This study aimed to report the outcomes of active surveillance(AS)in the management of low-risk prostate cancer(PCa).It recruited87 men who were prospectively followed up according to the Prostate Cancer Research International Active Surveillance(PRIAS)protocol with local adaptation at SH ho Urology Centre,Prince of Wales hospital,hong Kong,*** investigated the predictorsof disease progression and found that baseline prostate-specific antigen density(PSAD)and the presence of the highest ProstateImaging-Reporting and Data System(PI-RADS)score 5 lesion on magnetic resonance imaging(MRI)are significantly correlatedwith disease ***,men with PSAD>0.2 ng ml^(−2)or PI-RADS 4 or 5 lesions had significantly worse upgradingfree survival compared to those with PSAD≤0.2 ng ml−2 and PI-RADS 2 or 3 *** study concludes that AS is a safe andeffective management strategy for selected patients to defer radical treatment and that most disease progression can be detectedafter the first repeated *** combination of PSAD>0.2 ng ml^(−2)and PI-RADS 4 or 5 lesions may serve as a useful predictorof early disease progression and provide a guide to optimize follow-up protocols for men in different risk groups.

目的探讨贝母素甲(PME)对慢性阻塞性肺疾病(COPD)小鼠的作用和相关机制。方法将80只小鼠随机分为4组(每组20只):对照组、PME组、COPD组和治疗组。使用脂多糖联合烟雾诱导小鼠COPD动物模型。通过组织病理学、超微结构、小鼠肺组织湿/干重比值分析PME对COPD模型鼠结构的影响;ELISA和Western blotting分析PME对肺组织中炎症因子肿瘤坏死因子(TNF)-α、白细胞介素(IL)-6和IL-1β表达的影响;二氢乙锭(DHE)染色和Western blotting分析PME对肺组织中氧化应激反应的影响;Western blotting分析PME对核因子κB(NF-κB)通路以及核因子2相关因子2(Nrf2)通路相关蛋白表达的影响。结果与COPD组相比,PME治疗可明显减轻小鼠肺组织的损伤和减少炎症细胞数量,降低肺组织湿/干重比。与对照组相比,COPD组小鼠的肺泡灌洗液中TNF-α、IL-6及IL-1β水平明显增加,经PME治疗后小鼠肺泡灌洗液中TNF-α、IL-6及IL-1β水平明显降低。另外,与对照组相比,COPD组小鼠肺组织中TNF-α和IL-1β水平显著升高,经PME治疗后小鼠肺组织中TNF-α和IL-1β蛋白水平明显下降。免疫组织化学和Western blotting实验显示,与对照组相比,COPD组SOD2水平明显降低,而PME治疗后能够提高超氧化物歧化酶(SOD)蛋白水平。对肺组织丙二醛(MDA)含量分析发现,与COPD组比较,PME治疗后明显抑制COPD小鼠肺组织中MDA的产生。Western blotting结果显示,PME治疗后能够阻止肺组织中的NF-κB抑制蛋白(IκBα)磷酸化以及NF-κB p65向细胞核转移,PME处理后的小鼠肺组织中Nrf2及其主要下游靶点血红素加氧酶1(ho-1)的表达明显升高。结论PME能够抑制COPD小鼠体内的炎症和氧化应激反应,改善脂多糖联合烟雾诱导肺组织损伤,其机制可能与激活Nrf2通路和抑制NF-κB通路相关。

Catheter-Assisted Interlaminar Approach for Cervical Epidural Steroid Injection in Patient with Cervical Stenosis Caused by Ossification of Posterior Longitudinal Ligament: A Case Report

We present the case of a 64-year-old man with cervical ossification of the posterior longitudinal ligament (OPLL) experiencing chronic neck pain and radiculopathy for 6 months. A catheter-assisted interlaminar Cervical Epidural Steroid Injection (CESI) was performed under fluoroscopic guidance, targeting the affected C2-C6 levels. Significant improvement was observed after this procedure, with decreased pain scores (visual analogue scale (VAS) 8 to 2) and improved mobility. This technique not only enhances the effectiveness of CESI but also reduces the likelihood of complications such as stroke or epidural hematoma and thus provides an alternative treatment option for patients with multiple stenotic levels who are unsuitable for surgery or are unresponsive to conservative therapy such as medication or physical therapy.

