Anlotinib as third- or further-line therapy for short-term relapsed small-cell lung cancer: subgroup analysis of a randomized phase 2 study (ALTER1202)

Patients with small-cell lung cancer (SCLC) relapse within months after completing previous therapies. This study aimed to investigate the efficacy and safety of anlotinib as third- or further-line therapy in patients with short-term relapsed SCLC from ALTER1202. Patients with short-term relapsed SCLC (disease progression within 3 months after completing ≥ two lines of chemotherapy) in the anlotinib (n = 67) and placebo (n = 34) groups were analyzed. The primary endpoint was progression-free survival (PFS). The secondary endpoints included overall survival, objective response rate (ORR), disease control rate, and safety. Anlotinib significantly improved median PFS/OS (4.0 vs. 0.7 months, P he ORR was 4.5%/2.9% in the anlotinib/placebo group (P = 1.000). The DCR in the anlotinib group was higher than that in the placebo group (73.1% vs. 11.8%, P he most common adverse events (AEs) were hypertension (38.8%), loss of appetite (28.4%), and fatigue (22.4%) in the anlotinib group and gamma-glutamyl transpeptidase elevation (20.6%) in the placebo group. No grade 5 AEs occurred. For patients with short-term relapsed SCLC, third- or further-line anlotinib treatment was associated with improved survival benefit. Further studies are warranted in this regard.

Lung cancer is associated with a heavy cancer-related burden in terms of patients’physical and mental health worldwide.Two randomized controlled trials,the US-National Lung Screening Trial(NLST)and Nederlands-Leuvens Longkanker Screenings Onderzoek(NELSON),indicated that low-dose CT(LDCT)screening results in a statistically significant decrease in mortality in patients with lung cancer,LDCT has become the standard approach for lung cancer screening.However,many issues in lung cancer screening remain unresolved,such as the screening criteria,high false-positive rate,and radiation exposure.This review first summarizes recent studies on lung cancer screening from the US,Europe,and Asia,and discusses risk-based selection for screening and the related issues.Second,an overview of novel techniques for the differential diagnosis of pulmonary nodules,including artificial intelligence and molecular biomarker-based screening,is presented.Third,current explorations of strategies for suspected malignancy are summarized.Overall,this review aims to help clinicians understand recent progress in lung cancer screening and alleviate the burden of lung cancer.

Objective： To observe the efficacy of the inhalation of an aerosonzeo group ~ sueloto^u v / preparation in treating orthotopic lung cancer in mouse models and assess the feasibility, safety, and effectiveness of this administration mode for lung cancer. Methods： Lewis lung carcinoma （LLC） cell strains were administered via intrathoracic injection to establish orthotopic lung cancer mouse models. After the tumor-bearing models were successfully established, as confirmed by computed tomography, the mice were administered by inhalation with an aerosolized GAS preparation （GAS group） or aerosolized normal saline （control group）. The anti-tumor effect of the aerosolized GAS preparation was evaluated histologically; meanwhile, the survival and quality of life were compared between these two groups. Results： The aerosolized GAS preparation showed remarkably anti-tumor effect, causing the necrosis of the orthotopic lung cancer cells in tumor-bearing mice. Furthermore, mice in the GAS group had significantly better qualitT of life and longer survival than those in control group. Conclusions： The inhalation of aerosolized GAS preparation may be a feasible, safe and effective solution for lung cancer

Regional Variation in Carbon Sequestration Potential of Forest Ecosystems in China

Enhancing forest carbon(C) storage is recognized as one of the most economic and green approaches to offsetting anthropogenic CO_2 emissions. However, experimental evidence for C sequestration potential(C_(sp)) in China's forest ecosystems and its spatial patterns remain unclear, although a deep understanding is essential for policy-makers making decisions on reforestation. here, we surveyed the literature from 2004 to 2014 to obtain C density data on forest ecosystems in China and used mature forests as a reference to explore C_(sp). The results showed that the C densities of vegetation and soil(0–100 cm) in China's forest ecosystems were about 69.23 Mg C/ha and 116.52 Mg C/ha, respectively. In mature forests, the C_(sp) of vegetation and soil are expected to increase to 129.26 Mg C/ha(87.1%) and 154.39 Mg C/ha(32.4%) in the coming decades, respectively. Moreover, the potential increase of C storage in vegetation(10.81 Pg C) is estimated at approximately twice that of soil(5.01 Pg C). Higher C_(sp) may occur in the subtropical humid regions and policy-makers should pay particular attention to the development of new reforestation strategies for these areas. In addition to soil nutrients and environment, climate was an important factor influencing the spatial patterns of C density in forest ecosystems in China. Interestingly, climate influenced the spatial patterns of vegetation and soil C density via different routes, having a positive effect on vegetation C density and a negative effect on soil C density. This estimation of the potential for increasing forest C storage provided new insights into the vital roles of China's forest ecosystems in future C sequestration. More importantly, our findings emphasize that climate constraints on forest C sequestration should be considered in reforestation strategies in China because the effects of climate were the opposite for spatial patterns of C density in vegetation and soil.Enhancing forest carbon(C) storage is recognized as one of the most economic and green approache

