Background: Human African trypanosomiasis (HAT) occurs in three historical foci in the Central African Republic. Objective: To describe the morbi-mortality of childhood HAT in rural Central African Republic with the aim of early management. Methods: Descriptive and analytic cross-sectional study conducted from January 1, 2017 to March 30, 2018 at nola prefectural hospital. This study was included all children seen as outpatients and/or hospitalized in the pediatric wards, in whom the diagnosis of HAT was confirmed. Statistical analysis was performed using Epi-info software. Results: Forty children were included from Bilolo (60%), nola (30%) and Salo (10%). The sex ratio was 0.66 with a median age of 8.65 ± 12.48 years. Fever (82.5%), nocturnal insomnia (75%), daytime somnolence (67.5%), headache (65%), polyarthralgia (62.5%), convulsions 52.5% (n = 21), tremor (27.5%), trypanidism (7.5%) and delirium (2.5%) were the main functional signs. Examination signs were adenopathy (52.5%), paresthesia (30%), decreased cutaneous-abdominal reflexes (25%) and osteoarticular reflexes (17.5%), hyperesthesia (20%), extrapyramidal hypertonia (15%) and depression (2.5%). The children were in the lymphatic-blood phase in 65% of cases, and meningoencephalitis in 35%. Pentamidine was administered in 65% of cases. Four children died and 6 had neurological sequelae. There was an association between age under 5, Bilolo’s focus, the children’s history and the severity of the disease. Age under 5, gender, household, children’s activity, history and occurrence of sequelae were also associated. Conclusion: HAT remains a permanent threat to Central African children. Any clinical presentation combining long-term infectious signs and unexplained neurological or neuropsychological disorders must be treated with caution.

Bendamustine and rituximab as frontline therapy in extranodal marginal zone lymphoma:a single-institution experience

The addition of surface acoustic wave(SAW)technologies to microfluidics has greatly advanced lab-on-a-chip applications due to their unique and powerful attributes,including high-precision manipulation,versatility,integrability,biocompatibility,contactless nature,and rapid ***,the development of SAW microfluidic devices is limited by complex and time-consuming micro/nanofabrication techniques and access to cleanroom facilities for multistep photolithography and vacuum-based *** simplify the fabrication of SAW microfluidic devices with customizable dimensions and functions,we utilized the additive manufacturing technique of aerosol jet *** successfully fabricated customized SAW microfluidic devices of varying materials,including silver nanowires,graphene,and poly(3,4-ethylenedioxythiophene)polystyrene sulfonate(PEDOT:PSS).To characterize and compare the acoustic actuation performance of these aerosol jet printed SAW microfluidic devices with their cleanroom-fabricated counterparts,the wave displacements and resonant frequencies of the different fabricated devices were directly measured through scanning laser Doppler ***,to exhibit the capability of the aerosol jet printed devices for lab-on-a-chip applications,we successfully conducted acoustic streaming and particle concentration ***,we demonstrated a novel solution-based,direct-write,single-step,cleanroom-free additive manufacturing technique to rapidly develop SAW microfluidic devices that shows viability for applications in the fields of biology,chemistry,engineering,and medicine.

Mercury ion(Hg^(2+)),as one of the most toxic heavy metal ions,accumulates easily in the environment,which can generate potential hazards to the ecosystem and human *** effectively detect and remove Hg^(2+),we fabricated four types of carbon dots(CDs)using carboxymethyl nanocellulose as a carbon source doped with different elements using a hydrothermal *** the CDs exhibited a strong fluorescence emission,excitation-dependent emission and possessed good water ***,the four fluorescent CDs were used for Hg^(2+)recognition in aqueous solution,where the CDs-n exhibited better sensitivity and selectivity for Hg^(2+)detection,with a low limit of detection of 8.29×10^(-6)mol/*** was determined that the fluorescence quenching could be ascribed to a photoinduced charge-transfer processes between Hg^(2+)and the *** addition,the CDs-n were used as a smart invisible ink for anticounterfeiting,information encryption and ***,the CDs-n were immersed into a cellulose(CMC)-based hydrogel network to prepare fluorescent hydrogels capable of simultaneously detecting and adsorbing Hg^(2+).We anticipate that this research will open possibilities for a green method to synthesize fluorescent CDs for metal ion detection and fluorescent ink production.

