after adj.sting for epidural anaesthesia and pregestational BMI, the association was no longer significant. No difference was found in mode of delivery and total priming-to-delivery interval (median 21.3 hours vs. 22.0, p = 0.897). Furthermore, FC, compared with oral misoprostol, showed a longer duration of cervical ripening and hence a shorter duration of active labour (p 0.001). Apart from the 1-min Apgar score, secondary maternal and neonatal outcomes did not differ between the groups. Dendritic Cells as a Pharmacological Target of Traditional Chinese Medicine

Dendritic cells （DCs） represent a heterogeneous population of professional antigen-presenting cells （APCs） that play a central role in the initiation and regulation of immune responses. There is considerable evidence that DCs can be used as therapeutic targets for pharmacological modulation of immune responses. Traditional Chinese medicine （TCM） has a long-standing history of using herbal medicine in the treatment of variety of human diseases. Many of the clinical effects of TCM have reportedly been attributed to the up- or down-regulation of immune responses. Accumulating evidence indicates that TCM and its components can interfere with immune responses at the earliest stage by targeting key functions of DCs. Here, we review those published studies of TCM with respect to their effects on immunobiological functions of DCs. Investigations based on both chemical entities derived from TCM as well as TCM herbal mixtures are presented. These studies suggest that various TCM herbal medicines have the capacity to inhibit or promote maj.r functions of DCs, such as differentiation, maturation, cytokine production, survival, antigen uptake and presentation as well as trafficking. These studies have revealed novel biological effects of TCM and documented the utility of this approach to discover novel biological modifier of DC functions derived from natural sources.

Equivalences between Categories of Modules and Categories of Comodules

We show the close connection between apparently different Galois theories for comodules introduced recently in [j. Gomez-Torrecillas and j. Vercruysse, Comatrix corings and Galois Comodules over firm rings, Algebr. Represent. Theory, 10 （2007）, 271 306] and [Wisbauer, On Galois comodules, Comm. Algebra 34 （2006）, 2683-2711]. Furthermore we study equivalences between categories of comodules over a coring and modules over a firm ring. We show that these equivalences are related to Galois theory for comodules.

Prevention of overuse: A view on upper gastrointestinal endoscopy

Many upper gastrointestinal(GI) endoscopies worldwide are performed for inappropriate indications. This overuse of healthcare negatively affects healthcare quality and puts pressure on endoscopy services. Dyspepsia is one of the most common inappropriate indications for upper GI endoscopy as diagnostic yield is low. Reasons for untimely referral are: unfamiliarity with dyspepsia guidelines,uncertainty about etiology of symptoms, and therapy failure. Unfiltered openaccess referrals feed upper GI endoscopy overuse. This review highlights strategies applied to diminish use of upper GI endoscopies for dyspepsia. First,we describe the impact of active guideline implementation. We found improved guideline adherence, but resistance was encountered in the process. Secondly, we show several forms of clinical assessment. While algorithm use reduced upper GI endoscopy volume, effects of referral assessment of individual patients were minor. A third strategy proposed Helicobacter pylori test and treat for all dyspeptic patients. Many upper GI endoscopies can be avoided using this strategy, but outcomes may be prevalence dependent. Lastly, empirical treatment with Proton pump inhibitors achieved symptom relief for dyspepsia and avoided upper GI endoscopies in about two thirds of patients. Changing referral behavior is complex as contributing factors are manifold. A collaboration of multiple strategies is most likely to succeed.

The paucity of naturally occurring CD4^＋FoxP3^＋ regulatory T cells （Tregs） and difficulties in the identification and isolation of such cells are maj.r obstacles in the study of human Tregs. The availability of human Treg-like cell lines is likely to facilitate a better understanding of the molecular basis of Treg function and aid the discovery of pharmacological reagents to regulate Treg activity in a disease state.

