Fulminant liver failure following a marathon: Five case reports and review of literature

BACKGROUND The growing popularity of marathon and half-marathon runs has led to an increased number of patients presenting with exertion-induced heat stroke. Mild hepatic involvement is often observed in these patients;however, fulminant liver failure may occur in approximately 5% of all cases. Liver transplantation is a potentially curative approach for exertion-induced liver failure, although there is a lack of consensus regarding the criteria and optimal timing of this intervention. CASE SUMMARY This paper describes 5 patients (4 men and 1 woman) who were referred to the department where this study was performed with the diagnosis of exertioninduced acute liver failure. Three patients underwent liver transplantation, 1 recovered spontaneously, and 1 patient died on day 11 following the exertion. CONCLUSION Exertion-induced heat stroke may present as fulminant liver failure. These patients may recover with conservative treatment, may require liver transplantation, or may die. No definitive criteria are available to determine patient suitability for a conservative vs surgical approach.

Knowledge about the population dynamics within a specific spatial area is vital for the effective planning and management of important fishery resources.Population parameters,including the asymptotic length(L∞),growth coefficient(K),mortalities(Z,F,and M),recruitment pattern,exploitation level(E),and yield per recruit(Y’/R),of the tropical oyster Magallana bilineata in Mengkabong Bay,Tuaran,Malaysia were analyzed by using the FiSAT software,using on the monthly shell length-frequency data collected from April 2019 to March 2020.A total of 435 oysters with shell lengths ranging from 3.64 to 11.16 cm were analyzed.Habitat water temperature,salinity,dissolved oxygen,hydrogen ion concentration(pH),and chlorophyll concentration were measured at 29.0-33.7◦C,26.68-35.74 psμ,3.18-6.99 mg/L,6.57-8.29 and 1.51-98.93μg/L,respectively.The logarithmic relationship between the length and weight of Log W=1.9055 Log L+0.1049(R2=0.5431)with exponent b of 1.9055 for combined sexes showed a negative allometric growth(b<3),indicating that the oysters in Mengkabong Bay are skinny.Several management measures have been proposed to promote high meat growth,including the use of genetically selected seeds,selection of new farming sites and adoption of grow-out methods that favour high meat growth.Asymptotic length(L∞)and growth coefficient(K)of the von Bertalanffy Growth Formula(VBGF)for M.bilineata were estimated at 12.13 cm and 1.00 yr􀀀1,respectively.The theoretical ages at length zero(t0)(0.0487)were estimated by substituting L∞and K in Pauly’s equation.The sizes attained by M.bilineata were 3.76,5.04,6.13,7.05,7.83,and 8.49 cm at the end of 2,4,6,8,10,and 12 months,respectively.The calculated growth performance index(φ)for M.bilineata in Mengkabong Bay was 2.168,while the estimated lifespan was 3 years.The recruitment pattern was observed year-round,with higher recruitment activities in September-October 2019 and January 2020.The total mortality(Z)was analyzed using the length-converted catch curve and estimated at 3.04 yr^(-1),fishin

The retina plays a fundamental role in the process of vision,serving as the primary interface between external visual stimuli and the central nervous system.Because the retina is exposed to a variety of environmental stresses and deleterious insults,it is susceptible to a spectrum of pathological conditions that can detrimentally affect vision.This often leads to irreversible vision loss due to the injury of specific cell types.For instance,inherited retinal degeneration and age-related macular degeneration can lead to the death of photoreceptors,while conditions like glaucoma and optic nerve injury can result in the loss of ganglion cells.The precise pathological mechanisms driving retinal degeneration remain largely elusive,although research utilizing mouse models suggests that disruptions in intracellular signal transduction pathways may play a pivotal role.Signaling pathways within the retina orchestrate various aspects of retinal physiology,including phototransduction,synaptic transmission,and neuronal survival.

AIM: To investigate whether liver steatosis reduction due to a six-month dietary intervention results in significant changes in the concentrations of fatty acids.METHODS: A group of 35 Caucasian individuals diagnosed with different levels of steatosis were prospectively enrolled in the present study.Analysis of the fatty acid profiles was performed according to changes in liver steatosis(liver steatosis reduction by one or two degrees) after a six-month dietary intervention.The diet helped reduce body mass in obese and overweight patients,and stabilize both glycemia and dyslipidemia.Fatty acids were extracted according to the Folch method and analyzed by gas chromatography.RESULTS: This study showed significant changes in fatty acid profiles in patients who had reduced liver steatosis by one as well as two degrees.A reduction in liver steatosis by one degree caused a significant increase in the level of the n-3 family: eicosapentaenoic acid(P < 0.055),docosapentaenoic acid-C 22:5(P < 0.05) and docosahexaenoic acid(P < 0.05).A reduction in liver steatosis by two degrees caused a significant decrease in serum palmitoleic acid-C 16:1(P < 0.05).CONCLUSION: Liver steatosis reduction is associated with changes in fatty acid profiles,and these changes may reflect an alteration in fatty acid synthesis and metabolism.These findings may help better understand regression of nonalcoholic fatty liver disease.

