Background Understanding the evolution of circadian rhythm dysfunction and psychopathology in the high-risk population has important implications for the prevention of bipolar ***,some of the previous studies on the emergence of psychopathologies and circadian dysfunction among high-risk populations were inconsistent and *** To examine the prevalence rates of sleep and circadian dysfunctions,mental disorders and their symptoms in the offspring of parents with(O-BD)and without bipolar disorder(O-control).Methods The study included 191 O-BD and 202 O-control subjects aged 6-21 years from the Greater Bay Area,*** diagnoses and symptoms of sleep/circadian rhythm and mental disorders were assessed by the Diagnostic Interview for Sleep Patterns and Disorders,and the Schedule for Affective Disorders and Schizophrenia for School-Age children-Present and Lifetime Version,*** estimating equations and shared frailty proportional hazards models of survival analysis were applied to compare the outcomes in the *** Adjusting for age,sex and region of recruitment,there was a significantly higher risk of delayed sleep phase symptoms(9.55%vs 2.58%,adjusted OR:4.04)in O-BD than in O-control.O-BD had a nearly fivefold higher risk of mood disorders(11.70%vs 3.47%,adjusted OR:4.68)and social anxiety(6.28%vs 1.49%,adjusted OR:4.70),a fourfold higher risk of depressive disorders(11.17%vs 3.47%,adjusted OR:3.99)and a threefold higher risk of mood symptoms(20.74%vs 10.40%,adjusted OR:2.59)than *** analysis revealed that O-BD children(aged under 12 years)had a nearly 2-fold higher risk of any mental and behavioural symptoms than O-control,while there was a nearly 4-fold higher risk of delayed sleep phase symptoms,a 7.5-fold higher risk of social anxiety and a 3-fold higher risk of mood symptoms in O-BD adolescents(aged 12 years and over).Conclusions There was an increase in delayed sleep phase symptoms in O-BD

Comparing 11 early warning scores and three shock indices in early sepsis prediction in the emergency department

BACKGROUND:This study aimed to evaluate the discriminatory performance of 11 vital sign-based early warning scores(EWSs)and three shock indices in early sepsis prediction in the emergency department(ED).METHODS:We performed a retrospective study on consecutive adult patients with an infection over 3 months in a public ED in Hong *** primary outcome was sepsis(Sepsis-3 definition)within 48 h of ED *** c-statistics and the DeLong test,we compared 11 EWSs,including the National Early Warning Score 2(NEWS2),Modified Early Warning Score,and Worthing Physiological Scoring System(WPS),etc.,and three shock indices(the shock index[SI],modified shock index[MSI],and diastolic shock index[DSI]),with Systemic Inflammatory Response Syndrome(SIRS)and quick Sequential Organ Failure Assessment(qSOFA)in predicting the primary outcome,intensive care unit admission,and mortality at different time ***:We analyzed 601 patients,of whom 166(27.6%)developed ***2 had the highest point estimate(area under the receiver operating characteristic curve[AUROC]0.75,95%CI 0.70-0.79)and was significantly better than SIRS,qSOFA,other EWSs and shock indices,except WPS,at predicting the primary ***,the pooled sensitivity and specificity of NEWS2≥5 for the prediction of sepsis were 0.45(95%CI 0.37-0.52)and 0.88(95%CI 0.85-0.91),*** discriminatory performance of all EWSs and shock indices declined when used to predict mortality at a more remote time ***:NEWS2 compared favorably with other EWSs and shock indices in early sepsis prediction but its low sensitivity at the usual cut-off point requires further modification for sepsis screening.

