Detachable string magnetically controlled capsule endoscopy for the noninvasive diagnosis of esophageal diseases:A prospective,blind clinical study

BACKGROUND Traditional esophagogastroduodenoscopy(EGD),an invasive examination method,can cause discomfort and pain in patients.In contrast,magnetically controlled capsule endoscopy(MCE),a noninvasive method,is being applied for the detection of stomach and small intestinal diseases,but its application in treating esophageal diseases is not widespread.AIM To evaluate the safety and efficacy of detachable string MCE(ds-MCE)for the diagnosis of esophageal diseases.METHODS Fifty patients who had been diagnosed with esophageal diseases were pros-pectively recruited for this clinical study and underwent ds-MCE and conven-tional EGD.The primary endpoints included the sensitivity,specificity,positive predictive value,negative predictive value,and diagnostic accuracy of ds-MCE for patients with esophageal diseases.The secondary endpoints consisted of visualizing the esophageal and dentate lines,as well as the subjects'tolerance of the procedure.RESULTS Using EGD as the gold standard,the sensitivity,specificity,positive predictive value,negative predictive value,and diagnostic accuracy of ds-MCE for esophageal disease detection were 85.71%,86.21%,81.82%,89.29%,and 86%,respectively.ds-MCE was more comfortable and convenient than EGD was,with 80%of patients feeling that ds-MCE examination was very comfortable or comfortable and 50%of patients believing that detachable string v examination was very convenient.CONCLUSION This study revealed that ds-MCE has the same diagnostic effects as traditional EGD for esophageal diseases and is more comfortable and convenient than EGD,providing a novel noninvasive method for treating esophageal diseases.

NLRP3 inhibitor dopamine receptors agonist as a potential therapeutic strategy for treatment of overactive immune responses in COVID-19

Patients infected with coronavirus disease 2019(COVID-19)have high serum levels of proinflammatory cytokines.The"cytokine storm"has become one of the major causes of death for critically ill patients infected by COVID-19.Glucocorticoids,plasma from convalescent patients,blood purification,and tocilizumab are currently recommended for use when the body’s inflammatory response is overactivated.However,there are limitations in terms of medicinal effects,equipment reserves,and treatment expense.These challenges prompted us to assess classical agents with good safety and mature production technology.A recent study showed that nucleotide-binding oligomerization domain(NOD)-,leucine-rich repeat(LRR)-,and pyrin domain-containing protein 3(NLRP3)inflammasomes drive COVID-19 pathology.We speculate that suppression of NLRP3 inflammasome-derived cytokine production may be beneficial in COVID-19-infected patients.Dopamine receptors are present in almost all immune cells and can modulate their activation,proliferation,and cytokine production of immune cells.Previous studies have shown that dopamine receptor agonists can control systemic inflammation through inhibition of the NLRP3 inflammasome.This suggests that dopamine receptor agonists may be a new strategy for the treatment of overactive immune responses in COVID-19 patients.This is worthy of further investigation in clinical practice.

5S-Heudelotinone alleviates experimental colitis by shaping the immune system and enhancing the intestinal barrier in a gut microbiota-dependent manner

Aberrant changes in the gut microbiota are implicated in many diseases,including inflammatory bowel disease(IBD).Gut microbes produce diverse metabolites that can shape the immune system and impact the intestinal barrier integrity,indicating that microbe-mediated modulation may be a promising strategy for preventing and treating IBD.Although fecal microbiota transplantation and probiotic supplementation are well-established IBD therapies,novel chemical agents that are safe and exert strong effects on the gut microbiota are urgently needed.Herein,we report the total synthesis of heudelotinone and the discovery of 5S-heudelotinone(an enantiomer)as a potent agent against experimental colitis that acts by modulating the gut microbiota.5S-Heudelotinone alters the diversity and composition of the gut microbiota and increases the concentration of short-chain fatty acids(SCFAs);thus,it regulates the intestinal immune system by reducing proinflammatory immune cell numbers,and maintains intestinal mucosal integrity by modulating tight junctions(TJs).Moreover,5S-heudelotinone(2)ameliorates colitis-associated colorectal cancer(CAC)in an azoxymethane(AOM)/dextran sulfate sodium(DSS)-induced in situ carcinoma model.Together,these findings reveal the potential of a novel natural product,namely,5S-heudelotinone,to control intestinal inflammation and highlight that this product is a safe and effective candidate for the treatment of IBD and CAC.

