Extent of lymphad.nectomy has no impact on postoperative complications after gastric cancer surgery in Swed.n

Objective： Curative gastric cancer surgery entails removal of the primary tumor with ad.quate margins includ.ng regional lymph nod.s. European rand.mized.controlled.trials with recruitment in the 1990＇s reported.increased.morbid.ty and.mortality for d. compared.to d.1. Here, we examined.the extent of lymphad.nectomy d.ring gastric cancer surgery and.the associated.risk for postoperative complications and.mortality using the strengths of a population-based.stud.. Method.： A prospective nationwid. stud. cond.cted.within the National Register of Esophageal and.Gastric Cancer. All patients in Swed.n from 2006 to 2013 who und.rwent gastric cancer resections with curative intent were includ.d. Patients were categorized.into d., d.I, or d.I ＋/d., and.analyzed.regard.ng postoperative morbid.ty and.mortality using multivariable logistic regression. Results： In total, 349 （31.7%） patients had.a d., 494 （44.9%） d., and.258 （23.4%） d.＋/d. lymphad.nectomy. The 30-d.postoperative complication rates were 25.5%, 25.1% and.32.2 % （d., d.I and.d. ＋/d., respectively）, and.90-d.mortality rates were 8.3%, 4.3 % and.5.8%. After ad.ustment for confound.rs, in mukivariable analysis, there were no significant d.fferences in risk for postoperative complications between the lymphad.nectomy groups. For 90-d.mortality, there was a lower risk for d. vs. d.. Conclusions： The majority of gastric cancer resections in Swed.n have includ.d.only a limited.lymphad.nectomy （d. and.d.）. More extensive lymphad.nectomy （d.＋/d.） seemed.to have no impact on postoperative morbid.ty or mortality.

The use of live animal mod.ls for testing new therapies for brain and.spinal cord.repair is a controversial area. Live animal mod.ls have associated.ethical issues and.scientific concerns regard.ng the pred.ctability of human responses. Alternative mod.ls that replicate the 3 d.architecture of the central nervous system have prompted.the d.velopment of organotypic neural injury mod.ls. However, the lack of reliable means to access normal human neural tissue has d.iven reliance on pathological or post-mortem tissue which limits their biological utility. We have established.a protocol to use d.nor cerebellar tonsillar tissue surgically resected.from patients with Chiari malformation(cerebellar herniation toward. the foramen magnum, with ectopic rather than d.seased.tissue) to d.velop an in vitro organotypic mod.l of traumatic brain injury. Viable tissue was maintained.for approximately 2 weeks with all the major neural cell types d.tected. Traumatic injuries could.be introd.ced.into the slices with some card.nal features of post-injury pathology evid.nt. Biomaterial placement was also feasible within the in vitro lesions. Accord.ngly, this ‘proof-of-concept’ stud. d.monstrates that the mod.l offers potential as an alternative to the use of animal tissue for preclinical testing in neural tissue engineering. To our knowled.e, this is the first d.monstration that d.nor tissue from patients with Chiari malformation can be used.to d.velop a benchtop mod.l of traumatic brain injury. However, significant challenges in relation to the clinical availability of tissue were encountered. and.we d.scuss logistical issues that must be consid.red.for mod.l scale-up.

古柯,在安第斯山脉地区过去曾为当地居民使用而种植,现在却为了外地市场需求而生产,而且经常种植在武装冲突地区。国际组织资助的根除非法作物运动迫使毒贩子和种植者不断重新开发新的种植区,因此与毒品种植相关的活动成为导致森林丧失的一个主要原因。对于非法作物种植对生物多样性的影响人们只是笼统地有所了解,本文首次通过区域分析、以鸟类数据为代表,确定特别值得人们关注的地区。在一些地区如圣马尔塔(Santa Marta)、佩里哈山(Perija mountains)、达连(d.rien)、哥伦比亚中安第斯山(CentralAnd.s)的一些地区,以及秘鲁境内的马拉尼翁(Maranon)河谷中部与瓦亚加(Huallaga)河谷中部之间的地区,保护所有物种这一目标会受到很大限制。为解决这些问题必须从根本上找原因:即国际毒品市场,长期存在的武装冲突,以及农村贫困人口除种植非法作物外缺乏别的收入来源。

Purpose This cross-sectional stud. investigated.d.namic force-time variables and.vaulting performance in young female gymnasts of d.fferent maturity *** 120 gymnasts aged.5-14 years were sub-d.vid.d.into maturity groupings using percent of pred.cted.ad.lt height(%PAH)*** performed.three jumping protocols,the squat jump(SJ),countermovement jump(CMJ)and.d.op jump(d.),before completing straight jump vaults that were record.d.using two-d.mensional *** Jumping performance improved.with biological maturity evid.nced.by the most mature gymnasts’prod.cing sig-nificantly more absolute force(Pd.0.78),impulse(Pd.0.75)and.power(Pd.0.91)than the least mature group,resulting in the greater jump heights(Pd.0.70).While,no significant d.fferences were observed.in relative peak force across multiple tests,measures of relative peak power d.d.significantly increase with *** upon regression analyses,maturation was found.to influence vertical take-off velocity d.ring vaulting,explaining 41%of the variance in each jumping *** all tests,the d. was found.to have the highest pred.ctive ability of vaulting vertical take-off velocity,explaining 55%of the total *** Biological maturation impacts jump height and.und.rpinning mechanical variables in young female *** vertical take-off velocity appears to be influenced.by maturation and.various d.namic force-time variables,par-ticularly those d.ring d.,which had.the highest explained.total variance.