Tumor-stroma ratio as prognostic factor for survival in rectal adenocarcinoma: A retrospective cohort study

AIM To evaluate the prognostic value of the tumor-stroma ratio(TSR) in rectal *** TSR was determined on hematoxylin and eosin stained histological sections of 154 patients treated for rectal adenocarcinoma without prior neoadj.vant treatment in the period 1996-2006 by two observers to assessreproducibility. Patients were categorized into three categories: TSR-high [carcinoma percentage(CP) ≥ 70%], TSR-intermediate(CP 40%, 50% and 60%) and TSR-low(CP ≤ 30%). The relation between categorized TSR and survival was analyzed using Cox proportional hazards *** Thirty-six(23.4%) patients were scored as TSR-low, 70(45.4%) as TSR-intermediate and 48(31.2%) as TSRhigh. TSR had a good interobserver agreement(κ = 0.724, concordance 82.5%). Overall survival(OS) and disease free survival(DFS) were significantly better for patients with a high TSR(P = 0.01 and P = 0.02, respectively). A similar association existed for disease specific survival(P = 0.06). In multivariate analysis, patients without lymph node metastasis and an intermediate TSR had a higher risk of dying from rectal cancer(HR = 5.27, 95%CI: 1.54-18.10), compared to lymph node metastasis negative patients with a high TSR. This group also had a worse DFS(HR = 6.41, 95%CI: 1.84-22.28). An identical association was seen for OS. These relations were not seen in lymph node metastasis positive patients. CONCLUSION The TSR has potential as a prognostic factor for survival in surgically treated rectal cancer patients, especially in lymph node negative cases.

The IBI1 Receptor ofβ-Aminobutyric Acid Interacts with VOZ Transcription Factors to Regulate Abscisic Acid Signaling and Callose-Associated Defense

Extemal and internal signals can prime the plant immune system for a faster and/or stronger response to pathogen attack.β-aminobutyric acid(BABA)is an endogenous stress metabolite that induces broad-spectrum disease resistance in *** perception in Arabidopsis is mediated by the aspartyl tRNA synthetase IBI1,which activates priming of multiple immune responses,including callose-associated cell wall defenses that are under control by abscisic acid(ABA).However,the immediate signaling components after BABA perception by IBI1,as well as the regulatory role of ABA therein,remain ***,we have studied the early signaling events controlling IBI1-dependent BABA-induced resistance(BABAIR),using untargeted transcriptome and protein interaction *** analysis revealed that IBI1-dependent expression of BABA-IR against the biotrophic oomycete Hyaloperonospora arabidopsidis is associated with suppression of ABA-inducible abiotic stress *** interaction studies identified the VOZ1 and VOZ2 transcription factors(TFs)as IBI1-interacting partners,which are transcrip-tionally induced by ABA but suppress pathogen-induced expression of ABA-dependent ***,we show that VOZ TFs require nuclear localization for their contribution to BABA-IR by mediating augmented expression of callose-associated ***,our study indicates that the IBI1-VOZ signaling module channels pathogen-induced ABA signaling toward cell wall defense while simultaneously suppressing abiotic stress-responsive genes.

背景与目的目前行为医学的定义与范畴仍是25年以前的版本，因此，有必要对国际行为医学会章程中行为医学的定义和范畴进行修改、更新。本文旨在：（1）对建议修改行为医学定义与范畴的国际性讨论加以总结；（2）诚邀中国同道参与讨论。方法首先确定需要澄清或修改的问题，然后提出对行为医学定义与范畴进行更新和修改的建议。结果需要澄清或修改的问题包括：行为医学的范畴，行为医学的应用，以及行为医学、精神卫生学、健康心理学和心身医学之间的关系与区别。结论建议将行为医学的定义与范畴修改为：行为医学（behavioralmedicine）是以多学科协作为特点的、涉及与健康和疾病相关的生物医学知识和行为科学知识的发展与整合及其在预防、健康促进、诊断、治疗、康复和护理中应用的领域。行为医学的范畴从生物一行为机制（即与健康和疾病相关的生物医学、心理、社会和文化环境等进程之间的相互作用），延伸至临床诊断与干预以及公共卫生领域。诚邀中国同仁对行为医学的定义与范畴的后续讨论发表观点。
