AIM: To assess the impact of transjugular intrahepatic portosystemic shunt (TIPS) on pulmonary gas exchange and to evaluate the use of TIPS for the treatment of hepatopulmonary syndrome (HPS).METHODS: Seven patients, three of them with advanced HPS, in whom detailed pulmonary function tests were performed before and after TIPS placementat the University of Alabama Hospital and at the Hospital Clinic, Barcelona, were considered.RESULTS: TIPS patency was confirmed by hemodynamic evaluation. No changes in arterial blood gases were observed in the overall subset of patients. Transient arterial oxygenation improvement was observed in only one HPS patient, early after TIPS, but this was not sustained 4 mo later.CONCLUSION: TIPS neither improved nor worsened pulmonary gas exchange in patients with portal hypertension. This data does not support the use of TIPS as a specific treatment for HPS. However, it does reinforce the view that TIPS can be safely performed for the treatment of other complications of portal hypertension in patients with HPS.

The BETA application-specific integrated circuit(ASIC)is a fully programmable chip designed to amplify,shape and digitize the signal of up to 64 Silicon photomultiplier(SiPM)channels,with a power consumption of approximately~1 mW/channel.Owing to its dual-path gain,the BETA chip is capable of resolving single photoelectrons(phes)with a signal-to-noise ratio(SNR)>5 while simultaneously achieving a dynamic range of~4000 phes.Thus,BETA can provide a cost-effective solution for the readout of SiPMs in space missions and other applications with a maximum rate below 10 kHz.In this study,we describe the key characteristics of the BETA ASIC and present an evaluation of the performance of its 16-channel version,which is implemented using 130 nm technology.The ASIC also contains two discriminators that can provide trigger signals with a time jitter down to 400 ps FWHM for 10 phes.The linearity error of the charge gain measurement was less than 2%for a dynamic range as large as 15 bits.

Freshwater reservoirs are essential owing because of their ecological, economic, and social importance.They are particularly vulnerable to contamination, as of metal and metalloids, derived from anthropogenic activities like mining. The temporal variations in trace element concentrations(arsenic(As),mercury(Hg), chromium(Cr), copper(Cu), nickel(Ni), lead(Pb), and zinc(Zn)), enrichment, fluxes, and possible sources were evaluated by studying two sediment cores from the La Angostura(ANG) Reservoir(northwest Mexico), using210Pb geochronology. The enrichment factors showed from null to minor enrichment for most elements, but moderate to severe enrichment of mercury(Hg). Most trace element concentrations had a detrital origin, and notable Hg concentration increases since the past decade were associated with severe drought periods, likely resulting from wildfires. The observed sediment concentrations of As and Hg can cause adverse effects on biota in the ecosystem since they are above the probable effect level(PEL). Development of strategies for metal attenuation in this reservoir is recommended and metals should be controlled until specific ecotoxicological studies are performed.

BACKGROUND The Impella pump has emerged as a promising tool in patients with cardiogenic shock(CS).Despite its attractive properties,there are scarce data on the specific clinical setting and the potential role of Impella devices in CS patients from routine clinical practice.METHODS This is an observational,retrospective,single center,cohort study.All consecutive patients with diagnosis of CS and undergoing support with Impella 2.5?,Impella CP?or Impella 5.0?from April 2015 to December 2020 were included.Baseline characteristics,management and outcomes were assessed according to CS severity,age and cause of CS.Main outcome measured was in-hospital mortality.RESULTS A total of 50 patients were included(median age:59.3±10 years).The most common cause of CS was acute coronary syndrome(ACS)(68%),followed by decompensation of previous cardiomyopathy(22%).A total of 13 patients(26%)had profound CS.Most patients(54%)improved pulmonary congestion at 48 h after Impella support.A total of 19 patients(38%)presented significant bleeding.In-hospital mortality was 42%.Among patients with profound CS(n=13),five patients were previously supported with venoarterial extracorporeal membrane oxygenation.A total of eight patients(61.5%)died during the admission,and no patient achieved ventricular recovery.Older patients(≥67 years,n=10)had more comorbidities and the highest mortality(70%).Among patients with ACS(n=34),35.3%of patients had profound CS;and in most cases(52.9%),Impella support was performed as a bridge to recovery.In contrast,only one patient from the decompensated cardiomyopathy group(n=11)presented with profound CS.In 90.9%of these cases,Impella support was used as a bridge to cardiac transplantation.There were no cases of death.CONCLUSIONS In this cohort of real-life CS patients,Impella devices were used in different settings,with different clinical profiles and management.Despite a significant rate of complications,mortality was acceptable and lower than those observed in other series.

Two lead-210(210Pb) dated sediment cores from Chapala Lake were studied to identify the mineral composition of the sediments and to discuss its relation with trace metals. Minerals and trace metals reflected the domain of volcanic rocks(i.e. basalts, rhyolite and andesite) that characterize Chapala Lake and the Lerma River watershed. Redundancy analyses(RDA) were used to identify the sedimentary variables(magnetic susceptibility, organic matter, sand content, and mineral composition) that could be related to elemental composition. Despite the distance between the two cores(7.3 km) and hydrodynamic circulation the RDA showed that the main mechanism that controls the input of mineral species,and, therefore, element distributions in the lacustrine sediment of Chapala Lake, is related to the weathering of volcanic rocks in the Lerma-Chapala watershed, the consequent runoff and transport of fine grained catchment materials, and later in-lake processes. These findings highlight the importance of controlling watershed erosion to contribute to the improvement of the environmental quality of the lake.

