One-year survival in recipients older than 50 bridged to heart transplant with Impella 5.5 via axillary approach

BACKGROUND Optimizing patients with advanced heart failure before orthotopic heart transplantation(OHT), especially in patients greater than 50 years old, is imperative to achieving successful post-transplant outcomes. Complications are well-described for patients bridged to transplant(BTT) with durable left ventricular assist device(LVAD) support. Given the lack of data available in older recipients after the recent increase in mechanical support use, we felt it crucial to report our center’s one-year outcomes in older recipients after heart transplantation with percutaneously placed Impella 5.5 as a *** Forty-nine OHT patients were supported with the Impella 5.5 intended as a bridge between December 2019 and October 2022 at Mayo Clinic in Florida. Data were extracted from the electronic health record at baseline and during their transplant episode of care after Institutional Review Boards approval as exempt for retrospective data *** Thirty-eight patients aged 50 or older were supported with Impella 5.5 as BTT. Ten patients underwent heart and kidney transplantation within this cohort. The median age at OHT was 63(58–68) years, with 32 male(84%) and six female patients(16%). Etiology was divided into ischemic(63%) and non-ischemic cardiomyopathy(37%). The baseline median ejection fraction was 19%(15–24). Most patients were in blood group O(60%), and 50% were diabetic. The average duration of support was 27 days(range 6–94). The median duration of follow-up is 488 days(185–693). For patients that have reached the 1-year follow-up timeframe(22 of 38, 58%), the 1-year post-transplant survival is 95%.CONCLUSION Our single-center data provides awareness for using the Impella 5.5 percutaneously placed axillary support device in older heart failure patients in cardiogenic shock as a bridge to transplantation. One-year survival outcomes after heart transplantation are excellent despite the older recipient’s age and prolonged pre-transplant support.

Carbon monoxide（CO） has recently been well-established as a promising therapeutic modality against cancer. The development of transition-metal-free and tumor-targeted delivery platforms for CO is highly demanded but remains very sporadic. Herein, we developed a polyprodrug system for the co-delivery of CO and a chemotherapeutic agent combretastin A-4（CA-4） to the tumor site based on a chemiexcitation mechanism. Such a poly-prodrug successfully achieved the co-activation of CO and CA-4 in response to H2O2at the tumor site both in vitro and in vivo. Importantly, a synergist effect between CO and CA-4 was observed in suppressing tumor growth in a mouse tumor model. It is anticipated that such a poly-prodrug system could be potentially adapted for the co-delivery of CO and another drug payload for combinational therapy for other indications.

Projective Dirichlet Boundary Condition with Applications to a Geometric Problem

Given a domain Ω R^n, let λ 〉 0 be an eigenvalue of the elliptic operator L ：= ∑i,j^n= 1δ/δxi on Ω for Dirichlet condition. For a function f ∈ L2（Ω）, it is known that the linear resonance equation Lu ＋ λu = f in Ω with Dirichlet boundary condition is not always solvable. We give a new boundary condition Pλ（u｜δΩ） = g, called to be projective Dirichlet condition, such that the linear resonance equation always admits a unique solution u being orthogonal to all of the eigenfunctions corresponding to λ which satisfies ｜｜u｜｜2,2 ≤ C（｜｜f｜｜2 ＋｜｜g｜｜2,2） under suitable regularity assumptions on δΩ and L, where C is a constant depends only on n, Ω, and L. More a priori estimates, such as W^2~＇P-estimates and the C^2,α-estimates etc., are given also. This boundary condition can be viewed as a generalization of the Dirichlet condition to resonance equations and shows its advantage when applying to nonlinear resonance equations. In particular, this enables us to find the new indicatrices with vanishing mean （Cartan） torsion in minkowski geometry. It is known that the geometry of indicatries is the foundation of Finsler geometry.

