Acupuncture for migraine without aura:a systematic review and metaanalysis

Background： Migraine without aura （MWoA）, the most common type of migraine, has great impacts on quality of life for migraineurs. Acupuncture is used in the treatment and prevention of migraine for its analgesic effects. Objective： The aim of this systematic review and meta-analysis is to systematically assess the therapeutic and preventive effect of acupuncture treatment and its safety for MWoA,Search strategy： Nine electronic databases （PubMed, MEDLINE, Cochrane Library, Lilacs, Embase, China National Knowledge Infrastructure （CNKI）, Chongqing VIP （CQV1P）, Wanfang Data and Chinese Clinical Trial Registry （ChiCTR）） were systematically searched from their beginning through June 2017 using MeSH terms such as ＂acupuncture, acupuncture therapy, electro-acupuncture, ear acupuncture, acupuncture points, acupuncture analgesia,＂ and ＂migraine disorders, cluster headache.＂ Manual searching included other conference abstracts and reference lists. Inclusion criteria： Randomized controlled trials （RCTs） with a clinical diagnosis of MWoA, which were treated with acupuncture versus oral medication or sham acupuncture treatment. Data extraction and analysis： Two evaluators screened and collected literature independently; they extracted information on participants, study design, interventions, follow-up, withdrawal and adverse events and assessed risk of bias and quality of the acupuncture intervention. The primary outcomes were frequency of migraine （FM） and number of migraine days （NM）. Secondary outcomes included the visual analogue scale （VAS） score, effective rate （ER） and adverse events. Pooled estimates were calculated as mean difference （MD） with 95% confidence interval （CI） for continuous data and relative risk （RR） with 95% CI for dichotomous data. Results： Overall, 14 RCTs including 1155 participants were identified. The analysis found that acupunc- ture had a significant advantage over medication in reducing FM （MD）=-1.50; 95% CI： -2.32 to -0.68; P〈 0.001） and VAS score （MD =0.97; 95% CI： 0.63-1.31; P〈 0.00001） and

Myofibrillogenesis regulator-1 overexpression is associated with poor prognosis of gastric cancer patients

AIM: To investigate the expression of myofibrillogenesis regulator-1 (MR-1) in relation to clinicopathological parameters and postoperative survival in a group of Chinese patients with gastric cancer. METHODS: In our previous study of human wholegenome gene expression profiling, the differentially expressed genes were detected in the gastric cancer and its adjacent noncancerous mucosa. We found that MR-1 was associated with the location and differentiation of tumors. In this study, MR-1 protein expression was determined by immunohistochemistry in specimens of primary cancer and the adjacent noncancerous tissues from gastric cancer patients. A set of real-time quantitative polymerase chain reaction assays based on the Universal ProbeLibrary-a collection of 165 presynthesized, fluorescence-labeled locked nucleic acid hydrolysis probes-was designed specifically to detect the expression of MR-1 mRNA. The correlation was analyzed between the expression of MR-1 and other tumor characteristics which may influence the prognosis of gastric cancer patients. A retrospective cohort study on the prognosis was carried out and clinical data were collected from medical records. RESULTS: MR-1 mRNA and protein could be detected in gastric cancer tissues as well as in matched noncancerous tissues. MR-1 was up-regulated at both mRNA (5.459 ± 0.639 vs 1.233 ± 0.238, P < 0.001) and protein levels (34.2% vs 13.2%, P = 0.003) in gastric cancer tissues. Correlation analysis demonstrated that high expression of MR-1 in gastric cancer was significantly correlated with clinical stage (P = 0.034). Kaplan-Meier analysis showed that the postoperative survival of the MR-1 positive group tended to be poorer than that of the MR-1 negative group, and the difference was statistically significant (P = 0.002). Among all the patients with stageⅠ-Ⅳ carcinoma, the 5-year survival rates of MR-1 positive and negative groups were 50.40% and 12.70%, respectively, with respective median survival times of 64.27 mo (95%CI: 13.41-115.13) and 16.77 mo (95%CI: 8.

Evolution of viral RNA in a Chinese patient to interferon/ribavirin therapy for hepatitis C