AIM: To study the presence of sustained low diffusing capacity (DLCO) after liver transplantation (LT) in patients with hepatopulmonary syndrome (HPS).METHODS: Six patients with mild-to-severe HPS and 24 without HPS who underwent LT were prospectively followed before and after LT at mid-term (median, 15 mo). HPS patients were also assessed at long-tem (median, 86 mo).RESULTS: Before LT, HPS patients showed lower PaO2 (71 ± 8 mmHg), higher AaPO2 (43 ± 10 mmHg) and lower DLCO (54% ± 9% predicted), due to a combination of moderate-to-severe ventilation-perfusion (VA/Q) imbalance, mild shunt and diffusion limitation, than non-HPS patients (94 ± 4 mmHg and 19 ± 3 mmHg, and 85% ± 3% predicted, respectively) (P < 0.05 each). Seven non-HPS patients had also reduced DLCO (70% ± 4% predicted).At mid- and long-term after LT, compared to pre-LT, HPS patients normalized PaO2 (91 ± 3 mmHg and 87 ± 5 mmHg), AaPO2 (14 ± 3 mmHg and 23 ± 5 mmHg) and all VA/Q descriptors (P < 0.05 each) without changes in DLCO (53% ± 8% and 56% ± 7% predicted, respectively). Post-LT DLCO in non-HPS patients with pre-LT low DLCO was unchanged (75% ± 6% predicted).CONCLUSION: While complete VA/Q resolution in HPS indicates a reversible functional disturbance, sustained low DLCO after LT also present in some non-HPS patients, points to persistence of sub-clinical liver-induced pulmonary vascular changes.

Increased posterior tibial slope (PTS), medial tibial plateau slope (MTPS) and lateral tibial plateau slope (LTPS) have been proposed as potential risk factors for anterior cruciate ligament (ACL) tear. However, prior literature is inconclusive. The primary goal of the study was to investigate the relationship of MTPS, LTPS and PTS with ACL tears. The secondary goal was to determine whether injury mechanisms, age, gender, tobacco, obesity, extreme PTS (>12°) or previous injuries are risk factors in ACL tears. Three groups were identified: a study group of patients with ACL injury;a second group diagnosed with meniscal injury;and a third group of non-injured patients. Demographic data was collected. MTPS and LTPS were measured in MRI and PTS in lateral radiographs by two blinded observers. Data was analyzed using SPSS. 416 patients were included in the study. Objective: To evaluate the efficacy of endorectal Magnetic Resonance Imaging (MRI) and Magnetic Resonance Spetroscopic Imaging (MRSI) combined with total prostate-specific antigen (tPSA) and free prostate-specific antigen (fPSA) in selecting candidates for biopsy. Subjects and Methods: 246 patients with elevated tPSA (median: 7.81 ng/ml) underwent endorectal MRI and MRSI before Transrectal Ultrasound (TRUS) biopsy (10 peripheral + 2 central cores);patients with positive biopsies were treated with radical intention;those with negative biopsies were followed up and underwent MRSI before each additional biopsy if tPSA rose persistently. Mean follow-up: 27.6 months. We compared MRI, MRSI, tPSA, and fPSA with histopathology by sextant and determined the association between the Gleason score and MRI and MRSI. We determined the most accurate combination to detect prostate cancer (PCa) using receiver operating curves;we estimated the odds ratios (OR) and calculated sensitivity, specificity, and positive and negative predictive values. Results: No difference in tPSA was found between patients with and without PCa (p = 0.551). In the peripheral zone, the risk of PCa increased with MRSI grade;patients with high-grade MRSI had the greatest risk of PCa over time (OR = 328.6);the model including MRI, MRSI, tPSA, and fPSA was more accurate (Area under Curve: AUC = 95.7%) than MRI alone (AUC = 85.1%) or fPSA alone (AUC = 78.1%), but not than MRSI alone (94.5%). In the transitional zone, the model was less accurate (AUC = 84.4%). The association (p = 0.005) between MRSI and Gleason score was significant in both zones. Conclusions: MRSI is useful in patients with elevated tPSA. High-grade MRSI lesions call for repeated biopsies. Men with negative MRSI may forgo further biopsies because a significantly high Gleason lesion is very unlikely.

背景:乙肝疫苗的广泛接种和外部移民的不断增多已经改变了西班牙乃至整个欧洲乙型肝炎的流行病学数据。因此,本研究旨在评估乙型肝炎的人口统计学特征,并探讨不同来源(移民与非移民)和治疗路径患者间可能存在的差异。方法:这是一项多中心横断面研究(2014年6月至2015年5月),研究对象来自四个肝病中心收治和随访的HBsAg阳性门诊患者。收集西班牙加泰罗尼亚两个地区(国家卫生系统全面覆盖、无任何治疗障碍的地区)患者人口统计学特征、临床资料、治疗适应证及获治情况。结果:共对951例患者进行评估,其中男性占48.1%。46.6%为移民(其中87.7%出生于非洲),其较非移民更为年轻。在非移民患者中,酗酒、体重过重以及合并其他代谢性疾病者的比例相对较高。在937例接受HBeAg检测者中,91.7%为阴性。慢性HBeAg阳性肝炎在移民患者中的比例更高(3.9%vs 0.6%,P=0.001),但慢性HBeAg阴性肝炎则在非移民患者中的比例更高(31.7%vs 21.4%,P<0.001)。无论是满足治疗标准需要治疗的比例(38.4%vs 29.2%,P=0.003),还是需要治疗者中实际接受了治疗的比例(83.2%vs 57.8%,P<0.001),非移民患者均高于移民者。结论:相较于非移民,移民乙肝患者年龄相对较轻,代谢性合并症患病率较高,慢性HBeAg感染更为多见。尽管可以获得治疗且具有治疗指征,但部分移民者由于局部适应等问题并没有得到充分的治疗。
