Transjugular intrahepatic portosystemic shunt in liver transplant recipients

AIM： To evaluate the efficacy of transjugular intrahepatic portosystemic shunts （TIPSs） after liver transplantation （LT）. METHODS： Between November 1996 and December 2005, 10 patients with severe recurrent hepatitis C virus infection （n = 4）, ductopenic rejection （n = 5） or portal vein thrombosis （n = 1） were included in this analysis. Eleven TIPSs （one patient underwent two TIPS procedures） were placed for management of therapy-refractory ascites （n = 7）, hydrothorax （n = 2） or bleeding from colonic varices （n = 1）. The median time interval between LT and TIPS placement was 15 （4-158） mo. RESULTS： TIPS placement was successful in all patients. The mean portosystemic pressure gradient was reduced from 12.5 to 8.7 mmHg. Complete and partial remission could be achieved in 43% and 29% of patients with ascites. Both patients with hydrothorax did not respond to TIPS. No recurrent bleeding was seen in the patient with colonic varices. Nine of 10 patients died during the study period. Only one of two patients, who underwent retransplantation after the TIPS procedure, survived. The median survival period after TIPS placement was 3.3 （range 0.4-20） too. The majority of patients died from sepsis with multiorgan failure. CONCLUSION： Indications for TIPS and technical performance in LT patients correspond to those in non-transplanted patients. At least partial control of therapy-refractory ascites and variceal bleeding could be achieved in most patients. Nevertheless, survival rates were disappointing, most probably because of the advanced stages of liver disease at the time of TIPS placement and the high risk of sepsis as a consequence of immunosuppression.

A betatron tune measurement system based on bunch-by-bunch transverse feedback at the Duke storage ring

To combat electron beam instabilities, a digital bunch-by-bunch transverse feedback （TFB） system has been developed for the Duke storage ring. while it is capable of suppressing transverse beam instabilities for multibunch operation, the TFB system has not been needed for typical operation of the Duke storage ring. To explore the great potential of this system, we have developed beam diagnostic techniques using the TFB, in particular, the TFB based tune measurement techniques. The tune measurement technique allows us to conduct fast chromaticity measurements, compared with the existing chromaticity measurement system using a network analyzer. This new tune measurement system also enables us to measure the bunch tune for multibunch operation of the Duke storage ring. with the TFB based tune measurement system, we have studied the tune stability of the electron beam in the Duke storage ring. This tune system has also been used to calibrate the tune knob for the Duke storage ring.

weldability and Microstructure of Nickel-Silicon Based Alloys

The NiSix based alloy typically has poor weldability due to its lower ductility. A limited amount of work has been performed on the weldability of NiSix based alloys. Therefore, the effect of heat treatment and welding parameters on weldability of the alloys, and the relationship between the weldability and microstructure were studied. The results show that the as-cast Ni-Si-Nb-B alloy （Ni 76. 5%, Si 20%, Nb 3%, and B 0. 5%） could be successfully welded after preheating at 600 ℃. The welding procedure should be performed on the alloys before any heat treatment and a preheating at 600 ℃ should be used. The fusion zone is harder than the matrix due to a large amount of 7 phase and a finer microstructure. The cracks are predominantly intergranular in heat affected zone and associated with the needle-like ）, phase. The heat treatment before welding increases the tendency of cracking in the fusion zone.

The impact of the Rural Minimum Living Standard Guarantee(Rural Dibao)Program on child nutrition outcomes

The Rural Minimum Living Standard Guarantee(Rural Dibao)is an important unconditional cash transfer program to alleviate poverty in rural *** the importance of children’s nutrition in breaking poverty cycles,little is known about the impact of Rural Dibao on child nutrition *** China Family Panel Studies(CFPS),this paper examines the effects of Rural Dibao on child nutrition outcomes and investigates potential pathways and heterogeneous *** exploit propensity score matching and difference-in-differences techniques to evaluate the effects of the Rural Dibao program on child nutrition *** results suggest that Rural Dibao significantly impacts the nutrition outcomes of children up to 15 years of ***,our results suggest that Rural Dibao improves child height-to-age z-scores by 1.05 standard deviations and lowers the probability of stunting by 11.9 percentage *** analyses suggest that increased protein intake is the main pathway through which Rural Dibao participation contributes to better nutrition *** also find that the effect of the program is more pronounced among girls,children who are non-left-behind or live with highly educated mothers,and those from low-income families and poor *** findings suggest that Rural Dibao participation helps improve child nutrition outcomes through improving diet quality.