Objective： The combination of interferon （IFN） and ribavirin （RBV） is the standard therapy for hepatitis C virus （HCV） infection. HCV genotype 2a has proved more amenable to the therapy, but its efficacy is yet fimited. This study aimed to investigate the mechanism of the poor response in a case ofHCV genotype 2a infection. Methods： We analyzed dynamic change of HCV RNA from a patient, infected with HCV genotype 2a, showing a poor virological response to 1FN/RBV as judged 12 weeks after initiation of the therapy by HCV clone sequencing. Then we constructed subgenomic Japanese fulminant hepatitis-1 （JFH1） replicon and different chimeric replicons with humanized Gaussia luciferase gene. The chimeric replicons were derived from subgenomic JFH1 replicon, in which the NS5A region was replaced by the patient＇s sequence from the pre/post- treatment, and the chimeric replicons＇ susceptibility to IFN were evaluated by relative Gausia Luciferase activity. Results： The pretreatment HCV sequences appeared almost uniform, and the quasispecies variation was further more simplified after 12 weeks of therapy. Besides, the quasispecies variation seemed to be more diversified in the NS5A, relatively, a region crucial for IFN response, and each of chimeric replicons exhibited distinct response to IFN. Conclusions： During the course of the chronic infection, HCV population seems to be adapted to the patient＇s immunological system, and further to be selected by combination of 1FN/RBV therapy, indicating quasispecies may completely eliminated by addition of other drugs with targets different from those of IFN. In addition, each different response of chimeric replicon to IFN is most likely related to amino acid changes in or near the IFN-sensitivity determining region （ISDR） of NSSA during chronic infection and IFN/RBV therapy.

Objective: Tumor heterogeneity renders identification of suitable biomarkers of gastric cancer(GC)challenging. Here, we aimed to identify prognostic genes of GC using computational analysis.Methods: We first used microarray technology to profile gene expression of GC and paired nontumor tissues from 198 patients. Based on these profiles and patients’ clinical information, we next identified prognostic genes using novel computational approaches. Phosphoglucose isomerase, also known as glucose-6-phosphate isomerase(GPI), which ranked first among 27 candidate genes, was further investigated by a new analytical tool namely enviro-geno-pheno-state(E-GPS) analysis. Suitability of GPI as a prognostic marker, and its relationship with physiological processes such as metabolism, epithelial-mesenchymal transition(EMT), as well as drug sensitivity were evaluated using both our own and independent public datasets.Results: We found that higher expression of GPI in GC correlated with prolonged survival of patients.Particularly, a combination of CDH2 and GPI expression effectively stratified the outcomes of patients with TNM stage Ⅱ/Ⅲ. Down-regulation of GPI in tumor tissues correlated well with depressed glucose metabolism and fatty acid synthesis, as well as enhanced fatty acid oxidation and creatine metabolism, indicating that GPI represents a suitable marker for increased probability of EMT in GC cells.Conclusions: Our findings strongly suggest that GPI acts as a novel biomarker candidate for GC prognosis,allowing greatly enhanced clinical management of GC patients. The potential metabolic rewiring correlated with GPI also provides new insights into studying the relationship between cancer metabolism and patient survival.

The role of perioperative chemotherapy for gastric cancer has been established for gastric cancers in their advanced stage.In most parts of the world,even in Japan and Korea,local recurrence of gastric cancer following curative resection remains a problem.Should radiation be added to chemotherapy to achieve better local and regional control? What is the current evidence? What are the concerns regarding neoadjuvant chemoradiation in terms of safety,efficacy and survival benefit? After a serious review of the literature,the authors conclude that it is still too early to get a definitive answer but radiation seems promising.It may bring a higher pathological response rate.Rationally,more high level clinical trials are needed to confirm the role of radiotherapy in the neoadjuvant setting or to ascertain subsets of patients who may benefit from it.It is of note that surgeons should pay attention to possible complicated circumstances following radiotherapy,maintain proper nutrition status and minimize the occurrence of postoperative complications.As few data are available in Japan and Korea,interpretation and implementation of neoadjuvant radiation or chemoradiation should be done with caution.

Forensic Study on Objective Evaluation of Visual Acuity of Ametropia with the Event-related Potential P3

Objective In this study,we aimed to assess the characteristics of the P3 component from an event-related potential(ERP)that was induced by visual acuity(VA)processing.furthermore,we sought to provide electrophysiological evidence for the objective evaluation of VA.Methods We recruited 32 participants with myopia-related ametropia.They reported no other ocular diseases and had an uncorrected VA of 4.0 in both eyes.We used the block letter“E”at different visual angles and orientations as the graphic stimuli.The oddball paradigm,consisting of 4 modules,was used for ERP analysis.The standard stimuli of each module were identical,with a visual angle of 1°15′.The visual angles of the target stimuli were 1°15′,55′,24′,and 15′.The VA test was performed on each eye separately for all participants,and all characteristics of the P3 component were analyzed.Results There was no significant difference in the P3 peak letencies between the target stimulation angle 1°15′group and the 55′group,or between the target stimulation angle 24′group and the 15′group.There was a significant difference in the P3 peak letencies between the target stimulation angle 1°15′group and the 24′group as well as the 15′group.There was a significant difference in the P3 peak letencies between the target stimulation angle 55′group and the 24′group as well as the 15′group.No significant differences were observed in the P3 amplitude between modules.Conclusion In the oddball paradigm,P3 elicitation indicated a cognitive response to the target stimuli.These data showed that the characteristics of P3 can be used as an objective evaluation of VA.

