Severe thrombocytopenia before liver transplantation is associated with delayed recovery of thrombocytopenia regardless of donor type

AIM： To compare the recovery of thrombocytopenia and splenomegaly during long-term follow-up after liver transplantation in patients receiving a living donor transplant or a cadaveric donor transplant. METHODS： This was a retrospective cohort study of 216 consecutive liver transplant patients who survived for 〉 6 mo after transplantation; 169 received a liver transplant from a living donor and 47 from a cadaveric donor. The platelet counts or spleen volumes were examined before transplant, i, 6, and 12 mo after transplant, and then annually until 5 years after transplant. RESULTS： The mean follow-up period was 49 mo （range, 21-66）. Platelet counts increased continuously for 5 years after orthotopic liver transplant. The restoration of platelet counts after transplant was significantly slower in patients with severe pretransplant thrombocytopenia （〈 50000/μL） until 4 years after transplant （P = 0.005）. Donor type did not significantlyaffect the recovery of platelet count and spleen volume in either patient group. In multivariate analysis, pretransplant severe thrombocytopenia （〈 50000/μL） was an independent factor associated with sustained thrombocytopenia （P 〈 0.001, odds ratio 6.314; confidence interval, 2.828-14.095）. Thrombocytopenia reappeared after transplant in seven patients with portal flow disturbance near the anastomosis site. CONCLUSION： Our study suggests that severe thrombocytopenia before transplant is closely associated with delayed recovery of platelet count after transplant and donor type did not affect the recovery of thrombocytopenia. The reappearance of thrombocytopenia after transplant should be considered a possible indicator of flow disturbance in the portal vein.

Multiple primary malignancies in a single patient are relatively rare but have increase in frequency in recent decades. This may be a result of medical advancements in diagnostic and therapeutic strategies, a possible effect of new carcinogens in the industrial environment, and longer life span allowing another primary cancer to develop. Among those with multiple primary malignancies, double cancer is commonly seen, while triple cancers occur in 0.5% of patients, and quadruple or quintuple cancers occur in only less than 0.1% of the population.

BACKGROUND： An increasing number of studies have shown the effects of aging in basic cognitive processing and higher cognitive functions using functional magnetic resonance imaging （fMRI）. However, little is known about the aging effects in diverse cognitive abilities, such as spatial learning and reasoning. OBJECTIVE： To investigate the effect of aging on spatial cognitive performance and regional brain activation based on fMRI. DESIGN, TIME, AND SETTING： A block design for fMRI observation. This study was performed at the fMRI Laboratory, Brain Science Research Center, Korea Advanced Institute of Science and Technology from March 2006 to May 2009. PARTICIPANTS： Eight right-handed, male, college students in their 20s （mean age 21.5 years） and six right-handed, male, adults in their 40s （mean age 45.7 years）, who graduated from college, participated in the study. All subjects were healthy and had no prior history of psychiatric or neurological disorders. METHODS： A spatial task was presented while brain images were acquired using a 3T fMRI system （ISOL Technology, Korea）. The spatial tasks involved selecting a shape that corresponded to a given figure using four examples, as well as selecting a development figure of a diagram. MAIN OUTCOME MEASURES： The accuracy rate （number of correct answers/total number of items x 100%） of spatial tasks was calculated. Using the subtraction procedure, the activated areas in the brain during spatial tasks were color-coded by T-score. The double subtraction method was used to analyze the effect of aging between the two age groups （20s versus 40s）. RESULTS： The cerebellum, occipital lobe, parietal lobe, and frontal lobe were similarly activated in the two age groups. Increased brain activations, however, were observed in bilateral parietal and superior frontal lobes of the younger group. Sequence-related amplified polymorphism（SRAP） is a novel molecular marker technique designed to amplify open reading frames（ORFs） . Here,for the first time,SRAP was used to identify mulberry germplasm. Twenty-three mulberry accessions were screened using SRAP techniques,with 12 primer combinations selected for their reproducibility and polymorphism. Out of 83 PCR fragments scored,59（71.1%） were polymorphic,with an average of 4.9 polymorphic bands and 6.9 bands per primer combination. Mean gene diversity and PIC were 0.1611 and 0.1353,respectively. The genetic similarity coefficient ranged from 0.6905 to 0.9524,with an average of 0.8330. The phylogenetic tree was obtained using the UPGMA method using the total number of amplified SRAP fragments. The results from cluster analysis were in general agreement with our morphologic classification.
