Clinical outcome after pulmonary metastasectomy from primary hepatocellular carcinoma:Analysis of prognostic factors

AIM： To review the surgical outcomes in terms of the surgical indications and relevant prognostic factors. METHODS： Sixteen patients underwent therapeutic lung surgery between March 1999 and May 2006. The observation period was terminated on May 31, 2007. The surgical outcomes and the clinicopathological factors were compared. RESULTS： There was no mortality or major morbidity encountered in this study. The mean follow-up period after metastasectomy was 26.7 ＋ 28.2 （range： 1-99 mo）, and the median survival time was 20 mo. The 1- and 5-year survival rates were 56% and 26%, respectively. At the end of the follow-up, 1 patient died from hepatic failure without recurrence, 6 died from hepatic failure with a recurrent hepatocellular carcinoma （HCC）, and 4 died from recurrent HCC with cachexia. Among several clinical factors, Kaplan Meier analysis revealed as a treatment for the that liver transplantation primary lesion, grade of cell differentiation, and negative evidence HBV infection were independent predictive factors. On Cox＇s proportional hazard model, there were no significant factors affecting survival after pulmonary metastasectomy in patients with HCC. CONCLUSION： A metastasectomy should be performed before other treatments in selected patients, Although not significant, patients with liver transplantation of a primary HCC survived longer, Liver transplantation might be the most beneficial modality that can offer patients better survival, A multi- institutional and collaborative study would be needed for identifying clinical prognostic factors predicting survival in patients with HCC and lung metastasis.

Objectives： TO evaluate the effect of Uhwang Chungsim Won [UC, Niuhuang Qingxin Pill （牛黄清心丸] on the blood oxygen level-dependent （BOLD） functional magnetic resonance imaging （fMRI） signal response to a motor stimulation task using a hand grasping paradigm. Methods： Ten healthy right-handed male volunteers were recruited. Their BOLD fMRI signal characteristics during hand grasping were determined before and 1 h after consuming UC. Results： With the administration of UC, BOLD responses to the right hand motor task showed a decrease in both hemispheres including the right cerebellar culmen, right and left medial frontal gyrus, right parahippocampal gyrus, right and left sublobar insula, right and left posterior cingulate, left precuneus, and right precentral gyrus. Decreased BOLD responses to the left hand motor task were shown in the right lingual gyrus, right cerebellar culmen, left superior temporal gyrus, right and left middle frontal gyrus, and right precentral gyrus after UC administration. Increased BOLD responses to the left hand motor task were shown in the left and right posterior cingulate, while there was no increased response to the right hand motor task after UC administration. Conclusions： UC administration significantly decreased fMRI BOLD signal responses to hand grasping in most of the brain areas including the primary motor cortex, but it is not clear whether these decreased responses were caused by a decrease in neuronal activities or by an increase in cerebral blood flow due to the cerebral vasodilatory effects of UC. Further studies using positron emission tomography or single photon emission computed tomography are needed to fully elucidate these findings.

To apply a new airway treatment to humans, preclinical studies in an appropriate animalmodel is needed. Canine, porcine and leporine tracheas have been employed as animal airwaystenosis models using various methods such as chemical caustic agents, laser, and ***, existing models take a long time to develop (3- 8 weeks) and the mechanism of stenosisis different from that in humans. The aim of the present study was to establish a new and fasttracheal stenosis model in pigs using a combination of cuff overpressure intubation (COI) andelectrocautery. Fourteen pigs were divided into three groups: tracheal cautery (TC) group (n=3),COI group (n=3), and COI-TC combination group (n=8). Cuff overpressure (200/400/500 mmHg)was applied using a 9-mm endotracheal tube. Tracheal cautery (40/60 watts) was performed usinga rigid bronchoscopic electrocoagulator. After intervention, the pigs were observed for 3 weeks andbronchoscopy was performed every 7 days. When the cross-sectional area decreased by > 50%, itwas confirmed that tracheal stenosis was established. The time for tracheal stenosis was 14 days inthe TC group and 7 days in the COI-TC combination group. In the COI group, no stenosis *** the COI-TC group, electrocautery (40 watts) immediately after intubation for>1 h with a cufpressure of 200 mmHg or more resulted in suficient tracheal stenosis within 7 days. Moreover, thedegree of tracheal stenosis increased in proportion to the cuff pressure and tracheal intubation *** combined use of cuf overpressure and electrocautery helped to establish tracheal stenosis inpigs rapidly.

