Predictors of pain resolution after varicocelectomy for painful varicocele

Varioocelectomny is a management option for patients with painful varicocele. In this study, we assessed the effectiveness of varicocelectomy for painful varicocele and examined the factors that might be predictive of outcome. All patients who underwent a varicocelectomy for pain between February 2007 and July 2009 were includecL A review of patient medical records was conducted; patient age,body mass index （BMI）, uede, iocatien of the vericocele, tasticular volume, duration and quality of the pain （dull, draaing, throbbing or sharp） and surgical technique （ingulnal versus subinguinal） were documented. All parameters were compared with the rasolution of pain （complete, partial or failure）. We followed up on 53 of 104 patients （51.0%）. Complete postopanntive resolution of pain was reported by 28 patients （52.8%）, whereas 22 （41.5%） reported partial resolution, Only three patients （5.7%） mpaded failure. No relationship was observed between postoperative pain resolution and age, BMI, grade of varicoceio, location of varicocele, ipsilateral testicular, quality of pain or surgical technique. The duration of pain before surllery was the only factor that correlated with postopaative pain resolution （univariate, P=-0.004; multivariate, P=0.002）. Our rasults indicatethat varicocelectomy isan effective treatmentfar painful varicocele in properly selected patients, and that duration of pain before surgery may be predictive of outcome.

Bi_(2)Te_(3)-based materials have drawn much attention from the thermoelectric community due to their excellent thermoelectric performance near room ***,the stability of existing n-type Bi_(2)(Te,Se)_(3)materials is still low due to the evaporation energy of Se(37.70 kJ mol^(-1))being much lower than that of Te(52.55 kJ mol^(-1)).The evaporated Se from the material causes problems in interconnects of the module while degrading the ***,we have developed a new approach for the high-performance and stable n-type Se-free Bi_(2)Te_(3)-based materials bymaximizing the electronic transport while suppressing the phonon transport,at the same *** generated FeTe_(2)nanoinclusions within the matrix during the melt-spinning and subsequent spark plasma sintering is the key to simultaneous engineering of the power factor and lattice thermal *** nanoinclusions change the fermi level of the matrix while intensifying the phonon scattering via *** a fine-tuning of the fermi level with Cu doping in the n-type Bi_(2)Te_(3)-0.02FeTe_(2),a high power factor of∼41×10^(-4)Wm^(-1)K^(-2)with an average zT of 1.01 at the temperature range 300-470 K are achieved,which are comparable to those obtained in n-type Bi_(2)(Te,Se)_(3)*** proposed approach enables the fabrication of high-performance n-type Bi_(2)Te_(3)-based materials without having to include volatile Se element,which guarantees the stability of the ***,widespread application of thermoelectric devices utilizing the n-type Bi_(2)Te_(3)-based materials will become possible.

Design of a novel 50kW fast charger for electric vehicles

A novel 50 kW fast charger was proposed for electric vehicles. The proposed fast charger is divided into two main sections an AC-DC converter performing a PFC function and a DC-DC converter performing a charging function. A transformer including leakage inductances was used in the AC-DC converter in order to obtain isolation and inductance. A series-connection topology was used in the DC-DC converter between the DC-bus and outlet. This topology enables high power conversion efficiency up to 95% for the DC-DC converter. In order to reduce the impact of the 50 kW charging on the AC grid, the proposed fast charger system includes a buffering battery unit between the two main power conversion units. This leads to reductions in the power installation costs of power companies and to improvements in the power quality were verified through simulations and experimental results. on the AC grid. High rigidity twenty-high Sendzimir mills （ZRMs） are widely used for rolling stainless steels, silicon sheets, etc. A ZRM uses a small diameter work roll to produce massive rolling forces. Since a work roll with a small diameter can be bent easily, strips often have complex shapes with mixed quarter and deep edge waves in the shape of plates. In order to solve this problem, fuzzy neural network controls are generally used for shape： recognition in ZRM control systems. Among various neural network types, the multi-layer perceptron （MLP） is typically used in current ZRMs. However, an MLP causes the loss of a large amount of shape recognition data. To improve the shape recognition per- formance of ZRM control systems, echo state networks （ESNs） are proposed to be used. Through simulation re- sults, it is found that shape recognition performance could be improved using the proposed ESN method.

Prediction of Cold and Heat Patterns Using Anthropometric Measures Based on Machine Learning

Objective： To examine the association of body shape with cold and heat patterns, to determine which anthropometric measure is the best indicator for discriminating between the two patterns, and to investigate whether using a combination of measures can improve the predictive power to diagnose these patterns. Methods： Based on a total of 4,859 subjects （3,000 women and 1,859 men）, statistical analyses using binary logistic regression were performed to assess the significance of the difference and the predictive power of each anthropometric measure, and binary logistic regression and Naive Bayes with the variable selection technique were used to assess the improvement in the predictive power of the patterns using the combined measures. Results： In women, the strongest indicators for determining the cold and heat patterns among anthropometric measures were body mass index （BMI） and rib circumference; in men, the best indicator was BMI. In experiments using a combination of measures, the values of the area under the receiver operating characteristic curve in women were 0.776 by Naive Bayes and 0.772 by logistic regression, and the values in men were 0.788 by Naive Bayes and 0.779 by logistic regression. Conclusions： Individuals with a higher BMI have a tendency toward a heat pattern in both women and men. The use of a combination of anthropometric measures can slightly improve the diagnostic accuracy. Our findings can provide fundamental information for the diagnosis of cold and heat patterns based on body shape for personalized medicine.

Variability and Risk Analysis of Hong Kong Air Quality Based on Monsoon and El Nino Conditions

This study presents an exploratory analysis aimed at improving understanding of the variability of Hong Kong air quality associated with different climate conditions. Significantly negative correlations were found when Nifio 3 led particulate matter 〈10 p.m PM10） and NO2 by 2 3 months over the Hong Kong territory, while the other pollutants （e.g., Oa, SO2） showed modest correlations. A significant decreasing trend in visibility was observed during the autumn and winter, which has potential implications for the air-quality degradation and the endangerment of human health in Hong Kong. In an E1 Nifio summer, the visibility was relatively better, while visibility in other seasons was diminished. On the other hand, in La Nifia events, significant changes occurred in visibility in winter and autumn. Air pollution indices were less sensitive to the South China Summer Monsoon （SCSM）, but a relatively high correlation existed between the East Asian Winter Monsoon （EAWM） and air pollutants. Rainfall was lower during most of the strong EAWM years compared to the weak years. This result suggests that the pollutants that accumulate in Hong Kong are not easy to wash out, so concentrations remain at a higher level. Finally, based on the conditional Air Pollution Index （API） risk assessment, site-specific vulnerabilities were analyzed to facilitate the development of the air-quality warning systems in Hong Kong.