Critical role of corrosion inhibitors modified by silyl ether functional groups on electrochemical performances of lithium manganese oxides

Lithium manganese oxides(Li Mn2 O4, LMO) have attracted significant attention as important cathode materials for lithium-ion batteries(LIBs), which require fast charging based on their intrinsic electrochemical properties. However, these properties are limited by the rapid fading of cycling retention, particularly at high temperatures, because of the severe Mn corrosion triggered by the chemical reaction with fluoride(F-) species existing in the cell. To alleviate this issue, three types of silyl ether(Si–O)-functionalized task-specific additives are proposed, namely methoxytrimethylsilane, dimethoxydimethylsilane, and trimethoxymethylsilane. Ex-situ NMR analyses demonstrated that the Si-additives selectively scavenged the F-species as Si forms new chemical bonds with F via a nucleophilic substitution reaction due to the high binding affinity of Si with F-, thereby leading to a decrease in the F concentration in the cell. Furthermore, the addition of Si-additives in the electrolyte did not significantly affect the ionic conductivity or electrochemical stability of the electrolyte, indicating that these additives are compatible with conventional electrolytes. In addition, the cells cycled with Si-additives exhibited improved cycling retention at room temperature and 45 °C. Among these candidates, a combination of MTSi and the LMO cathode was found to be the most suitable choice in terms of cycling retention(71.0%), whereas the cell cycled with the standard electrolyte suffered from the fading of cycling retention triggered by Mn dissolution(64.4%). Additional ex-situ analyses of the cycled electrodes using SEM, TEM, EIS, XPS, and ICP-MS demonstrated that the use of Si-additives not only improved the surface stability of the LMO cathode but also that of the graphite anode, as the Si-additives prevent Mn corrosion. Computed tomography and magnetic resonance imaging findings of metastatic rectal linitis plastica from prostate cancer:A case report and review of literature

Linitis plastica is a rare condition showing circumferentially infiltrating intramural anaplastic carcinoma in a hollow viscus, resulting in a tissue thickening of the involved organ as constricted, inelastic, and rigid. While most secondary rectal linitis plastica(RLP) is caused by metastasis from stomach, breast, gallbladder, or bladder cancer, we report an extremely rare and unique case of secondary RLP due to prostate cancer with computed tomography(CT) and magnetic resonance imaging(MRI) findings, including diffusion weighted imaging(DWI). A 78-year-old man presented with approximately a 2-mo history of constipation and without cancer history. On sigmoidoscopy, there was a luminal narrowing and thickening of rectum with mucosa being grossly normal in its appearance. On contrast-enhanced CT,marked contrast enhancement with wall thickening of rectum was noted. On pelvic MRI, rectal wall thickening showed a target sign on both T2-weighted imaging and DWI. A diffuse infiltrative lesion was suspected in the prostate gland based on low signal intensity on T2-weighted imaging and restricted diffusion. A transanal full-thickness excisional biopsy revealed metastasis from a prostate adenocarcinoma invading the submucosa to the muscularis propria consistent with metastatic RLP. We would like to emphasize the CT and MRI findings of metastatic RLP due to prostate cancer.

Wire-Shaped 3D-Hybrid Supercapacitors as Substitutes for Batteries

We report a wire-shaped three-dimensional(3D)-hybrid supercapacitor with high volumetric capacitance and high energy density due to an interconnected 3D-configuration of the electrode allowing for large number of electrochemical active sites,easy access of electrolyte ions,and facile charge transport for flexible wearable *** interconnected and compact electrode delivers a high volumetric capacitance(gravimetric capacitance)of 73 F cm−3(2446 F g−1),excellent rate capability,and cycle *** 3D-nickel cobalt-layered double hydroxide onto 3D-nickel wire(NiCo LDH/3D-Ni)//the 3D-manganese oxide onto 3D-nickel wire(Mn3O4/3D-Ni)hybrid supercapacitor exhibits energy density of 153.3 Wh kg−1 and power density of 8810 W kg−*** red lightemitting diode powered by the as-prepared hybrid supercapacitor can operate for 80 min after being charged for tens of seconds and exhibit excellent electrochemical stability under various deformation *** results verify that such wire-shaped 3D-hybrid supercapacitors are promising alternatives for batteries with long charge–discharge times,for smart wearable and implantable devices.