Background: Alterations of rest-activity pattern are common among the patients with bipolar disorder and increasingly seen among their family members. There is emerging evidence of rest-activity pattern alterations with regard to the emergence of psychopathologies in the high-risk population. However, the genetic associations between rest-activity pattern and bipolar disorder are largely unknown. The current study aimed to determine the genetic associations of objective rest-pattern and bipolar disorder by using a large-scale pedigree ***: A total of 835 participants(58.6% female;mean age: 28.93 ± 16.53 years old) of 320 families were recruited in this study from a bipolar offspring study, of whom 120 participants had bipolar disorder. All the participants were invited to accomplish the screening clinical interviews to determine the diagnosis of bipolar disorder. One-week Actigraphy was employed to continuously measure rest-activity pattern, including rest pattern parameters, activity pattern parameters, and circadian rhythm parameters. The activity data were cleaned and analyzed carefully by the "GGIR" package(version 1.11-0) in R project. The Sequential Oligogenic Linkage Analysis Routines(SOLAR) program was used to analyze the genetic associations of objective rest-activity pattern with bipolar ***: Totally, 9 rest pattern parameters, 8 activity pattern parameters, and 7 circadian rhythm parameters derived from Actigraphic data were examined. Eight rest pattern parameters, 7 activity pattern parameters, and 3 circadian rhythm parameters were found to have significant heritability(h2 range: 0.22 – 0.49). Three activity pattern parameters and two circadian rhythm parameters were found to be genetically associated with bipolar disorder. After further adjustment by the Benjamini-Hochberg false discovery rate control procedure, only the genetic associations of start time of active period(ρg = 0.51, Q =.03) and daytime total sustained inactivity

Background:Surface-Enhanced Laser Desorption/ionization Time-of-Flight Mass Spectrometry Protienchip system(SELDI-TOF-MS)is a frequent used technique for cancer biomarkers *** specificity of biomarkers detected from SELDI can be influenced by coexistent with inflammatory *** study aimed to increase the detection accuracy by a two-stage analysis ***:Serums from 118 lung cancer patients,72 healthy individuals and 31 inflammatory disease patients were divided into training group and testing group randomly by 3:2 *** stage training method use SELDI profiles analysis to form a decision making tree to directly distinguish lung cancer patients from all the serum in the training *** twostage analysis is to increase the steps of partitioning analysis from distinguishing the healthy people and non-healthy patients(1st-stage)and then differentiating cancer patients from inflammatory disease patients(2nd-stage)to minimize the influence of inflammation.A decision tree by combination of this two-stage training process was validated in the test *** the test group,one stage method has 87.2%sensitivity,37.5%specificity and 64.4%*** two-stage method has lower sensitivity(>70,1%),but higher specificity(80%)and accuracy(74.7%)with statistically *** predominately expressed protein peak at 11480 Da was the primary splitter regardless one-stage or two-stage *** peak was most likely to be SAA(Serum Amyloid A)with average m/z countered around this *** significance of this important primary splitter was further confirmed by ELISA ***:The inflammatory disease severely interfere the detection accuracy of lung cancer by SELDI *** a two-stage training process will improve the specificity and accuracy in detection of lung cancer.

目的　两个线粒体突变 32 4 3A→G和 3316G→A已经被报道发现在糖尿病患者中 ,本研究旨在调查这个可能的糖尿病相关区域 3116→ 335 3在中国人二型糖尿病中的角色。方法　应用聚合酶链反应 ,直接的DNA测序 ,限制性长度片段多态性和等位特异聚合酶链反应等方法筛查 2 77个二型糖尿病患者和 2 4 1个正常对照的线粒体目的基因区域。对检出的序列改变进行Fisher精确统计分析 ,并应用RNA折叠软件mfold预测 16SrRNA基因区域的变异所引起的最小自由能二级结构改变以确定其功能意义。结果　检出 4个同质体碱基取代 :32 0 0T→C ,32 0 6C→T ,32 90T→Cand 3316G→A。其中 32 0 0T→C只出现在患者中 ,另外三个在患者和正常人中均有检出 ,统计分析显示其频率的不同无显著意义。 32 0 0T→C和 32 0 6C→T位于16SrRNA ,为该研究首次检出 ,前者引起了最小自由能二级结构模型的极大改变 ,而后者几乎没有引起结构变化。结论　本结果建议 32 0 0T→C与二型糖尿病的发展相关 ,而其他几种变异的影响是中性的。不同于日本人的研究 ,3316C→T与二型糖尿病无关。
