The development of 3d spiral microfluidics has opened new avenues for leveraging inertial focusing to analyze small fluid volumes,thereby advancing research across chemical,physical,and biological *** traditional straight microchannels rely solely on inertial lift forces,the novel spiral geometry generates dean drag forces,eliminating the necessity for external fields in fluid ***,fabricating 3d spiral microfluidics remains a labor-intensive and costly endeavor,hindering its widespread ***,conventional lithographic methods primarily yield 2d planar devices,thereby limiting the selection of materials and geometrical *** address these challenges,this work introduces a streamlined fabrication method for 3d spiral microfluidic devices,employing rotational force within a miniaturized thermal drawing process,termed as *** innovation allows for rapid prototyping of twisted fiber-based microfluidics featuring versatility in material selection and heightened geometric *** validate the performance of these devices,we combined computational modeling with microtomographic particle image velocimetry(uTPiM)to comprehensively characterize the 3d flow *** results corroborate the presence of a steady secondary flow,underscoring the effectiveness of our *** 3d spiral microfluidics platform paves the way for exploring intricate microflow dynamics,with promising applications in areas such as drug delivery,diagnostics,and lab-on-a-chip systems.

BACKGROUNd Screening for iron deficiency anemia(IdA)is important in managing pediatric patients with inflammatory bowel disease(IBd).Concerns related to adverse reactions may contribute to a reluctance to prescribe intravenous(IV)iron to treat IdA in this *** To track the efficacy and safety of IV iron therapy in treating IdA in pediatric IBd patients admitted to our *** A longitudinal observational cohort study was performed on 236 consecutive pediatric patients admitted to our tertiary IBd care center between September 2017 and december 2019.92 patients met study criteria for IdA,of which 57 received IV iron,17 received oral iron,and 18 were discharged prior to receiving iron *** Patients treated with IV iron during their hospitalization experienced a significant increase of 1.9(±0.2)g/dL in mean(±SE)hemoglobin(Hb)concentration by the first ambulatory follow-up,compared to patients who received oral iron 0.8(±0.3)g/dL or no iron 0.8(±0.3)g/dL(P=0.03).One out of 57(1.8%)patients that received IV iron therapy experienced an adverse *** Our findings demonstrate that treatment with IV iron therapy is safe and efficacious in improving Hb and iron levels in pediatric patients with IdA and active IBd.

Introduction: Vaginal birth after cesarean (VBAC) plays an essential role in lowering cesarean rates. despite endorsement, trial of labor after cesarean (TOLAC) attempt rates remain low, in part due to fear of lawsuits. Zavanelli maneuver is a last resort procedure in the management of shoulder dystocia. We discuss a case of a woman determined to have a vaginal birth after her prior birth was complicated by shoulder dystocia requiring a Zavanelli maneuver. Her physicians were reluctant to allow her a TOLAC given her prior obstetric history. Case: A 34-year-old para 1 with prior cesarean delivery due to shoulder dystocia that required Zavanelli maneuver presents determined to pursue VBAC in her current pregnancy. She considered her delivery route options and addressed her modifiable risk factors. She consulted with multiple perinatologists who agreed that a TOLAC was reasonable, however she had to travel more than 70 miles (from Pennsylvania to New Jersey) to find an obstetrical practice and hospital willing to consider VBAC. She transferred care and the remainder of her prenatal course was uncomplicated. She went into labor at 41 weeks and had a successful VBAC without complication. In a thank you letter to her obstetrician, she described her birth experience as euphoric. Conclusion: This case illustrates how a woman’s choice of delivery route may be impacted by fear of litigation. Local providers focused on her prior delivery instead of her overall improved risk profile. delivery route decisions should be based on a thorough evaluation of all risk factors and individualized to meet the reproductive goals of each woman. .

Glioblastoma multiforme(GBM) is the most common malignant primary brain tumor with a survival prognosis of 14-16 mo for the highest functioning patients. despite aggressive, multimodal upfront therapies, the majority of GBMs will recur in approximately six months. Salvage therapy options for recurrent GBM(r GBM) are an area of intense research. This study compares recent survival and quality of life outcomes following Gamma Knife radiosurgery(GKRS) salvage therapy. Following a Pub Med search for studies usingGKRS as salvage therapy for malignant gliomas, nine articles from 2005 to July 2013 were identified which evaluated rG BM treatment. In this review, we compare overall survival following diagnosis, overall survival following salvage treatment, progression-free survival, time to recurrence, local tumor control, and adverse radiation effects. This report discusses results for rG BM patient populations alone, not for mixed populations with other tumor histology grades. All nine studies reported median overall survival rates(from diagnosis, range:16.7-33.2 mo; from salvage, range:9-17.9 mo). Three studies identified median progression-free survival(range:4.6-14.9 mo). Two showed median time to recurrence of GBM. Two discussed local tumor control. Six studies reported adverse radiation effects(range:0%-46% of patients). The greatest survival advantages were seen in patients who received GKRS salvage along with other treatments, like resection or bevacizumab, suggesting that appropriately tailored multimodal therapy should be considered with each rG BM patient. However, there needs to be a randomized clinical trial to test GKRS for rG BM before the possibility of selection bias can be dismissed.

The conversion of biomass into sugar platform compounds is very important for the biorefinery *** is essential to the biomass of the sugar platform,however,the lignin obtained by pretreatment,as a key part of lignocellulose,generally has a passive effect on the enzymatic hydrolysis of cellulose into *** this study,p-TsOH(p-toluenesulfonic acid),dES(deep eutectic solvent)and CAOSA(cooking with active oxygen and solid alkali)pretreatment ways were used to fraction lignin from bamboo *** CAOSA treatment,the hydrolysis efficiency of the pulp was 95.57%.Moreover,the effect of different treatment methods on lignin properties was studied and the promotion effect of lignin was investigated by adding it to the cellulose enzymatic hydrolysis *** this work,the results showed that CAOSA-extracted lignin with lower d(1.31-1.25)had a better adsorption effect on the enzyme protein.p-TsOH-extracted lignin with a larger S/G ratio enhanced the inhibition of enzymatic *** addition,the presence of-COOHs in lignin could reduce its inhibitory effect on cellulose saccharification.