Effects of different Ca^(2+) behavior patterns in the electric field on membrane fouling formation and removal of trace organic compounds

The effects of Ca^(2+) on membrane fouling and trace organic compounds(TrOCs)removal in an electric field-assisted microfiltration system were investigated in the presence of Na^(+) alone for *** the electric field,negatively charged bovine serum albumin(BSA)migrated towards the anode far away from the membrane surface,resulting in a 42.9%transmembrane pressure(TMP)reduction in the presence of Na^(+) at 1.5 *** contrast,because of the stronger charge shielding of Ca^(2+),the electrophoretic migration of BSA was limited and led to a neglectable effect of the electric field(1.5 V)on membrane ***,under 3 V applied voltage,the synergistic effects of electrochemical oxidation and bridging interaction between Ca^(2+) and BSA promoted the formation of denser settleable flocs and a thinner porous cake layer,which alleviated membrane fouling with a 64.5%decrease in TMP and nearly 100%BSA *** Tr OCs elimination increased with voltage and reached29.4%–80.4%at 3 *** electric field could prolong the contact between Tr OCs and strong oxidants generated on the anode,which enhanced the Tr OCs ***,a stronger charge shielding ability of Ca^(2+) weakened the electric field force and thus lowered the Tr OCs removal.

β-Arrestin 2 suppresses the activation of YAP by promoting LATS kinase activity

Dysregulation of the Hippo pathway has been frequently identified in various human cancers. The Hippo pathway is a highly complex pathway involving multiple types of proteins, and the activation of YAP by LATS kinase is the final effector step in the transcription process. In this study, we linked the roles of the multifunctional adapter proteins β-arrestins (β-arrestin 1 and 2) in cooperation with other signaling pathways such as GPCR and Wnt to essential cellular functions involved in carcinogenesis, including the regulation of cell proliferation, migration, and differentiation as well as stem cell properties.1 Although β-arrestin 1 and 2 have high structural similarities, they have different roles in carcinogenesis as β-arrestin 1 aids cancer cell survival and metastasis2 and β-arrestin 2 inhibits tumor growth.1 In the Hippo signaling pathway, β-arrestins function as scaffold proteins that mediate the phosphorylation of key molecules, and their association with human cancers is a major research topic. In this study, we demonstrated that β-arrestin 2 inhibits YAP activation through the formation of the β-arrestin 2-LATS-YAP trimeric complex, which results in the promotion of the kinase activity of LATS in cancer cell lines and patient-derived colon cancer organoids.

Motor outcomes of patients with a complete middle cerebral artery territory infarct

Detailed knowledge of motor outcomes enables to establish proper goals and rehabilitation strate-gies for stroke patients. Several previous studies have reported functional or motor outcomes in patients with a middle cerebral artery territory infarct. However, little is known about motor outcome in patients with a complete middle cerebral artery territory infarct. In this study, we investigated the motor outcomes in 23 patients with a complete middle cerebral artery territory infarct. All of these patients received comprehensive rehabilitative management, including movement therapy and neuromuscular electrical stimulation of the affected finger extensors and ankle dorsiflexors, for more than 3 months. Motor outcomes were measured at 6 months after stroke onset using the Medical Research Council, Motricity Index, the modified Brunnstrom Classification, and Functional Ambula-tion Category scores. The motor function of the lower extremities was found to be better than that of the upper extremities. After receiving rehabilitation treatments for 3–6 months, about 70% of these patients were able to walk independently （Functional Ambulation Category scores 3）, but no pa-tient achieved functional hand recovery.
