目的哮喘-慢性阻塞性肺疾病肺重叠症(Asthma-chronie obst ructive pulmonary disease overlap,A-CO)患者的生活质量较低,肺功能较差,病情经常加重。由于暴露于污染环境会加重其症状,增加发病率和病死率,环境管理对其至关重要。旨在通过系统性综述,寻找证据,以指导制订护理干预措施,促进ACO患者对空气质量的自我管理。方法2023年3月 ,系统检索CInAHL、Medline、Academie Search Complete .Scopus、WebofScience、Cochrane图书馆、乔安娜布里格斯研究所(JoannaBriggsInstitute,JBI)循证实践数据库,以及GoogleScholar。对纳人研究进行数据提取和综合,并对护理干预措施进行分类。采用JBI研究质量评估工具进行方法学质量评估。结果共纳入5篇来自不同国家的研究。促进ACO患者自我监控空气质量的干预措施包括:警觉性干预措施(医务人员定期巡诊、进行症状评估),监测干预措施(监测室内外诱发因素) ,教育干预措施(空气质量警报、避免接触过敏原),以及相关政策的影响,如无烟政策和改善空气质量的综合战略。尽管这些研究报告了不同的干预措施,方法质量也存在差异,但这些干预措施显示了预防患者病情恶化、尽量减少急诊就诊和住院治疗的潜力。这些关键点可作为护理干预措施的关键组成部分。结论有必要采取全面措施,让护士参与促进患者自我监控空气质量的多学科团队.提高认识、制订和实施护理干预,从而使ACO患者受益。同时,需要进--步开展研究,以扩大证据基础并完善这些干预措施。

Lanthanide complexes with β-diketones and coumarin derivates:synthesis,thermal behaviour,optical and pharmacological properties and immobilisation

The paper presents the results of the synthesis of complexes of the ty pe Eu(DBM) 3 and Eu(DBM) 3·Q(DBM-dibenzoylmethane,Q-1,10-phenantroline or 4,7-d iphenyl-1,10-phenantroline) and of Tb-and nd-complexes with the newly-synthesis ed coumarin derivates 3,3’-[(4-chlorphenyl) methylene) bis(4-hydroxy-2H-chromen-2-one) ,3,3’-[(3,5-dimethoxy-4-hydroxy) methylene) bis(4-hydroxy-2H-chromen-2-one) ,etc. Elemental and thermogravimetric analysis,Ir,UV,nMr and fluorescence spe ctroscopy and X-ray analysis were applied for characterisation of the complexes. Some peculiarities of the synthetic procedures for both types of complexes were discussed and the influence of the synthetic approach,pH of the reaction mediu m,temperature of synthesis and drying of the complexes on the composition,stab ility and optical properties was reported. The immobilisation of the complexes i n thin films based on sol-gel produced SiO2 and on polymethylmethacrylate was st udied. The optimal conditions for preparation of the matrices(composition of th e starting system,temperature and time of sol aging,etc.) were recommended. Th e film morphology was evaluated by fluorescence,scanning electron and atomic fo rce microscopy. The interaction of the lanthanide ions with the matrices and the influence of their nature,the effect of the in-situ polymerisation and other f actors on photoluminescent excitation and emission spectra and excited state lif e-times of the complexes were followed. The effect of the second ligand on the p hotoluminescence properties of the immobilised diketonates was further elucidate d. Processes involved in the thermal decomposition of the complexes and microcom posites produced on their base were proposed. Preliminary results on the pharmac ological properties of the coumarin complexes reported showed unambiguously high er cytotoxicity of the nd complex in comparison with that of the respective ligand.

BACKGrOUnD neurogenic heterotopic ossification is an acquired serious complication described in patients with central nervous system disorders and defined by bone formation in non-osseous *** SUMMArY We present an unusual case of a 13-yr-old boy presenting with hip pain and severe gait impairment 5 mo after the diagnosis of hemiplegia following a spontaneous intracerebral *** tomography revealed bilateral heterotopic ossification of both the paretic and the non-paretic limbs,with entrapment of the sciatic *** choice of surgical or nonsurgical management of such patients depends on the timing of diagnosis,the symptoms,and the extent of maturation of the ossified *** resection remains the only treatment with proven,evidence-based *** choice of surgical approach largely depends on the location of the ossified *** We believe the plane of dissection presented is a satisfactory option for resection of a posteromedial mass and sciatic nerve release.

BACKGrOUnD Merkel cell carcinoma(MCC)is a rare and aggressive cutaneous neuroendocrine neoplasia,with high risk of recurrence and metastasis and poor *** checkpoint inhibitors,like the anti-programmed death-ligand 1 agent avelumab,were recently approved for the treatment of advanced ***,herein,report the first case of advanced MCC with oligoprogression managed with avelumab and local radical *** SUMMArY A 61-year-old man was presented to the hospital with sporadic fever and an exudative malodorous mass(10 cm of diameter),located on the right gluteal *** final diagnosis was MCC,cT4n3M1c(AJCC,TnM staging 8th edition,2017),with invasion of adjacent muscle,in-transit metastasis,and bone *** started chemotherapy(cisplatin and etoposide),and after six cycles,the main tumor increased,evidencing disease *** months later,the patient started second line treatment with avelumab(under an early access program).After two cycles of treatment,the lesion started to decrease,achieving a major *** progression was documented after 16 ***,as the tumor became resectable,salvage surgery was performed,while keeping the systemic treatment with *** the patient developed bilateral pneumonia,immunotherapy was *** than 2.5 years after surgery(last 19 mo without systemic therapy),the patient maintains complete local response and stable bone *** This report highlights the efficacy and long-term response of avelumab on the management of a chemotherapy resistant advanced MCC,with evidence of oligoprogression,in combination with local radical treatment.
