Early screening and diagnosis strategies of pancreatic cancer:a comprehensive review

Pancreatic cancer is a highly malignant digestive system tumor with a poor *** pancreatic cancer patients are diagnosed at an advanced stage or even metastasis due to its highly aggressive characteristics and lack of typical early ***,an early diagnosis of pancreatic cancer is crucial for improving its ***,screening is often applied in high-risk individuals to achieve the early diagnosis of pancreatic *** understanding the risk factors of pancreatic cancer and pathogenesis could help us identify the high-risk population and achieve early diagnosis and timely treatment of pancreatic ***,accumulating studies have been undertaken to improve the detection rate of different imaging methods and the diagnostic accuracy of endoscopic ultrasound-guided fine-needle aspiration(EUS-FNA)which is the golden standard for pancreatic cancer *** addition,there are currently no biomarkers with sufficient sensitivity and specificity for the diagnosis ofpancreatic cancer to be applied in the clinic. As the only serum biomarkerapproved by the United States Food and Drug Administration, carbohydrateantigen 19-9 (CA19-9) is not recommended to be used in the early screeningof pancreatic cancer because of its limited specificity. Recently, increasingnumbers of studies focused on the discovering of novel serum biomarkersand exploring their combination with CA19-9 in the detection of pancreaticcancer. Besides, the application of liquid biopsy involving circulating tumor cells(CTCs), circulating tumor DNA (ctDNA), microRNAs (miRNAs), and exosomesin blood and biomarkers in urine, and saliva in pancreatic cancer diagnosis aredrawing more and more attention. Minimally invasive techniques have been widely applied in general surgery for decades,but the generalization in pancreatic resection underwent a hard process for the complexity of the *** current studies suggested that the safety and feasibility of minimally invasive pancreatic surgery(MIPS)were approximately equivalent to that of open surgery,but the overall benefit and the oncological outcomes of MIPS remained to be *** outcomes were observed in the initial learning curve phase and low-volume hospitals,thus establishing a program that can well assess the learning process and minimize the harm to patients was *** ensure safety,the patients should be strictly selected in the early phase of the learning curve,and a careful and judicious stepwise should be considered to broaden the indications.

MiR-135a biogenesis and regulation in malignancy:a new hope for cancer research and therapy

MicroRNAs(miRNAs)are evolutionarily conserved small non-coding RNAs that affect posttranscriptional regulation by binding to the 3′-untranslated region of target messenger ***-135a is a critical miRNA that regulates gene expression,and many studies have focused on its function in cancer ***-135a is dysregulated in various cancers and regulates cancer cell proliferation and invasionvia several signaling pathways,such as the MAPK and JAK2/STAT3 ***-135a has also been found to promote or inhibit the epithelial-mesenchymal transition and chemoresistance in different *** studies have discovered the value of miR-135a as a novel biomarker for cancer diagnosis and *** studies have suggested the potential of therapeutically manipulating miR-135a to improve the outcome of cancer *** these findings have demonstrated the role of miR-135a in cancer progression and clinical applications,a number of questions remain to be answered,such as the dual functional roles of miR-135a in *** this review,we summarize the available studies regarding miR-135a and cancer,including background on the biogenesis and expression of miR-135a in cancer and relevant signaling pathways involved in miR-135a-mediated tumor *** also focus on the clinical application of miR-135a as a biomarker in diagnosis and as a therapeutic agent or target in cancer treatment,which will provide a greater level of insight into the translational value of miR-135a.

We introduce a modified weak value that is related to the mean value of input meter *** the help of the modified weak value,the validity conditions for various modified versions of weak value formalism are investigated,in terms of the dependence of the pointer shift on the mean value of the input *** weak value formalism,often used to represent the pointer shift,with the modified weak value is of great use in simplifying calculations and giving guidance of practical experiments whenever the mean value of the input meter variable is *** simulation in a qubit system is presented and coincident well with our theoretical result.

Pre-and post-selected(PPS) measurement, especially the weak PPS measurement, has been proved to be a useful tool for measuring extremely tiny physical parameters. However, it is difficult to retain both the attainable highest measurement sensitivity and precision with the increase of the parameter to be measured. Here, a modulated PPS measurement scheme based on coupling-strength-dependent modulation is presented with the highest sensitivity and precision retained for an arbitrary coupling strength. This idea is demonstrated by comparing the modulated PPS measurement scheme with the standard PPS measurement scheme in the case of unbalanced input meter. By using the Fisher information metric, we derive the optimal pre-and post-selected states, as well as the optimal coupling-strength-dependent modulation without any restriction on the coupling strength. We also give the specific strategy of performing the modulated PPS measurement scheme, which may promote practical application of this scheme in precision metrology.

We theoretically introduce the statistical uncertainty of photon number and phase error to discuss the precision of parameters to be measured based on weak measurements. When the photon counting scheme is used, we discuss the relative accuracy of the system in the presence of phase error by using the orthogonal and nonorthogonal pre-and postselected states, respectively. When using the measurement scheme of pointer shift, we discuss the measurement accuracy in the presence of phase error, pointer resolution, and statistical uncertainty. These results give a guide way to get the smallest relative precision and deepen our understanding about weak measurement.

胰腺癌是高病死率的消化系统恶性肿瘤,手术技术的进步并未明显改善病人预后。随着综合治疗理念的普及,胰腺癌的外科治疗也由“surgery first”进入多学科诊断与治疗协作时代,在术前评估、新辅助治疗及微创技术的应用、术后辅助治疗等方面均取得较大进展。上述治疗的新理念、新方法可在一定程度上改善病人预后,但临床应用还存在争议。如何推动胰腺癌外科治疗向个体化、微创化、精准化方向发展仍是学术界的重要课题。笔者总结近年来多学科诊断与治疗协作理念下胰腺癌外科治疗的新进展,探讨目前其存在的局限和争议,针对胰腺癌外科治疗的未来发展提出思考,以期进一步促进其发展,改善病人预后。

胰腺癌是高度恶性的消化系统肿瘤,手术技术的进步未能明显改善患者预后。随着多学科诊疗理念的普及,新辅助治疗在胰腺癌中的地位逐渐凸显。新辅助治疗旨在缩小肿瘤,消除微转移病灶,进而提高R0切除率,减少复发转移。由于胰腺癌独特的肿瘤微环境,其对放疗、化疗敏感性较低,新辅助治疗并非对所有胰腺癌患者有效,且在患者选择、方案制定、治疗后再评估等方面仍存在诸多争议,临床应用仍不十分成熟。当前应重视新辅助治疗在胰腺癌中的作用,鼓励开展高质量临床研究,充分发挥多学科诊疗优势,为胰腺癌患者制定个体化新辅助治疗方案,以进一步提升患者预后。
