Performance and comparison of assessment models to predict 30-day mortality in patients with hospital-acquired pneumonia

Background:Hospital-acquired pneumonia(HAP)is the most common hospital-acquired infection in China with substantial morbidity and *** no specific risk assessment model has been well validated in patients with *** aim of this study was to investigate the published risk assessment models that could potentially be used to predict 30-day mortality in HAP patients in non-surgical ***:This study was a single-center,retrospective *** total,223 patients diagnosed with HAP from 2012 to 2017 were included in this *** and laboratory data during the initial 24 hours after HAP diagnosis were collected to calculate the pneumonia severity index(PSI);consciousness,urea nitrogen,respiratory rate,blood pressure,and age≥65 years(CURB-65);Acute Physiology and Chronic Health Evaluation II(APACHE II);Sequential Organ Failure Assessment(SOFA);and Quick Sequential Organ Failure Assessment(qSOFA)*** discriminatory power was tested by constructing receiver operating characteristic(ROC)curves,and the areas under the curve(AUCs)were ***:The all-cause 30-day mortality rate was 18.4%(41/223).The PSI,CURB-65,SOFA,APACHE II,and qSOFA scores were significantly higher in non-survivors than in survivors(all P<0.001).The discriminatory abilities of the APACHE II and SOFA scores were better than those of the CURB-65 and qSOFA scores(ROC AUC:APACHE II ***-65,0.863 vs.0.744,Z=3.055,P=0.002;APACHE II ***,0.863 vs.0.767,Z=3.017,P=0.003;SOFA ***-65,0.856 vs.0.744,Z=2.589,P=0.010;SOFA ***,0.856 vs.0.767,Z=2.170,P=0.030).The cut-off values we defined for the SOFA,APACHE II,and qSOFA scores were 4,14,and ***:These results suggest that the APACHE II and SOFA scores determined during the initial 24 h after HAP diagnosis may be useful for the prediction of 30-day mortality in HAP patients in non-surgical *** qSOFA score may be a simple tool that can be used to quickly identify severe infections.

Spontaneous symmetry breaking(SSB)plays a central role in understanding a large variety of phenomena associated with phase transitions,such as superfluid and *** far,the transition from a symmetric vacuum to a macroscopically ordered phase has been substantially *** process bridging these two distinct phases is critical to understanding how a classical world emerges from a quantum phase transition,but so far remains unexplored in *** here report an experimental demonstration of such a process with a quantum Rabi model engineered with a superconducting *** move the system from the normal phase to the superradiant phase featuring two symmetry-breaking field components,one of which is observed to emerge as the classical *** results demonstrate that the environment-induced decoherence plays a critical role in the SSB.

Extending Structures for Gel'fand-Dorfman Bialgebras

Gel'fand-Dorfman bialgebra,which is both a Lie algebra and a Novikov algebra with some compatibility condition,appeared in the study of Hamiltonian pairs in completely integrable *** also emerged in the description of a class special Lie conformal algebras called quadratic Lie conformal *** this paper,we investigate the extending structures problem for Gel'fand-Dorfman bialgebras,which is equivalent to some extending structures problem of quadratic Lie conformal ***,given a Gel'fand-Dorfman bialgebra(A,o,[.,.]),this problem is how to describe and classify all Gel'fand-Dorfman bialgebra structures on a vector space E(A⊂E)such that(A,o,[.,.])is a subalgebra of E up to an isomorphism whose restriction on A is the identity *** by the theories of extending structures for Lie algebras and Novikov algebras,we construct an object gH2(V,A)to answer the extending structures problem by introducing the notion of a unified product for Gel'fand-Dorfman bialgebras,where V is a complement of A in *** particular,we investigate the special case when dim(V)=1 in detail.

One-year results for myopia control of orthokeratology with different back optic zone diameters: a randomized trial using a novel multispectral-based topographer

AIM:To present the 1-year results of a prospective cohort study investigating the efficacy,potential mechanism,and safety of orthokeratology(ortho-k)with different back optic zone diameters(BOZD)for myopia control in ***:This randomized clinical study was performed between Dec.2020 and *** were randomly assigned to three groups wearing ortho-k:5 mm BOZD(5-MM group),5.5 mm BOZD(5.5-MM group),and 6 mm BOZD(6-MM group).The 1-year data were recorded,including axial length,relative peripheral refraction(RPR,measured by multispectral refractive topography,MRT),and visual *** contrast sensitivity(CS)was evaluated by CSV-1000 instrument with spatial frequencies of 3,6,12,and 18 cycles/degree(c/d);the corneal higher-order aberrations(HOAs)were measured by iTrace aberration *** one-way ANOVA was performed to assess the differences between the three *** correlation between the change in AL and RPR was calculated by Pearson’s correlation ***:The 1-year results of 20,21,and 21 subjects in the 5-MM,5.5-MM,and 6-MM groups,respectively,were *** were no statistical differences in baseline age,sex,or ocular parameters between the three groups(all P>0.05).At the 1-year visit,the 5-MM group had lower axial elongation than the 6-MM group(0.07±0.09 vs 0.18±0.11 mm,P=0.001).The 5-MM group had more myopic total RPR(TRPR,P=0.014),with RPR in the 15°–30°(RPR 15–30,P=0.015),30°–45°(RPR 30–45,P=0.011),temporal(RPR-T,P=0.008),and nasal area(RPR-N,P<0.001)than the 6-MM *** 15–30 in the 5.5-MM group was more myopic than that in the 6-MM group(P=0.002),and RPR-N in the 5-MM group was more myopic than that in the 5.5-MM group(P<0.001).There were positive correlations between the axial elongation and the change in TRPR(r=0.756,P<0.001),RPR 15–30(r=0.364,P=0.004),RPR 30–45(r=0.306,P=0.016),and RPR-N(r=0.253,P=0.047).The CS decreased at 3 c/d(P<0.001),and the corneal HOAs increased in the 5-MM group(P=0.030).CONCL
