Introduction:The histological diagnosis of prostate cancer is commonly based on morphological *** presence of malignant tissue mixed with benign tissue,or the presence of carcinoma that mimics benignity may generate difficulty in the diagnostic ***,the application of immunohistochemistry contributes its diagnostic ***:To evaluate the 34βE12 marker in the detection of adenocarcinoma(ADn),atypical small acinar proliferation(ASAp),regular prostatic tissue(RPT)and regular prostatic tissue alternated by atrophy spotlights(RPTa)in transrectal biopsy guided by ultrasonography of patients with suspected prostate ***:Analysis of 34 patients who underwent ultrasound-guided transrectal biopsy with subsequent analysis by H&E staining and 34βE12 labeling for elucidation of neoplasms or diseased tissues with doubtful ***:The marker 34βE12 showed negativity in 100%of the neoplasms ADn,positivity in 100%of the benign prostatic tissues(RPT and RPTa);the patients with ASAp presented positivity(20%)and negativity(80%).The chi-square test(χ2)showed that there is an association(χ2=29.55 and p<0.0001)between the groups,that is,the 34βE12 marker has a significant value(p<0.0001)in the elucidation of patients with prostatic neoplasia and benign prostatic *** and Conclusion:With the early screening of prostate cancer in the modern era,pathologists have become increasingly challenged to diagnose small outbreaks of cancer when only a few atypical glands are present in transrectal biopsy-guided *** 34βE12 marker becomes an important tool in elucidating diagnoses such as ADn and ASAp.

Diagnostic Value between 1984 and 2018 of Transrectal Biopsy Guided by Ultrasonography after Radical Prostatectomy

Objectives—to determine correlation between GSs(Gleason scores)on needle biopsy and RP(radical prostatectomy),evaluating diagnostic tests on biopsy and RP within the last years,between 1984 and ***—analysis of 100 patients,diagnosed with PCa(prostate cancer)needle biopsy using 18-gauge needle,who underwent RP with lymphadenectomy and for which preoperative and postoperative GSs were *** group analysis used three categorization schemes for differentiation:mild,moderate and poor for the whole group and we determined SE(sensitivity),SP(specificity),PVPR(positive predictive value),negative predictive value and ***—we found that 42%of the patients had no changes between GS on biopsy and prostatectomy,while 20%were overgraded and 38%undergraded by needle *** of+1 point in GS(32%)or-1 point(17%)was the most *** patients were classified as moderately differentiated by biopsies(78 and 35%in scheme 1 and 2 or 3,respectively),while 43%of patients received an intermediate differentiation *** accuracy varied from 44 to 76%for the analysis of all three ***—there are differences in correlation between GS on biopsy and on surgical specimen,and Gleason’s graduation also depends on the experience of the *** have shown that sextant biopsies using 18-gauge and a same group of pathologists showed acceptable concordance values(42%)between the GS on biopsy and prostatectomy.

Entrepreneurship and Unemployment Cycles： A Delay Differential Equation Approach

This paper is an application of delay differential equations to entrepreneurship and unemployment. It shows that the results of empirical studies can be replicated through a simple DDE. The paper also carries out a comparative study between a solution of a DDE and the observation of cyclic behaviors. The calculated delay （and growth rate） of the entrepreneurship DDE for the US and Spain is 1.25 years （1.25）, for Ireland is 1.75 years （0.89）, and for the UK is 2 years （0.78）. The delay （and growth rate） of the unemployment DDE for the US and Ireland is 1.5 years （1.04）, for Spain is 2 years （0.78）, and for the UK is 2.5 years （0.62）.

Systemic mastocytosis: A rare cause of non-cirrhotic portal hypertension

Mastocytosis is a clonal neoplastic disorder of the mast cells(MC) that can be limited to the skin(cutaneous mastocytosis) or involve one or more extracutaneous organs(systemic mastocytosis). The clinical manifestations of mastocytosis are heterogeneous ranging from indolent disease with a long-term survival to a highly aggressive neoplasm with survival of about 6 mo. Although liver involvement in aggressive systemic mastocytosis(ASM) is relatively common, the development of portal hypertension with or without cirrhosis is rare. We report a case of ASM without skin involvement in a 72-year-old caucasian male who presented with non-cirrhotic portal hypertension based on clinical, analytical, imagiological and endoscopic findings. Given the hematological picture, the correct diagnosis was established based on ancillary tests for MC using bone marrow aspirates and biopsy. Extensive involvement of the liver and gastrointestinal tract was histologically documented. The disease progressed rapidly and severe pancytopenia and recurrent upper gastrointestinal bleeding became the dominant problem. This case illustrates the challenge in establishing a diagnosis of ASM especially when the clinical picture is atypical and without skin involvement. Gastroenterologists should consider infiltrative disease, particularly systemic mastocytosis, as a differential diagnosis in a clinical case of portal hypertension of unknown etiology.

An updated overview on the development of new photosensitizers for anticancer photodynamic therapy

Photodynamic therapy(PDT), based on the photoactivation of photosensitizers(PSs), has become a well-studied therapy for cancer. Photofrin~?, belonging to the first generation of PS, is still widely used for the treatment of different kinds of cancers; however, it has several drawbacks that significantly limit its general clinical use. Consequently, there has been extensive research on the design of PS molecules with optimized pharmaceutical properties, with aiming of overcoming the disadvantages of traditional PS, such as poor chemical purity, long half-life, excessive accumulation into the skin, and low attenuation coefficients. The rational design of novel PS with desirable properties has attracted considerable research in the pharmaceutical field. This review presents an overview on the classical photosensitizers and the most significant recent advances in the development of PS with regard to their potential application in oncology.

Ischaemic heart disease(IHD)is a major cause of morbidity and mortality *** there have been major advances in this field,these patients are still a higher risk *** such,strategies to mitigate risk and tailor secondary prevention measures are of the utmost *** rehabilitation(CR),encompassing several domains including exercise training,cardiovascular risk factor optimization,nutritional and psychological assessments,as well as other ancillary interventions has shown to be one of the pillars in the contemporary management of patients with ***,CR is associated with several benefits in this population,ranging from functional capacity to improvements in *** this,there are still several issues concerning the optimal application of CR which are still not fully ascertained,such as lack of referral and completion,as well as questions related to programme design(particularly among patients with multiple comorbidities).In this review,we aim at presenting a pragmatic overview on the current role of CR in the management of individuals with IHD,while also discussing some of the caveats in the current data,as well as future concepts which could help improve the uptake and personalization of this pivotal time-tested intervention.

The Importance of the Three-Dimensional Image in the Early Diagnosis of Cementoblastoma

The cementoblastoma is a very rare odontogenic neoplasm characterised by the formation of cementum-like tissue in connection with the root of a tooth. An 18-year-old boy was referred to the Clinic of Oral Pathology of the School of Dentistry, Universidade Federal de Minas Gerais, Brazil, complaining of an irradiating, itching pain from the region of the lower right first molar. He reported that the symptoms began three months prior, with a gradual increase in intensity during this period. The pain episodes were of short duration, with characteristics of toothache originating from the pulp. Upon intra-oral clinical examination, an open bite and slight increase in volume at the buccal alveolar region of the lower right first molar was observed. He also reported palpation and percussion pain symptoms in this region. Electrical, heat and cold pulp vitality tests were performed, and the tooth was vital. Periapical and panoramic radiographs showed radiolucent images with diffuse, poorly defined borders at the mesial root periapex extending to the interradicular region suggesting an inflammatory periapical lesion. In this early presentation, the three-dimensional image was fundamental to the final diagnosis of cementoblastoma.