Comparison of the Clinical Profile and Severity Factors of COVID-19 during the First 4 Waves in a Paediatric Population of Yaounde

Introduction: COVID-19 is an infectious disease that has been causing a global pandemic since 2019. Although clinical forms are generally less severe in children than in adults, children nevertheless present polymorphous clinical forms and severe cases that can lead to death. Objective: To describe the clinical presentations found in the different waves of COVID-19, and to highlight the different factors of severity. Materials and Methods: We conducted a cross-sectional study with retrospective and prospective data collection which lasted 7 months (from november 2021 to June 2022) and covered a study period from 6 March 2020 to 22 June 2022, i.e. 27 months. All patients aged 0 to 18 years, suspected of having COVID-19, confirmed by real-time RT-PCR or an antigenic Rapid Diagnostic Test or antibody were included. These patients were to be managed in the Mother and Child Centre of the Chantal Biya Foundation, as well as in the Specialised Centre for the Management of COVID patients, annex number II of the Yaoundé Central Hospital. The results were analysed using IBM SPSS.23 software. Results: We included 163 patients in our study. no paediatric patients were registered during the 3rd wave. The mean age of the patients in the study population was 13 ± 5 years with extremes from 15 days to 18 years. We had a female predominance with a sex ratio of 0.83. The most common comorbidity was asthma. The first wave presented mainly with respiratory symptoms such as dry cough and signs of respiratory distress. The second wave presented mainly with digestive symptoms such as diarrhoea, abdominal pain and vomiting. The fourth wave presented with EnT signs such as sore throat, and rhinorrhea. Hospital Outcome of newborns with a Health Cheque System in Comparison to Those Without

Introduction: The health cheque system is a prepayment mechanism aimed at reducing neonatal and maternal mortality through improving the management of pregnant women. The pregnant woman with the health cheque system that she pays at six thousand francs XAF (African financial community) is covered free of charge for all the care provided by the cheque system in the health facilities accredited to the health cheque project. We did a study, with objective to determine the hospital outcome of newborns with a health cheque system (HCS) compared to those without health cheque system. Method: A descriptive cross-sectional study with retrospective data collection was carried out at the ngaoundere Regional Hospital from January 2018 to September 2021. Results: During our study period, 2985 newborns were received. We saw an increase in admissions over the years, particularly in the group of newborns with the health cheque system. Comparatively, the percentage of newborns cured in the health cheque system group was 76.73% (n = 1643) versus 77.72% (n = 656) those in the non-health cheque system group. Those who died were 8.96% (n = 192) in the health cheque system group compared to 6.27% (n = 53) in the non-health cheque system group. Conclusions and Recommendations: Most patients admitted to our service have the health cheque system. We notice an increase in hospital attendance with the health cheque project. The outcome of the newborn under the health cheque system is not different from that without health cheque system. The health cheque system was successful in getting the larger number of newborns into care. The next step is to put strategies in place to keep these patients in care for the duration of hospitalization.

Paths and cycles of hypergraphs

Hypergraphs are the most general structures in discrete mathematics. Acyclic hypergraphs have been proved very useful in relational databases. new systems of axioms for paths, connectivity and cycles of hypergraphs are constructed. The systems suit the structure properties of relational databases. The concepts of pseudo cycles and essential cycles of hypergraphs are introduced. They are relative to each other. Whether a family of cycles of a hypergraph is dependent or independent is defined. An enumeration formula for the maximum number of independent essential cycles of a hypergraph is given.

Comparative Analysis of Community-Based Management of Anemia in Children before and during the COVID Pandemic

Introduction: In low-income countries, severe anemia represents a significant risk of mortality. It is generally the consequence of a lack of correct community-based management of mild and moderate anemia and its etiologies. Our study aimed to evaluate the community practice in confronting anemia by mothers of children aged 0 - 5 years old before and during the COVID-19 period. Methods: We conducted a descriptive cross-sectional study in two phases at the pediatric ward of the regional hospital of ngaoundere in Cameroon;before the period of the COVID-19 pandemic from December 2018 to May 2019 and during the pandemic period from December 2020 to May 2021. It was a volunteer sample. Were included in this study all mothers of children aged 0 - 5 years present in the pediatric ward at the time of the survey who agreed to freely answer our questionnaire. Results: Of the 152 mothers surveyed in the “Before COVID-19” group, 69% went to a health facility when they suspected their children were anemic, 20% gave grenadine juice (Red colored soda), 7% went to marabous, 2% bought street medicines and 2% used other home-made potions. Of the 92 mothers included in the “During COVID-19” group, the majority did not go to a health facility: 36% gave grenadine juice and other potions, 26% went to marabouts, 23% bought medicines from the street and only 13% went to a health facility;2% did nothing. The average Group rank is not statistically significantly different between the before and during the COVID-19 periods (p = 0.89). Conclusion: Community behavior and practice in confronting anemia deteriorated during the COVID-19 pandemic, with the majority of mothers resorting to non-effective out-of-hospital solutions. The consequence can be an increase in infant mortality. While waiting for the end of the pandemic, it is judicious and urgent to review the whole health system in Africa and especially in countries with limited resources.
