Growth hormone Replacement Therapy in Adult Growth hormone Deficiency and Risk of Cancer: A Meta-Analysis

The growth hormone (GH) replacement therapy in adult growth hormone deficiency (AGHD) is now well developed, nevertheless, the safety of GH replacement, especially the incidence of cancer in these patients remains to be further clarified. To summarize the evidence on the safety of using GH in AGHD, we conduct this meta-analysis to assess the relationship between the risk of cancer and GH replacement therapy. Randomized controlled trials and cohort studies involved in GH therapy for AGHD were selected. Meta-analysis was performed and risk ratio (RR) was pooled with 95% confidence interval (CI) to investigate the relationship between GH replacement and the risk of cancer. The result indicated that there was no evidence to draw a conclusion that GH replacement therapy will increase the risk of cancer (P = 0.001, RR = 0.77, 95% CI [0.65, 0.90]). Meanwhile, according to the calculated analysis, the replacement therapy might even reduce the risk of cancer. Furthermore, subgroup analysis demonstrated that there was no correlation between replacement therapy of GH and the risk of cancer both in prospective and retrospective cohort design research, and in prospective group, the risk of cancer even decreased (P = 0.0002, RR = 0.71, 95%CI [0.59, 0.85]). In conclusion, our study corroborates evidence from previous studies showing that GH replacement therapy in AGHD patients would not increase the risk of cancer;instead, it might be even decrease cancer risk. The results suggested that GH replacement therapy in AGHD patients was safe.

背景:烧创伤、糖尿病溃疡和静脉性溃疡等各种急慢性因素极易影响人体最大的器官——皮肤,可造成不同程度的皮肤缺损。深度皮肤缺损的面积超过一定的范围,就会难以自愈,给患者及其家庭带来巨大的痛苦和沉重的经济负担。因此,治疗深度皮肤缺损的关键就是尽早封闭创面。最常见的临床治疗方法是自体皮肤移植,但是,感染风险、供皮区的匮乏和二次损伤等限制了自体皮肤移植的应用。真皮替代物是真皮再生的模板,在皮肤重建/再生中至关重要,它的出现为临床上深度皮肤缺损的治疗提供了一条新的途径。创面的修复主要由炎症细胞、血管内皮细胞、角质形成细胞、成纤维细胞、细胞外基质、生长因子及细胞因子等共同参与,其过程大致可以分为炎症期,增生期和重塑期,这三个过程相互重叠。皮肤损伤后,真皮成纤维细胞作为重要的修复细胞被激活、增殖、迁移、分化并合成多种细胞外基质(包括胶原和多糖类等)及分泌多种细胞因子,这种生物学行为被认为是创面修复的重要部分。基于前期的研究发现,采用具有良好生物相容性、可降解性的多孔结构胶原-壳聚糖(CCS)支架作为真皮支架材料与PLGA经编网(PLGAm)整合成强化型真皮支架,具有较高的机械强度,能够在体内诱导组织再生功能,具有巨大的临床应用潜在前景。目的:本项目在前期研究的基础上,体外部分通过在强化型聚乳酸-羟基乙酸编织网胶原壳聚糖支架(PLGAm/CCS)上接种人真皮成纤维细胞,与胶原壳聚糖支架(CCS)作对比,在体外环境条件下,考察PLGAm/CCS支架对真皮成纤维细胞生物学行为的影响。体内研究,将PLGAm/CCS支架移植到动物创面,考察真皮替代物在体内诱导组织再生情况及转归情况,为进一步研究真皮替代物的临床转化提供理论基础。方法:(1)强化型真皮再生模板的制备:将PLGA网置于模具中,注入0.5%胶原-壳聚糖溶液(质量比为9:1),4℃孵育过夜,-20℃下冷冻2h,然后冻干24h,得到厚度为2mm的PLGAm/CCS多孔支架;(2)体外细胞接种实验:随后将原代提取的人真皮成纤维细胞接种到PLGAm/CCS支架上,通过体外ELISA检测α-羟脯氨酸(α-Hp)和Ⅰ型胶原羧基末端肽(CICP)量的变化观察成纤维细胞的胶原合成能力;通过激光共聚焦(CLSM)观察PLGAm/CCS支架对成纤维细胞的影响;采用MTT法检测PLGAm/CCS支架对成纤维细胞的毒性作用;(3)体内实移植PLGAm/CCS支架入SD大鼠体内,通过HE染色和免疫组织化学法观察移植PLGAm/CCS支架对大鼠创面的影响;(4)采用RT-qPCR和蛋白质免疫印迹法(WB)观察移植PLGAm/CCS支架后大鼠新生组织中CD31、Col Ⅰ、Col Ⅲ和Elastin的RNA和蛋白质表达量的影响。结果:(1)PLGAm/CCS支架具有良好的机械强度,和合适的三维多孔结构;(2)PLGAm/CCS和CCS支架接种真皮成纤维细胞后,其上清液中α-Hp和CICP含量随培养时间的延长而升高,且两组支架间比较有显著性差异(P<0.05),表明PLGAm/CCS支架能促进真皮成纤维细胞的胶原合成与分泌;(3)通过CLSM观察到PLGAm/CCS支架中的成纤维细胞含量明显高于其他组支架(P<0.05),结果表明,PLGAm/CCS支架能促进成纤维细胞的黏附和生长;(4)MTT结果显示,PLGAm/CCS支架的生物相容性良好,PLGA网的存在对细胞功能基本没有影响;(5)体内移植实验,HE染色和免疫组织化学法结果显示,PLGAm/CCS支架能够与周围组织紧密结合,新生血管的数目显著增多(P<0.05),且胶原排列有序,更有利于组织再生;(6)RT-qPCR和WB结果显示,术后5d后,PLGAm/CCS支架组大鼠组织中CD31、ColⅠ、ColⅢ和Elastin的mRNA和蛋白质表达量均显著高于CCS支架组(P<0.05)。结果表明,PLGAm/CCS支架可以诱导细胞快速长入、促进血管新生,促使新生组织形成。结论:强化型真皮再生模板(PLGAm/CCS)的机械强度高,具有良好的生物相容性,对真皮成纤维细胞的增殖能力基本没有影响且能促进人真皮成纤维细胞合成分泌胶原等细胞外基质。体内移植PLGAm/CCS支架促进血管新生,以及胶原纤维的生成和沉积,同时使胶原高度有序排列,促进创面修复再生。PLGAm/CCS具有潜在的临床转化价值。

