standardizing MI-TLIF, a proposal for a Reproducible Technique

Background: Minimally invasive transforaminal lumbar interbody fusion (MI TLIF) is a widely known and performed technique, however its versatility among different physicians continues to hinder its replication and results. Therefore, this study aimed to provide a step-by-step surgical guide to perform a safe MI-TLIF, based on the results obtained in patients operated on by a single surgeon over a period of 12 years. patients and methods: A retrospective, single center, longitudinal, and observational cohort study was conducted with 931 patients who underwent MI TLIF by a single surgeon between 2010 and 2022 using the technique described on this paper, each with a minimum follow-up of 12 months. Criteria included schizas classification, listhesis according to Meyerding classification, number of levels treated, cage size, and complications (screw repositioning or cerebrospinal fluid leak). patient clinical outcomes were assessed using the Oswestry Disability Index (ODI), Visual Analog scale (VAs) for pre- and postoperative radicular pain. Thin slice CT scans were used to assess the progression of the fusion using the Bridwell classification. In the statistical analysis, percentages, median, and interquartile range (IQR) were calculated. Results: Nine hundred and thirty one patients underwent MI TLIF using the technique described, eight hundred and eighty (94.5%) had a single level treated and fifty one (5.5%) had a 2 level procedure (982 levels), an 8mm cage was placed on five hundred and seventeenlevels (52.7%), six hundred and sixty three levels(67.6%) achieved grade I fusion, two hundred and sixty six levels (27.1%) achieved grade II fusion, 52 levels (5.3) achieved grade III fusion and one level (0.1) achieved a grade IV fusion or non-union. Revision surgery was performed on 3 patients (0.3%) for screw repositioning, cerebrospinal fluid leak was present on 2 patients during surgery and treated before closure. Academic stress Assessment Using Virtual Reality as an Educational Tool in spine surgery

Introduction: The evaluation of academic stress in medical students and residents is a topic of significant interest, given the considerable challenges they face during their learning process with traditional teaching methods. The use of technologies like virtual reality presents an opportunity to enhance their skills through simulations and training. The main objective of this study is to qualitatively assess the stress levels experienced by medical students and residents by integrating virtual reality into their current learning methods, aiming to improve their ability to manage stressors in their practice. Material and Methods: A questionnaire was conducted with 12 medical students and 12 Traumatology and orthopedics residents. The purpose of the questionnaire was to evaluate the levels of academic stress using the sIsCO inventory. The stress levels were calculated by transforming average values into percentages, and the following criteria were assigned: 0 to 33% for Mild stress, 34 to 66% for Moderate stress, and 67 to 100% for Deep stress. Then, a virtual reality class focused on spine surgery was provided. Both medical students and residents were trained using the Non Nocere sharpsurgeon software platform and Oculus Quest 2 virtual reality glasses. At the end of the session, a second questionnaire related to the practice with virtual reality was conducted with the same evaluation criteria and a comparative analysis was carried out. Results: 12 undergraduate students from Hospital Angeles Mexico, CDMX and 12 traumatology and orthopedics residents at Hospital santa Fe, Bogota were evaluated. The students in CDMX reported an average qualitative stress of 28.50% during habitual practices, which decreased to an average of 14.67% after virtual reality practice. Residents in Bogotá experienced an average qualitative stress of 30.50% with their current learning methods but this reduced to an average of 13.92% after using virtual reality. Endotoxin-induced alterations of adipose tissue function:a pathway to bovine metabolic stress

During the periparturient period, dairy cows exhibit negative energy balance due to limited appetite and increased energy requirements for lactogenesis. The delicate equilibrium between energy availability and expenditure puts cows in a state of metabolic stress characterized by excessive lipolysis in white adipose tissues(AT), increased production of reactive oxygen species, and immune cell dysfunction. Metabolic stress, especially in AT, increases the risk for metabolic and inflammatory diseases. Around parturition, cows are also susceptible to endotoxemia. Bacterial-derived toxins cause endotoxemia by promoting inflammatory processes and immune cell infiltration in different organs and systems while impacting metabolic function by altering lipolysis, mitochondrial activity, and insulin sensitivity. In dairy cows, endotoxins enter the bloodstream after overcoming the defense mechanisms of the epithelial barriers, particularly during common periparturient conditions such as mastitis, metritis, and pneumonia, or after abrupt changes in the gut microbiome. In the bovine AT, endotoxins induce a pro-inflammatory response and stimulate lipolysis in AT, leading to the release of free fatty acids into the bloodstream. When excessive and protracted, endotoxin-induced lipolysis can impair adipocyte's insulin signaling pathways and lipid synthesis. Endotoxin exposure can also induce oxidative stress in AT through the production of reactive oxygen species by inflammatory cells and other cellular components. This review provides insights into endotoxins' impact on AT function, highlighting the gaps in our knowledge of the mechanisms underlying AT dysfunction, its connection with periparturient cows' disease risk, and the need to develop effective interventions to prevent and treat endotoxemia-related inflammatory conditions in dairy cattle.

