Increasing Incidence of Diabetes Mellitus, Systemic Hypertension and Germ Cells with Endogenous Estrogen

Background: Era of contraception, abortions, [20th, 21st centuries] implemented as family welfare schemes, witnessed increased global incidence of Type 2 diabetes mellitus, systemic hypertension. Objectives: Altruistic association of contraception [if any], with increasing diabetes mellitus, systemic hypertension, was sought after. Methods: In 2012, retrospective analysis of, prevalence of diabetes mellitus, systemic hypertension, in 350 patients of 20 - 35 years, 35 - 50 years, >50 years, age groups, data collected by convenient, stratified random sampling, from different geographical locations, between 2003-2012 and its association with presence, absence of contraception, abortion was undertaken;simultaneously, serum estrogen levels obtained from 105 patients, were also analysed. Results: 10 - 45 fold increase in Type 2 diabetes mellitus was seen in contraceptive users of >20 years to >50 years;15 - 50 fold increase in incidence of systemic hypertension was seen in contraceptive users of >20 years to >50 years. Endogenous estrogen was reduced below normal in 75% of contraceptive users, and 25% of contraceptive users had low normal serum estrogen. Cholesterol deprived diet, due to decreased synthesis of endogenous estrogen, androgen also was associated with 50% increase in diabetes mellitus, systemic hypertension. Conclusion: Concept is acquired contraception, abortion status, with smashed fragmentation of germ cells, reduced endogenous estrogen, androgen, results in defaulted genomic repertoire, deranged cell metabolism, increased incidence of diabetes mellitus, systemic hypertension as part of metabolic syndrome. Contraception reversal declined the diseases as a cause and effective phenomenon.

Role of seed size, phenology, oogenesis and host distribution in the specificity and genetic structure of seed weevils (Curculio spp.) in mixed forests

Synchrony between seed growth and oogenesis is suggested to largely shape trophic breadth of seed-feeding insects and ultimately to contribute to their co-existence by means of resource partitioning or in the time when infestation *** we investigated:(i)the role of seed phenology and sexual maturation of females in the host specificity of seed-feeding weevils(Curculio spp.)predating in hazel and oak mixed forests;and(ii)the consequences that trophic breadth and host distribution have in the genetic structure of the weevil *** analyses were used to establish unequivocally host specificity and to determine the population genetic *** identified 4 species with different specificity,namely Curculio nucum females matured earlier and infested a unique host(hazelnuts,Corylus avellana)while 3 species(Curculio venosus,Curculio glandium and Curculio elephas)predated upon the acorns of the 2 oaks(Quercus ilex and Quercus pubescens).The high specificity of *** coupled with a more discontinuous distribution of hazel trees resulted in a significant genetic structure among *** addition,the presence of an excess of local rare haplotypes indicated that *** populations went through genetic expansion after recent ***,these effects were not observed in the more generalist Curculio glandium predating upon ***,co-existence of weevil species in this multi-host-parasite system is influenced by both resource and time *** what extent the restriction in gene flow among *** populations may have negative consequences for their persistence in a time of increasing disturbances(*** in Mediterranean areas)deserves further research.

Increasing Prevalence of Osteoporosis, Hypothyroidism and Endogenous Estrogen with Germ Cells

Background: Women were expressing disabling low back aches and weight gain attributing to puerperal sterilization, when contraception implementation as Family Welfare Program was progressing between ~1983 and ~1989. Hence we sought to identify any correlation existing with contraception. Methods: We performed a retrospective analysis of prevalence of osteoporosis and hypothyroid status, in 350 patients of 20-35 years, 35-50 years, >50 years age groups, from data collected by convenient, stratified random sampling of different geographical locations, between 1995-2012. We also analyzed its association with presence or absence of contraception and abortion. Simultaneously, serum estrogen levels obtained from 105 patients were also analyzed. Results: ~15-25 fold increase in osteoporosis was seen in contraceptive users with a p value of 20->50 years age group;we also found a 6 fold increase in hypothyroidism among contraceptive users aged >35 years [p 20 years age group [p 50 years age group [p raception with a p value of raception prevents traversal of normal path by germ cells, resulting in smashed fragmentation of germ cells, and consequent decreased endogenous estrogen or androgen leading to defaulted genomic repertoire, deranged cell cycle and cell metabolism resulting in metabolic syndrome, with consequent increase in osteoporosis and hypothyroid status.

Uterine Fibroids Operated in the Obstetric Gynecology Department of the Mother-Child Department at the University Hospital of Tengandogo: About 109 Cases

Background: Uterine fibroid is the most common benign gynecological tumor in women of childbearing age and is common in our context. It can be discovered incidentally or by metrorrhagia. Objective: To study the epidemiological, clinical, therapeutic and prognostic aspects of uterine fibroids operated in the gynaecology and obstetrics department of the CHU-T. Patients and Method: This was a descriptive cross-sectional study including all patients operated on for uterine fibromyoma in the gynaecology-obstetrics department. The collection mode was retrospective, over a 5-year period from January 1, 2017 to December 31, 2021 in the mother-child department of the CHU-T. Data entry and analysis were carried out on a microcomputer using Epi info 7.2.5 software. Results: We collected 109 cases of uterine fibroids which represented 42.5% of the surgical activities of the gynaecology department. The average age of the patients was 38.9 years ± 7.8. Married women represented 77.1%. Salaried women accounted for 65.1%. The main reasons for consultation were uterine haemorrhage (53.1%), pelvic pain (40.4%) and hypofertility (31.2%). Ultrasound was performed in all patients to help map the fibroid nuclei. The main indication for surgery was haemorrhagic myoma (43.1%). The surgical treatment was conservative (myomectomy) in 58.7% of cases and radical (hysterectomy) in 27.4% of cases. The most frequent postoperative complication was vulvar haemorrhage and the average hospital stay was 4 days ± 1.4. Anatomical pathological examination of the surgical excision specimen carried out on 30 operated patients concluded that uterine leiomyoma was diagnosed in 100% of cases. Conclusion: Patient education for early detection, universal health insurance and cost subsidies could improve the management of this condition.