Acute lower gastrointestinal bleeding from a dieulafoy lesion proximal to the anorectal junction post-orthotopic liver transplant

A 67-year-old woman underwent an orthotopic liver transplantation for end stage liver disease secondary to chronic autoimmune hepatitis. She developed sudden massive hematochezia on post-operative day 23 with hemodynamic compromise. The source of hemorrhage was found at colonoscopy after careful irrigation and inspection to be a dieulafoy lesion situated just proximal to the anorectal junction. Hemostasis was achieved with epinephrine injection and thermal coagulation.

Previously we have designed and implemented new image browsing facilities to support effective offiine image contents on mobile devices with limited capabilities： low bandwidth, small display, and slow processing. In this letter, we fulfill the automatic production of cartoon contents fitting small-screen display, and introduce a clustering method useful for various types of cartoon images as a prerequisite stage for preserving semantic meaning. The usage of neural networks is to properly cut the various forms of pages. Texture information that is useful for grayscale image segmentation gives us a good clue for page layout analysis using the multilayer perceptron （MLP） based x-y recursive algorithm. We also automatically frame the segment MLP using agglomerative segmentation. Our experimental results show that the combined approaches yield good results of segmentation for several cartoons.

Real-world comparison of non-vitamin K antagonist oral anticoagulants and warfarin in Asian octogenarian patients with atrial fibrillation

Background The efficacy and safety of non-vitamin K antagonist oral anticoagulants （NOACs） and warfarin in Asian octogenarian atrial fibrillation （AF） patients have not been established in a real-world setting. We aimed to evaluate the efficacy and safety of NOACs and warfarin in Korean octogenarian patients. Methods A total of 293 consecutive patients aged 〉 80 years with non-valvular AF who had taken either NOACs 048 cases, 50.5%） or warfarin （145 cases, 49.5%） were retrospectively reviewed. The efficacy outcome was the com- posite of stroke or systemic embolism. The safety outcome was major bleeding. Results The follow-up duration was 375 patient-years （ 172 patient-years with NOACs and 203 patient-years with warfarin）. Patients on NOACs were slightly older （P = 0.006） and had slightly higher HAS-BLED scores （P = 0.034）. The efficacy of both anticoagulants was high （1.16% for NOACs vs. 2.98% for warfarin per 100 pa- tient-years, P = 0.46）. The safety outcome was relatively high in both NOACs and warfarin groups （8.96% vs. 12.46%, P = 0.29）. The effi- cacy and safety outcomes tended to decrease non-significantly in low dose NOACs than in common dose NOACs or warfarin （0.85% vs. 1.84% vs. 2.98% in efficacy outcome, P = 0.69; and 6.97% vs. 13.29% vs. 12.46% in safety outcome, P = 0.34）. Conclusions NOACs were highly effective for prevention of stroke or systemic embolism in Asian octogenarian AF patients. however, major bleeding occurred excessively high in both anticoagulant groups. Further study is required on the optimal anticoagulant regimen in octogenarian population.

Second to fourth digit ratio： a predictor of adult lung function

Sex and sex hormones play a major role in lung physiology. It has been proposed that the ratio of the second to fourth digits （digit ratio） is correlated with fetal sex hormones. We therefore hypothesized that digit ratio might help predict lung function. We investigated the relationship between digit ratio and pulmonary function test （PFT） findings. A total of 245 South Korean patients （162 male, 83 female） aged from 34 to 90 years who were hospitalized for urological surgery were prospectively enrolled. Before administering the PFTs, the lengths of the second and fourth digits of the right hand were measured by a single investigator using a digital Vernier caliper. In males （n = 162）, univariate and multivariate analysis using linear regression models showed that digit ratio was a significant predictive factor of forced vital capacity （FVC） and forced expiratory volume in I second （FEV1）（FVC： r = 0.156, P = 0.047; FEVI： r = 0.160, P = 0.042）. In male ever-smokers （n = 69）, lung functions （FVC and FEV1） were correlated with smoking exposure rather than digit ratio. In female never-smokers （n = 83）, lung functions （FEV1 and FEVI/FVC ratio） were positively correlated with digit ratio on univariate analysis （FEVI： r = 0.242, P = 0.027; FEVI/FVC ratio： r = 0.245, P = 0.026）. Patients with lower digit ratios tend to have decreased lung function. These results suggest that digit ratio is a predictor of airway function.