Effects of an Urban park and Residential Area on the Atmospheric CO_2 Concentration and Flux in Seoul,Korea

The CO2 concentrations and fluxes over an urban forest site （Namsan） and an urban residential region （Boramae） in Seoul, Korea, during the non-growing season （2 4 March 2011）, the growing season （10-12 June 2011）, and the late-growing season （22-24 September 2011） were analyzed. The CO2 concentrations of two sites showed nearly the same diurnal variation, with a maximum value occurring during the night and a minimum value occurring during daytime, as well as the same seasonal variation, with a maximum value during the non-growing season （early spring） and a minimum value during the growing season （summer）. The CO2 flux over the urban forest did not show any typical diurnal variation during the non-growing season, but did show diurnal variation with a small positive value during the night and a large negative value during daytime in the growing and late-growing seasons due to photosynthesis in the urban forest. The CO2 flux over the urban residential region showed a positive daily mean value for all periods, with large values during the non-growing season and small values during the growing season, and it also showed diurnal variation with two maxima at 0600-1000 LST and 1800-2400 LST, and two minima at 0300-0600 LST and 1100-1500 LST, and was strongly correlated with the use of liquefied natural gas for cooking and heating by surrounding houses.

BACKGROUND Paragonimiasis is a food-borne parasitic infection caused by lung flukes of the genus Paragonimus. Although the most common site of infection is the pleuropulmonary area, the parasite can also reach other parts of the body on its journey from the intestines to the lungs, ending up in locations such as the brain,abdomen, skin, and subcutaneous tissues. Ectopic paragonimiasis is difficult to diagnose due to the rarity of this *** SUMMARY Here, we report a rare case of simultaneous breast and pulmonary paragonimiasis in a woman presenting painless breast mass and lung nodule with a history of eating raw trout. To confirm the diagnosis, serologic testing and tissue confirmation of the breast mass were performed. As a new class of two-dimensional materials, two-dimensional (2D) heterostructures constructed from metal chalcogenides (MCs) have been gaining tremendous attention due to their unprecedented physical and chemical phenomena, mainly originated from their distinct structural features such as composition, architecture type, spatial arrangement of each component, crystal structure, exposed facet and interface, dimensionality in their heterostructures. Towards the realization of practical applications, synthetic approaches need a rational design with a variety of architecture types including laterally-combined, vertically-aligned, and conformally-coated 2D MC heterostructures. Among various synthetic routes, solution-based synthesis is thought of as an alternative to fabrication through high-cost setups since it can control those structural features in a cheap fashion. This review presents recent progress on solution-based synthesis to produce various 2D MC heterostructures with a focus on the synthetic fundamentals in terms of thermodynamic and kinetic aspects related to the growth mechanism. Four different synthetic approaches are reviewed: seeded growth, cation exchange reaction, colloidal atomic layer deposition, direct synthesis including one-step process and modified electrochemical method. We also provide some representative applications of 2D MC heterostructures and their hybrid composites in various fields including optoelectronics, thermoelectrics, catalysis, and battery. Finally, we offer an insight into challenges and future directions in a synthetic improvement of 2D MC heterostructures.

Mungbean （Vigna radiata （L.） Wilczek） is a unique species in its ability to fix atmospheric nitrogen, with early maturity, and relatively good drought resistance. We used 454 sequencing technology for transcriptome sequencing. A total of 150 159 and 142 993 reads produced 5 254 and 6 374 large contigs （〉_500 bp） with an average length of 833 and 853 for Sunhwa and Jangan, respectively. Functional annotation to known sequences yielded 41.34% and 41.74% unigenes for Jangan and Sunhwa. A higher number of simple sequence repeat （SSR） motifs was identified in Jangan （1 630） compared with that of Sunhwa （1 334）. A similar SSR distribution pattern was observed in both varieties. A total of 8 249 single nucleotide polymorphisms （SNPs） and indels with 2 098 high-confidence candidates were identified in the two mungbean varieties. The average distance between individual SNPs was -860 bp. Our report demonstrates the utility of transcriptomic data for implementing a functional annotation and development of genetic markers. We also provide large resource sequence data for mungbean improvement programs.