The scientific community faces the challenge of measuring progress toward biodiversity targets and indices have been traditionally used.However,recent inventories in secondary tropical mountain forests using traditional biodiversity indices have yielded results that are indistinct with primary ones.This shows the need to develop complementary indices that goes beyond species count but integrates the distribution and conservation status of the species.This study developed endemicity and conservation importance index for tropical forest that incorporated the distribution and conservation status of the species.These indices were applied to Mt.Natoo,a remnant primary mossy forest in Buguias,Benguet,Philippines,that resulted to endemicity index of 81.07 and conservation importance index of 42.90.Comparing these with secondary forest sites with comparable Shannon-Wiener,Simpson,Evenness and Margalef’s indices,our endemicity and conservation indices clearly differentiates primary forest(our study site)with higher values from secondary forests with much lower values.Thus,we are proposing these indices for a direct but scientifically-informed identification of specific sites for conservation and protection in tropical forests.Additionally,our study documented a total of 168 vascular plant species(79 endemic and 12 locally threatened species)in Mt.Nato-o.Majority are of tropical elements for both generic and species levels with some temperate elements that could be attributed to the site's high elevation and semi-temperate climate.These are important baseline information for conservation plans and monitoring of tropical mossy forests.

Since ChatGPT emerged on November 30, 2022, Artificial Intelligence (AI) has been increasingly discussed as a radical force that will change our world. People have become used to AI in which such ubiquitous technologies as Siri, Google, and Netflix deploy AI algorithms to answer questions, impart information, and provide recommendations. However, many individuals including originators and backers of AI have recently expressed grave concerns. In this paper, the authors will assess what is occurring with AI in Visual Arts Education, outline positives and negatives, and provide recommendations addressed specifically for teachers working in the field regarding emerging AI usage from kindergarten to grade twelve levels as well as in higher education.

Synchronous and metachronous neoplasms in gastric cancer patients:A 23-year study

AIM:To determine the prevalence and characteristics of additional primary malignancies in gastric cancer(GC)patients.METHODS:GC patients(862 total;570 men,292women;mean age 59.8±12.8 years)diagnosed at the Department of Gastroenterology at Pomeranian Medical University over a period of 23 years were included in this retrospective analysis of a prospectively maintained database.Mean follow-up time was 31.3±38.6 mo(range 1-241 mo).The following clinicopathological features of patients with synchronous tumors were compared to those with metachronous tumors:age,sex,symptom duration,family history of cancer,tumor site,stage(early vs advanced),histology,and blood group.GC patients with and without a second tumor were compared in terms of the same clinicopathological features.RESULTS:Of 862 GC patients,58(6.7%)developed a total of 62 multiple primary tumors,of which 39(63%)were metachronous and 23(37%)synchronous.Four(6.9%)of the 58 multiple GC patients developed two or more neoplasms.The predominant tumor type of the secondary neoplasms was colorectal(n=17),followed by lung(n=9),breast(n=8),and prostate(n=7).Age was the only clinicopathological feature that differed between GC patients with synchronous vs metachronous malignancies;GC patients with synchronous neoplasms were older than those with metachronous neoplasms(68.0±10.3 years vs 59.9±11.1years,respectively,P=0.008).Comparisons between patients with and without a second primary cancer revealed that the only statistically significant differences were in age and blood group.The mean age of the patients with multiple GC was higher than that of those without a second primary tumor(63.4±11.4years vs 59.5±13.0 years,respectively,P=0.026).GC patients with a second primary tumor were more commonly blood group O than those without(56.2%vs 31.6%,respectively,P=0.002).CONCLUSION:GC patients may develop other primary cancers;appropriate preoperative and postoperative diagnostic modalities are thus required,particularly if patients are older and blood group O.

BACKGROUND Pelvic fractures(PF)with concomitant injuries are on the rise due to an increase of high-energy trauma.Increase of the elderly population with age related comorbidities further complicates the management.Abdominal organ injuries are kindred with PF due to the proximity to pelvic bones.Presence of contrast blush(CB)on computed tomography in patients with PF is considered a sign of active bleeding,however,its clinical significance and association with outcomes is debatable.AIM To analyze polytrauma patients with PF with a focus on the geriatric population,co-injuries and the value of contrast blush.METHODS This retrospective cohort study included 558 patients with PF admitted to level 1 trauma center(01/2017-01/2023).Analyzed variables included:Age,sex,mechanism of injury(MOI),injury severity score(ISS),Glasgow coma scale(GCS),abbreviated injury scale(AIS),co-injuries,transfusion requirements,pelvic angiography,embolization,laparotomy,orthopedic pelvic surgery,intensive care unit and hospital lengths of stay,discharge disposition and mortality.The study compared geriatric and non-geriatric patients,patients with and without CB and abdominal co-injuries.Propensity score matching was implemented in comparison groups.RESULTS PF comprised 4%of all trauma admissions.89 patients had CB.286(52%)patients had concomitant injuries including 93(17%)patients with abdominal co-injuries.Geriatric patients compared to non-geriatric had more falls as MOI,lower ISS and AIS pelvis,higher GCS,less abdominal co-injuries,similar CB and angio-embolization rates,less orthopedic pelvic surgeries,shorter lengths of stay and higher mortality.After propensity matching,orthopedic pelvic surgery rates remained lower(8%vs 19%,P<0.001),hospital length of stay shorter,and mortality higher(13%vs 4%,P<0.001)in geriatric patients.Out of 89 patients with CB,45(51%)were embolized.After propensity matching,patients with CB compared to without CB had more pelvic angiography(71%vs 12%,P<0.001),higher embolization rates(64%vs 22%,P=0.02)and com