Clinicopathological and prognostic implications of endoglin (CD105) expression in hepatocellular carcinoma and its adjacent non-tumorous liver

AIM: The expression pattern of endoglin (CD105) in hepatocellular carcinoma (HCC) has not been reported so far. we hypothesized that CD105 could differentially highlight a subset of microvessels in HCC, and intratumoral microvessel density (IMVD) by CD105 immunostaining (IMVD-CD105) could provide better prognostic information than IMVD by CD34 immunostaining (IMVD-CD34).METHODS: Paraffin blocks of tumor and adjacent nontumorous liver tissues from 86 patients who underwent curative resection of HCC were used for this study. Serial sections were stained for CD105 and CD34, respectively,to highlight the microvessels. IMVD was counted according to a standard ***: In the HCC tissues, CD105 was either negatively or positively stained only in a subset of microvessels. In contrast, CD34 showed positive and more extensive microvessel staining in all cases examined. However, in the adjacent non-tumorous liver sections, CD105 showed a diffuse pattern of microvessel staining in 20 of 86 cases,while CD34 showed negative or only focal staining of the sinusoids around portal area. Correlation with clinicopathological data demonstrated that lower scores of IMVD-CD105 were found in larger sized tumors [mean 41.4/0.74 mm2 (＞5 cm tumor) vs 65.9/0.74 mm2(≤ 5 cm tumor), P = 0.043] and more aggressive tumors,as indicated by venous infiltration [36.8/0.74 mm2 (present)vs 64.2/0.74 mm2 (absent), P = 0.020], microsatellite nodules [35.1/0.74 mm2 (present) vs 65.9/0.74 mm2(absent), P = 0.012], and advanced TNM tumor stage [38.8/0.74 mm2 (stage 3 or 4) vs 68.3/0.74 mm2 (stage 1or 2), P = 0.014]. No prognostic significance was observed when median values were used as cut-off points using either IMVD-CD105 or IMVD-CD34. However, the presence of the diffuse pattern of CD105 expression in the adjacent non-tumorous liver tissues predicted a poorer disease-free survival (median 8.6 vs 21.5 mo, P = 0.026).CONCLUSION: Our data demonstrate that a lower IMVDCD105 is associated with larger

Control of the formation and stability of reverted austenite is critical in achieving a favorable combination of strength,ductility,and toughness in high-strength *** this work,the effects of Cu precipitation on the austenite reversion and mechanical properties of maraging stainless steels were investigated by atom probe tomography,transmission electron microscopy,and mechanical *** results indicate that Cu accelerates the austenite reversion kinetics in two manners:first,Cu,as an austenite stabilizer,increases the equilibrium austenite fraction and hence enhances the chemical driving force for the austenite formation,and second,Cu-rich nanoprecipitates promote the austenite reversion by serving as heterogeneous nucleation sites and providing Ni-enriched chemical conditions through interfacial *** addition,the Cu precipitation hardening compensates the strength drop induced by the formation of soft reverted *** tensile deformation,the metastable reverted austenite transforms to martensite,which substantially improves the ductility and toughness through a transformation-induced plasticity(TRIP)*** Cu-added maraging stainless steel exhibits a superior combination of a yield strength of~1.3 GPa,an elongation of~15%,and an impact toughness of~58 J.

PRECIPITATION BEHAVIOR OF M_(2)N IN A HIGH-NITROGEN AUSTENITIC STAINLESS STEEL DURIng ISOTHERMAL AGIng

The precipitation behavior of M2N and the microstructural evolution in a Cr-Mn austenitic stainless steel with a high nitrogen content of 0.43mass% during isothermal aging has been investigated using optical microscopy （ OM）, scanning electron microscopy （SEM）, and transmission electron microscopy （TEM）. The aging treatments have led to the decomposition of nitrogen supersaturated austenitic matrix through discontinuous cellular precipitation. The precipitated cells comprise alternate lamellae of M2N precipitate and austenitic matrix. This kind of precipitate morphology is similar to that of pearlite. However, owing to the non-eutectoidic mechanism of the reaction, the growth characteristic of the cellular precipitates is different from that of pearlite in Fe-C binary alloys. M2N precipitate in the cell possesses a hexagonal crystal structure with the parameters a = 0.4752nm and c = 0.4429nm, and the orientation relationship between the M2V precipitates and austenite determined from the SADP is [01^-10]M2N//[101]γ, [2^-1^-10]M2N//[010]γ.