Case 1: In 1990, a 23-year-old woman, married for two years, with primary infertility, was brought by her husband, with ultrasonography of abdomen, pelvis report stating multiple tiny cysts in both ovaries, infantile uterus;so husband claimed he was cheated to marry a woman with an infantile uterus, he wanted to divorce her on medical grounds. Analysing the problem revealed the woman had irregular menstruation before marriage due to polycystic ovaries;the husband took a prescription of oral contraceptive pills from a clinician, for one cycle to regularise menstruation of his wife;which he continued to administer for 2 years, with a desire to enable conception of his wife not understanding oral contraceptive pills with their exogenous oestrogen, suppress endogenous oestrogen preventing ovulation to conceive, produce withdrawal bleeding, due to suppressed endogenous oestrogen-suppressed uterine development resulting in infantile uterus. Case 2: In 1996, a 25-year-old woman underwent lower segment Cesarian section, 10 days prior to her EDC [expected date of child birth], as per the request of her husband who desired to see the baby before boarding his flight overseas;lower segment Cesarian section was performed by a urologist, general surgeon, but the mother expired on the theatre table, probably because the woman’s expected date of childbirth range would have fallen into the 15 days after expected date of childbirth norm, when her oestrogen would not have dipped, oxytocin would not have been released, to prevent postpartum haemorrhage. Case 3: In 1998, a 27-year-old woman presented with postpartum haemorrhage of one hour duration, following vaginal delivery of foeto placental unit;with haemoglobin of 3 gm%;immediately hysterectomy of the soft uterus was performed, mobilising 10 units of blood;once bleeding uterus was severed, all the 10 units of blood were transfused immediately, she survived. Background: Autoimmunity has increased significantly, alarmingly secondary to global contraception, abortion, fragmenting germ cells;herewith three types of life threatening manifestations, are analysed. Case-1: 28-year-old afebrile lady, vegetarian, presented to emergency with impaired responsiveness, deep jaundice, menorrhagia of one day duration and she had been taking native treatment with leaves for primary infertility for the past five years;she had a few ecchymotic patches, investigations showed severe anaemia, thrombocytopenia, haemolytic jaundice, peripheral smear confirmed thrombotic thrombocytopenic purpura;she developed profound hematuria;she received treatment with intravenous immunoglobulin, fresh frozen plasma, platelet transfusions, packed red blood cells transfusions, plasmapheresis was planned;she improved gradually. In her sincere effort to conceive a baby, for the past five years, due to cultural ignorance which teaches only three days abstinence, instead of seven days abstinence after last menstrual periods, exposure of germ cells to raw endometrial surface, from 4th to 7th days of menstrual cycle, resulted in the cumulative autoimmunity-thrombotic thrombocytopenic purpura status. Case-2: 24-year-old lady who had undergone puerperal sterilisation, on the fourth day after last menstrual periods, presented, with unresponsiveness, to emergency, her investigations revealed prolonged prothrombin time, prolonged activated partial thromboplastin time, more than a minute, thrombocytopenia, reduced fibrinogen levels suggesting disseminated intravascular coagulation;her ultra sonogram abdomen showed, absence of gestational sac in the uterus;she received fresh frozen plasma transfusions, but succumbed within few hours. Background: Electroacupuncture(EA) may reduce the severity of acute pancreatitis(AP) and provide additional pain relief in patients with chronic pancreatitis. However, the ability of EA to relieve pain in patients with AP has not been well ***: This study was undertaken to compare the pain-relieving effects of EA and conventional treatment in patients with ***, setting, participants and interventions: This study was conducted using a randomized, controlled,three-arm, parallel-group and multi-center design. Patients diagnosed with AP were randomly and equally assigned to EA1, EA2 or control groups. All participants received conventional standard-of-care therapy for AP. Local EA alone was administered in EA1, and local plus distal EA was given in *** EA included two abdominal acupoints, while distal EA included twelve peripheral acupoints. EA groups underwent one session of EA daily for 4 days(days 1–4), or until pain was resolved or *** outcome measures: The primary outcome measure was the change in the visual analogue scale(VAS;0–100) pain score between baseline and day ***: Eighty-nine participants were randomized into EA1, EA2 and control groups, and 88(EA1, 30;EA2, 29;control, 29) were included in the full-analysis set. VAS score change(median [interquartile range]) on day 5 was(12.3 ± 22.5) in the EA1 group,(10.3 ± 21.5) in the EA2 group, and(8.9 ± 15.2) in the control group. There were not significant differences in the change in VAS score among treatments(P = 0.983). However, time to food intake was significantly shorter in the EA group(EA1 + EA2) than in the control group(median 2.0 days vs 3.0 days), with a hazard ratio of 0.581(P = 0.022;95% CI,0.366–0.924). No significant adverse events ***: EA treatment did not significantly reduce pain after 4 days of treatment in patients with AP-associated abdominal pain but significantly reduced time to first food intake.