BACKGROUND This case study describes an atypical presentation of avascular necrosis(AVN)of the first metatarsal head,which is largely unfounded in the *** SUMMARY A healthy 24-year-old female initially presented with pain at the first metatarsophalangeal joint(MTPJ)and was diagnosed with AVN by physical examination and magnetic resonance *** patient demonstrated atypically poor progress in recovery,despite being in otherwise good health and being of young age,with no history of corticosteroid or alcohol *** patient also did not have any history or clinical features of autoimmune disease or vasculitis,such as systemic lupus *** patient was managed with conservative treatment for 18 mo,which allowed for gradual return of full range of motion of the first MTPJ and subsiding pain,permitting the patient to return to highintensity sports training and full *** her recovery,many differential diagnoses were ruled out through specific investigations leading to further reinforcement of the diagnosis of AVN of the 1st metatarsal *** Atypical AVN may occur with no predisposing risk *** is mainly conservative,with unclear guidelines in literature on management.

AIM： To assess the incidence of and risk factors for gallstone disease （GSD） among type 2 diabetics in Kinmen, Taiwan. METhoDS： A screening program for GSD was performed by two specialists who employed real-time abdominal ultrasound to examine the abdominal region after patients had fasted for at least eight hours. Screening, which was conducted in 2001, involved 848 patients diagnosed with type 2 diabetes. After exclusion of 63 subjects with prevalent GSD, 377 participants without GSD were invited in 2002 for a second round of screening. A total of 281 （74.5%） subjects were re-examined. RESULTS： Among the 281 type 2 diabetics who had no GSD at the first screening, 10 had developed GSD by 2002. The incidence was 3.56% per year （95%CI： 1.78% per year-6.24% per year）. Using a Cox regression model, age （RR = 1.07, 95% CI： 1.00-1.14）, waist circumference （RR = 1.12, 95% CI： 1.01-1.29）, and ALT （RR = 1.13, 95%CI： 1.01-1.26） appeared to be significantly correlated with development of GSD. CONCLUSION： Older age is a known risk factor for the development of GSD. Our study shows that greater waist circumference and elevated ALT levels are also associated with the development of GSD among type 2 diabetics in Kinmen.

BACKGROUND Acral persistent papular mucinosis(APPM)is a rare idiopathic subtype of localized lichen *** date,there have been 40 APPM cases reported worldwide;however,only 7 cases have been reported in the Korean *** SUMMARY A 70-year-old man was referred to our hospital with a solitary pinkish nodule on the dorsum of his right *** the absence of symptoms,the patient wanted to know the exact diagnosis;thus,a biopsy was *** examination of a biopsy specimen obtained from the nodule on the dorsum of his hand revealed orthokeratotic hyperkeratosis with patchy parakeratosis,prominent hypergranulosis,and diffuse dissecting mucinous deposition between collagen bundles,along with some bland-looking spindle cells throughout the *** nodule was histologically diagnosed as an APPM,and an intralesional triamcinolone injection(2.5 mg/mL)was started every 2 *** three sessions of treatment,the patient showed marked *** To the best of our knowledge,this is the first case of a Korean APPM presenting as a solitary nodule that showed a marked response to triamcinolone intralesional *** it is a rare disease,we report this case to contribute to future research on the pathogenesis and treatment of APPM.

The efficient production of hydrogen through electrocatalytic decomposition of water has broad prospects in modern energy ***,the catalytic efficiency and durability of hydrogen evolution catalyst are still very deficient,which need to be further *** in this work,we prove that introducing a graphene layer(Gr)between the molybdenum disulfide and nickel foam(Ni-F)substrate can greatly improve the catalytic performance of the *** to the excitation of local surface plasmon resonance(LSPR)of gold nanoparticles(NPs),the electrocatalytic hydrogen releasing activity of the MoS_(2)/Gr/Ni-F heterostructure is greatly *** results in a significant increase in the current density of AuNPs/MoS_(2)/Gr/Ni-F composite material under light irradiation and in the dark at 0.2 V(versus reversible hydrogen electrode(RHE)),which is much better than in MoS_(2)/Gr/Ni-F composite *** enhancement of hydrogen release can be attributed to the injection of hot electrons into MoS_(2)/Gr/Ni-F by AuNPs,which will improve the electron density of MoS_(2)/Gr/Ni-F,promote the reduction of H_(2)O,and further reduce the activation energy of the electrocatalyst hydrogen evolution reaction(HER).We also prove that the introduction of graphene can improve its stability in acidic catalytic *** work provides a new way of designing efficient water splitting system.