Testosterone is closely associated with lipid metabolism and known to affect body fat composition and muscle mass in males.However,the mechanisms by which testosterone acts on lipid metabolism are not yet fully understood,especially in teleosts.In this study,cyp17a1-/-zebrafish(Danio rerio)exhibited excessive visceral adipose tissue(VAT),lipid content,and up-regulated expression and activity of hepatic de novo lipogenesis(DNL)enzymes.The assay for transposase accessible chromatinwithsequencing(ATAC-seq)results demonstrated that chromatin accessibility of DNL genes was increased in cyp17a1-/-fish compared to cyp17a1+/+male fish,including stearoyl-CoA desaturase(scd)and fatty acid synthase(fasn).Androgen response element(ARE)motifs in the androgen signaling pathway were significantly enriched in cyp17a1+/+male fish but not in cyp17a1-/-fish.Both androgen receptor(ar)-/-and wildtype(WT)zebrafish administered with Ar antagonist flutamide displayed excessive visceral adipose tissue,lipid content,and up-regulated expression and activity of hepatic de novo lipogenesis enzymes.The Ar agonist BMS-564929 reduced the content of VAT and lipid content,and down-regulated acetyl-CoA carboxylase a(acaca),fasn,and scd expression.Mechanistically,the rescue effect of testosterone on cyp17a1-/-fish in terms of phenotypes was abolished when ar was additionally depleted.Collectively,these findings reveal that testosterone inhibits lipid deposition by down-regulating DNL genes via Ar in zebrafish,thus expanding our understanding of the relationship between testosterone and lipid metabolism in teleosts.

Microbial Fertilizer: A Sustainable Strategy for Medicinal Plants Production

Medicinal plants have aroused considerable interest as an alternative to chemical drugs due to the beneﬁcial effects of their active secondary metabolites.However,the extensive use of chemical fertilizers and pesticides in pursuit of yield has caused serious pollution to the environment,which is not conducive to sustainable devel-opment in theﬁeld of medicinal plants.Microbial fertilizers are a type of“green fertilizer”containing speciﬁc microorganisms that can improve the soil microbial structure,enhance plant resistance to biological and abiotic stresses,and increase the yield of medicinal plants.The root exudates of medicinal plants attract different micro-organisms to the rhizosphere,which then migrate further to the plant tissues.These microbes can increase the levels of soil nutrients,and improve the physical and chemical properties of soil through nitrogenﬁxation,and phosphorus and potassium solubilization.In addition,soil microbes can promote the synthesis of hormones that increase plant growth and the accumulation of active compounds,eventually improving the quality of med-icinal plants.In 2022,the total value of the global microbial fertilizer market was$4.6 billion and is estimated to reach$10.36 billion by 2030.In this review,we have summarized the types of microbial fertilizers,the coloniza-tion and migration of microorganisms to plant tissues,and the beneﬁcial effects of microbial fertilizers.In addi-tion,the prospects of developing microbial fertilizers and their application for medicinal plants have also been discussed.It aims to provide a reference for the rational application of microbial fertilizers in theﬁeld of med-icinal plants and the green and sustainable development of medicinal plant resources.

Production induced fracture closure of deep shale gas well under thermo-hydro-mechanical conditions

Deep shale gas reservoirs have geological characteristics of high temperature, high pressure, high stress,and inferior ability to pass through fluids. The multi-stage fractured horizontal well is the key to exploiting the deep shale gas reservoir. However, during the production process, the effectiveness of the hydraulic fracture network decreases with the closure of fractures, which accelerates the decline of shale gas production. In this paper, we addressed the problems of unclear fracture closure mechanisms and low accuracy of shale gas production prediction during deep shale gas production. Then we established the fluid—solid—heat coupled model coupling the deformation and fluid flow among the fracture surface,proppant and the shale matrix. When the fluid—solid—heat coupled model was applied to the fracture network, it was well solved by our numerical method named discontinuous discrete fracture method.Compared with the conventional discrete fracture method, the discontinuous discrete fracture method can describe the three-dimensional morphology of the fracture while considering the effect of the change of fracture surface permeation coefficient on the coupled fracture—matrix flow and describing the displacement discontinuity across the fracture. Numerical simulations revealed that the degree of fracture closure increases as the production time proceeds, and the degree of closure of the secondary fractures is higher than that of the primary fractures. Shale creep and proppant embedment both increase the degree of fracture closure. The reduction in fracture surface permeability due to proppant embedment reduces the rate of fluid transfer between matrix and fracture, which has often been overlooked in the past. However, it significantly impacts shale gas production, with calculations showing a 24.7% cumulative three-year yield reduction. Programmed cell death 1 ligand 1(PD-L1)is an important immunosuppressive molecule,which inhibits the function of T cells and other immune cells by binding to the receptor programmed cell death-1.The PD-L1 expression disorder plays an important role in the occurrence,development,and treatment of sepsis or other inflammatory diseases,and has become an important target for the treatment of these diseases.Mesenchymal stem cells(MSCs)are a kind of pluripotent stem cells with multiple differentiation potential.In recent years,MSCs have been found to have a strong immunosuppressive ability and are used to treat various inflammatory insults caused by hyperimmune diseases.Moreover,PD-L1 is deeply involved in the immunosuppressive events of MSCs and plays an important role in the treatment of various diseases.In this review,we will summarize the main regulatory mechanism of PD-L1 expression,and discuss various biological functions of PD-L1 in the immune regulation of MSCs.

