Low-dose aspirin in the prevention of pre-eclampsia in China: postpartum hemorrhage in subgroups of women according to their characteristics and potential bleeding risk

Background:The APPEC study is a large-population randomized controlled trial in China evaluating the role of low-dose aspirin prophylactic treatment for pre-eclampsia.There was no statistically significant difference in postpartum hemorrhage(PPH)incidence between the aspirin and control groups.This study aimed to evaluate the potential bleeding risk of 100 mg aspirin in high-risk pregnant women and the difference in the incidence of PPH according to maternal characteristics.Methods:This is a secondary data analysis of the APPEC study.Platelet counts and coagulation test results were collected at five follow-up visits.Subgroups defined by maternal age(<35 years and≥35 years),pre-pregnancy body mass index(pre-BMI,<28 kg/m 2 and≥28 kg/m 2),parity,gestational age at enrollment,and medical history,including pre-eclampsia,chronic hypertension,and diabetes mellitus,were analyzed.Logistic regression analysis was used to determine the statistical significance of the difference in the incidence of PPH after aspirin administration in pregnant women in each subgroup.Adjustment using multiple logistic regression models followed these analyses.Binary logistic regression was used to determine the relationship between pre-BMI and PPH.Results:There was no significant difference between the aspirin and control groups in bleeding risk(3.4%[16/464]vs.3.0%[13/434],T=0.147,P=0.701).No significant difference was found in the incidence of PPH in total(relative risk=1.220,95%confidence interval[CI]=0.720–2.067,P=0.459;aspirin group vs.control group,6.5%[30/464]vs.5.3%[23/434],P=0.459)or in subgroup analysis.A significant correlation between pre-BMI and PPH was found in the aspirin group,while in the control group there was no significant correlation(aspirin group,odds ratio[OR]=1.086,95%CI=1.004–1.175,P=0.040;control group,OR=1.060,95%CI=0.968–1.161,P=0.209).Conclusions:A dosage of 100 mg of aspirin per day,initiated from 12 to 20 gestational weeks until 34 weeks of gestation,did not increase the risk of potential bleeding and PPH regard

Impact of digital economy development on carbon productivity:An empirical analysis based on threshold effect and spatial spillover effect

Utilizing provincial panel data from 2014 to 2020,this study employs a fixed effect model,a threshold effect model,and a spatial lag model to empirically examine the correlation between digital economic development and carbon productivity.The findings indicate that digital economic development significantly contributes to the enhancement of carbon productivity in the long term.Furthermore,through instrumental variable method,replacement of explanatory variables and other methods to test its endogeneity and stability,the results remain robust.In terms of regional heterogeneity,the impact of digital economic development on carbon productivity is less pronounced in the central and western regions compared to the eastern region.Additionally,further investigation reveals that industrial structure upgrading and science and technology investment level exhibit different threshold effects on the influence of digital economy development level on carbon productivity.Moreover,there is a significant spatial spillover effect of digital economy development on carbon productivity with H-H and L-L agglomeration spatial correlation.

Additive manufacturing technology is highly regarded due to its advantages,such as high precision and the ability to address complex geometric challenges.However,the development of additive manufacturing process is constrained by issues like unclear fundamental principles,complex experimental cycles,and high costs.Machine learning,as a novel artificial intelligence technology,has the potential to deeply engage in the development of additive manufacturing process,assisting engineers in learning and developing new techniques.This paper provides a comprehensive overview of the research and applications of machine learning in the field of additive manufacturing,particularly in model design and process development.Firstly,it introduces the background and significance of machine learning-assisted design in additive manufacturing process.It then further delves into the application of machine learning in additive manufacturing,focusing on model design and process guidance.Finally,it concludes by summarizing and forecasting the development trends of machine learning technology in the field of additive manufacturing.

In this study,a novel method is proposed for producing perovskite rare-earth manganese oxide magnetic refrigeration materials that exhibit superior properties compared to similar materials.The maximum magnetic entropy change values for this sample are 1.44 and 3.75 J·kg-1·K-1for magnetic fields of 2 and 7 T,respectively.Additionally,its relative cooling power has been calculated to be 101.96 and 404.78 J·kg-1,indicating good performance.Both results were obtained using the sample synthesized under a pressure of 4 GPa,and the sample has undergone a second phase transition.The critical behavior of the sample fits well with the mean field model.For different magnetic fields,there is a significant overlap between the values of the maximum magnetic entropy change and the temperature-averaged entropy change,indicating that the refrigeration performance of the sample after high pressure can approach its maximum capability.