Multi-azimuth walkaway vertical seismic profiling is an established technique for the estimation of in situ slowness surfaces and inferring anisotropy ***,this technique requires the assumption of lateral homogeneity,which makes the horizontal slowness components at depths of downhole receivers equal to those measured at the *** violations of this assumption,such as lateral heterogeneity or nonzero dip of intermediate interfaces,lead to distortions in reconstructed slowness surfaces and,consequently,to errors in estimated anisotropic *** this work,we relax the assumption of lateral homogeneity and discuss how to correct vertical seismic profile data for weak lateral *** describe a procedure of downward continuation of recorded traveltimes that accounts for the presence of both vertical inhomogeneity and weak lateral heterogeneity,which produces correct slowness surfaces at depths of downhole receivers,noticing that sufficiently dense receiver coverage along a borehole is required to separate influences of vertical and lateral heterogeneity on measured traveltimes and obtain accurate estimates of the slowness *** the slowness surfaces are found and a desired type of anisotropic model to be inverted is selected,the corresponding anisotropic parameters,providing the best fit to the estimated slownesses,can be *** invert the slowness surfaces of p-waves for parameters of the simplest anisotropic model describing dipping fractures(transversely isotropic medium with a tilted symmetry axis).Five parameters of this model,namely,the p-wave velocity V0 in the direction of the symmetry axis,Thomsen's anisotropic coefficients e and d,the tilt n,and the azimuth b of the symmetry axis,can be estimated in a stable manner when maximum source offset is greater than half of receiver depth.

Remediation of amide pesticide-polluted soils by combined solarization and ozonation treatment

Agriculture has a close relationship with nature,but it can also be the source of negative and permanent environmental *** use of pesticides in modern agriculture is a common practice,but their side effects on the environment cannot be *** this study,we evaluated a combination of solarization and ozonation techniques for the elimination of six amide pesticides(boscalid,chlorantraniliprole,cyflufenamid,fluopyram,napropamide,and propyzamide)in *** experiments were performed with four different soils to assess the efficiency of this methodology at different soil temperatures and ozone dosages under laboratory conditions,and then a greenhouse pot experiment was conducted under controlled conditions during *** days after the onset of the experiments,higher degradation percentages of amide pesticides were observed in ozonized soils than in other treated soils,particularly when ozone was applied at 10 cm soil *** results show that the utilization of ozonation,along with solarization,represents a valid method for degrading residues of the studied pesticides and suggest that this combined technology may be a promising tool for remediating pesticide-polluted soils.

Corrosion testing of X52 and X80 steels immersed in stimulated emulsions using a real petroleum sample

The aim of this study is to evaluate the internal corrosion process on X52 and X80 steels/real petroleum interfaces containing condensed hydrocarbon plus oilfield-produced water,which were subjected to stimulated emulsions using 50/50 vol ratio mixtures at 45℃,different hydrodynamic conditions,1 h,and 24 h.A washing process by using deionized water was proposed to simulate and identify the corrosiveness of the hydrocarbon phase after 24 h of exposure *** characterization by electrochemical impedance spectroscopy and the monitoring of the polarization curves indicated that X80 steel/oilfield-produced water interfaces were more susceptible to corrosion than X52 steel exposed to oilfield-produced *** combined speed rotation of 600 rpm using a magnetic stirrer+600 rpm using a rotating disk electrode decreased the corrosion rate on X52 *** stimulated emulsions made of hydrocarbon+oilfield-produced water and hydrocarbon+deionized water at 24 h increased the corrosion rate on X80 steel(0.34 mm/year and 0.43 mm/year,respectively),promoting the formation of erosion and pitting *** types of corrosion depended mainly on the physicochemical properties of the hydrocarbon,oilfield-produced water,exposure times,and hydrodynamic systems in which the hydrocarbon was studied.

Historical fluxes of metal and metalloids in an aquatic ecosystem affected by land-use change and mining activities in northwestern Mexico

Freshwater reservoirs are essential owing because of their ecological, economic, and social *** are particularly vulnerable to contamination, as of metal and metalloids, derived from anthropogenic activities like mining. The temporal variations in trace element concentrations(arsenic(As),mercury(Hg), chromium(Cr), copper(Cu), nickel(Ni), lead(pb), and zinc(Zn)), enrichment, fluxes, and possible sources were evaluated by studying two sediment cores from the La Angostura(ANG) Reservoir(northwest Mexico), using210pb geochronology. The enrichment factors showed from null to minor enrichment for most elements, but moderate to severe enrichment of mercury(Hg). Most trace element concentrations had a detrital origin, and notable Hg concentration increases since the past decade were associated with severe drought periods, likely resulting from wildfires. The observed sediment concentrations of As and Hg can cause adverse effects on biota in the ecosystem since they are above the probable effect level(pEL). Development of strategies for metal attenuation in this reservoir is recommended and metals should be controlled until specific ecotoxicological studies are performed.