Nomogram for predicting the prognosis of tumor patients with sepsis after gastrointestinal surgery

BACKGROUND There were few studies on the prognosis of tumor patients with sepsis after gastrointestinal surgery and there was no relevant nomogram for predicting the prognosis of these patients.AIM To establish a nomogram for predicting the prognosis of tumor patients with sepsis after gastrointestinal surgery in the intensive care unit(ICU).METHODS A total of 303 septic patients after gastrointestinal tumor surgery admitted to the ICU at Peking University Cancer Hospital from January 1, 2013 to December 31, 2020 were analysed retrospectively. The model for predicting the prognosis of septic patients was established by the R software package.RESULTS The most common infection site of sepsis after gastrointestinal surgery in the ICU was abdominal infection. The 90-d all-cause mortality rate was 10.2% in our study group. In multiple analyses, we found that there were statistically significant differences in tumor type, septic shock, the number of lymphocytes after ICU admission, serum creatinine and total operation times among tumor patients with sepsis after gastrointestinal surgery(P < 0.05). These five variables could be used to establish a nomogram for predicting the prognosis of these septic patients. The nomogram was verified, and the initial C-index was 0.861. After 1000 internal validations of the model, the C-index was 0.876, and the discrimination was good. The correction curve indicated that the actual value was in good agreement with the predicted value.CONCLUSION The nomogram based on these five factors(tumor type, septic shock, number of lymphocytes, serum creatinine, and total operation times) could accurately predict the prognosis of tumor patients with sepsis after gastrointestinal surgery.

Involvement of Opioid Peptides in the Analgesic Effect of Spinal Cord Stimulation in a Rat Model of Neuropathic Pain

Spinal cord stimulation(SCS)-induced analgesia was characterized,and its underlying mechanisms were examined in a spared nerve injury model of neuropathic pain in rats.The analgesic effect of SCS with moderate mechanical hypersensitivity was increased with increasing stimulation intensity between the 20%and 80%motor thresholds.Various frequencies(2,15,50,100,10000 Hz,and 2/100 Hz dense-dispersed)of SCS were similarly effective.SCS-induced analgesia was maintained without tolerance within 24 h of continuous stimulation.SCS at 2 Hz significantly increased methionine enkephalin content in the cerebrospinal fluid.The analgesic effect of 2 Hz was abolished byμorκopioid receptor antagonist.The effect of 100 Hz was prevented by aκantagonist,and that of 10 kHz was blocked by any of theμ,δ,orκreceptor antagonists,suggesting that the analgesic effect of SCS at different frequencies is mediated by different endorphins and opioid receptors.

Bread wheat(Triticum aestivum L.)is one of the most important staple crops worldwide.The phytohormone auxin plays critical roles in the regulation of plant growth and development.However,only a few auxin-related genes have been genetically demonstrated to be involved in the control of plant architecture in wheat thus far.In this study,we characterized an auxinrelated gene in wheat,TaIAA15,and found that its ectopic expression in rice decreased the plant height and increased the leaf angle.Correlation analysis indicated that TaIAA15-3B was associated with plant height(Ph),spike length(SL)and 1000-grain weight(TGW)in wheat,and Hap-II of TaIAA15-3B was the most favored allele and selected by modern breeding in China.This study sheds light on the role of auxin signaling on wheat plant architecture as well as yield related traits.

A first-principles study on electronic structures and elastic properties of metal dopedα-Fe(N)high nitrogen steel

The binding energies,electronic structures and elastic properties of Ti,V,Cr,Mn,Co,Ni and Mg dopedα-Fe（N）systems have been investigated using a first-principles method.The calculated results show that the dopings of Ti,V,Cr and Co improve the stability ofα-Fe（N）,and the stability ofα-Fe（N）is slightly weakened by Mn and Ni,and the doping of Mg is disadvantageous.For Ti,V,Cr and Mn dopedα-Fe（N）systems in which the doping metals are on the left side of Fe in the element periodic table andα-Fe（N）systems doped by Co and Ni on the right side of Fe,their corresponding cohesive forces decrease with decreasing atomic radius of the doping species.The obvious interaction exists among M3 d,Fe4s3p3d and N2 p.In these doping systems,metal atoms lose electrons,while N gains electrons.Dopings of Ti,V,Cr and Mn inα-Fe（N）strengthen the interaction between N and the surrounding metals,and it is not apparent for the dopings of Co,Ni and Mg.Elastic calculations of Fe15 MN systems show that,except for the Fe15 MgN system,shear modulus G and Young modulus E of Fe15 MN systems are improved,and the bulk modulus Bslightly decreases,namely,total elastic properties are enhanced.The magnitude change rule of E reflecting the cohesive force between atoms is consistent with that for the binding energies.