BACKGROUND Genetic factors of chronic intestinal ulcers are increasingly garnering attention.We present a case of chronic intestinal ulcers and bleeding associated with mu-tations of the activin A receptor type II-like 1(ACVRL1)and phospholipase A2 group IVA(PLA2G4A)genes and review the available relevant literature.CASE SUMMARY A 20-year-old man was admitted to our center with a 6-year history of recurrent abdominal pain,diarrhea,and dark stools.At the onset 6 years ago,the patient had received treatment at a local hospital for abdominal pain persisting for 7 d,under the diagnosis of diffuse peritonitis,acute gangrenous appendicitis with perforation,adhesive intestinal obstruction,and pelvic abscess.The surgical treat-ment included exploratory laparotomy,appendectomy,intestinal adhesiolysis,and pelvic abscess removal.The patient’s condition improved and he was dis-charged.However,the recurrent episodes of abdominal pain and passage of black stools started again one year after discharge.On the basis of these features and results of subsequent colonoscopy,the clinical diagnosis was established as in-flammatory bowel disease(IBD).Accordingly,aminosalicylic acid,immunotherapy,and related symptomatic treatment were administered,but the symptoms of the patient did not improve significantly.Further investigations revealed mutations in the ACVRL1 and PLA2G4A genes.ACVRL1 and PLA2G4A are involved in angiogenesis and coagulation,respectively.This suggests that the chronic intestinal ulcers and bleeding in this case may be linked to mutations in the ACVRL1 and PLA2G4A genes.Oral Kangfuxin liquid was administered to promote healing of the intestinal mucosa and effectively manage clinical symptoms.CONCLUSION Mutations in the ACVRL1 and PLA2G4A genes may be one of the causes of chronic intestinal ulcers and bleeding in IBD.Orally administered Kangfuxin liquid may have therapeutic potential.

Different Effects of Pravastatin on Preeclampsia-like Symptoms in Different Mouse Models

Background： Pravastatin （Pra） exerts protective effects on preeclampsia. Preeclampsia is a multifactorial and pathogenic pathway syndrome. The present study compared the effects of Pra on clinical manifestations of preeclampsia in different pathogenic pathways. Methods： Two different preeclampsia-like mouse models used in this study were generated with Nω-nitro-L-arginine methyl ester （L-NAME） and used lipopolysaccharide （LPS） from day 7 of gestation, respectively. Pra treatment was administered on day 2 after the models were established in each group （L-NAME ＋ Pra, LPS ＋ Pra, and Control ＋ Pra, n = 8） or normal saline （NS） for the control group （L-NAME ＋ NS, LPS ＋ NS, and Control ＋ NS, n = 8）. Maternal weight, serum lipids, the histopathological changes, and lipid deposition in the liver and placenta were observed. The pregnancy outcomes were compared. The blood pressure analysis was carried out on repeated measurements of variance. Student＇s t-test was used for comparing the two groups. The enumeration data were compared by Chi-square test. Results： The mean arterial pressure （MAP） and 24-h urinary protein in the L-NAME ＋ NS and LPS ＋ NS groups were significantly higher than the Control ＋ NS group （F = 211.05 and 309.92 for MAP, t = 6.63 and 8.63 for 24-h urinary protein; all P 〈 0.05） and reduced in the L-NAME ＋ Pra group as compared to the L-NAME ＋ NS group （F = 208.60 for MAP, t = 6.77 for urinary protein; both P 〈 0.05）. Urinary protein was decreased in the LPS ＋ Pra group as compared to the LPS ＋ NS group （t = 5.33; P 〈 0.05）, whereas MAP had no statistical significance （F = 3.37; P 〉 0.05）. Compared to the Control ＋ NS group, the placental efficiency in the L-NAME ＋ NS and LPS ＋ NS groups decreased significantly （t = 3.09 and 2.89, respectively; both P 〈 0.05）; however, no significant difference was observed in L-NAME ＋ Pra and LPS ＋ Pra groups （t = 1.37 and 0.58, respectively; both P 〉 0.05）. Free fatty acid was elevated in the L-NAME ＋ NS group as compared to the Control ＋ NS group （t = 3.99; P 〈 0.05）

Effects of Rapamycin on Clinical Manifestations and Blood Lipid Parameters in Different Preeclampsia-like Mouse Models