Exploring Paths for the High-Quality Development of Forest-Based Wel ness Industry:A Case Study of Sichuan Province

The forest-based wellness industry,as a rapidly growing sector that integrates various business forms with extensive coverage and an extended industrial chain,is undergoing rapid development due to the increasing aging population and people's suboptimal health conditions.As a forerunner in developing the forest-based wellness industry,Sichuan province is known for its early development,proactive efforts,diverse models,and significant impact in this industry.It has achieved certain milestones in terms of top-level design,pilot demonstration,standardized guidance,and public awareness campaigns to promote the development of this industry.Therefore,this paper utilizes Sichuan as a case study to systematically summarize and analyze the key practices made by the province in promoting the rapid development of the industry by investigating the development trajectory of the forest-based wellness industry.Additionally,it examines the development trends of this industry from the perspectives of supply,demand,and consumption.Finally,this paper proposes several measures to facilitate the high-quality development of the forest-based wellness industry.These measures encompass nurturing specialized talent in forest-based wellness,enhancing market players'capabilities in this domain,conducting extensive research on technologies that promote this industry,actively seeking support from relevant policies,and promoting integrated development across diverse sectors.

Soil phosphorus (P) plays a vital role in both ecological and agricultural ecosystems, where total P (TP) in soil serves as a crucial indicator of soil fertility and quality. Most of the studies covered in the literature employ a single or narrow range of soil databases, which largely overlooks the impact of utilizing multiple mapping scales in estimating soil TP, especially in hilly topographies. In this study, Fujian Province, a subtropical hilly region along China’s southeast coast covered by a complex topographic environment, was taken as a case study. The influence of the mapping scale on soil TP storage (TPS)estimation was analyzed using six digital soil databases that were derived from 3 082 unique soil profiles at different mapping scales, i.e., 1:50 000 (S5),1:200 000 (S20), 1:500 000 (S50), 1:1 000 000 (S100), 1:4 000 000 (S400), and 1:10 000 000 (S1000). The regional TPS in the surface soil (0–20 cm) based on the S5, S20, S50, S100, S400, and S1000 soil maps was 20.72, 22.17, 23.06, 23.05, 22.04, and 23.48 Tg, respectively, and the corresponding TPS at0–100 cm soil depth was 80.98, 80.71, 85.00, 84.03, 82.96, and 86.72 Tg, respectively. By comparing soil TPS in the S20 to S1000 maps to that in the S5map, the relative deviations were 6.37%–13.32%for 0–20 cm and 0.33%–7.09%for 0–100 cm. Moreover, since the S20 map had the lowest relative deviation among different mapping scales as compared to S5, it could provide additional soil information and a richer soil environment than other smaller mapping scales. Our results also revealed that many uncertainties in soil TPS estimation originated from the lack of detailed soil information, i.e., representation and spatial variations among different soil types. From the time and labor perspectives, our work provides useful guidelines to identify the appropriate mapping scale for estimating regional soil TPS in areas like Fujian Province in subtropical China or other places with similar complex topographies. The non-selective cytotoxicity of toxins limits the clinical relevance of the toxins.In recent years,toxins have been widely used as warheads for antibody-drug conjugates(ADCs)due to their eff-cient killing activity against various cancer cells.Although ADCs confer certain targeting properties to the toxins,low drug loading capacity,possible immunogenicity,and other drawbacks also limit the po-tential application of ADCs.Recently,non-ADC delivery strategies for toxins have been extensively investigated.To further understand the application of toxins in anti-tumor,this paper provided an over-view of prodrugs,nanodrug delivery systems,and biomimetic drug delivery systems.In addition,toxins and their combination strategies with other therapies were discussed.Finally,the prospect and challenge of toxins in cancertreatmentwerealso summarized.