Solid oxide fuel cell is a promising energy conversion system which converts chemical energy into electrical energy directly. Electrolyte is the key component and determines the working temperature. In this paper,ceria and scandia co-doped zirconia electrolytes sintered from 1300 to 1550 ℃ were chosen as research objects. Scanning electron microscopy, X-ray diffraction and transmission electron microscopy were performed to characterize the ceramic samples. The effects of grain size and grain boundary element segregation on the electrical conductivity were focused. Electrochemical impedance spectroscopy was used to calculate the bulk, grain boundary and specific grain boundary conductivity. Results show that the bulk and grain boundary ionic conductivity increases with the increasing grain size.However, the specific grain boundary conductivity decreases with the increasing grain size. This is explained by the fact that Sc^(3+) is segregated at the grain boundary, which leads to higher oxygen vacancy concentration when sintered at lower temperature.

稳定的土壤pH对维持生态系统的结构和功能至关重要.中国是酸沉降最严重的区域,长期高剂量的酸沉降可能将导致土壤pH显著降低,甚至造成严重的区域生态环境问题.然而,目前有关酸沉降对中国森林土壤酸化的影响还未见报道.本文利用土壤酸化过程模型结合长期监测的大气沉降、植物-土壤养分含量数据、社会发展、气候和污染控制情景,评估并预测1980~2050年间大气沉降导致的森林土壤酸化趋势.结果表明:1980~2019年,表层土壤pH和碱基饱和度分别平均下降0.56和18%.氮沉降的贡献小于硫沉降,但它仍在持续增加.预测结果表明,2020~2050年可持续发展结合高强度减排情景相较于不采取措施情景可使土壤pH下降减少60%,但仍不能逆转土壤pH降低.因此,在未来不仅需要继续控制酸性污染物的排放,还需要加强氮的控制,并在土壤酸化高风险区采取相应恢复措施.

Reconstruction of the thoracic tracheal defects with portions of deepithelialized myocutaneous flaps after resection of a large tumor

Objective： To study the possibility of using portions of deepithelialized myocutaneous flaps to the reconstruction of thoracic tracheal defects after resection of a large tumor. Methods： From June 2007 to June 2012, five cases of defects of the thoracic trachea were reconstructed by applying portions of deepithelialized myocutaneous flaps. The patients were 27-61 years old with 4 male cases and 1 female. The cervical trachea ranged in diameter from 4-8.5 cm with circumferences of approximately 1/3-2/5 of the bronchial circumference. Results： M1 five patients with thoracic tracheal defects after resection of a large tumor were cured of portions of deepithelialized myocutaneous flaps, with no tracheal stricture remaining and vomica successfully eliminated. During the first 1 to 3 months after the operation, bronchoscopy showed that the tracheal lumens were smooth, and the visible skin of the musculocutaneous flaps became gray and exhibited a small amount of white discharge. Conclusions： Despite this being a small series and short follow-up, this thoracic tracheal reconstruction with portions of deepithelialized myocutaneous flaps shows encouraging preliminary results and could be an alternative to other methods for the treatment of carefully selected patients with thoracic tracheal defects.

Objective： The objective of the current study was to evaluate the feasibility and safety of nonintubated nniportal video-assisted thoracoscopic surgery （VATS） for the management of primary spontaneous pneumothorax （PSP）. Methods： From November 2011 to June 2013, 32 consecutive patients with PSP were treated by nonintubated uniportal thoracoscopic bullectomy using epidnral anaesthesia and sedation without endotracheal intubation. An incision 2 cm in length was made at the 6th intercostal space in the median axillary line. The pleural space was entered by blunt dissection for placement of a soft incision protector. Instruments were then inserted through the incision protector to perform thoracoscopic bullectomy. Data were collected within a minimum follow-up period of 10 months. Results： The average time of surgery was 49.0 rain （range, 33-65 rain）. No complications were recorded. The postoperative feeding time was 6 h. The mean postoperative chest tube drainage and hospital stay were 19.3 h and 41.6 h, respectively. The postoperative pain was mild for 30 patients （93.75%） and moderate for two patients （6.25%）. No recurrences ofpneumothorax were observed at follow-up. Conclusions： The initial results indicated that nonintubated uniportal video-assisted thoracoscopic operations are not only technically feasible, but may also be a safe and less invasive alternative for select patients in the management of PSP. This is the first report to include the use of a nonintubated uniportal technique in VATS for such a large number of PSP cases. Further work and development of instruments are needed to define the applications and advantages of this technique.