Background： The pathogenesis of some types ofpreeclampsia is related to fatty acid oxidation disorders. Rapamycin can regulate fatty acid metabolism. This study aimed to investigate the effects of rapamycin on the clinical manifestations and blood lipid parameters in different preeclampsia-like mouse models. Methods： Two preeclampsia-like mouse models and a control group were established： L-NA （injected with Nco-nitro-L-arginine methyl ester）, LPS （injected with lipopolysaccharide）, and the control group with normal saline （NS）. The mouse models were established at preimplantation （PI）, early- and late-pregnancy （EP, LP） according to the time of pregnancy. The administration of rapamycin （RA; L-NA＋RA. LPS＋RA, and NS＋RA） or vehicle as controls （C; L-NA＋C, LPS＋C, NS＋C） were followed on the 2^nd day after the mouse models＇ establishment, Each subgroup consisted of eight pregnant mice. The mean arterial pressure （MAP）, 24-h urinary protein, blood lipid, fetus, and placental weight were measured. The histopathological changes and lipid deposition of the liver and placenta were observed. Student＇s t-test was used for comparing two groups. Repeated measures analysis of variance was used for blood pressure analysis. Qualitative data were compared by Chi-square test. Results： The MAP and 24-h urinary protein in the PI, EP, and LP subgroups of the L-NA＋C and LPS＋C groups were significantly higher compared with the respective variables in the NS＋C group （P 〈 0.05）. The preeclampsia-like mouse models were established successfully. There was no significant difference in the MAP between the PI, EP, and LP subgroups of the L-NA＋RA and L-NA＋C groups and the LPS＋RA and LPS＋C groups. The 24-h urine protein levels in the PI and EP subgroups of the L-NA＋RA group were significantly lower compared with the respective levels in the L-NA＋C groups （1037 ± 63 vs. 2127± 593 μg; 976 ±42 vs. 1238 ±72 μg; both P 〈 0.05）, also this effect appeared similar in the PI and EP subgroups of the LPS＋RA and LPS＋C groups （1022 ± 246 vs. 2141 ± 432 μg;

Role of mammalian target of rapamycin signaling pathway in regulation of fatty acid oxidation in a preeclampsia-like mouse model treated with pravastatin

Background: Fatty acid oxidation (FAO) disorder is involved in the pathogenesis of some cases of preeclampsia (PE). Several show that mammalian target of rapamycin (mTOR) signaling pathway is related to FAO. Pravastatin (Pra) can promote FAO in Nio-nitro-L-arginine methyl ester (L-NAME) PE-like mouse model in our previous study. This study aimed to investigate the effect of mTOR signaling pathway in PE-like model treated with Pra. Methods: Pregnant mice were randomly injected with L-NAME as PE-like model group or saline as control group respectively, from gestational 7th to 18th day. Giving Pra (L-NAME + Pra, Control + Pra, n= 8) or normal saline (NS;L-NAME + NS, Control + NS, n = 8) from gestational 8th to 18th day, the mice were sacrificed on day 18 and their liver and placental tissues were collected. Then the activation of mTOR and its substrates in the liver and placenta were detected. And the association between mTOR activation and mice were randomly injected with L-NAME as PE-like model group or saline as control group respectively, from serum free fatty acid (FFA) levels and the expression of long-chain 3-hydroxyacyl-coenzyme A dehydrogenase (LCHAD) were evaluated using Pearson correlation test. Differences between groups were analyzed using independent t-test or one-way analysis of variance (ANOVA). Results: Both in the maternal liver and placenta, the activation of mTOR protein and its effect on substrates increased significantly in the L-NAME + NS group and decreased significantly in the L-NAME + Pra group. The p-mTOR/mTOR protein ratio decreased in the L-NAME + Pra group significantly than that in the L-NAME + NS group both in liver and placenta (liver: 0.74±0.08 vs. 0.85± 0.06, t=2.95, P<0.05;placenta: 0.63±0.06 vs.0.77±0.06, t=4.64, P<0.05). The activation of mTOR protein in the liver and placenta negatively correlated with the expression of LCHAD in the L-NAME + NS group (liver: r=—0.745, P<0.05;placenta: r=-0.833, P< 0.05) and that in the maternal liver negatively correlated with the expression of

Currently,with the advent of high-repetition-rate laser-plasma experiments,the demand for online diagnosis for the X-ray spectrum is increasing because the laser-plasma-generated X-ray spectrum is very important for characterizing electron dynamics and applications.In this study,scintillators and silicon PIN(P-type–intrinsic-N-type semiconductor)diodes were used to construct a wideband online filter stack spectrometer.The X-ray sensor and filter arrangement was optimized using a genetic algorithm to minimize the condition number of the response matrix.Consequently,the unfolding error was significantly reduced based on numerical experiments.The detector responses were quantitatively calibrated by irradiating the scintillator and PIN diode with various nuclides and comparing the measuredγ-ray peaks.A prototype 15-channel spectrometer was developed by integrating an X-ray detector with front-and back-end electronics.The prototype spectrometer could record X-ray pulse signals at a repetition rate of 1 kHz.Furthermore,an optimized spectrometer was employed to record the real-time spectra of laser-driven bremsstrahlung sources.This optimized spectrometer offers a compact solution for spectrum diagnostics of ultrashort X-ray pulses,exhibiting improved accuracy in terms of spectrum measurements and repetition rates,and could be widely used in next-generation high-repetition-rate high-power laser facilities.
