Effect of Radiant Energy Vacuum on Physical and Microbial Properties of Beef Jerky

To test the feasibility of radiant energy vacuum （REV） dehydration, a technology that couples microwave cooking with vacuum pressure, on beef jerky preparation, the physical and microbial properties of the final jerky product was compared to conventional preparation methods. Physical characteristics assessed using puncture and shear tests of samples prepared using REV dehydration compared to the traditional method were not statistically different （P 〈 0.05）. Moisture content and water activity levels were also very similar between the two products, To test microbiological quality, samples were homogenized in a stomacher and a variety of 3 M Petrifilms were used to evaluate the microbial load. Raw beef harboured low numbers of microbes, but the post-marination pasteurization/smoking step used in both treatments eliminated all culturable microorganisms tested for. To further investigate the ability of REV dehydration to kill microbes, samples were spiked with Listeria innocua after the pasteurization/smoking step but prior to REV dehydration. Samples were taken at different time intervals for microbial enumeration, and a decimal reduction time of 1 min was calculated, with 99.99% of 1.98 × 10^7 CFU g^-1 Listeria being killed in five min. Improved drying times were observed for jerky samples prepared using the REV method offering potential energy savings during jerky preparation.

Osteomyelitis is a devastating disease caused by microbial infection of bone. While the frequency of infection following elective orthopedic surgery is low, rates of reinfection are disturbingly high. Staphylococcus aureus is responsible for the majority of chronic osteomyelitis cases and is often considered to be incurable due to bacterial persistence deep within bone. Unfortunately, there is no consensus on clinical classifications of osteomyelitis and the ensuing treatment algorithm. Given the high patient morbidity,mortality, and economic burden caused by osteomyelitis, it is important to elucidate mechanisms of bone infection to inform novel strategies for prevention and curative treatment. Recent discoveries in this field have identified three distinct reservoirs of bacterial biofilm including: Staphylococcal abscess communities in the local soft tissue and bone marrow, glycocalyx formation on implant hardware and necrotic tissue, and colonization of the osteocyte-lacuno canalicular network(OLCN) of cortical bone. In contrast, *** intracellular persistence in bone cells has not been substantiated in vivo, which challenges this mode of chronic osteomyelitis. There have also been major advances in our understanding of the immune proteome against S. aureus, from clinical studies of serum antibodies and media enriched for newly synthesized antibodies(MENSA), which may provide new opportunities for osteomyelitis diagnosis, prognosis, and vaccine development. Finally, novel therapies such as antimicrobial implant coatings and antibiotic impregnated 3D-printed scaffolds represent promising strategies for preventing and managing this devastating disease. Here, we review these recent advances and highlight translational opportunities towards a cure.

Overprescription of opioid analgesia is common following ambulatory Otolaryngology—Head and Neck surgery procedures: A multicenter study

Background: The rise in the use of prescription opioids for postoperative analgesia within surgery has mirrored an increased trend of opioid-related morbidity within Canada and the United States. This study prospectively studied daily pain levels and medication requirements postoperatively in patients undergoing elective Otolaryngology—Head and Neck surgery ***: Patients were asked to prospectively document their pain level and medication use daily for 7 days postoperatively. A final survey was used to quantify unused medication left at home and clarify each patient's disposal plan. We included patients undergoing elective outpatient or short stay surgeries from three tertiary care centers in Toronto, Ontario from September 2016 to September 2017. Previous opioids users or patients suffering from chronic pain were ***: A final cohort of 56 eligible adult patients were included in the study. The most common procedures were thyroidectomy (n = 19), endoscopic sinus surgery (n= 10), tympanoplasty/ossiculoplasty (n= 7), and cochlear implant (n = 5). Most patients received a prescription for acetaminophen/codeine (n = 29, 51.8%) or acetaminophen/oxycodone (n = 22, 39.3%) and used on average 29% of their initial prescription. Patients most commonly opted to keep their unused narcotics at home (n = 23, 41%). A total of 710 tablets of narcotics were overprescribed in our study population, 351 of which were kept in patients'' home for future ***: There is a clear tendency to overestimate postoperative pain resulting in significant overprescription of opioids among Otolaryngologists.

The relationship of high-intensity cross-training with arterial stiffness

Background'.Central arterial stiffness is a cardiovascular risk factor that can be readily affected through engagement in physical exercise training,with resistance and aerobic exercise having disparate *** the growing popularity of high-intensity cross-training(HICT),little is currently known about the effects of this mixed modality exercise stimulus on arterial ***,the purpose of this study was to characterize the arterial stiffness of habitual HICT participants *** active and sedentary controls using a cross-sectional ***'.A total of 30 participants were recruited:10 middle-aged long-term participants of HICT(CrossFit)and 20 age,sex,and height matched controls(10 recreationally active,10 sedentary).Central and peripheral pulse wave velocities were measured for the carotid-femoral and femoral-dorsalis pedis arterial *** fitness(maximal oxygen uptake,VChmax)was measured and typical exercise participation rates were self-reported for each ***'.HICT participants manifested central pulse wave velocity(PWV)(5.3±1.0 m/s,mean±SD)and VChmax(43±6 mL/kg/min)values nearly identical to active *** active groups had significantly better values than sedentary controls(7.1±1.0 m/s,p≤0.001;and 32±7 mL/kg/min,p=0.01).No differences were observed in peripheral PWV between ***'.Habitual participation in HICT exercise was not associated with increased central nor peripheral arterial ***-term HICT participants presented with similar fitness and arterial stiffness as compared with participants who practiced traditional aerobic *** to sedentary living,HICT may offer musculoskeletal and cardiovascular health benefits without negatively impacting arterial stiffness.
